Dear Friends of Ravenswood Family Health Center,

I would like to take this opportunity to thank you for your support of Ravenswood Family Health Center. It is because of your generosity and our shared commitment to the ideal of equal access to health care for all that we have been able to achieve so much in our 15 years as a community health center here in East Palo Alto. Our new 38,300 sq. ft. facility, which opened in May 2015, has allowed us to grow and offer more services under one roof to our patients. We have implemented all of the new services we proposed for our permanent site. We now offer pharmacy, optometry, optical, screening mammography, ultrasound, and general x-ray services to our patients. We brought integrated behavioral health services into the same building, and expanded family medicine, pediatrics, prenatal care, women’s health care, and dental care as well.

In May 2017, we became nationally recognized by the National Committee for Quality Assurance (NCQA) as a Level Three Patient-Centered Medical Home (PCMH), which is the highest level achievable. At a Level Three PCMH like Ravenswood, our patients receive medical care from their personal primary care provider (PCP), who coordinates the various parts of their care with other doctors, follows up on lab results, and connects them with the care they may receive at a hospital. The members of every patient’s care team include the patient, health coaches, pharmacists, and behavioral health experts. Together, they address the many factors that impact health. Patients are able to access medical advice around the clock due to expanded clinic hours and an after-hours telephone advice line. Patients can also trust that the healthcare they receive is of the highest quality, as we at Ravenswood are continuously working to improve our services and provide positive experiences in patients’ health journeys.

This model of team care has helped us become more outcome-oriented and assess the effects that our services have had on our patients, both individually and as a population, over time. Team care provides our patients with tools and additional support services to empower them to take control of their own health, and make proactive choices that will benefit their health outcomes in the long-term. We have increased our focus on addressing the social determinants of health—of which the greatest is poverty—by connecting our patients to resources for food, shelter, clothing, special education, intimate partner violence, and parenting support.

Despite 15 years of promising development, we find ourselves facing uncertain times ahead. Our federal grant funding, which accounts for 15% of our revenue, is at risk due to a funding cliff. Our Medi-Cal reimbursement, tied to the continuation and solvency of the Affordable Care Act, is threatened by block grants. We need to meet these challenges by holding together as a community, by continuing our relentless advocacy with our legislators, by dialoguing with one another respectfully to find common ground, and by living by the universal value to “do unto others as we do unto ourselves”. We are our brothers’ keepers.

Thank you for your encouragement, your support, your contributions, your advice, your advocacy, and your faith in our abilities to perform. Thank you for your willingness to collaborate with us, and the love you have shared with us along the way.

As Ravenswood moves towards a future full of ambiguity and questions, we remain proud of all that we have accomplished and still hope to achieve. Dear Friends of Ravenswood Family Health Center,

To look back over the past 15 years is to recognize the many blessings associated with being a part of the Ravenswood Family Health Center Family. It has truly been a source of opportunities that have opened up endless other possibilities for growth. Some of us, who have been fortunate enough to have participated in this journey from the beginning, have real cause for gratitude.

Take a drive down Bay Road towards the Bay in East Palo Alto, and you can’t help but notice the tall, pale yellow and brick colored, new building that is home to Ravenswood Family Health Center’s main clinical campus. I remember as a brand new board member being given permission to begin services in portables, with the requirement that there would be a permanent building in five years! It was difficult enough acquiring the portables! Ravenswood Family Health Center responded to that requirement, despite challenges (expected and unexpected). The need for local and accessible community health care, combined with a lot of good will and hard work, enabled Ravenswood Family Health Center to construct a beautiful new clinic, albeit a little more than five years later.

The new clinic building is a physical reminder of the many years of partnerships and support that Ravenswood Family Health Center has enjoyed and drawn from to become a provider of high-quality health care in this community today. I would, therefore, like to take yet another opportunity to express my deep gratitude on behalf of the organization to government agencies, to philanthropic entities and individuals, to our partner health organizations, to all community support, to our wonderful staff, and lastly to my fellow Board members for all the support and good will we have enjoyed over the past 15 years.

As we move forward into the future, we clearly see the need for developing more programs and services in the quest to meet the health needs in our community. FOUR GENERATIONS OF CARE

When Ravenswood Family Health Center opened its doors in December 2001, Claudia, her parents, and her grandparents were among the first patients served. As a child, Claudia had chronic headaches, and her parents often brought her to Ravenswood for care. When Claudia got older, Ravenswood was there to help her navigate her pregnancies. During her pregnancy with her youngest child, Claudia was diagnosed with gestational diabetes and high blood pressure. Ravenswood’s perinatal care staff worked with Claudia to connect her to Lucile Packard Children’s Hospital at Stanford, to enable her to receive the specialty prenatal high-risk pregnancy care she would need to deliver a healthy baby boy.

