Seventy-one representatives attended the TDA Leadership Conference on Saturday, June 16, 2012, in Austin. TDA President Dr Michael L. Stuart facilitated the 1-day conference, hosted by TDA Financial Services, Inc. (TDA Perks). e conference combined meetings of the TDA Presidents Presidents-elect Conference, the TDA Council, Committee and Affiliate Training, and the TDA Component Society Presidents Committee. e morning session included presentations from TDA Perks and the TDA Smiles Foundation. Presidents from each component society gave an overview of society activities. Each council and committee also gave brief reports about current and future projects. DENPAC Chair Dr Phil Nauert reported on the current activities of DENPAC and the necessity for TDA Holds Leadership Conference Changes to Texas Medicaid Dental Program As of March 1, 2012, most children and young adults age 20 and younger enrolled in Medicaid and the Children’s Health Insurance Program (CHIP) began receiving their dental services through one of the 3 state selected dental plans: Delta Dental, DentaQuest, and MCNA Dental. On June 29, 2012, the TDA participated in a successful Dental Implementation Roundtable meeting hosted by the Health and Human Services Commission (HHSC). e meeting showed HHSC’s continued commitment to working with dental stakeholders to address issues that impede efficiency and effectiveness of the Medicaid and CHIP dental programs. Following the meeting, the TDA suggested 3 priority items for future discussion with HHSC: (1) continued support of the Main Dentist (ie, dental home) defined as the ongoing relationship between the dentist, who is the primary care provider, and the patient or parent (eg, legal guardian) in the instance of a minor child; (2) creating a “Dental Advisory Workgroup” to offer organized dentistry and other stakeholders an opportunity to give valuable clinical input about Medicaid/CHIP dental benefits prior to any final benefit decisions by HHSC; and (3) improving procedural consistency among the dental plans. Although the very nature of commercial administration means that the dental plans have flexibility in developing their own claims processing requirements etc., it is extremely helpful for practicing dentists when as many processes as possible are standardized among the dental plans. e TDA Committee on Access, Medicaid and CHIP (CAMC) is still designing a Texas Dental Program webpage that may be accessed on the member side of the TDA’s website in August 2012. CAMC intends the webpage to be a “one-stop shop” for Medicaid/CHIP dental information. Let us know the information you would like included on the webpage. Dr Maria Lopez Howell Helps Launch ADA’s New Consumer Website American Dental Association (ADA) Spokesperson Dr Maria Lopez Howell of San Antonio participated in a 28-city satellite media tour on June 28, 2012, in Chicago, IL, to help launch the ADA’s new consumer website, mouthhealthy.org. e website is available in English and Spanish and provides timely and credible oral health information on prevention, care, and treatment. Dr Howell participated in 11 taped and 17 live 2- to 3-minute English and Spanish interviews on the satellite media tour with television stations around the country. A general dentist since 1983 and ADA consumer advisor since 1991, Dr Howell hosted for 14 years the ADA Dental Minute, a 1-minute public service announcement available on ada.org. It provides dental history and education for the public. Many of the Dental Minutes were revised for use on mouthhealthy.org and are called Mouth Healthy Moments. In March 2012, after more than 1 million airings and 467 episodes, the ADA Dental Minute and Dr Howell were honored as the longest running live action public service announcement (PSA) series in the history of public service broadcasting, according to Nielsen Media Research. Dr Howell has appeared in more PSAs than any other performer in the history of public service broadcasting — the crew taped about 40 shows in 2 days each year since 1998, according to the ADA. e Dental Minute has aired in every U.S. market, as well as in Mexico, Panama, and England. ADA Spokesperson Dr Maria Lopez Howell of San Antonio participates in a 28-city satellite media tour on June 28, 2012, in Chicago, IL, to help launch the ADA’s new consumer website, mouthhealthy.org. Black moderated a panel discussion on recent and upcoming legislative activity. e panel included TDA Director of Public Affairs Jess Calvert, Policy Manager Diane Rhodes, and Policy Analyst Danielle Taylor. Also in the afternoon, Future Focus Committee Chair Dr Arthur Morchat reviewed the Association’s strategic plan, TDA 2014, and reported on the necessity for input from all councils and committees to ensure its success. TDA President-elect Dr David Duncan conducted a roundtable discussion with the TDA Component Society Presidents Committee.

