    After the huge success of the Taiwanese Dermatological Association Spring Meeting 2013 and the 1st Taiwan Dermatology Aesthetic Conference, every member of the Taiwanese Dermatological Association is looking forward to the inspiring meeting in 2014. The well-organized meeting covers the latest advances in aesthetic dermatology, dermatologic surgery, pediatric dermatology, and medical dermatology. Eleven new sessions from symposium to workshop and medical education reveal the great spirit of progress of dermatologists.

    Dozens of world known international speakers and local experts were invited to join the meeting. The meeting does not only provide our member with the best continuous medical education but also a globalized platform of networking. I would like to thank the organizing committee Po-Han Huang, MD, Ren-Yeu Tsai, MD, Ming-Tsan Wang, MD, MSc, MOH, Nai-Jen Hsu, MD, Chao-Chin Wang, MD and those behind this huge event for your effort in making every detail perfect, educational and productive. Kaohsiung International Airport Kaohsiung Airport is next to the city center with excellent metro and taxi connections.Airlines fly to over 11 Asian destinations, including Hong Kong, Tokyo, Bangkok and Singapore and 20 major cities in China, and over 70 northbound trains of high speed rail per day connect Kaohsiung with Taipei from where connections radiate worldwide.     About TDACAesthetic dermatology is a combination of skin physiology, skin pathology, clinical diagnosis, skin therapeutics and skin surgery. It is a marriage of science and aesthetics. Doctors can easily diagnose the status of skin and its problems due to the intensive and strict training programs for specialized dermatologists. By combining the most advanced therapeutic technique with precise aesthetic concept it allows dermatologists to enhance the health of skin and its beauty.A dermatologist can achieve his/her best skin care medical practice by combining the progress of medical technology along with more precise diagnostic methods, innovative biological medical device and invention of advanced therapeutic equipment. However, Taiwanese Dermatological Association, the largest and prestigious aesthetic medical society in Taiwan, has the obligation to provide the physicians other than dermatologists to pursuit the knowledge of the aesthetic medicine in order to enhance the quality of care to a higher level. Not only do we hope to elevate the health of skin and the aesthetic standard of our people but we also hope to correct the chaos of other non-specialized doctors who are taking charge of most of the aesthetic medicine in Taiwan.Therefore, conducting a Taiwan Dermatology Aesthetic Conference (TDAC) will allow physicians who are interested in this new medical field further learning and discussion. This symposium will provide the best international experts with interactions, live demonstrations and varies choices of clinical aesthetic dermatology learning courses. It is our goal to enhance the standard of our national aesthetic dermatology as well as to contribute to the continuation and the growth of medical education and to become an exchange corner of clinical researches and expert experiences.

    Prof Cindy Wang is currently in the School of Art, Design and Media; she joined as an Assistant Professor of Visual Communication in 2007. Cindy holds a BFA from the School of Visual Art and a MA in New York University (NYU), Dept. of Graphic Communication Management & Technology.

    Cindy's research focuses on use various principles and elements of graphic design, and combining with modern printing techniques to enhance visual communications. One of the key findings of her research was how to use illusions to further stimulate the visual expression of an artwork. The research revolves around the utilization of illusions and modern printing technologies to create new, innovative and unique illustration of visual presentation.

    On the other hand, Cindy also research on type forms that combines elements of Asia and Western typography is innovative from as Asian perspective and meaningful as bridge between East and West. This fusion typography is gathering tremendous interest across the globe and her work in this area establishes her as a pioneer in this exciting new area of design research. She has been successful in securing research grants to support her work.

    Cindy's reputation is more and more well known on both international and national level. She has been prolific in publishing five books on her research themes, won numerous international awards and with numerous of exhibitions locally and internationally. All these awards are only honored to highly talented designer from Hong Kong, New York, and Germany. This explains that Cindy's creativity, talents, and ability are recognized throughout the continents of Asia, North America, and Europe.

    Welcome to TDA Spring Meeting 2014 and TDAC! We are here to assist you through your time at the conference and have assembled some important information for you.Please register before attending all sessions.

    Attendees without registration badges will not be granted entry and to sessions.Pre-registration is strongly suggested as space is limited.

    Those who have not completed advanced registration before the registration web page closed may register for the meeting at the On-Site Registration Desk.The registration area will be located on the first floor of the the and will be open during the following hours:

    Photo identification will be required.

    Official LanguageThe official languages of Spring Meeting 2014 are Mandarian and English. All aesthetic lectures of TDAC are in English.

    ExhibitsTechnical Exhibits the will be located in the lobby and between the Conference Halls at KEC. Admittance into the Technical Exhibit Hall is limited to attendees only. Infants and children under the age of 16 will NOT be admitted into the Technical Exhibit Hall under any circumstances. This policy has been created for the safety of the children of TDA member and guests. Your support and compliance are appreciated.

