Bringing the Best Together2016 REPORT TO OUR DONORS

Dear Friends,

The work we completed in 2016 reflects on this agency’s long history of community service, excellence, and innovation. VNA of Middlesex-East was initially formed when five community home care agencies, The Woburn VNA, The Reading VNA, The Wakefield VNA, The Stoneham VNA, and The North Reading VNA, were invited to participate in a demonstration project sponsored by a federal government grant in 1962. The project’s purpose was to explore the advantages of combining operations. It proved to be a great success and in 1967 the agencies formally united into one agency called VNA of Middlesex-East.

Fifty years later, our agency once again found advantages by combining with other high performing organizations to better serve our community. After joining the Lahey Health system in 2014, it was our goal to integrate with the system’s two other homecare agencies, Lahey Health at Home and Winchester Hospital Home Care. This integration was completed on January 1, 2017 when the three organizations came together as one home care agency under the Lahey Health at Home name. Hospice and palliative care will continue to be an important and integral part of our service and will retain the Middlesex-East Visiting Nurse Hospice and Sawtelle Family Hospice House names.

The demand for home health, hospice and palliative care services is growing dramatically. Over the past five years VNA of Middlesex-East experienced a 67% increase in patient referrals. These services reduce healthcare spending while providing care in the patient preferred setting of the home. As a combined organization we expect this growth to continue as we play a larger role in Lahey Health’s efforts to bring the cost of healthcare down while improving patient outcomes.

The Annual Report gives us the opportunity to express our gratitude to our family of donors. Gifts of support have made it possible for us to touch the lives of thousands with care and compassion when it is needed most. We thank you for your past generosity and ask for your continued support as we move forward to improve the health and well being of our patients and community.

2016 NUMBERS TO NOTE

VNA of Middlesex-East saw continued growth and achievement in 2016. We are proud to be among the highest performing home health agencies in the nation, focused on bringing the best of care to our patients.

l Patient referrals increased by 7%

l Made 87,879 homecare visits to 4,415 patients

l Provided hospice in the home to 419 patients

l Cared for 201 patients at our Sawtelle Family Hospice House

l Served 65 Greater Boston communities

l Named a top 500 HomeCare Elite agency, one of 110 organizations nationwide to receive this status every year since the award’s inception in

2006

l Placed telehealth monitors in 1,071 patient homes to track vital signs on a daily basis

l Provided respite care free of charge through our Memory Care Program

that totaled 2,955 hours of in-home care and 165 days of adult

daycare

l Gave bereavement support to 741 families dealing with the loss of a loved one

l Received 2,200 hours of donated time from 38 dedicated Hospice volunteers

SAWTELLE FAMILY HOSPICE HOUSECONTINUES GOOD WORK

When patients are unable or prefer not to receive hospice care in the home, our Sawtelle Family Hospice House offers an alternative. It is a “home away from home” where individuals receive end-of-life care in a restful, intimate setting. Since opening in 2003, the house has consistently operated at full capacity.

The many donations we receive in appreciation of the care given at the House help us maintain it in the highest of standards. In 2016 we were able to change the layout of the Sunroom and Great Room to accommodate the increasing number of families and visitors who look for gathering space outside patient rooms. New furniture was purchased and arranged to create more intimate seating areas for small groups. The updated décor has a fresh and peaceful feel conducive to quiet conversation and relaxation.

Also in 2016, a new patio was added to our landscaped grounds thanks to volunteer Austin Paul who designed and built the space for his Eagle Scout project. During the warm months our patients and families make good use of this extra outdoor seating area.

Because of our family of donors we have been able to meet a great need in the community for residential end of life care. Charitable support allows us to remain the lowest cost hospice residential facility in the nation. It also assures we are able to provide care to all in need, regardless of ability to pay for service.

A HOSPICE TEAM AND LOVING FAMILY Fill a Father’s Final Days with Grace and Dignity

Domenic Caruso is described by his family as a wonderful, happy man who loved life and people. His friends called him a gentle soul. He was a member of numerous community organizations, proud of his Italian heritage and always enjoyed a game of golf or Bocce. As an active 84 year old retiree it was difficult for him to lose his independence after developing prostate cancer. But throughout his illness he remained as upbeat as possible, never complained, and kept his great sense of humor while being cared for by his daughter Anita and her husband Michael in their home. With the help of the VNA of Middlesex-East he and his family shared final days filled with love, respect, and dignity.

Domenic first used VNA of Middlesex-East services as a homecare patient in January 2015 during the early stages of the disease. A team of nurses and therapists helped him manage the cancer, controlling his pain and preserving his strength and mobility. His daughter Anita is especially grateful to his nurse Barbara McGillvray, RN whose visits her dad looked forward to. She says that when the cancer metastasized and spread to his bone and kidneys, “It was Barbara’s skilled nursing that enabled her to see it was time for our Dad to be transferred into the Hospice program.”

Hospice focuses on providing comfort and support to people dealing with a terminal illness. Stacie Lage, RN and Paula Lunn, RN were Domenic’s primary caregivers during this time. “Stacie was attentive, extremely skilled, and very willing to teach and guide us in Dad’s care,” notes Anita. “Paula always took extra time to come up with creative ways to keep him comfortable,” she adds. “She was a lifesaver and in our minds Paula went above and beyond what was expected of her. The comfort and compassion that she gave us is something we will never forget.”

To receive hospice care in the home a patient needs 24-hour support. “They were amazing”, Paula says adding, “We should all be so lucky to have family like that.” When Domenic’s daughter Anita was working, her husband Michael, a retired firefighter, was his father-in-law’s primary caregiver. Paula says, “He tended to him so lovingly it made your heart smile.”

VNA of Middlesex-East Social Worker Tammy Pacyna was also an important part of the hospice team helping coordinate the additional services needed during the final weeks of Domenic’s life. The family greatly appreciated not only her professional advice, but also her kindness.

When a life-limiting illness no longer responds to curative treatment, choosing to receive hospice rather than continue with aggressive treatments can often improve both the patient’s quality and length of remaining life. This is a very difficult decision, however, and does not come easy to patients and their families. Lahey Hospital and Medical Center physician Elizabeth Collins, MD, a palliative and hospice care specialist, explained the program to Domenic and his children and with her careful guidance they took the step to begin hospice. Anita says “It was a very difficult time for my Dad as well as his family being his primary caretakers. It was extremely emotional but we can honestly say that without the support and skilled care given by the VNA of Middlesex-East team, we are not sure we could have cared for Dad at home surrounded by his family’s love. There are not enough words to say how thankful we are for that.”
