SECTION I: 21st CCLC INTRODUCTION & OVERVIEWOur Mission

To provide a safe and caring environment where students can improve their 21st Century skills, including, but not limited to:

Our Goals1. Maintain and improve student academic achievement in

language arts and math.a. Seventy-five percent (75%) of students earning a passing

grade after the first grading period will maintain a passing grade by the close the academic year.

b. Fifty percent (50%) of students earning below a passing grade after the first grading period will raise their grade by the close of the academic year.

c. Assessment results from short cycle assessment selected by the district should be recorded with the goal of a one point/grade level increase for 80% of students by the close of the academic year.

2. Design and deliver a 21st CCLC program which meets the needs of all constituent groups.

a. Eighty-five percent (85%) of all survey participants will agree or strongly agree that the 21st CCLC program being offered in their community is high quality.

3. Implement a program that engages students.a. Seventy-five percent (75%) of the target enrollment goal will be met each

academic year.b. An average attendance rate of 75% will be maintained throughout the

Our Hours of Operation

Hours of Operation: Monday - Thursday: 2:30pm – 4:30pm

The 21st CCLC program begins on August 28, 2016 and ends on December 15th, 2017. The program will resume January 16th through May 19th, 2018. We will notify parents in March if University Hills ES will be a site for summer programs.

After SchoolThe out of school time program operates from 2:30pm – 4:30pm Monday – Thursday. The last day of program during the academic year will be on Thursday, May 18th 2018.

Summer ProgramIf University Hills is selected as a central school to house the summer program, it will begin on, June 4th and end on June 22, 2018. The summer program operates from 8:30am – 2:30pm Monday – Friday.

Holiday and Storm Day PoliciesDuring the regular academic year, the calendar for the 21st CCLC program follows the calendar of the school district. On days when there is no school, including snow days, the 21st CCLC programs (morning and afternoon) are not in session. If a late start for school is announced due to weather conditions or any reason, there will be no program before school. If school is released due to bad weather, program will not be provided after school hours. On school days when regular classes are in session, but bad weather is predicted for late afternoon, the afternoon program hours may be canceled, and parents will be notified.

SECTION II: MEDICAL INFORMATIONAllergies and Other Conditions

Each student attending our learning center has a 21st CCLC Medical Authorization form on file. This form lists allergies, medications, and pertinent medical conditions. If any enrolled student has an issue of which you need to be aware (allergies,

EpiPens, etc.), the learning center coordinator will inform you directly. This is done to help protect students’ privacy. Medical information is shared only on a need to know basis. Should an emergency arise during which the Medical Authorization form is needed and the learning center coordinator is not present, the files are with each academic enrichment program teacher. Due to legal regulations, this filing cabinet is locked. If the learning center coordinator is not present, but the files are needed due to an emergency, the substitute coordinator, each program teacher will be able to gain access to the students’ medical files.

First Aid Kits21st CCLC First Aid Kits are purchased at the start of each academic year and are replenished as needed. Each instructor has access to the first aid kits, which are located with each DiMA teacher. If you notice a kit is running low, please let the coordinator know.

Medication AdministrationThe 21st CCLC program is not responsible for medication administration. Families are aware that arrangements must be made to have medications administered to students prior to their arrival at the program (this information is shared in the Family Handbook). Exceptions to this rule include EpiPens for allergic emergencies or inhalers for asthma treatment. Students who require these supplies should have the items with them. The 21st

CCLC program does not keep EpiPens or inhalers stocked. If a student uses an inhaler or an EpiPen, the appropriate instructors will be notified.

Medical Emergencies21st CCLC team members are trained in First Aid and cardiopulmonary resuscitation (CPR). We have to be, because during out of school time hours, there will not usually be a school nurse on site. In case of an accident or illness, first use your First Aid skills to assess the situation. Use CPR if the situation requires. After assessing the situation, take appropriate action (band aid, icepack, bathroom pass issued, etc.) and notify the Learning Center Coordinator who will determine if the parent/guardian of the child will be called. Whenever any injury occurs during program hours, an Incident Form must be completed and kept on file. It can’t be emphasized enough how important proper documentation really is! In serious emergencies the parent will be contacted and the coordinator may call 911. Directions from the Emergency Medical Technician (EMT) will be followed until the parent/guardian has arrived. If the EMT determines the need to transport the student and the parent/guardian has not yet arrived, then the student will be transported to the hospital.