    Lunch and DinnerThe Gala Dinner will be held in Room 305 and the Exhibit Hallway at Kaohsiung Exhibition Center from 17:30 to 19:00 on May 3. In addition to two luncheon symposia and one luncheon forum during the lunch time on May 4 , the participants may network with peers and enjoy Chinese lunch boxes in the dining area from 12:00 to 13:00. Vegetarian lunch boxes are also available. Please book in advance at Registration Desk.

    KEC is located at the hub of the Kaohsiung Asian New Bay Zone and is easily reachable by air, sea, and land transportation, including hi-speed rail from Taipei and other cities in the north of Taiwan. Free shuttle transportation will be provided during the meeting.

    Skin aging is the result of natural physiological aging processes (chronoaging) as well as damage caused by exogenous environmental factors, such as climate, pollution, sun. Amongst these, sun is the main cause of wrinkles, loss of tone and firmness, and pigmentation disorders (photoaging). Similarly, skin rejuvenation and lightening procedures such as laser, peeling and dermoabrasion, act by creating a controlled damage to skin, reducing or eliminating inaesthetisms and setting in motion natural regeneration mechanisms which help restore skin to its original conditions. Dermo-cosmetic formulations can be useful adjuvants in helping repair damaged skin and accelerate generation processes. Extensive scientific evidence supports the efficacy of SCA, a biologically-active glycoprotein solution secreted by the mollusk Cryptomphalus aspersa, as an anti-aging and post-procedure treatment. The secretion is obtained under a patented process which ensures a solution rich in proteins, including growth factors and haemocyanin, enzymes, anti-oxidants, proteoglycans and GAGs, and minerals (calcium). In vitro studies have identified its action mechanisms: fibroblast and keratinocyte proliferation and migration, fibroblast morphology rejuvenation, renewal and assembly of the extra-cellular matrix, inhibition of MMPs and anti-oxidant activity. Clinical work, including histological assays, have demonstrated SCAs ability to reduce lines and wrinkles, increase firmness and elasticity and recently, improve in the skins fibrotic component using confocal microscopy. Improvements have been achieved in sagging, skin texture and uniformity of skin tone in such difficult-to-treat areas as neck and dcolletage. During and following dermatological procedure p r o g r a m s S C A h a s r e d u c e d i r r i t a t i o n a n d inflammation, accelerated healing, and helped ensure optimal procedure outcome. Scientific data demonstrating SCAs skin regeneration activity will be discussed, and experience will be shared regarding its implementation in cosmetic antiaging regimes and in combination with dermatological procedures.

    Many patients with chronic disease dont improve with treatment as expected. One of the most important, and often forgotten, factors in the management of patients with any disease is adherence to treatment. Electronic monitors provide a window on the various patterns of adherence exhibited by different patients. Patients are often poorly adherent to treatment. Concrete approaches to enhancing patients adherence behavior include the timing of office visits, patient education, building a strong physician patient relationship, and the choice of treatment. Physicians have a lot of control over patients adherence and the resulting outcomes of treatment.

    Aesthetic medicine is emerging and booming these years worldwide. Several new technologies and prodedures have been or will be introduced into Taiwan in the year of 2014.     Statistic studies revealed that eyelid surgery accounted for 32.72%, Botox comprised 10.80%, and rhinoplasty constituted 9.21% of all the cosmetic procedures in Taiwan in 2004 (ISAPS 2005). Besides that, nevus resection, scar revision, autogenous fat injection, filler injeciton, tattoo removal, body hair removal, hair transplantation, pigmentation lesion management, dermabrasion are all considered commonly performed methods. Facial contouring, orthognathic surgery, mandible angle reduction, genioplasty, malar bone reshaping, breast augmentation, facelift, and liposuction are among the more imposing procedures in the aesthetic surgical field. It is noticeable that the advancement in navigation technique, computerized simulation, image analysis system as well as innovative instrumentation contribute a lot to the progress in surgical outcome, with better control of the operative effect, enhanced safety and efficiency of the surgical procedures. The ideal proportion of the human body and aesthetic features of human face will be reviewed; and the risk, benefit and technical pitfalls as well as the principles of perioperative cares would be elaborated in the lecture.

