VERTICAL EYE MOVEMENT and RAPID AUTOMATIZED NAMING (RAN) TEST - AGE: 6 to 13 years AT RISK ___ - Tests vertical eye tracking and ability to quickly name numbers aloud - Predicts single-word reading skills and spelling (independent of phonological awareness) - Read the two vertical rows of number from top to bottom aloud - Time the number of seconds to complete and compare to Age-norms - Higher than Age-norms indicates a risk for vision and learning problems 5. HORIZONTAL EYE MOVEMENT TEST AT RISK ___ - AGE: 6 to 13 years - Tests horizontal eye tracking and probes quality of reading eye movements - Read aloud the horizontal rows of numbers like reading a paragraph of words - Time the number of seconds to complete and compare to Age-norms - Higher than Age-norms indicates a risk for vision and learning problems 6. FOUNDATIONS FOR LEARNING CENTRE QUESTIONNAIRE (FLCQ) - AGE: 8 and up (or younger with parents help) AT RISK ___ - Asks patient to report on behavioral occurrence of vision and learning problems - Select the column that best represents the occurrence of each symptom - Total-score higher than 11 indicates a risk for vision and learning problems Please note that these tests are assessing risks of vision and learning problems for the grades and ages listed. Further tests at our office are available for older children. The test results are NOT diagnostic; they only indicate risk to determine whether further testing should be considered.

3. Find Test Card (2nd page)

4. On Test Card, draw a line through the first “O” you see, and then without lifting the pencil up, continue drawing lines through the “O”s you see on the paper, for one minute.
