On behalf of myself and Meeting Host, Phil Hess, I would like to enthusiastically invite you to the

48th Annual Meeting of SOAP this spring in Boston. The theme of this year’s meeting is care coordination and we have created a forum for discussing the management of obstetric problems from the perspective of multiple disciplines. The goal as always is to deliver the highest quality educational and scientific forum for anesthesiologists who practice obstetrics. Specifically we made an effort to highlight our society’s best speakers and incorporate more experts from other specialties and partners in the international obstetric anesthesiology community into the SOAP 2016 Annual Meeting program. Naturally, Boston provided a rich environment to recruit local speaking talent.

Several innovations are being employed this year in the program. This will be the first SOAP meeting using electronic versus paper posters. In addition, we would like to extend the discussion around abstracts beyond the meeting itself. SOAP members will be able to review and discuss abstracts in a blog

format in the weeks preceding and following the annual meeting. We are once again providing Spanish language venues for some of international attendees including translation of the main program.

The SOAP Annual Meeting is the highlight of my professional year and I am proud to present what I’m sure will be a wonderful program. On a personal note, this year’s meeting is particularly meaningful to me, as Boston is the city where I trained and where I was lucky enough to be introduced to obstetric anesthesia. I’m looking forward to seeing all of you in Boston.

Program InformationMission Statement The Society for Obstetric Anesthesia and Perinatology (SOAP) was founded in 1968 to provide a forum for discussion of problems unique to the peripartum period. SOAP is comprised of anesthesiologists, obstetricians, pediatricians, and basic scientists who share an interest in the care of the pregnant patient and the newborn.

The mission of this Society is to improve the pregnancy-related outcomes of women and neonates through the support of obstetric anesthesiology research, the provision of education to its members, other providers, and pregnant women, and the promotion of excellence in clinical anesthetic care.

A membership in SOAP is an opportunity to meet people who share your interests, and to stimulate improvements in health care for pregnant patients.

ACCME Accreditation and Designation Statements This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the American Society of Anesthesiologists and the Society for Obstetric Anesthesia and Perinatology. The American Society of Anesthesiologists is accredited by the ACCME to provide continuing medical education for physicians.

The American Society of Anesthesiologists designates this live activity for a maximum of 31.75 AMA PRA Category 1 Credit ™*. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Target Audience The SOAP 48th Annual Meeting is intended for anesthesiologists, obstetricians, neonatologists, obstetric medicine specialists, maternal-fetal medicine specialists, residents, fellows and medical students. The Society supports the attendance by associate members in the educational sessions of the annual meeting. The program is generated from member requests and an assessment of need by the Program Committee. Attendance at this meeting does not guarantee competency or proficiency in the performance of any procedures which may be discussed or taught during the course.

Mission of SOAP Program Committee The mission of the Society’s Program Committee is to provide anesthesiologists, obstetricians, and other physicians and members of related allied health specialties with the knowledge that will reinforce past learning as well as disseminate new concepts, practices, and skills involving anesthesia and analgesia for the pregnant woman.

Participation in the SOAP 48th Annual Meeting Attendance shall be open to all health practitioners, provided that they have registered for the meeting. CME credit will only be offered to M.D.s, D.O.s or equivalent.

Educational Format CME activities may include the following formats: plenary sessions, debates, lectures, poster discussions, oral abstracts, problem-based learning, and skill-set workshops.

