Good day po here are the questions related to SF5:

1. Red color po ba ang general average if failed? - black pa rin po, wala pong gagamiting red2. Same data po ba sa SF5 ang ilalagay sa form 138 and form 137? - dapat po magkakapareho sila esp sa gen. ave. & descriptive equiv., action taken, and incomplete subject/s (if any)3. Ang MAPEH grade 7 po ay may 4 areas what if nagfailed daw po sa isang area pano daw po ang general average? - kahit may failed na isang subject or component, ganun pa rin ang pag-compute ng gen. ave. since by subject pa rin ang basis for promotion/retention4. If one subject lang po ang pinasa ng bata irregular po sya pero for promotion? - as per deped order, yes but student must undergo extensive intervention; maglalabas daw po ang central office ng clarifications regarding dito, until then, ganun po ang mangyayari.

1. Yung form 7 po ba gagamitin na ngayong end of school year or next SY? sa memo po kasi form 5 and 6 lang pero nabangit din po na as of March 31,2014 data.  - as per deped order, form 5 and 6 ang gagamitin this end of sy 2013-14 pero mag-aaccomplish din po kayo ng sf-7 as part of yearend reports with data as of march 31, 20142. Pano po yung ngsummer then pumasa na tapos ngenrol na po as regular student iddeclare pa po ba dun sa column ng incomplete subject yung nasummer nya since as regular student po sya ngenrol?  - if the student is enrolled this sy, yung ipinasa nyang subjects summer 2013 ay iindicate sa incomplete subject/s-first column3. Yung level of proficiency  na may 74% and below na average is for retained students lang po diba since wala naman po general average ang irregular students?  - yes4. Pano po kpag fourth year na student then naging irregular sya sa action taken for promotion pa din po ba yung student?  - since rbec ang curriculum ng 4th year, kapag may bagsak, status is retained.
