今日の目標

• 新しい Risk of Bias tool の使い方が説明出来る

変更の背景

用語の整理( 以前は同じ名前で違うバイアスを示すような混乱があった e.g. selection bias)

研究デザイン (cluster-randomized trials ) によってドメインを追加する必要性があった

GRADE に合わせて ( 一部は ) アウトカムごとには RoB 評価する必要性が出てきた

改善点への感想

より評価が clear に文面化されるようになった！迷わなくなったかもしれない！しかし大変！

変更点：目的ごとにツールを分けた　２種類

• “Intention to treat の効果”を見たい場合the effect of assignment to the interventions at baseline (regardless of whether the interventions are received during follow-up)

• “Per-protocol の効果”を見たい場合the effect of starting and adhering to the interventions as specified in the trial protocol.

変更点：研究デザインごとにツールを分けた　３種類

• Individually randomized, parallel group trials• Cluster randomized, parallel group trials• Individually randomized, cross-over trials

変更点：研究の結果を意識して評価するようになった

RoB ツールに以下を記述するようになった

• どのアウトカムを評価するのか• そのアウトカムの効果推定値

全部のアウトカムに対して個別に評価するのか？というのはまだ決まっていない！

ドメインが 7 個から、 5 個に減って、総合評価をする！

1. ランダム化プロセスでの Bias (arising from the randomization process)

2. 介入効果に影響する Bias (due to deviations from intended interventions)

3. 欠測アウトカムによる Bias (due to missing outcome data)

4. アウトカム測定に関する Bias (in measurement of the outcome)

5. アウトカム報告に関する Bias (in selection of the reported result)