Membership growth is vital to any thriving organization. The Texas Dental Association (TDA) Board is strongly committed to the Membership goal within the “TDA 2009” strategic plan, which addresses continued growth. I am proud to report that, as evidence of that commitment and for the second year in a row, the TDA received the following ADA awards in 2008:• Greatest Percentage Membership Net Gain: Texas had a net gain of 198 members in the year 2007.• Top Constituent Dental Society with Greatest Percentage of Non-Members to Membership: Represents a conversion of 4.7 percent of non-members into membership at end of year 2007.• Top Constituent Dental Society to Improve Retention Rate Percentage in 2007: Represents a 97 percent Association retention rate, which is an increase of 7 percent from the previous year.• Greatest Percentage of New Dentist Members: Represents a gain of 111 new dentist members (those dentists who are 1-9 years out of school).

Another example of the commitment to the growth of our Association is the TDA Externship Program, which builds student awareness of organized dentistry. This innovative program, created in 2007, offers students the opportunity to gain insight into the different components of organized dentistry including legislative and regulatory affairs, membership, governance, continuing education, and charitable dentistry and to gain a better understanding of their role as future leaders of the dental team. The program is open to dental students from each of the three dental schools in Texas. This year the externship program will take place June 20 – June 27 in Austin at the TDA central office. As an ongoing effort to enhance the value and benefits of the Association, the TDA Council on Membership developed a survey that was sent to all members in 2006. The results clearly showed that members have a high satisfaction rating with benefits such as the TEXAS Meeting, TDA publications, and the ability to participate philanthropically in the TDA Smiles Foundation. A great number of the discounted programs offered to members through the TDA Perks Program also received a high satisfaction rating. In accordance with the strategic plan, the Council on Membership, will survey members again in 2009. Participation from all members and a collaborative effort between the ADA, TDA, and local component societies are the keys to successful membership recruitment and retention. We encourage every member to get involved whether it is by attending local meetings, taking part in Give Kids a Smile! Day activities, volunteering at a Texas Mission of Mercy, or participating in TDA Legislative Day activities in Austin. Each member matters and is essential to the continued growth of our Association and of organized dentistry. This is the seventh and final in a series of articles focused on the TDA’s accomplishments guided by the strategic plan. Seventy percent of active licensed dentists in TX will be members of TDA by 2009 (baseline of 68.5% in January 2007); increase

membership retention rate to 97 percent by April 2007 (baseline 96% in 2006). a-d. Increase the number of dentists serving on component peer review and ethics and judicial affairs committees to attain a

participation rate of 2-5 percent relative to the total number of component society members by August 2009.2. Ninety percent of TX dental school graduates actively licensed in TX will be members of TDA five years after graduation (89%

dental student conversion from the graduating class of 2005 in Texas).3. Increase the number of member dentists actively participating in TDA by 1 percent a year (as measured by a variety of means

– participation at TX Meeting, TMOM, TXDDS, FSI programs, DENPAC participations, TDA legislative day) with annual report to TDA Board by the Council on Membership.

4. Establish baseline of nonmember participation at TDA activities (CAS, TDA Smiles Foundation, Membership) 5. Established baseline of members’ understanding of and satisfaction with the benefits of their TDA membership through Member

The TDA will update it’s strategic plan with another statewide retreat in August of 2008.

In 2003, Texas Dental Association members from across the state develop TDA’s first strategic plan, TDA 2009.

Health Insurance Exchanges This is the fifth in a series of articles to keep you informed about the Health Insurance Exchange (HIX) in Texas. The HIX is an online marketplace tool enabling consumers to shop, compare, and enroll in a health insurance plan. Future articles will focus on helpful reference websites and other information about the HIX.

On June 28, 2012, a divided (5-4) United States Supreme Court (SCOTUS) upheld the majority of the Patient Protection and Affordable Care Act including provisions establishing state health insurance exchanges, minimum health insurance benefit packages including a pediatric oral health benefit, and mandating individuals to either purchase health insurance or pay a penalty.

The SCOTUS ruling makes no changes to the Essential Health Benefit (EHB) package for the individual commercial insurance and small employer insurance group markets. Non-grandfathered health insurance plans in the individual and small group markets, Medicaid benchmark coverage, and the Basic Health Plan must include EHBs beginning in 2014.

