This project shall consist of furnishing and installation of positive air system, heating and

B. EQUIPMENT INSTALLATION
• Installation of all air handling equipment
• Foundation shall be provided and shall conform to the recommendation of the manufacturers of the equipment. Equipment shall be levelled on foundation. All spaces between equipment bases and concrete shall be filled with cement mortar.
• For skid type equipment, anchor the equipment to the concrete slab (10 ins. Thick) using log screw. Clearance around equipment base should not be less than 12 inches

1.0 PROJECT SUMMARY:
1.1 This project requires the following activity: CONSTRUCTION OF LEAD BLOOD FACILITY (P1)
1.2 The work requires the furnishing of all labor, supervision, administration and management, supply of equipment, tools and services necessary for the satisfactory and expeditious completion of the Project.
2.0 PROJECT DESCRIPTION:
2.1 The work shall be carried out strictly in accordance with the requirement of the Work Orders attached as required by the Proponent.
2.2 The Project shall be carried out in all respect to the requirements of the Contract Documents and applicable Philippine Standards, Code and Specifications.
3.0 GENERAL SCOPE OF WORK:
3.1 Verification of Dimension: Authorized Contractor’s personnel shall visit the project location of the above mentioned Work Orders to familiarize themselves with all details of the work and working conditions and to verify all the stated Project requirements with the Proponent. Contractor shall advise DAVAO REGIONAL MEDICAL CENTER project representative of any perceived discrepancy before performing any work
3.2 Work Order and Work Description
3.2. A. CONSTRUCTION OF LEAD BLOOD FACILITY (P1)
1. Staking and Lay-out This activity includes the following:
• Installation of batter boards for lay-out and perimeter fence.
• Installation of Temporary facility.
2. Demolition and Dismantling Works The Contractor shall demolish, dismantle and/or remove any miscellaneous existing structures, buildings or parts thereof which occupy or obstruct the Permanent Works, all as shown on the Drawings. Unsuitable materials from demolishing and dismantling remain the property of the Contractor and shall be disposed of in a manner and at a location to the approval of the Engineer. Where partial demolition of structures is required, the Contractor shall take the utmost care not to damage any part of the structure which is to remain in place. If any such damage may occur the Contractor shall repair and make good the damage at his own expense.
• Demolition, hauling and clearing of area affected by the lay-out.
• Demolition of concrete pavements and structures which are affected by the construction works.
• Dismantling of existing (road) catwalk.
• Dismantling and turn-over of the dismantled materials to the agency.
3. Earthworks
This activity includes the following:
• Earthwork consists of excavation, back filling and disposal of surplus material. Work of this section includes all measures and materials required to complete the design supply, support, use, construction, removal of earth work.
• Boulders of 6” to 8” dia. should be used as column foundation beddings.

4. Formworks and Scaffoldings This item consist of:
• Installation of Forms and Bracings for columns, beams, parapets and slabs.
5. Concrete Works/ Structural Steel Works
• Concrete work shall consist of mixing, conveying, placing of concrete and reinforcement Work, (it is inclusive of all measures and materials required removal of concrete forms and reinforcement).
• Construction of concrete columns, footings, tie beams and zoccalos with a minimum of 3,000 psi concrete strength in 28 days;
• Reinforcing bar should be deformed bars for all reinforced concrete work,use Grade 40 ( 275 MPa) for 16mm dia reinforcing bars ; use Grade 60 (415 MPa) for 20mm & 25mm dia. Reinforcing bars in accordance to (ASTM A 184 and A 185). Present samples of all steel reinforcement that the contractor proposes to use in the Works together with manufacture's certificate stating clearly for each sample, the place of manufacture expected date and size of deliveries to site, and all relevant details as regards composition manufacture, strength and other qualities of the steel shall be submitted to the Engineer for written approval
• Test of Concrete -Work cylinder test shall be made on concrete
sampled during the works. Samples shall be taken for each new
grade concrete, from each 100 m3
of concrete when the same grade is being used continuously, except for lean concrete and other non load bearing concrete. The number of specimens taken shall not less than 3 for each compressive strength test. All tests shall be performed in accordance with ASTM C. 39, B.S, 1881 and shall be carried out in an authorized laboratory at the contractor's own expense. If the results of the 28 days test are unsatisfactory, all concrete work shall be stopped at contractor's expense and shall not proceed further without the written permission of the Engineer. Should the test prove that the concrete is not satisfactory or the Engineer Judge any section to be defective the condemned concrete shall be cut out, removed and replaced by the contractor;
• Concreting of suspended slabs and on-fill flooring, second floor beams, column beams, parapets and roof beams.
• Construction of concrete stairs.
• Construction of concrete canopy
6. Masonry Works
• This item consists of the construction of 150mm thk. CHB for perimeter and zocalo walls, and 100mm thk. CHB for interior walls, T&B/CR walls and concrete counter stands smooth plastered finished.
• All Concrete Hollow Blocks shall be machine-made and all CHB walls shall be cement plastered (12mm thk.) and smooth finished and are reinforced with 10mm dia. deformed bars with spacing of 600mm o.c. for vertical reinforcement and every 3 layers for horizontal reinforcement.
• Mortar filler shall be Class “B” mixture while Cement Plastering shall be Class “A” mixture;
• Construction of new toilets;
• Construction of Septic Tank using 150mm CHB (1 unit- 2m x 4m x 2m depth).

