Histo logic ally, carci nosar coma s comprise of e pithe lial as w ell a s mesenchymal components, which are eithe r homologous (normally found in ovary) or heterologous (not normally seen in ovary). Here, we report a case of a 50 year o ld female patient who presented with abdominal distension and was diagnosed as malignant mixed mullerian t umor of bilateral ovaries histopathologically. Carcinosarcomas of the ovary are extrem ely rare and agg ressi ve. We want ed to draw the at tention that alth ough it is mor e freque ntly unilateral and seen among the postmenopausal nullipara women, malignant mixed müllerian tumors can also be bilateral and seen among multiparas in the reproductive period as with this case report. INTRODUCTION Ovarian carcinosarcoma, also ca lled malig nant mixed muller ian tumour (MMMT) , i s a ver y r are ovaria n neoplasm, wi th an incidence of l ess than 1% of all ov ari an tumors, and les s t han 400 case report s i n literature 1 .Histologically, carcinosarcomas comprise of epithelial(carcinomatous) as well as mesenchymal (sarcomatous) components both, which are either homologous (norma lly found in ov ary ) or heterologous (not normally seen in ovary) 2,3 These tumo rs are ofte n seen in t he 5th to 7th dec adeie in postmenopau sal wo men wh o are nulli parou s. They are usually asymptomatic. Only 10% of them are bilateral. 2-4 Despite aggressive treatment which includes surgery and chemotherapy, patients have an increased risk of death compared to women with epithelial ovarian cancer 4 .Her e we re port a case of ma lig nant mi xed mull er ian tum our of the bil at er al ovari es in a 50 year o ld fema le with complains ofabdominal distension where a total abdominal hysterectomy and bilateral salpingo-oophorectomy was performed. CASE REPORT A 50-y ear-o ld woman, gravi da2, para2, cons ulted to the outpatient department of Obstetrics and Gynecology with complaints of abdominal distention since t wo months. Physical examination was unrema rkable excep t for bi lateral ma ss es per abdo me n. There wer e no other comp lain ts and sh e was previo usly doing well. Histo ry o f ir regula r ble edi ng or any suc h s ignifi cant pa st his tor y was negat ive. Blood , urine and b iochem ical inves tigatio ns of patien t were with in normal limits. Howev er only an increa se in CA- 125 (60.2m /ml) and CA- 19- 9(2 0.4m/mL) was noted. USG rev eale d c yst ic les ions in both ovarie s le ft ovar y measurin g 14x7cms and right ovary meas uring9 x7 cms. Ovarian cancer was suspecte d, so t he pat ient underwent expl or at or y lap arotomy. Optimal debulking su rgery was performed, and specimen was sent for histopathological study.
