By signing this agreement, I agree to be present and have my products on display at the “Ladies' Night Out” Event to be held at the Murphysboro Event Center, 1329 Walnut Street, as follows: Thursday, July 29, 2010 from 6:00 p.m. to 9:00 p.m. I further agree that my display will be completely set-up and ready for viewing no later than 5:45 p.m. on Thursday, July 29, 2010. Dismantling of display will NOT be allowed until after 9:00 p.m. Set-up may begin at 4:00 p.m. Booth space (1) 6' table & (2) chairs (approximately 10' x 10' in size) You will NOT be allowed to exceed your space limits. If you require additional space, please contact FOM for additional space pricing. Fee for booth space is $25.00 (IF POSTMARKED BY JAN 30, 2010) Fee after this date is $30. Final postmark deadline is March 1, 2010. Spaces will be assigned as entries are received. Any contracts received after the FINAL postmark date may be accepted if space allows.