Annual Meeting Objectives At the completion of this conference the participants should be able to:

1. Identify and critically evaluate current research related to obstetric anesthesia, obstetrics, perinatology, and related medical disciplines;

2. Design and implement research investigations related to obstetric anesthesia that are built upon the foundations of current research;

3. Compare recent findings related to obstetric anesthesia to the prevailing standard of care, and adjust patient care plans accordingly;

4. Learn the details of new obstetric guidelines for thromboembolic prophylaxis and their impact on administering neuraxial anesthesia;

5. Review the applications of the discipline of blood management to obstetric practice and the use of point of care coagulation testing;

6. Identify the benefits and consequences of utilizing various obstetric anesthesia quality measures;

7. Formulate appropriate anesthetic care plans for parturients with sepsis with the goal of reducing related mortality;

8. Recognize the applications of focused cardiac ultrasound in the management of obstetric patients;

9. Determine if investigations in the human biome has a role in improving obstetric outcomes;

10. Review changes in the health care policy and their impact on practice and outcomes in obstetrics.

Session DescriptionsOral and Poster Sessions:

Oral Presentations 1 These are our first oral presentations, a sampling of peer-reviewed scientific research related to obstetric anesthesia, followed by a question and answer session.

Oral Presentations 2 Oral presentations of peer-reviewed scientific research related to obstetric anesthesia continue in this second session. A short question and answer period follows.

Poster Sessions 1 - 4 These are electronic poster reviews. Designated moderators will highlight several poster presentations of peer-reviewed scientific research.

General Sessions/Other:

Best Paper Session Presentation of obstetric anesthesia research judged by peers as the best quality research currently taking place, moderated by a leader in the field. A question and answer period will be included. This is a judged competition.

Best Practice Panel - Case Report Review with the Experts Dr. Katie Arendt will moderate a panel including Drs. Robert D’Angelo, Brendan Carvalho and Roshan Fernando discussing common clinical dilemmas in obstetric anesthesia. Dr. Arendt will use questions prompted by submitted case reports for this session.

Fred Hehre Lecture This is an honorary lecture, in memory of Fred Hehre, and awarded to that person judged by his peers to have made significant contributions to the field of obstetric anesthesia.

Gertie Marx/FAER Lecture Dr. Mary D’Alton, the Chair of Obstetrics at Columbia University, will provide this keynote lecture on the value of coordinating high risk obstetric care between maternal fetal medicine physicians, anesthesiologists, nurses and other specialists.

Gertie Marx Research Competition In memory of Gertie Marx, this session is a presentation obstetric anesthesia research judged by peers as the best

quality research done by medical students, residents, and fellows, moderated by Dr. Richard Smiley. This is a judged competition.

Gerard W. Ostheimer Lecture - What’s New in Obstetric Anesthesia? Always a highlight of the meeting, the Gerard W. Ostheimer lecture is a review of important, relevant, and/or practice-changing literature related to obstetric anesthesia, obstetrics, perinatology, and allied medical disciplines that was published in calendar year 2015. This presentation will bring you up to date on the latest evidence-based advances in the anesthetic management of pregnant women.

Obstetric Anesthesia Quality Measures Panel Dr. Jill Mhyre will moderate a panel including Drs. Scott Segal and Barbara Scavone who will review three clinical metrics being considered as quality measures in obstetric anesthesia. The metrics include unintended dural puncture/epidural blood patch, maternal satisfaction, and neuraxial anesthetic replacement/conversion to general anesthesia in cesarean delivery.

Panel on Applications of Echocardiography and Echocardiography Education Dr. Alicia Dennis, an anesthesiologist from the University of Melbourne in Australia, and a widely published author on the use of transthoracic echocardiography in obstetrics, will present clinical applications of this technique. Dr. Feroze Mahmood, a cardiac anesthesiologist at Beth Israel Hospital in Boston and an experienced instructor in transthoracic echocardiography, will review educational methodology for teaching anesthesiologists transthoracic echocardiography.

Panel on Blood Management and Point of Care Coagulation Testing Blood management refers to the broad medical and surgical discipline with the goals of maintaining hemoglobin concentration, optimizing hemostasis, and minimizing blood loss, in an effort to improve patient outcomes. Dr. Walter Dzik, a pathologist specializing in transfusion medicine from Massachusetts General Hospital, will review applications of blood management in the obstetric population. Dr. Roshan Fernando, anesthesiologist and President of the Obstetric Anesthetists’ Association in the United Kingdom, will discuss the role of point of care

coagulation testing in obstetrics.

