The following documentation is enclosed for Dr. King to review and sign: the dose calibrator accuracy test, linearity test, sealed source inventory, leak test and this letter.
.The sealed source inventory was verified and leak tested. The results of the leak tests were acceptable.
Please note that your NRC license expires on May 31 , 201 1. The NRC must receive the license renewal 30 days prior to the expiration date. The NRC renewal application and the required documentation for the transfer of ownership were emailed to Autumn on January 11 , 201 1.
All available Nuclear medicine personnel exposure reports were reviewed. The following Nuclear Medicine personnel exposures were above ALARA level I limits of 125 mremlquarter for the whole body, 1250 mredquarter for the extremity and skin, and 375 mrem for the lens of eye.

There was one spill of radioactive material recorded since my last visit. Nudear Medicine personnel appropriately followed spill pmcedures and decontaminated the area with no spread of contamination. All required papennrork has been filled out and filed appropriately.
No accidental administrations of radioactive material were recorded since my last visit.
During my visit, I tested the CAPRAC well counter and found it to be operating properly.

8. At the time of my visit, I was able to review the camera QC uniformity and resolution images. Review of the uniformity images and resolution images from Camera A and B reveal no significant non-uniformities and the resolution is within the range of expected values.
