Don Jones CSP, PE, MBA President and Principal Consultant Comprehensive Safety Solutions, LLC Don has extensive safety experience having worked in the petrochemical industry for over 40 years at a local, regional, national and international levels for several major companies: Exxon, Dow Chemical, and BP. During the time he worked for these companies, he has held many managerial positions including Site Safety Manager; Global Regulatory & Personal Safety Leader; and Site Health, Safety, Security and Environmental (HSSE) Manager. Don has also worked

for OSHA as a Special Government Employee (SGE). He is currently President and Principal Consultant of Comprehensive Safety Solutions. Don holds a BS degree in Civil Engineering from Louisiana State University. He also holds a Master of Business Administration (MBA) Degree from Texas A& M University. Don is a Professional Member of the American Society of Safety Engineers (ASSE), Certified Safety Professional (CSP) and Registered Professional Engineer (PE) in the State of Louisiana. He has held many offices in the ASSE including ASSE Society President. Don has received numerous awards and honors over his career. Tim is an inspirational presenter who brings experiences from his over 19 years of Occupational Health, Safety and Environmental experience. As a United States Marine, Tim earned the Humanitarian Service Medal and is a veteran of Operation Desert Shield/Storm. Tim’s approach intertwines his military combat and HS&E exposure to provide a more memorable experience for any audience. Tim is the Past President of the Arizona Chapter of the American Society of Safety Engineers. He also volunteers as the Committee Chair for Safety, Security and Health for the Water Environment Federation (WEF). He is currently working on two books, a Safety Engineering manual for WEF Press through McGraw/Hill and a Self Help Book titled the Core of FourTM. He

has also authored several published articles featured in industry magazines including Coatings Pro Magazine, Professional Safety Magazine and The Journal of Environmental Management. His industry accomplishments include certification as an OSHA Training Institute instructor, Asbestos Inspector/Management Planner, Hazwoper Instructor, California Specialized Training Institute Instructor and California Fire Marshal Instructor. He is also a Certified Safety Professional (CSP) from the Board of Certified Safety Professionals. While Tim’s history is impressive, it is his personal passion for safety that makes him such a motivating speaker. As a father of teenagers, he realizes the importance of safety not just at work, but also at home and on the road. This passion makes Tim’s training sessions entertaining, humorous and informative. Keynote Friday, February 24, 2017 12:00 PM - 1:15 PM Lunch / Keynote Speaker Changes In OSHA Dorinda Folse Join us as an OSHA investigator discusses four cases and their outcomes. In the first, you’ll learn about how employees at a frozen food production facility were exposed to carbon dioxide, and how OSHA’s Health Response Team assisted in identifying the source. The second case involves a 125-foot fall by an employee working on a zipline attraction on the island of Maui. The third case, titled “Lock Out Before You Clean Out – Make it a Two-Handed Job,” details how the unexpected energization of a meat blender entangled a hose and an employee, resulting in a double amputation. Finally, “From Ashes to Excellence – Applying Lessons Learned from Catastrophe” describes how violations of the Process Safety Management and Emergency Response standards at an adhesive plant led to a catastrophic explosion that injured four workers.

Dorinda Folse Area Director U.S. Department of Labor - OSHA Baton Rouge, LA Dorinda is a 1986 graduate of the University of Southern Mississippi. She began working for the Occupational Safety and Health Administration (OSHA) in 1991 as an Industrial Hygienist in the

Baton Rouge Area Office (BRAO). She served as the Compliance Assistance Specialist for three years and as the Assistant Area Director for a year before becoming the Area Director of the Baton Rouge Area Office in 2008.

William (Bill) Phillips Bill Phillips, P. E., is Managing Consultant with New Source Security, Treasure Island FL a multi-risk consulting group with emphasis on business safety, security and liability. His specialty with New Source Security is assisting Chief Security and Chief Safety Officers, directors and managers to set strategies and plans that add significant value to their companies and help them advance in their careers. For 18 years he was Vice President and Chief Security and Safety Officer (CSSO) for CNA Financial/Insurance with world-wide responsibilities for security, crisis