SECTION III: STUDENT SAFETYBuilding Access

During program hours, the front door of the main building will remain unlocked for entry. Other doors (side, back and loading) will all be locked. This protocol is in place to ensure all incoming traffic is directed to the front office. If you take your students outside, please do not leave doors propped

open. Ensure your exit door is fully closed, and reenter the building through the designated entry door during program hours.

Identification BadgesAll 21st CCLC team members should wear their identification badge during program hours. This pinpoints you as a staff member to students, volunteers and guests. All visitors must wear temporary identification badges while in the building. If you see a visitor or volunteer in the building without an identification badge, please direct them to the front office so they can sign-in and receive a visitor’s badge.

Check-In ProceduresStudents will arrive at the out of school time (OST) program by going to the cafeteria for snack. Students will be escorted to the cafeteria by the traditional learning day teachers. Initial attendance is taken at the start of program by each 21st CCLC instructor. Instructors should take attendance for their assigned team of students. When there is an unexpected absence, it is very important the Learning Center Coordinator be aware of this so the family of the absent student can be contacted. If a student is not in program, and the family, school

administration, or the Learning Center Coordinator has not notified you of this absence in advance; please notify the Learning Center Coordinator within the first ten minutes of program. This policy is extremely important because it impacts student safety. We would never want a family to assume their child is with us, while we assume a child is with their family.

Attendance should be taken for each activity session throughout the remainder of program using the student rosters provided. Submit the completed attendance sheets to the Learning Center Coordinator at the end of each program day.

Check-Out Procedures Each 21st CCLC student has a registration form on file which lists approved modes of travel home following program, as well as individuals who are authorized for student pick-up throughout the academic year. For students’ safety, instructions provided on these forms must be explicitly followed. The only exception to this expectation is if the Learning Center Coordinator has been contacted in advance. Each 21st CCLC instructor will receive the copies of registration forms for the students they will be “checking-out.” In order for a student to check out, they must complete their daily activity and be walked to the bus by the designated staff member if they ride

the bus or have approval to walk home, or have an individual authorized for pick-up sign them out. You will be provided with sign-out sheets at the beginning of each semester.

Early pick-up is discouraged, but will happen occasionally. If a student leaves the OST program after initial attendance has been taken, that student and the authorized individual who is picking-up them must sign the student out at the front office.

If you ever suspect a student is being picked-up by someone under the influence, do not allow that child to leave the building. Please distract the person you believe to be under the influence. Keep them talking as long as you can and send someone (this can be a student, if need be) to get the Learning Center Coordinator. The Learning Center Coordinator will handle the situation once they arrive.

Student DismissalThe “Walkers”Students who have permission to walk home will be walked to the main entrance of the school. If the school is located on a busy street or one that has traffic, designated staff will walk the students across the street to ensure safe crossing.

The “Bus Riders”Students taking the bus home will be taken to the designated bus loading zone by the academic enrichment instructor. The instructor will wait for the bus to be loaded before reentering the school.

The “Parent Pick-Up” StudentsStudents being picked-up will be taken to the main entrance of the school where designated adults will pick them up. If students are to be picked-up by persons other than those designated at the time of registration (those listed on the student’s registration form), the Learning Center Coordinator must be notified prior to pick-up time, or the student will not be released to the individual who has arrived for pick-up.

SECTION IV: LEARNING CENTER SAFETY PLANS

The possibility exists that we may need to implement emergency procedures during out of school time hours. For this reason, we will also conduct practice drills. There will be one emergency drills each semester. The drill conducted could be any of the following: fire, evacuation, shelter in place, or lockdown. The procedures for these drills will be the same as those followed during the traditional learning day, and are outlined on the following pages.