    Dermatologist: PRASETYO, Adri Dwi MDRhinoplasty is a typical area of interest in both dermatology and facial plastic surgery. Many aims of rhinoplasty can be successfully approached with dermatologic techniques using filler augmentation. Various methods had been described. Anatomical approach of fil ler rhinoplasty using a specific technique needs to be developed. This objective is to discuss and debate filler placement as a true rhinoplasty related to anatomical basics, details of the augmentation technique, also the limitation and potential complication compare to surgical method. Single entry point filler rhinoplasty using blunt tip cannula is described in details, focusing on its usage to place filler as a true implant. The related anatomy of nasal profile and cadaver nasal dissection of filler placement are reviewed. Limitation and potential complication are discussed. This technique provides access for continuous aesthetic correction of radix, dorsum, tip and columella from only a single entry point, thus resembling a true implant as in surgical rhinoplasty. Cadaver dissection showed filler placement is in continuous true implant form and in conjunction with anatomy of nasal profile and plane. With proper techniques, medical rhinoplasty using this novel method is safe and effective. This approach can be used in those cases where patients are reluctant to undergo surgery, or as a primary indication in the aesthetic nasal correction. In addition, this technique provides access for continuous radix, dorsum, tip and columella augmentation from only a single entry point. Correct anatomical placement of filler material is also essential for the success of this method.

    Rhinoplastic Surgeon: CHIU, Tony Yu-Hsun () MD MMSAsian rhinoplasty is augmentation rather than reduction. The nasal dorsum is augmented, and the tip definition is enhanced in most cases. We performed surgical rhinoplasty using autologous cartilages with or without alloplastic materials. Medical or injection rhinoplasty is one alternative method for enhancing the shape of Asian nose. The use of fillers in this purpose becomes very popular nowadays because of the improvement of safety and effectiveness. Through this lecture, I would like to discuss about 1.     The Pulsed Dye Laser was originally developed for the treatment of vascular lesions such as port wine stains and rosaceal telangiectasia as the target of the laser is oxyhaemaglobin. Clinical and laboratory research then demonstrated that a low level pulsed dye laser with a very short pulse duration, 585nm using a 7mm hand piece with fluence of 2.5 to 3.5 joules/cm2, could induce collagen production with a 60% increase in procollagen III production after a single treatment. Subsequent studies in patients with inflammatory acne have shown a high level of success in treating inflammatory acne. Scientific studies have demonstrated that the improvement in inflammatory acne was unrelated to a change in sebum production or to levels of Proprionibacterium acnes, but due to a massive up-regulation of transforming growth factor beta. This has led to the use of this laser in the treatment of other inflammatory dermatoses.

    SY3 Hair Removal with Lasers: An Update in 2014Speaker SBANO, Paolo MD PhD

    The Photoepilation, Vascular and Skin Rejuvenation procedures are the most requested by patients in the aesthetic field. The standard single wavelength laser systems are not able to treat all the skin types in the best conditions, taking sometimes limiting compromises to avoid side effects. The Mixing Technology are using 755 and 1064 nm laser (Quanta System, Italy) is the first technology able to delivers these two wavelengths also in a simultaneous and in a sequential emission, opening new treatment possibilities with improved safety. The Hair Removal treatment takes important benefits from the Mixing Technology treating all the border line skin types where the Alexandrite alone can be too aggressive and the Nd:Yag alone can be not enough effective. The simultaneous emission of both the lasers improves the outcomes and the safety in all the skin conditions. The Vascular treatment for the removal of facial and leg telangiectasia uses the sequential emission of the Mixing Technology to convert hemoglobin in methemoglobin to increase the efficiency of the treatment and to reduce the laser fluence. The Skin Rejuvenation takes advantages by the Mixing Technology due to the affect of the Alexandrite on pigmentations and to the effect of the Nd:Yag on the skin tightening just in one session. This presentation shows our experience with this laser device.

    My concept is that rosacea is primarily a disease of the facial vasculature leading to non-physiological facial flushing,, fixed erythema and in some patients the development of inflammatory acneiform lesions which may be follicular or interfollicular. Treatment of Type I, erythrotelangiectatic rosacea is to stimulate alpha sympathetic receptors leading to vasoconstriction or inhibit beta receptors preventing vasodilatation. Inflammatory Type II rosacea will respond to topical or systemic antibiotics but will also respond well to low level pulse dye laser treatment. Ablative pulsed dye laser treatment can then be used to treat telangiectasias or persistent erythema. Severe Type II rosacea can be very difficult to manage and the use of systemic metronidazole and dapsone has been used very successfully in combination with low level pulsed dye laser. Type I rosacea, particularly following treatment of acne with oral isotretinoin can be very difficult to manage but the use of antimalarial drugs has been successful in some patients.