Panel on New Thromoboprophylxais Guidelines and Neuraxial Anesthesia and Anticoagulation Dr. Errol Norwitz, Chair of Obstetrics at Tufts University will provide a review of newly released obstetric guidelines for the prevention of thromboembolism. Dr. Richard Smiley will discuss the impact of these guidelines on the administration of neuraxial anesthesia in obstetrics.

Panel on Sepsis and Advances in Sepsis Treatment Sepsis represents one of the increasing causes of maternal mortality. Dr. Nuala Lucas, an anesthesiologist from London and Honorary Secretary of the Obstetric Anesthetists’ Association will present a review of peripartum sepsis. Dr. Andrea Ciaranello, an infectious disease specialist from Massachusetts General Hospital, will present an update on new developments in the treatment of sepsis.

Research Hour Dr. Rich Smiley will moderate the research hour with guest speaker Dr. Caroline Mitchell, an obstetrician and clinical scientist at Massachusetts General Hospital. The focus of the session will be an area of increasing research interest, specifically, the relationships of the human microbiome, the population of bacteria in our bodies, to disease states including those affecting obstetric and neonatal outcomes.

What’s New in Neonatology? Dr. Terrie Inder, a dual boarded neonatologist and neurologist, originally from New Zeeland and currently Chair of Pediatric Newborn Medicine at Brigham and Women’s Hospital, will present an update on recent developments in neonatal medicine.

What’s New in Obstetric Medicine? Neel Shah, M.D., MPP, Assistant Professor of Obstetrics, Gynecology and Reproductive Biology at Harvard Medical School, and associate faculty at the Ariadne Labs for Health Systems Innovation, will provide an update and discuss the most current relevant topics in obstetric medicine.

Specifically tailored to the anesthesiologist with little or no prior experience, this workshop consists of a short didactic session followed by a hands-on workshop with volunteers to familiarize the participant with the use of bedside transthoracic echocardiography to assess volume status and cardiac function.

The workshop builds on published articles that participants can use to utilize for learning or teaching debriefing in their own contexts. This workshop will be followed by a tour of the Center for Medical Simulation.

What would a trip to Boston be without a visit to sites that are so vital to the heritage of anesthesiology? On Friday afternoon, May 20th, please join two OB anesthesiologists, Drs. William Camann and Andrew Miller, who have led many tours highlighting the deep and rich medical and anesthetic legacy of this historic city. The tour will feature a visit to the Ether Dome at Massachusetts General Hospital, where on October 16, 1846, surgical anesthesia was first demonstrated to the world. Every anesthesiologist must make a pilgrimage to this special site! Opportunities will exist for you to re-enact the famous scene and even take

photos. We will then visit the Ether Monument, in the beautiful Boston Public Garden. This monument to the specialty of anesthesiology is unrivaled by any structure anywhere else in the world. We will visit the Mt. Auburn Cemetery, one of the most scenic and picturesque settings in the world. Here, we will visit the graves of Dr. William T.G. Morton, who delivered the anesthetic for the Ether Day event, Dr. Charles Jackson, the pioneering chemist who created the ether compound for Dr. Morton, and Fanny Longfellow, wife of the famed poet Henry Wadsorth Longfellow. Fanny was the first woman in the USA to receive inhalation anesthesia for childbirth, in 1847. Each of these sites will be accompanied by much discussion, education, amazement, and fun. Your tour will be in very comfortable luxury vans and there will be a gourmet (yes, real gourmet, catered by a fine Boston restaurant!) lunch for the road. Tour will leave from the headquarters hotel shortly after the SOAP events, approximately 1pm. We anticipate a roughly 4-hour tour, and you will be returned to the hotel in time for your evening plans, full of great knowledge of Boston’s important role in the development of our specialty.
