1. INTRODUCTION

The purpose of this plan is to establish the procedures for the safe installation and dismantle of the below mentioned works facilities. The plan has been prepared in accordance with all current and relevant statutory legislation. An understanding of the contents of the Method Statement is mandatory for Harsco Infrastructure supervisors and employees at each project. It outlines the responsibilities, procedures and actions required of all employees and other persons at a workplace so as to achieve a high standard of workplace health and safety whilst working on a Harsco Infrastructure project. It is expected that all persons at the workplace will accept their responsibilities and obligations as prescribed in this plan. This will enable them to ensure compliance with all procedures outlined within

3.1 Nature of work:

Erection and dismantling of mobile scaffolding inside tank 501,502,503 and 504 on the pontoon deck to allow blasting and painting of the tank roof.

3.3 Personnel All squads engaged in this work activity will contain a balance of qualified operatives in accordance with the type of scaffolding required. All operatives working on the site will be deemed competent to carry out work of this nature and will have been briefed on the use of all fall arrest equipment for works on scaffolding projects.

3.4 Materials • Cuplok/ Tube and Fittings meet the standard requirement of BS 1139 1&2 (1991). • Timber Scaffold Boards meet the standard requirement for BS2482.

4 GENERAL REQUIREMENT

All Vendor‘s site activities shall comply with project HSE requirements.

Vendor is responsible for ensuring that an exclusion zone is in place where we are erecting scaffold on the Pontoon, with clearly displayed signage and barriers to warn other trades of the dangers of entry to the area. Contractor is responsible for enforcing the exclusion zone.

Lay down areas for materials will be established once men and materials are present at site and consideration will be taken for other trades and any restrictions to allow for clear access to all parties at all times as best is possible. Whilst undertaking any scaffolding works Vendor will barrier off the work area and display signage warning of the risks to other parties. Scaffolds will display a 'Do Not Use' Tag whenever they are incomplete or not fit to be used, and will display a Scafftag whenever they are fit to use. Only qualified Vendor scaffolders are authorized to carry out scaffolding works. Contractor should advise all other parties on the dangers of interfering with or altering scaffolds when they are not trained or authorized to do so.

Vendor will ensure that access/egress as well as emergency escape routes are clear of materials at all times.

All operatives are to obey all site specific rules and regulations regarding environmental considerations whilst on site. Vendor will, at all times ,take great care to prevent materials falls and to prevent materials falls and to prevent damage to any components/area of the site.

6 SAFETY WORK PROCEDURE

6.1 Preparation phase

Site inspection, risk Assessment and Rescue plan to be completed prior to the job start. Findings to be presented as ToolBox Talk prior to job start. Scaffold team will sign to say that they have received and understand the toolbox talk. Scaffold team will ensure that all PPE/Specialist rescue equipment is moved to the work location, and passed it for use, prior to job start. Scaffold Supervisor to carry out site Health & Safety Checklist prior to any .

6.2 Erection Phase.

Scaffolders will erect each scaffold to the correct specification as the job dictates e.gf standard spacing, lift height etc. Scaffold will be formed on firm and level bases .The foundations of the scaffold should be adequate to carry and dispose the load imposed both locally at each standard and in general to carry the whole weight of the scaffold. The foundation for the scaffold should be maintained in an adequate condition during the life of the scaffold. Tube & Fitting- All standards will be vertical and plumb and at correct centres for type of scaffold req. All joints shall be staggered using sleeve couplers. All ledgers will be horizontal ,level ,at the correct lift heights and fixed to standards using load bearing right angle couplers. All joints in ledgers will be staggered using sleeve couplers. Boards across the width of the scaffold will be of the same length, clean and clear of debris and of the same nominal thickness. Scaffold boards will be fixed into place by connecting to Transom's using board retaining coupler's or rope/wire. Overhang of boards will not exceed 4 times their thickness.

Cuplok- All scaffolds will be erected as per manufacturer’s recommendations. Scaffold beams will be fixed using doubles and will have the required amount of chords, knee bracing, plan bracing etc as per the job requirements. Ladders will be stand on a firm and level base and will be installed as required. Ladders will be fixed at an angle of 75 degrees (4 to 1) and will project at least 1m above the working platform. Scaffolders will at all times use safe systems of work and ensure compliance with NASC Guidelines SG4:05, to include the use of personal fall protection harness clipped at all times (whilst erecting, dismantling or modifying scaffolds) to suitable anchor points, using a minimum of 4 properly supported boards, the provision of single guardrails on non-working lifts, and using tunneling system

when erecting/dismantling scaffolds. Scaffolders will at all times use inertia reels attached to their harnesses whilst erecting scaffolds.