    SY6 Rosacea in TaiwaneseSpeaker LEE, Julia Yu-Yun () MD

    Rosacea is a chronic recurrent inflammatory disorder of unknown etiology mainly affecting the central facial skin and eye. It is more common in fair-skinned populations, esp. northern Europeans and Celtics, but much rarer among Asians and African Americans. Reports of rosacea affecting Asians, including Taiwanese, are scant. Rosacea appears not so rare in our practice. In 2004, we reported the first series in Taiwan, a retrospective study of 109 cases of rosacea diagnosed in our department during 1989.2~2010.10 (Dermatol Sinica 2004). Since then, we have additional clinical experiences in rosacea. This talk will cover the clinical features of rosacea observed in our first series, and the selected treatment experiences on some difficult cases, such as treatment of severe steroid rosacea with red face syndrome, severe granulomatous rosacea treated by azithromycin, and severe erythematotelangiectatic rosacea (ETR) satisfactorily controlled by the low dose carvedilol (a series of 11 cases, 1999-2010. JAAD Sept 2012). Briefly, the clinical features in our first series were comparable to those observed in Western countries, except for mean age younger than the Western series by about 10 years, possibly related to genetic and climate factors, and lower rate of ocular rosacea due to incomplete clinical documentation. With regards to the adrenergic blocker treatment for control of vasodilatation and adrenal flush, we have tried low dose of carvedilol in some patients with severe ETR. Carvedilol is a nonselectiveblocker that can blocks both1 &2 adrenergic receptors with infrequent hypotensive side effect. Carvedilol and its metabolites have potent antioxidant and anti-inflammatory properties that are unique amongblockers. Low dose (mostly 6.25mg tid, titrating up to 31.25mg/d) was well tolerated and effective in treating severe ETR with rapid onset of symptom control that eventually allowed tapering or discontinuation of other concurrent medications. These results are encouraging, but need further prospective controlled studies to determine the optimal dosage, long-term therapeutic effects and side effects.

    Patients with fungal infection having skin lesions may consult a dermatologist, which is a diagnostic and therapeutic challenge. Dermatologists take samples from the lesion to check the fungal elements under a microscope by KOH preparation and then treat the patient. This model has advanced from bedside to bench and from bench to bedside (B to B to B), which is defined as Translational Medical Mycology. Dermatologists have an advantageous position in finding, isolating and identifying the pathogenic fungi and treating the patient with antifungal drugs. Samples should be cultured in different media with or without chloramphenicol and cycloheximide and incubated at room temperature or 37C. Non-culture techniques such as polymerase chain reaction based molecular identification, transmission electron microscopy, scanning electron microscopy, biochemistry tests and histopathology are also necessary to confirm the identification of the species, especially when the routine culture is negative. We start treatment upon obtaining evidence of fungal infection, i.e., posit ive KOH examination. Antifungal drugs such as itraconazole, fluconazole, terbinafine and amphotericin B can be used alone or in combination based on the fungal species and the location of the lesion. The role of dermatologists practice on fungal infection includes screening of the patient, merging all of the laboratory techniques and methods from the microbiologists, pathologists, molecular researchers, identification of the pathogen and determination of the optimum antifungal drug.

    Epidemiology of superficial fungal infection in Japan has been drifting along with changing our living style. International or domestic sports activity, higher prevalence of keeping pet animals and changing demographics in the Japanese population increasing the ratio of elderly may be the major causes. In the last decade, we are confronting the outbreak of an epidemic of Trichophyton tonsurans infection. First case of culture proven outbreak of T. tonsurans infection had been reported in 2001 in a high school wrestling team, followed by high school and university judo teams, then high school sumo teams in 2004. At first the infected cases were restricted among these contact sports players. Now the infection is spreading in community, and cases who had no apparent relationship between these sports players were reported. Several studies using PCR-RFLP analysis of ribosomal RNA gene using MvaI and AvaI revealed epidemics among judo practitioners, sumo practitioners and wrestlers had characteristic mode of infection in Japan, for example, the epidemic among sumo wrestlers originated from an earlier epidemic among judo practitioners (K Anzawa Jpn J Infect Dis 2013:64:458). Prompt and simple identification of T. tonsurans is very important part for infection control. Standard culture based methods such as slide culture are time consuming, and molecular technology based methods require skill and equipment. Recently, chlamydospore-like structures (CLS) abundantly observed by light microscopy from reverse side of grown Petri dishes had been reported as a useful morphological marker characteristic for T. tonsurans (M Fujihiro, Jpn J Med Mycol 2008:49:191, T Mochizuki, J Dermatol 2013:40:1027). therapeutic challenge.

    The main difference of dermoscopic findings between Asian skin and Caucasian skin lies in melanin pigmentation, such as BCC, Poroma, Spitz nevi and Melanoma. Arbolizing vessels of BCC are often obscured by melanin pigmentation and hard to see. Poromas are of ten pigmented and look like BCC or melanoma clinically, but in this case, dermoscopy play an important role to reveal gromellular vessels of diagnostic value. More than half of the cases of Spitz nevi are darkly pigmented, sometimes confused with melanoma. And some of them are noticed even on palms and soles. Differential diagnosis between nail melamoma and infantile nail streaks is extremely difficult even by dermoscopy. Age of onset can be a clue. Typical examples of above conditions will be briefly presented.
