Nurse Rachel Tomczek is Tolt’s School Nurse and is at Tolt 5 days a week from 8:15a-3:15p Monday-Thursday and 8:15a-1:45p on early release Fridays.
Forms for Incoming 6th Graders The forms you need to turn in by the first day of school:
Annual Student Health Form – find on our district webpage under Health Services and page 4 of our Tolt Registration Packet available on our Tolt website.
Medication Authorization Forms – if your student will be taking any prescription medications during school hours you and your healthcare provider must complete this form. Available on our district webpage under Health Services and on our Tolt Website under Parents and Students.
Medications may be turned into Nurse Rachel on our “Back to Business Day” in August or before the first day of school.
If your student is capable and responsible of carrying their own medication, a medication authorization form must still be filled out for all prescription medications. For over-the-counter medications you must fill out a Permission to Self-Carry Form. Students may carry a one-daysupply of an OTC medication in the original container. Self-carry forms may be obtained from Nurse Rachel.
Your student may not be quite ready for this responsibility and that’s okay! We will keep their medication securely in the attendance office and your student may come to the health room or attendance office if they need their medication. Nurse Rachel will assess them to make sure everything is okay and then either herself, or a trained designated staff member will administer the medication.
Students are not allowed to carry liquid medications.
Students are not allowed to carry controlled substance medications, such as those used for ADD/ADHD.
If your student has a health care plan, Nurse Rachel will mail this to you over the summer. Complete the form, sign, and turn in with any applicable medications and forms before the first day of school. Nurse Rachel will contact you if a meeting needs to be arranged to go through your health plan.
Let’s Talk Immunizations!
Starting on August 1, 2020, the revised rule requires medically verified immunization records for school and childcare entry.
Here are some examples of medically verified immunization records:
A completed Certificate of Immunization Status (CIS) signed by a health care provider. Click on “Certificate of Immunization Status.”
A CIS filled out and signed by you, along with a copy of your child’s official immunization record from another state/country.
A CIS printed by a health care provider or school from the Washington State Immunization Information System.
A CIS printed from MyIR which is a free Department of Health online tool that allows families to view and print their official immunization records themselves. Go here to begin the sign-up process.
If you are requesting an exemption from one or more of the immunization requirements, you must provide us with a completed Certificate of Exemption. More exemption information and forms are available here.
The Tdap vaccine is due prior to students entering 7th grade. (Previously, this was required prior to students entering 6th grade).
Children may begin school or childcare only if they received all of the required immunizations they are due to receive and submitted medically verified records on or before the first day of attendance. If this requirement is not met, they may not begin school on the first day of attendance. Visit the Washington State Department of Health's website for more information.