Fire Evacuation

SIGNAL: Fire Alarm Bell

PROCEDURE 1. Students exit the room they are in (if they are in the building) and

WALK in a single line.2. Students evacuate by designated routes to the assembly area, which

is the field furthest away from the building. (Each room in the learning center has a fire evacuation map. Please be sure you are familiar with the exit route and designated assembly area for any space you use during program hours.)

3. There should be silence during the evacuation in case further instructions are provided.

4. Instructors:a. Take your attendance sheet/class roster with you.b. Check that all students are out of the classroom.c. Check that all exits are clear.d. Close classroom door. DO NOT LOCK. (Later entry may be required.)

5. Once in the assembly area, the instructor takes roll and accounts for each student.6. Students in classrooms other than their own are to remain with that class until given

permission to rejoin their class.7. Students should remain in orderly and silent lines until the all clear signal is given.8. After returning to the classroom, the instructor takes roll and accounts for each student. If

any students are not accounted for following the fire evacuation, this should be reported to the Learning Center Coordinator.

Learning Center Evacuation

A wide variety of emergencies, both man-made and natural, may require a learning center to be evacuated. These emergencies may include the following situations: fires, explosions, floods, earthquakes, tornadoes, toxic material releases, radiological and biological accidents, civil disturbances, and violence.

Instructors Learning Center Coordinator or Principal

2. Be clear on evacuation routes and sites. If our learning center is evacuated, we will go to the playground fence furthest from the building.

3. Close classroom doors and windows after everyone has been evacuated.

4. Leave the building quickly, quietly, and in an orderly manner.

5. Account for all students; keep track of and report all missing students to the Learning Center Coordinator.

6. Do not allow anyone to leave the secured area until the all-clear signal is given.

7. If the situation warrants vehicle and/or bus use, follow set procedures. The Learning Center Coordinator will share those procedures if the situation warrants.

Shelter-in-Place This is a precaution aimed to keep you safe while remaining

indoors, likely due to the release of chemical, biological, or radiological contaminants.

Principal/designee/Incident Commander receives information about a situation that requires sheltering-in-place rather than an evacuation or lock-down.

Different from Lock-Down procedures in that students/staff have some degree of discretion over movement, as directed by the Principal/designee/Incident Commander.

4. Turn off all ventilation that may bring in air from outside.

5. If the air within the shelter appears to be contaminated, inform the office of the problem.

6. Account for all students; keep track of and report all missing students to the office.

7. Discourage use of cell phones.

8. Continue to listen for and follow the directions given by the Principal’s Office. These instructions will be given through the PA system.

9. Do not allow anyone to leave the shelter until the all-clear signal is given. The all-clear signal will be given through the PA system.

1. Instruct staff to shelter-in-place by using the pre-designated PA system. This signal requires all persons outside to go indoors.

2. Call: 9-1-1 or emergency services.

3. Make sure persons with special needs are given assistance in getting to shelter.

4. Ensure that maintenance staff shut all exterior doors and shut off all ventilation systems. (Consider taping doors and windows.)

5. Notify superintendent/supervisor.

6. Make sure that people stay in shelters. (Use your best judgment in allowing visitors into a shelter.)

7. Communicate with police and fire departments to determine any change in the situation. Get/give periodic updates.

8. Communicate the all-clear signal by PA system once police and fire departments deem appropriate.

Lock Down Refers to protection from an internal or external threat by

excluding and/or isolating the threat. Protective cover is sought in locked sections of the building and

locked classrooms. Requires the ability to quickly secure all sections of the

building and campus, the classrooms, and all interior and exterior doors.

During a lockdown, limited use of all phones (including cell phones) is encouraged.

5. Do not allow anyone to enter/exit the secured area until the proper signal or response is given.In the event of any audible alarm occurring during a lockdown, all students/staff will remain in place. If evacuation alarms sounds, manually check and re-secure any unlocked doors or windows.