Now Claudia brings the fourth generation of her family, her sons King and Kayden, to Ravenswood for all of their health care needs. Claudia and Ravenswood’s pediatric staff work together to ensure that her boys are healthy and thriving. In addition to accessing resources to meet her children’s needs, Claudia utilizes the parenting and child development resources provided by Ravenswood. These resources have helped her understand the importance of reading and singing to her children. She has also learned techniques to encourage her children to eat healthy fruits and vegetables. Without Ravenswood, many mothers like Claudia who live in East Palo Alto, Belle Haven, and North Fair Oaks neighborhoods would not be able to access the health care and support services they need for their children. Claudia truly appreciates that Ravenswood treats her and her sons like family.

WHOLE PATIENT CARE | LIFE CHANGING HEALTH CARE

At Ravenswood, our staff works collaboratively in order to care for the whole patient. It is not enough to treat symptoms; we must understand and investigate underlying causes to ensure our patients have a healthy life trajectory. The teamwork between Ravenswood’s Pediatrics Department and Ravenswood Family Dentistry is one example of how we are looking after the holistic well-being of our patients.

At only six years old, Cecilia was displaying health anomalies atypical for her age range. Her family, concerned about her health, brought her to our Pediatrics Department to address her symptoms. Cecilia had high blood pressure, difficulty breathing, skin discoloration, and a severe toothache. Her pediatrician, Dr. Saad, observed that Cecilia had skeletal anomalies and referred her for additional testing. The test results revealed that Cecilia has a condition which can cause tumors to grow along her nerve cells.

To further prevent unnecessary discomfort for Cecilia, Dr. Saad immediately referred her to our dental clinic, Ravenswood Family Dentistry, to address her toothache. Dr. Thakur, our Chief Dental Officer, discovered that Cecilia had cavities—which were aggravating her condition and required immediate attention. Dr. Thakur and her team quickly went to work to treat Cecilia’s cavities. Following treatment, Dr. Thakur has continued to work with Cecilia and her family by providing oral health education to help prevent future cavities from exacerbating her health. The teamwork between our pediatric and dental care teams has enabled Cecilia to remain pain-free.

When Antonio’s leg was in pain he knew he would need medical attention. In his search for medical services, one of his relatives referred him to Ravenswood. Antonio met with a Ravenswood provider and received a thorough examination. His primary care provider (PCP) informed him that he had developed gout, and prescribed him medication to help clear up the gout and relieve the discomfort in his leg. In addition to medication, Antonio’s PCP stressed the importance of making healthy dietary changes in addition to taking medication in order to prevent his gout from returning in the future.

Antonio embraced the advice of his PCP and began choosing healthier foods to consume. He is proud to report that, since his treatment, he has stopped eating fried foods. Now, he chooses healthier meal options. He reports that he regularly prepares healthy meals using vegetables, chicken, and fish.

By partnering with his PCP, Antonio’s gout has been successfully treated, and he is taking control of his health by making alternative dietary choices. At Ravenswood, we do not just provide our patients with a cure—we work closely with them to make changes and help them find opportunities to keep themselves healthy and strong.

Community Impact Award: 2015On May 5, 2015 we opened the doors of Ravenswood Family Health Center’s new state-of-the-art facility in East Palo Alto. Our new facility is 38,300 square feet, and contains 53 exam rooms. Since the opening of our new facility, we have been able to offer new services, including optometry, optical, ultrasound, mammography, x-ray, and retail pharmacy. In addition, our new health center has Energy Star Certified features, such as LED lighting, low-flow plumbing fixtures, solar panels, and a rainwater collections system.

Our expansion has made it possible for us to serve an increasing number of patients in our community and the larger Silicon Valley Community. We were awarded the Silicon Valley Business Journal Community Impact Award in 2015, in recognition of our significant impact as a nonprofit organization on the Silicon Valley Community.

Bronze Building Award: 2015Ravenswood was awarded the Bronze Building Team Award from Building Design and Construction Magazine in recognition of our team collaboration, design, and construction solutions. The construction and design of our new facility was made possible thanks to our partnerships with Rudolph and Sletten (GC), INDE Architecture, Hohbach-Lewin, Inc., Interface Engineering, and CSDA Design.

Ravenswood Family Dentistry’s Excellence Award: 2016Since we opened the doors of Ravenswood Family Dentistry (RFD) in 2010, we have been committed to providing dental health care services to individuals and families with low incomes and special needs in our community. In recognition of our Chief Dental Officer, Dr. Yogita Thakur, and our entire dental team’s leadership and excellence in oral health practices, RFD was awarded the John Rossetti Center of Excellence Award (sponsored by the DentaQuest Institute’s Safety Net Solutions program). RFD was also honored at the National Network for Oral Health Access’s annual conference in November 2016.