Texas must now identify a “benchmark plan” for the individual and small employer markets chosen from one of 4 categories: 1) the largest plan of one of the 3 largest small-group products in the state; 2) one of the 3 largest state employee plans; 3) one of the 3 largest Federal Employee Health Benefit Plans; or 4) the state’s largest non-Medicaid HMO. Furthermore, Texas must select and supplement, if necessary, its commercial EHB benchmark plan by the end of September 2012.

The health benefits offered by the chosen benchmark plan, and supplemented by benefits from other plans if necessary to cover all categories of essential benefits listed in the PPACA, will become the EHB for Texas. All health insurance plans offered in the individual and small group markets must offer equivalent benefits starting in 2014.

The PPACA also authorized individual states to establish a HIX, but mandated that states participate in a federal exchange if they refuse to act. Since Texas delayed planning for an HIX, it now appears likely that Texas will have a Federally Facilitated Exchange in at least 2014, and possibly longer.

Given the limited time — September 2012 — to select an EHB benchmark, the TDA recently joined patient and consumer advocacy groups, health care provider associations, and other health care stakeholders in a letter to legislative and agency officials requesting the immediate creation of a Texas Department of Insurance and Health and Human Services Commission Essential Health Benefits Stakeholder Workgroup.

Intraoral Orthopedic Appliances for TMJ DisordersA systematic review including a meta-analysis evaluation about the use of orthopedic appliances to reduce pain associated with TMJ disorders was published in the Journal of Orofacial Pain in 2010 (1). Pain reduction of subjects with TMJ disorders involving joint and muscle pain were treated with orthopedic devices and compared with no treatment, placebo control, or other procedures. The conclusion was, “Hard stabilization appliances, when adjusted properly, have good evidence of modest efficacy in the treatment of TMJD pain compared to non-occluding appliances and no treatment. Other types of appliances, including soft stabilization appliances, anterior positioning appliances, and anterior bite appliances, have some RCT evidence of efficacy in reducing TMJD pain. However, the potential for adverse events with these appliances is higher and suggests the need for close monitoring in their use.”

Victoria hosted its first TMOM on June 22-23. This 23-chair TMOM included more than 50 dentists and hundreds of other dental and community volunteers to help make it a success.

Dr Chuck Rader and Dr Lisa Heinrich-Null chaired this TMOM. They mobilized their community and saw the event through to completion. Local dentists came to help, and dentists came from as far as El Paso and Mt. Pleasant to volunteer. Together, volunteer dentists and hygienists saw 573 patients and provided more than $391,000 worth of charitable care. Thanks to this community’s generosity, hundreds of Victoria residents are now out of pain and have something to smile about.

Speaking of members, I want to congratulate 3 of our members who are highlighted in this issue of TDA Today. Dr Alan Coleman was recently elected mayor pro-tem ofBeaumont. What an honor for Dr Coleman! Dr Gayle Glenn was elected president-elect of AAO! Congratulations, Dr Glenn! Also notice the story of one of our TDA vice-presidents, Dr Michael Goulding. Besides being a really good guy, Michael has a really interesting story to tell. This is the first of our TDA Member Spotlights, so expect to read about more of our members in the future. Many of our members give sacrificially to serve in variouscapacities. Dr Phil Nauert serves as both our chair ofDENPAC and the mayor of Bellaire. Way to go, Phil!How do you spend your life outside of the office? Maybeyou volunteer. If you do, please consider lending your skillsto a Texas Mission of Mercy (TMOM). The TDA Smiles Foundation organizes these outstanding events, and the next one is in Brady on July 28. The Dentist’s Creed (author unknown) encourages dentists to “take good trip of the joy of life … and to endeavor to grow as a Dentist and as a man or woman with the passage of time.” What a wonderful way to grow as a dentist as a volunteer at a TMOM. Dr Michael J. Goulding of Fort Worth says there is one event that changed his life forever: the day he collapsed at the New York City Marathon.

He might be best known throughout the Association as a member of the TDA Board of Directors. In his third year of service, he’s the current vice president of the northwest district. Dr Goulding graduated from the University of Texas Dental Branch in Houston in 1984 and owns a general dentistry practice in Fort Worth. Among the years of experience, fellowships, and memberships in professional societies, there is something else that keeps him busy. He’s an avid runner.