6. Do not allow anyone to leave the secured area until the all-clear signal is given. The all-clear signal will be given by PA system and verbally.

2. Call: 9-1-1, or emergency services.3. Make sure persons with special needs are

SECTION V: SHARED BEHAVIOR MANAGEMENT PLAN

The behavior policy at our learning center was established to help students develop self-discipline so they may function independently in a socially acceptable manner. Self-control and social skills are developed over time by participating in activities and interacting with others. The main reasons for establishing behavior expectations are:

1. to prevent possible harm to self or others,2. to prevent infringement on the rights of others, and 3. to prevent damage to equipment and property.

To provide a safe and secure environment for everyone, the following expectations have been developed which will be in effect at all times.

1. Follow rules of school district handbook.2. Follow the directions of instructors the first time they are given.3. Show respect and treat others with kindness. 4. Ask permission to leave an area.

Students will be taught to be responsible for their own actions; they make the choice to follow the rules and enjoy the rewards or to disregard the rules and accept the consequences.

Incentives

Cause and Effect: Positive Choices = Positive Outcomes

During our 21st CCLC program, we first and foremost acknowledge when students exceed expectations! During program hours, take note of who go above and beyond (socially, academically, perfect weekly attendance, etc.). Any student can earn recognition and will receive a small prize during the Student Showcase in December and the Award Ceremony in May.

Student incentives cannot exceed $5.00 per student per year, so all of our prizes are valued under $5.00, unless we receive a donation from a local store.

Instructors are also strongly encouraged to communicate good news to parents/guardians early and often! If a student makes a good choice – please let their family know about it!

ConsequencesCause and Effect: Poor Choices = Poor Outcomes

If a student has a discipline issue, please try to first work through a resolution within your own classroom (redirection, situation de-escalation, etc.). If the issue becomes problematic (a student’s choice[s] continuously interfere[s] with teaching and learning or creates a safety concern), please complete a Behavior Notification Form and make a copy of the completed form.

At the end of the program day, send the copy of the form home with the student and contact the parent/guardian to talk through what occurred during program hours. Ask the parent/guardian if they have any suggestions about ways to assist their child or ideas about how you can help redirect their son/daughter in the future. The conversation should feel more like team brain storming, not a negative call to place blame on their child. Please also follow-up with the Learning Center Coordinator and turn-in the original Behavior Notification Form so the Learning Center Coordinator can add it to the student’s 21st CCLC file.

If a student receives multiple Behavior Notification Forms may be removed from the program and prevented on participating in the program in future semesters. The Learning Center Coordinator or the academic enrichment coordinator will discuss the behavior issue with the student to determine what the cause is and what the best course of action will be. We want to encourage positive behavior and will model that through dialogue, genuine concern, and openness with parents.

We reserve the right to suspend or terminate a student from the 21st CCLC program immediately, if a student’s behavior warrants it. The Learning Center Coordinator is the only 21st CCLC team member who has the authority to move forward with this type of outcome. Overall, the priority of our program is to create a safe, welcoming space in which student learning can occur.

SECTION VI: REGULAR PROGRAMMING EXPECTATIONS

Supervise Participants at all times

Students should be supervised at all times, whether inside, outside, or during transition periods. Volunteers should not be left alone with

students. A paid 21st CCLC team member should always remain with students – even if a volunteer is present.

Instructor EngagementBe engaged with your students at all times:

Meal/Snack – Address students by name, talk informally with students, and ask how their school day went.

Homework Assistance – Circulate through the room; check to see if students are completing their assignments correctly. If students don’t have homework, have long term projects available for them to work on during the homework assistance time block. (A board game is not a long-term project.) Be sure to let families know the status of their child’s homework by using a homework folder. One side will be labeled, “COMPLETE,” and one side will be labeled, “INCOMPLETE.” Please ensure students are using their homework folders correctly.

Enrichment – If you are at an enrichment activity session and another instructor is leading an activity, you should be assisting with the activity. The OST program is not a time for adults to sit or stand around and chat or to catch up on their day school tasks. Every moment is to be spent teaching the students, assisting the students, and/or building relationships with the students.