management, safety and business continuity/disaster recovery. He has a broad professional background as both a corporate security and safety officer and as an independent consultant. Prior to CNA, he headed up the corporate security and safety services group for Continental Grain Company, was the COO for a Washington, DC based consulting company, was an officer and head of a practice unit for the Clayton Group and held corporate positions with several industrial corporations. He is a member of ASIS International having served on the advisory board for its CSO Roundtable. He a Fellow and former National President of ASSE and the “father’ of the ASSE Foundation, member of the Safety Executives of NY as well as other professional organizations. He participates with the Department of State Overseas Security Advisory Council (OSAC), served on the iJET International Client Advisory Council as well as the HID Advisory Council. He has testified before Congressional Committees on various safety issues and regulations and is active on several American Society of Mechanical Engineers safety standard committees. He has made presentations internationally and around the US including ASIS International and ASSE. He was recognized by several industry groups for his research and work in control and prevention of combustible dust fires and explosions in the grain industry. He received CSO Magazine’s Compass Award for Security Leadership and has a Masters Degrees in Occupational Safety and Health from New York University.

Kelly Ingram-Mitchell, MPT, CEAS, CWcHP President Unify Health Servies Kelly Ingram-Mitchell holds a Master’s degree in Physical Therapy and a Bachelor’s degree in Exercise and Sports Medicine. Her experience exceeds 20 years in prevention services and injury management. Kelly previously served as Executive Director of a national therapy provider network and Director of Operations for over 20 outpatient therapy centers. Mrs. Ingram-Mitchell spent many of her industrial years developing and implementing client-specific prevention and rehab programs

including post offer testing, physical demand analysis, ergonomics, early intervention, and onsite therapy models. In 2000 she developed a FCE Certification Program and trained those in her profession for many years. In mid 2016 she will be launching a University focused on Occupational Medicine, Workers’ Compensation & Disability. The passion for clinical education and effecting optimal outcomes influenced Kelly to leave the corporate world and begin this business venture. She recognizes the need for heightened education which provides our profession with the tools necessary to service both pre and post injury. She serves as an adjunct professor at the University of North Florida, is a Fellow for Claims Litigation Management Alliance, School of Workers’ Compensation and has been the invited presenter at state, regional and national conferences for over 15 years. Kelly’s goal is to operate an organized top tier group of experts and professional partners, which utilize these tools providing best in class services for Unify’s clients and future prospects. Thursday, February 23, 2017 Breakout Session #1C 9:30 AM - 10:30 AM Track C - Regulatory A New Fall Protection Standard For General Industry Chris Irwin New year, new rule. As we ring out the old in 2016, we ring in the first major general industry OSHA update since the 1970s for walking-working surfaces (subpart D) and fall protection systems standards (subpart I) when a new ruling took effect on January 2017.

Chris Irwin MSA Global Training Instructor Chris Irwin, ASHM, is a global fall protection instructor and course developer with MSA, having worked extensively in both fall protection and confined space entry training programs. Prior to entering the safety field Chris worked teaching Spanish at a high school in

Pennsylvania. He received a Bachelor of Sciences degree in Safety and Health Management, and began his career at an OSHA Voluntary Protection Program (VPP) Star manufacturing facility as an Environmental, Health, and Safety specialist. Chris has also worked as a site safety supervisor for a general contractor in construction, and then began full-time safety training, covering various safety related topics from basic fall protection, competent person, and train-the-trainer programs to tower, wind turbine, and confined space rescue. In addition to training, he has worked carrying out on-site OSHA-style safety inspections pertaining to all areas of the 1910 and 1926 regulations, consulting with safety managers to establish corporate programs and policies. Chris is a certified OSHA 10 and 30-Hour trainer for both General Industry and Construction.

Thursday, February 23, 2017 Breakout Session #2A 11:00 AM - 12:00 PM Track A - Culture OSHA Update Sheldon Primus The U.S. Occupational Safety and Health Administration (OSHA) has changed elements of the 29 Code of Federal Regulation (CFR) 1904 to include electronically submittal of injury and illness data. OSHA believes this is a behavioral economic move to provide a "nudge" to employer to focus on safety. The information for injury and illness will be available publicly and is believed to help accuracy of recordkeeping data. This rule will be phased into effect within a two-year period with certain high-risk employers being targeted for more frequent reporting than their low-risk counterparts. Whereas, employers with high injury rates will have injury records available for workers, job seekers, customers, researchers, and the general public that will affect the way they do business.

