ITP (Idiopathic Thrombocytopenia Purpura)
DefinisiSuatu kelainan autoimun di mana IgG autoantibodi yang
terbentuk mengadakan pengikatan dengan trombosit.Mekanisme
perusakan trombosit terjadi dalam limpa di mana makrofag limpa
dengan reseptor Fc terikat pada antibodi yang menyelimuti
trombosit.
Gejala klinisBiasanya terjadi pada masa kanak-kanak yang
terinfeksi dengan virus dan sembuh spontan. Bentuk dewasa
menunjukkan suatu penyakit yang berjalan kronis dan jarang akibat
infeksi virusInsidensi terbanyak pada:usia 20 50 tahun; wanita :
pria = 2 : 1Tidak didapatkan gejala sistemik dan biasanya tanpa
febrisGejala klinisPada pemeriksaan fisik didapatkan perdarahan
pada kulit (purpura, ptechiae), menoragia, perdarahan mukosa
(epistaksis, oral bleeding, hemorrhagic bullae pada mulut). Tidak
didapatkan kelainan fisik yang lain kecuali yang berhubungan dengan
perdarahan, seperti didapatkannya darah dalam urine atau berak.Bila
penderita ITP diperiksa secara fisik, maka biasanya keadaan umumnya
baik, tidak didapatkan demam, dan ada pembesaran limpa.

Bleeding manifestations were defined as: 0: no bleeding; 1: minimal
bleeding after trauma; 2: spontaneous but self-limited bleeding; 3:
spontaneous bleeding requiring special attention, such as nasal
packs for epistaxis; and 4: severe, life-threatening bleeding.
Spontaneous bleeding begins at platelet counts below 40,000/L,
while severe (grades 3 and 4) spontaneous bleeding was primarily

Types of ITPAcute ITP is a temporary or short-term type of ITP.
It generally lasts less than 6 months. It is the most common type
of ITP and occurs mainly in children, both boys and girls, usually
between 2 and 4 years of age. It often occurs after a child has an
infection or is sick with a virus (e.g. Rubeola, Rubella, Varicella
zoozter, Epstein Barr virus, or other kinds of viral-induced
respiratory diseases).Chronic ITP is long-lasting, usually causing
symptoms for 6 months or longer. It mostly affects adults, but
sometimes teenagers or children may develop it.Model of platelet

T-cell tolerance, which is a prerequisite for autoimmune disease to
occur. (a) As part of a normal physiological process, platelets are

histocompatibility complex (MHC) class II to T helper (Th) cells,
which become activated and secrete the Th1 cytokines interleukin
(IL)-2 and interferon (IFN)-g. (c) Th1 cytokines activate and drive
autoreactive B cells to differentiate into autoantibody-producing
cells.

thrombocytopenic purpura (ITP). (a) Antigen-presenting cells (APCs)
present platelet antigen in association with major
histocompatibility complex (MHC) class II to T helper (Th) cells,
which become activated and secrete the Th1 cytokines interleukin
(IL)-2 and interferon (IFN)-g. (b) APCs also present platelet
antigen associated with MHC class I to pre-cytotoxic T lymphocytes
(pre-CTLs). Pre-CTLs differentiate into active CTLs. (c) In the
case of chronic ITP in the active phase, CTLs release toxic
contents, such as granzyme B and perforin, and platelet lysis
occurs. (d) In the case of chronic ITP in remission, upregulation
of killer-cell immunoglobulin-like receptors (KIRs) on the surface
of CTLs could inhibit cell-mediated cytotoxicity in chronic ITP.
KIRs recognise MHC class I molecules, hinder presentation of
antigen to CTLs and thereby promote termination of cytolytic
activity by CTLs. This might explain the absence of platelet lysis
in ITP patients in remission.
LaboratoriumTrombositopenia merupakan tanda utama ITP dan dapat
menunjukkan hasil di bawah 10.000/mm3 Anemia ringan disebabkan oleh
perdarahan atau proses hemolitikTrombosit dapat menunjukkan
megatrombosit ringan yang merupakan trombosit muda yang diproduksi
untuk mengkompensasi kerusakan trombosit secara cepat10% penderita
ITP disertai autoimmune hemolytic anemia (AIHA) disebut sindroma
EVANSSumsum tulang normal, hanya didapatkan peningkatan
megakariosit. Tes koagulasi relatif normal.

TerapiBeberapa kasus dapat mengalami remisi spontanPengobatan
diawali dengan Prednison 1 2 mg/kgBB/hr. Perdarahan biasanya
berkurang walaupun trombosit belum menunjukkan peningkatan. Pada
80% penderita menunjukkan perbaikan klinis/lab. Dosis terkecil yang
mempertahanan jumlah trombosit perlu ditentukan secara
bertahapSplenektomi adalah terapi definitif; kasus dewasa biasanya
berakhir dengan terapi ini terutama yang tidak memperlihatkan
respons terhadap pemberian PrednisonTerapiPemberian imunoglobulin
dosis tinggi 1 g/kgBB selama 1 2 hari sangat efektif dalam
peningkatan trombosit. Terapi ini dirasakan sangat mahal, namun
hanya berlangsung 1 -2 mingguObat-obatan imunosupresif terutama
golongan Rituximab menunjukkan hasil yang baik terutama pada
kasus-kasus refrakterTerapiAutologous stem cell transplantation dan
penelitian terhadap molekul yang menyerupai trombopoetin memberikan
masa depan yang cerah terhadap terapi ITPTransfusi trombosit jarang
dilakukan mengingat umur trombosit eksogen hanya beberapa jam
kecuali pada kasus trombositopenia parah disertai perdarahan

PrognosisPada umumnya pemberian Prednison memberikan remisi yang
baikSplenektomi merupakan terapi definitifTrombosit < 5000/mm3
dapat memicu perdarahan otakJarang didapatkan episode perdarahan
yang fatal.
