Further to TB 435 regarding the incident at Ohio State Fair (July 2017), NAFLIC has now received further information from the HSE regarding what is suspected to be the circumstances leading up to the failure of the device. It is advised the structure appears to have failed between the sweep arm and the gondola and not at the bolted connection as was originally thought (see attached photos). It is understood from the HSE that the structure had suffered corrosion which had reduced the material thickness. It is further understood that an ingress of water occurs during the transport and storage that leads to the corrosion. It is understood that KMG, the manufacturers of these devices, has written to owner/operators and instructed that the machines are not used until the cause is fully understood. Any IBs that are asked by the owner/operators to undertake an inspection of these devices should arrange to inspect the inside of the affected structure. They should also undertake a detailed thickness check on the material, over the complete section, and record the results. It will then be for the manufacturer to provide advice on what the acceptable level of loss of material is, if any at all. Thereafter, it is expected that the NDT schedule will have to be revised. In relation to this, the HSE has also put out the following statement aimed at IBs (dated August 4 th , 2017): AFTERBURNERS You will by now have received the revised note from KMG. I have told the operators of the machines that so long as their machines pass all of the stages of the inspection required by that note, the prohibition notices will be deemed complied with and they can operate the machine again without further reference to HSE. I have also told them that I require a statement from them to confirm that they have required you to conduct the test as stated in that note. I require the same from yourselves please.