Sheldon Primus Sheldon Primus is a Certified Occupational Safety Specialist with a Masters of Public Administration with a concentration in Environmental Policy. He has been in the environmental and occupational safety field since 1994. Additionally, he is a trainer for the Certified Occupational Safety Specialist program of the Alliance Safety Council-Baton Rouge, Louisiana. Sheldon is an authorized OSHA General Industry and Construction trainer for the 10 and 30-hour Outreach program. Sheldon has served as a Subject Matter Expert for Health Safety Institute and Red Vector Learning for General Industry and Construction OSHA courses. Mr. Primus is a guest columnist for the online publications of

Treatment Plant Operator (TPO) and WaterOnline. He has written article regarding regulatory compliance, operator safety, and wastewater process control. Additionally, he conducts OSHA compliance webinars for Audio Solutionz, Red Vector, Online Compliance Panel, Compliance Online, Compliance World, Edupliance, and Compliance Key. Currently, Sheldon is the owner/CEO of OSHA Compliance Help, an international safety consulting, training, and a regulatory agency compliance assistance company. He has presented safety topics at the Louisiana Governors Safety Expo, Florida Water Resource Conference, Florida Pest Management Association, American Society of Safety Engineers (ASSE) National Leadership Conference, and the South Florida Chapter of ASSE. Approximately 676,000 workplaces will be affected, including sites in construction and general industry and maritime. About 2.3 million workers are exposed to respirable crystalline silica in their workplaces. The majority of these workers, about 2 million, work in construction. About 295,000 workers are exposed to respirable crystalline silica in more than 75,000 general industry and maritime workplaces. Exposure to respirable crystalline silica can cause silicosis, lung cancer, other respiratory diseases, and kidney disease. OSHA estimates that more than 100,000 workers in general industry and maritime are exposed to silica levels that exceed the new permissible exposure limit (PEL).

This culture change is, however, an abstract concept – how can a pragmatic and methodological culture change be executed? We can look to those who have worked in culture changes in the most austere and dangerous of conditions for this process. Using military “hearts and minds” counterinsurgency operational components, we can avoid being the ‘safety police’ and instead enable and empower our employees to own safety at their own choosing.

Mr. Douvillier enlisted in the United States Army in 1971 and served 20 years as a Medical Corpsman, Infantryman, and Paratrooper. He served 5 years as a paramedic with New Orleans EMS and 15 years with the New Orleans Fire Department, most of that on their rescue squads. He has also served for 12 years with Louisiana Task Force One, an Urban Search and Rescue team. He has spent 11 of those years as their Task Force Leader. Mr. Douvillier has deployed to events such as Hurricanes Katrina, Ike, Gustav, and Ike, as well as tornado strikes in New Orleans, Convent Louisiana, and Tuscaloosa Alabama. Mr. Douvillier was the Incident

Commander of most of the water rescue efforts during the flooding in the Baton Rouge area in 2016. Mr. Douvillier has been teaching technical rescue subjects for over a decade including a tour in Kosovo assisting them in building an Urban Search and Rescue team. He is an occasional contributor to the technical rescue supplement of Firehouse magazine. He is currently the Urban Search and Rescue Coordinator for the State of Louisiana.

Friday, February 24, 2017 Breakout Session #6A 11:00 AM - 12:00 PM Track A - Culture Analysis Of Active Shooters Rickey Miller Active shooter attacks are dynamic incidents that vary greatly from one attack to another. The Department of Homeland Security (DHS) defines an active shooter as “an individual actively engaged in killing or attempting to kill people in a confined and populated area.” In its definition, DHS notes that, “in most cases, active shooters use firearms(s) and there is no pattern or method to their selection of victims.” This session will provide an analysis of active shooters by a highly decorated 24 year veteran of law enforcement.

Rickey Miller was a founding member and team leader of his department’s SWAT team and is currently the commanding officer of the Training Division. Rickey is a certified instructor in active shooter incidents, defensive tactics, less lethal munitions, pepper spray, Taser, and police driving. He is also certified as an instructor in numerous weapons systems through the NRA and Safariland training group and is a U.S. State Department certified firearms instructor. Rickey holds the distinction of being his department’s first and only master

firearms instructor. Throughout his career, he has worked as a part of a fugitive apprehension task force with the U.S. Marshal’s Service and has also provided personal security for visiting dignitaries and Fortune 500 executives. In 2014, he was selected to attend the FBI National Academy session 258, in Quantico, Va. where he trained for 11 weeks with 225 police and military officers from 49 states and 17 foreign countries.