All scaffolds will be erected in accordance with BS EN-12811-1(2003) and NASC SG4:05

6.3 Sequence of work for Erecting Scaffold. 1) Scaffold material will be off loaded in a barricaded area near by the tank and lift to the roof

deck of the tank by using a crane.( Crane and banks man should provide Contrator ) 2) Material will be shift inside the tank manually from the roof deck. 3) Scaffolders will begin by fixing the castor wheel at the bottom of the standards. 4) Once 4 nos wheels are fix on the standards, ledgers will be installed. These ledgers will be

2.5M in length. Next transoms will be installed. 5) Additional Board supporting transoms will be installed at correct centers , then working

boards will be placed on the scaffold 6) Bracing will be installed accordingly progress of the scaffold structure erection. 7) The working platform will be fully boarded and the boards will secure with board retaining

couplers to prevent moving. 8) Double guard rails and toe boards will install to prevent falls of men and materials from the

working platform. Ladder access also will provide inside of the scaffold. 9) Once this scaffold will complete site Supervisor will inspect the scaffold and a green tag will

provide on the ladder of the scaffold. 10) A Hand over certificate will be issued to the Contrator.

7. HSE Requirement

The manual blasting activities shall be carried out as per 5638-E1-1500-SA-PRO-001Project HSE Plan . HSE requirement refer to Risk Accessment For Scaffolding Erection, Dismantle and Modification Inside Tank .

a) Give safety trainings to construction operators before construction begins. b) The scaffolding Supervisor shall carry out daily check before commence the work. c) Proper ventilation arrangement shall be made while scaffolding erection. All the vents ,

manhole and any other outlet inside shop or inside tank shall be completely open during manual blasting to keep air circulation .Contractor may installation exhaust fan at roof vent or manhole and run air fan outside the tank on the ground if necessary .

d) Site supervisor or HSE officer shall make sure no other activities shall be carried out in scaffolding erection area.

e) Workers shall wear appropriate protective grear when dealing with manual blasting equipment. Eye protection, respiratory protection, protective shoes, clothing and gauntlet gloves are some of the most important sandblasting safety accessories.

f) Lifeline must be checked and fixed firmly to scaffolding handrail before operating on them.

Do not attempt to lift or pass a weight that you feel is too heavy or that you are not comfortable with. Where possible , all short materials(Under 3M) should be passed up through the scaffold lift and not up the out side.

4 Site Hazard

Recognition. Unforeseen or unknown hazards.

Scaffold team Super visor/Manager to carry out pre-start site survey safety checklist prior to starting work on the project for the first time. Site safety plan comprising Method Statement and Risk Assessmentbased on findings to be produced,sent to site,disseminated to the workforce and recorded as received and understood by the workforce on a Tool box talk record. Job health and safety checklist to be copleted by foreman prior to work commecing for first time at site and at the start of each shift. Site Task Observation Reports are completed one per month per site by contracts supervisors. Scaffold team are encouraged to submit site hazard observations from all sites via near missing cards,increasing general and specific hazard awareness through the workforce. All persons are required to attend site induction training prior to entering site for the first time.

Always ensure that work areas are barricaded off using physical barriers/warning tape and clearly displayed signage. Set up additional exclusion zone within general exclusion zone to keep Harsco employees out of direct line of materials which may be being passed or lifted up the outside of the scaffold. Leave yourself enough room within area to carry out your task safely and easily. Ensure Gin wheels/ropes/lifting bags are fit for use Do not attempt to lift a weight that you or your colleages are not comfortable with. Where possible, all materials under 3m in length should be passed up internally through the scaffold, not externally Ensure good communication (verbal and action) and stay alert when passing materials hand to hand Keep stacked material on scaffold to a minimum. Ensure there are no gaps in the platform that material can fall through. Do not stack material higher than the toeboards. Stack short tubes at right angles to the boards. Ensure main contractor removes all debris from boarded lifts prior to dismantling. Do not overload scaffold with stored equipment.