7. Tiling Works
This activity includes the following:
• Supply and installation of 300mm x 300mm ceramic floor and wall tiles for toilets.
• Supply and installation of 600mm x 600mm granite tiles on main floorings (ground & second floor).
• Supply and installation of 400mm x 400mm ceramic tactiles on main entrance (ground floor), near stairs and elevator (second floor). (Ceramic Tactiles)
• Supply and installation of 600mm x 600mm granite tiles on stairs with bronze stair nosing at edge.
• Supply and installation of Granite Slab, 3/4" x 2' x 8' at concrete counters and mop wash.
• Sample of tiles should be presented to the agency’s representative for approval prior to installation.

• Supply and installation of electrical materials.

General Scope of Work: (MECHANICAL WORKS – HVAC SYSTEM)
This project shall consist of furnishing and installation of positive air system, heating and ventilating and air-conditioning system, and, 100% air flow system. This includes necessary electrical connections, pipes and condensate drains and all other necessary accessories and auxiliaries, ready for service in accordance with the plans and specifications. Installation of equipment and its accessories and auxiliaries shall conform to established standards. The project includes testing of all equipment and all other related activity needed for commissioning. Detailed Scope of Work:
• Installation of 1 inch thick fibre glass insulating material to all tubing/pipes
• Insulation shall be pre-formed fibre glass or its equivalent. The insulating
materials shall be covered with 4 inches x ½ inch thick poly ethylene film,
which shall be overlapped not less than 2 inches. Pipe insulation shall be
adequately protected at point of support by means of suitable metal shield to
avoid damage from compression. Insulated pipes, valves and fittings located
outdoors shall be provided with metal jacket
C. ELECTRICAL WORKS
• Provision of electrical supply cables necessary to all equipment
• Power supply shall be provided by the contractor at the pull box installed
inside the machine room and shall furnish and install the main circuit breaker
and starter with suitable ratings and capacities, conduits, wiring, fittings,
devices and all other equipment and electrical connections needed to
complete the electrical installation of the system. All electrical works shall
comply with the latest edition of the electrical code, with the applicable
ordinance of the local government and all the rules and requirements of the
local power company

unless specified in the drawing

G. PIPE PROTECTION
• Buried pipes shall be provided with bituminous material wrapped around its
circumferential
• Exposed pipes shall be applied with epoxy primer AB for two coatings
• Exposed pipes shall be applied with fire red paint for two coatings after
application of epoxy primer
H. CONSTRUCTION REQUIREMENT
• The fire fighting system shall be entirely semi-automatic in operation by just
assuring that all main gate valves are permanently open in nature (Main Gate
Valves means valves at supply and fire department sides)and shall be
connected to elevated tank
• The system shall be in conformance with NFPA standards
• The system shall have end drain for line removal of dirt and flow testing
• All materials shall be inspected upon delivery and shall be tested after
installation. Piping shall not be buried, concealed, or insulated until it has
been inspected, tested and approved. Walls, floors and other parts of the
building and equipment damaged by the contractor in the prosecution of the
work shall be replaced as shown in the plans
I. CONTRACTOR REQUIREMENT
• The contractor shall have ample experience on fire fighting system specially
on stand pipe system
J. OPERATING TEST
• The piping system shall be hydro tested 1.5 times the operating pressure or
as per recommended in the NFPA standards. Test shall be within 3 hours
without dropping of pressure.
• All leak/s or suspected leak/s shall be eliminated/repaired before acceptance
K. METHOD OF MEASUREMENT
• The work under this item shall be measured either by set, piece, length,
square foot actually placed and installed as shown in the plans
• FHC shall be measured by set; valves by piece, pipe/tubing by length
L. MISCELLANEOUS
• The owner shall be provided with three (3) bound copies “ AS BUILT “
diagram, shop drawings, parts lists, serial number and inventory of equipment
including manufacturers and maintenance manuals