Bathroom Procedures

Students will have an opportunity to use the restroom during snack and homework help time. If students must use the restroom, they will be allowed to go as per each teachers regular school day procedures whether it be signing out, using the bathroom pass, etc.

Classroom/Learning Space ResponsibilityThe OST instructor is responsible for ensuring that the indoor and outdoor spaces used by the OST program are left in the same condition or better than you found it. If the learning space is not your classroom during the traditional learning day, respect that teacher’s property and refrain from opening the desk drawers. Use the traditional learning day teacher’s computer ONLY with permission. Supervise your students at all times. You are responsible if something gets damaged or goes missing, so please teach your students to respect the room and equipment.

Attendance Policy If a student reaches three unexcused absences, Kelsey Moore, the site coordinator or the academic enrichment program instructor will contact the parents/guardians to determine if there are any challenges or barriers to attendance and problem solve with the family about how to remove these barriers. If the issue can’t be resolved, or the student continues to miss program

(unexcused), then the student may be dismissed from program in order to make room for a student on the waiting list. Please keep the Learning Center Coordinator updated if a student is close to reaching three unexcused absences.

21st CCLC EXCUSED ABSENCES:• Illness of student• Serious illness or death of family member• Head lice or nits• Doctor or dental appointments• Legal matters• Religious holiday or training• Unforeseen emergencies (e.g., natural disasters)• Prior commitment that has been documented in student file by the parent/guardian

Communication with FamiliesOST staff (instructors and site coordinators) will communicate on a regular basis with the parents of students in the OST program about their student’s experiences, behavior, successes, and achievements. Encourage family input and participation by inviting families to contribute their expertise and talents to the program and asking for feedback. Initial conversations should include identifying their child’s interests, abilities, and talents. Please document all communication with parents. Methods of communication may vary, but should include one or more of the following:

Verbal (face-to-face) Email Written Correspondence (mail) Meetings Phone Calls

Suggested items for discussions throughout the year are as follow: Respect for others (How does the student treat other

students and their instructor(s)?) Attendance (How many days has the student attended the OST program? Has their

attendance at program impacted their grades during the traditional learning day?) Academic (Is student completing homework? Is student experiencing any successes or

challenges with specific subjects?) Prepared (Does the student arrive at program with necessary supplies?) Attitude (Does the student seem ready to work hard on assignments?)

A newsletter will be sent home the last Friday of each month starting in November, 2017. A monthly program schedule, and list of contacts will be included in each newsletter.

Communication in Various Languages For those parents/families who do not speak English as their primary language, the flyers, applications, and parent and family workshop material will be provided in Spanish. We also have fluent Spanish speakers on staff so communication should not be an issue. We are an inclusive community striving to reach all families and parents!

Communication with StudentsHelp all participants feel welcomed, appreciated, and comfortable. Greet all participants by name. Communicate expectations clearly and frequently. Provide positive guidance and redirection that encourages the youth we serve to become self-disciplined. Encourage our

students to express their ideas and opinions regarding the activities that take place during program hours, and then follow-up with them regarding action steps based on their feedback.

Communication with Traditional Learning Day TeachersOST staff will communicate with the school day staff and instructors on a regular basis for the purpose of sharing information concerning the academic needs, homework assignments, and any discipline contracts or procedures used with students enrolled in the OST program. Please document all communication. Methods of communication may vary, but should include one or more of the following:

Reporting Fraud, Waste, or Abuse 21st CCLC is a federally funded program. The US Department of Education provides New Mexico (the grantee) with funding, and then the New Mexico Public Education Department distributes our state’s allocation to awarded subgrantees (NMSU STEM Outreach Center). 21st

CCLC funding has been made available at our learning center to assist our students and their families. Anyone suspecting fraud, waste, or abuse involving federal funds or programs should report their concerns. There are three different options for reporting. Please choose the method of contact which best suits you.