This is an advanced topic for participants who are already familiar with Safety Instrumented Systems, Safety Instrumented Functions, Safety Integrity Levels, and Layers of Protection Analysis. Brief discussion will also be made of the requirements concerning field verification appropriate for such systems. These techniques are applicable and beneficial to Chemical Plants, Refineries, Pipelines, and other non-aerospace / non-nuclear applications.

This presentation will explore where true losses occur and some future trends and demographics that will create new challenges for our industry. Future trends such as the graying workforce will also be discussed to identify new potential risks. Ideas, solutions and examples on how to mitigate these potential losses will be given. This timely and interesting presentation will conclude with open discussion to identify other actions and solutions relevant to the real threats we face today and the new threats we will face in the near future.

Melissa Black is the President of MsR3, LLC, a Safety and Risk Reduction Company. She is an Adjunct Professor for Columbia Southern University in dual Colleges; Occupational Safety and Health and Criminal Justice/Homeland Security. She is an Associate with the Back School of Atlanta and conducts Workzone Classes as an adjunct instructor for the National Safety Council. She retired from Gwinnett County Government. Prior to her career in government she was the Environmental and Safety Manager at Power Plus, a battery manufacturer. She was employed by Law Environmental Labs and did Superfund Site assessments. She also worked for Exxon Refinery in

Baytown, TX. and for Dow Chemical in Plaquemine, LA. Ms. Black presented on the topic of Workplace Violence at the ASSE PDC in Atlanta, June 2016. She presented the topic Crisis Communication at the Clean Pacific Conference in Seattle in June of 2016. In the past, we thought that workers did not recognize the hazard, or did not properly understand the extent of the risk presented by the hazard. When we look at RISK TOLERANCE, we find that individuals may properly identify the workplace hazards, but for a variety of reasons the workers accept the risk and proceed with the task in an unsafe manner.

William Garnes - a Safety Security Health and Environment (SSHE) Advisor at ExxonMobil Global Services Company, an affiliate of Exxon Mobil Corporation. During his 27 year career, he has worked in a variety of roles, including leading small and large workgroups and project teams. In his current role, Will is responsible for analyzing the effectiveness of existing personnel safety programs,

and providing expertise and support to teams around the world. Most recently, he was responsible for developing the safety program at a new company facility in a highly Risk Tolerant part of the world. Will is an ASSE member, and holds a B.S. in Electrical Engineering from Rensselaer, and a M.S. from California Institute of Technology (Caltech), also in Electrical Engineering.

It is only in recent decades that this paradigm has shifted to risk-based assessment, analysis and mitigation. Furthermore, safety management is now viewed as a true career, with college degrees, certifications, professional societies, conferences, work-shops and subject-matter experts. At a time when respected companies pride themselves on safety performance and when all employees are encouraged to be safety advocates, OSH professionals should recognize how the tools and approaches that define the safety profession, such as Root-Cause Analysis, Audits, Hazard Identification and Communication, can be used to employers' advantage.

Riad Efendi graduated from Moscow Institute of Physics and Technology with B.S. in Chemical Engineering and Ph.D. in Environmental Biophysics. After military training and service at a ballistic missiles control center, he worked as an OSH specialist for Russian national oil company, before joining faculty at the University of Texas Medical School. Riad is an Exemplar (former RAB-QSA) certified Lead Auditor in Environmental and Occupational Health & Safety

management systems. Currently with TechnipFMC, he is credited with creating and rolling out Technip’s HSE management system and taking it through ISO 14001 and OHSAS 18001 certifications, as well as developing its behavior-based safety system. In 2016 he received the Company’s most prestigious Franquelin Award and American Society of Safety Engineers’ Safety Management Innovation Award for development of mobile application for site visit reports and observations. During his industry and academic tenure, Riad has published more than 20 peer-reviewed articles and has been speaking at national and international professional meetings.
