3. The RSC shall evaluate the documents pertinent to thequalified cooperatives that responded to the notice givenand validate the same;
4. Based on the result of the RBOJ, the extension office shallendorse to the Central Office the top three (3) cooperativesper category. The following shall be the minimum ratings :

50%-micro 60% -small
70%-medium 80%- large

1. The CDA extension office shall come up with the list ofpotential nominees for primary cooperatives, federationsand unions which shall be derived from the database of theAuthority;
2. The Extension Office shall notify the concerned cooperativesand direct them to prepare and submit the necessarydocuments for the competition;
3. The RSC shall evaluate the documents pertinent to thequalified cooperatives that responded to the notice givenand validate the same;

5. The TWC shall evaluate the documents submitted by theextension offices and validate the veracity of documentsand information pertinent to the qualified entries andsubmit the results of validation to the ProgramManagement Committee (PMC);
6. The TWC shall present the result of the evaluation andvalidation to the PMC sub-committees for selection offinalists to each category;
7. The PMC Head or sub-committee representative shallpresent the result of the finalists to the BOJ;

8. The members of the sub-committee shall be present duringthe BOJ Meeting who shall present the result;
9. The TWC shall also be present during the BOJ Meeting toprovide technical support to the Sub-Committeerepresentative presenting the result
10. The PMC shall come up with the short list of nationalfinalists for each category for submission and presentationto the National Board of Judges (NBOJ);
11. The NBOJ shall proclaim the winners for each category.

1. The Leaders, CDOs and CDCs must be nominated by anorganization/s as identified in the qualifying criteria for eachcategory;
2. The accomplished nomination form together with thedocumentary requirements must be submitted to theExtension Office where the nominee resides/located;
3. The RSC shall evaluate the documents pertinent to thenominees;
4. The RSC shall recommend to the Regional Board of Judgesthe finalists for each category .
The required points as finalists: 70%
5. The Regional Board of Judges shall proclaim the winners foreach category.
6. The RSC shall endorse to the TWC the Regional Winner (1st
only)

Eligibility/Qualifying Criteria
• Must be a cooperative member in good standing for the past ten (10) years immediately preceding the nomination – With share capital not exceeding 10% of the total
subscribed capital – Actively participating in the SMCBU program of their
cooperative – Regularly attending General Assembly Meeting – Patronizing the services of cooperatives
• Presently or had been an officer of cooperatives for at least five (5) years whether elective or appointive
• With no derogatory record / No administrative or criminalrecord
• With exemplary contributions to cooperative development
• Must be nominated/endorsed by at least two (2) differenttype of organizations from the following : cooperative,government organizations (except CDA), local governmentunits, private sector, other institutions
• He/she is not receiving compensation from the cooperative
• The Nominee and nominator should be at same region
Eligibility/Qualifying Criteria
1. Nomination Form2. Profile of the candidate3. List and photos with descriptions of the accomplishments and
contributions to cooperative development for the past five (5)years.
4. List of Technical Assistance Rendered to cooperative for the past five (5) years.
5. List of innovative programs/projects introduced by the nominee which are being practiced by the cooperative (e.g. marketing system, new product develop) supported with BOD Minutes of Meetings, Annual Report, Board Resolutions, publications)MOV: minutes of the meeting, programs/projects must be proposed by the nominee; programs /projects must have impact to the coops and to the cooperative development
DOCUMENTARY REQUIREMENTS
6. Board Resolution approving affiliation of cooperative and proof of membership or affiliation of cooperative during his/her term
7. Proof of resource generated for the last three (3) years such as donations, grants, subsidies, loan assistance etc.
8. Number of technical assistance rendered to cooperatives for the past five (5) years;
9. List of materials developed and presented in fora/conferences/ seminars (i.e. Publications, researches, etc.)
10. Certification from LGUs of his/her membership/representation in the activities/planning bodies and his/her involvement/ participation in the LGU activities
11. Certification from other organization/s that he/she has been or is currently an officer or member of such organizations;
DOCUMENTARY REQUIREMENTS
12. Proof/evidence of cooperatives organized/served by thecooperative leader with increase in any of the followingaspects:• Asset/Networth/Income/Net Surplus/Membership
13. Certification from Cooperatives on the following: • His/her major involvement in the organization of
successful cooperatives• membership in good standing for the past ten (10) years;• share capital, savings, attendance to GA meeting and
patronage of cooperative’s business
14.Proof of Awards, Certificates of Recognition / Citationreceived for the past five (5) years
DOCUMENTARY REQUIREMENTS