In fact, Dr Goulding participated in the New York City Marathon 2 years ago; however, that race turned out to be one that he did not win, on many levels. As described in a New York Times article titled “At Mile 19, a Runner from Texas Is Resuscitated by Firefighters” on November 9, 2009, firefighters from Engine Company 35 in Harlem used a defibrillator to revive Goulding when he collapsed and stopped breathing. An ambulance took Dr Goulding, then 51 years old, to the hospital, and it was determined that he had sustained a minor heart attack.

Miraculously, and thanks to the chance location of the firefighters that day in 2009 and their diligence in resuscitating Dr Goulding, he is running again. Dr Goulding was invited by the New York City Fire Department (FDNY) to participate in the Stephen Siller Tunnel to Towers Run in September 2011. It benefits families of FDNY who died from the events of 9/11.And, Dr Goulding was able to thank his resuscitators in person.

“I had dinner with one of my rescuers and we had a long, nice chat about life,” Dr Goulding says. “I found it strange that she was so thankful to me that I returned. I explained that the run was the least I could do as I am still so grateful.”

TDA Member Elected Beaumont Mayor Pro TemThe Beaumont City Council voted unanimously to elect Alan B. Coleman,

DDS, FAGD, as mayor pro tem on July 3, 2012. Beaumont is the 24th largest city in Texas with a population of 118,296.

Dr Coleman has practiced general dentistry in Beaumont for 29 years and joined TDA in 1983.

Besides his involvement with city governance, Dr Coleman founded Southeast Texas Hands For God Ministry. This 501(c)(3) non-profit ministry has traveled within the U.S. and abroad to provide free dental care to those in need. Dr Coleman has served on numerous charitable boards. Last year, he and his wife were honored as the “Family of the Year” by Family Services of Southeast Texas, and he is a recipient of the Jefferson Award from the American Institute for Public Service.

“I passed Mile 18. Then Mile 19. I was tired but didn’t feel any pain. I saw the water stop and a group of firefighters. The next thing I remember is waking up in an ambulance.”

TDA Member Elected AAO President-electGayle Glenn, DDS, MSD, a Dallas orthodontist, was elected the 2012-13 president-elect of the American Association of Orthodontists (AAO) at its 112th annual session in Honolulu, HI, in May 2012. AAO is the oldest and largest dental specialty organization in the world.

She is an experienced leader in organized dentistry and orthodontics. She is a past president of the Southwestern Society of Orthodontists and the Texas Association of Orthodontists (TAO). Dr Glenn has also served on the TDA Council on Membership.

Dr Glenn is a member of the American Dental Association, the TDA, the Dallas County Dental Society, the TAO, the Greater Dallas Association of Orthodontists, the Texas Orthodontic Study Club, and the American Association of Women Dentists.

Dr Glenn earned her DDS at the University of Texas Health Science Center at San Antonio Dental School and her MSD at Baylor College of Dentistry in Dallas. She is a diplomate of the American Board of Orthodontics and a fellow of both the American College of Dentists and the International College of Dentists.

Texas State Board of Dental Examiners Considers Rule Changes

The Texas State Board of Dental Examiners (TSBDE) recently published proposed changes to the Business Promotion Rules (§§108.50 - 108.61) in the May 18, 2012, issue of the Texas Register. The TDA has been closely involved in the rule making process since its inception.

The TDA, along with numerous stakeholders, commented on the proposed rule changes resulting in a July 13, 2012, meeting of the TSBDE’s Advertising Ad-Hoc Committee to discuss stakeholder comments. Dr Rudolfo G. Ramos Jr chairs the committee. Members include Dr William R. Birdwell, Dr D. Bradley Dean, Dr Tamela L. Gough, and Mr Arthur Troilo III.

The proposed rules are modeled in part on the American Association of Dental Board’s (AADB) Guidelines on Advertising. In addition to the generally accepted guidelines promulgated by the AADB, the rules address the communication of specialty practices and dental credentials to the public, the inclusion of professional awards and honors in advertisements, and the requirements of website publications.

Based on stakeholder comments, the Advertising Ad-Hoc Committee made revisions to the proposed rules that will be presented for approval during the TSBDE’s August 10, 2012, board meeting. Once approved by the TSBDE and published for adoption in the Texas Register, future issues of the “TSBDE Rule Update” will be devoted to a section by- -section analysis of the new rules as they will significantly impact dental advertising and business promotion in Texas.