RFD was established to provide full-service dental care to patients of RFHC, low-income families, children with special needs, and homeless individuals residing in San Mateo County. Since opening RFD, we have increased the number of patients served from 2,000 patients in 2010 to 5,379 patients between July 2015 and June 2016. Today, RFD is at full capacity with 14 chairs on site. However, with the number of dental patients increasing each year, our next step is to expand RFD with nine additional chairs to serve more high-risk populations, including pregnant women, children, and individuals with diabetes.

Ravenswood’s Chief Executive Officer, Luisa Buada, has been recognized by various organizations for her drive and dedication to providing high-quality health care to underserved individuals in our community. Luisa’s commitment to improving the lives of the people around her was instilled in her from a young age. As she grew into adulthood, Luisa’s passion to make a change in the world did not diminish—it is what drove her to volunteer with the United Farm Workers’ Union during the grape workers’ strike, and prompted her to become a nurse.

After graduating with a nursing degree from the University of California, San Francisco, Luisa founded her first community health center, Clínica Popular del Valle de Salinas. This was only the beginning of her work with community health centers. Since 1980, Luisa has established and consulted with health centers throughout California, ensuring that individuals and families in underserved communities would have access to the health care they needed. She founded Berkeley Primary Care Access Clinic, and played an integral role in its merger with West Berkeley Clinic and Over 60 Health Center in order to establish what is now known as LifeLong Medical Care.

In September 2015, Luisa was honored with the Trailblazer Award from Project WeHOPE for her efforts to rebuild the lives of homeless and underserved individuals within our community. Individuals who are homeless and in need of care have access to a wide range of services, from medical and dental to health coverage enrollment support, all in one convenient location at Ravenswood.

In 2016, Luisa was awarded the John W. Gardner Award and the Silicon Valley Business Journal Excellence in Healthcare Award, and was nominated for the San Francisco Visionary of the Year Award. Individuals who receive the John W. Gardner Award are American Leadership Forum Senior Fellows who have been a source of inspiration to others to get involved, acted as bridge builders, and led others through complex issues successfully. Luisa was recognized for her leadership as a Silicon Valley hero.

As a nominee for the San Francisco Visionary of the Year Award, Luisa was recognized for her work towards making the world a better place. One of the many ways that Luisa has impacted our community is through her work as CEO of Ravenswood. When Luisa first started at Ravenswood in 2002, our health center was housed in a modular building and consisted of only 13 employees. Under her leadership, we have moved into a custom-built, state-of-the-art health center and now have over 200 employees serving our ever-growing patient population.






Through Luisa’s leadership, Ravenswood provides high-quality health care services to individuals from all walks of life. Luisa Buada’s commitment to the health and well-being of our community is truly inspiring. She is more than just a CEO—she is a friend and a mentor to every employee at Ravenswood and to every patient who walks through our doors. It is not only our community that is honored by Luisa’s influence—our staff at Ravenswood are honored to be guided by her vision.

December 2016 marked Ravenswood Family Health Center’s 15th anniversary. We are honored and privileged to have spent the past 15 years helping the individuals and families in our community grow and thrive, and we look forward to continuing to provide high-quality health care for years to come.

Since our humble beginnings as a modular health center, Ravenswood has expanded its staff, services, and space. The first five years of our existence were dedicated to ensuring that Ravenswood had a sound organizational structure, and that measures were in place to support our mission and the needs of our patients.

As Ravenswood matured, our need to provide health care began to extend beyond the four walls of our center. A mobile clinic, donated by Lucile Packard Children’s Hospital at Stanford, opened up new opportunities for us to physically reach out into the community and provide care through our Health Care for the Homeless program. The mobile clinic facilitated our ability to travel into our community to meet with individuals who had difficulties accessing our health center for care.

Over the years, we observed that additional services were necessary to help keep our patients healthy. Ravenswood Family Dentistry was established in 2010 in response to the






tremendous need for accessible oral health care. Our dental clinic has provided full-service dental care to adults and pediatric patients that includes preventive care, restorative and periodontal care, crowns, bridges and dentures, oral surgery, emergency dental services, and oral health education. In 2012, we received funding to launch our Virtual Dental Home program, enabling our dental team to provide oral health care services to children in local schools.

On May 5, 2015, we opened the doors to our new state-of-the-art health center. Moving from our modular offices to a permanent facility has allowed us to successfully expand the services we are able to provide our patients. Since the move, Ravenswood has introduced new services such as optometry and optical services, full-retail pharmacy, x-ray, ultrasound, and mammography.

Although Ravenswood has grown from a modular clinic with only 13 employees to a full-fledged health center with more than 200 employees among our six office locations in East Palo Alto, we strive to maintain our close relationship with our community. While we are looking to the future to grow and expand our reach, Ravenswood will always aim to be the medical coach, the confidant, and the friend our patients and our community look to, so that together, we may achieve a healthier tomorrow.
