1. Eligibility

Love Your Smile (the "Contest"J is open to legal residents of Hamilton, Boone, Madison,Hendricks, Marion, Hancock, Morgan, Johnson, and Shelby counties in Central Indiana and atleast eighteen (18) years old at the time of entry. Employees ofTeam Green Dentistry and

other companies associated with the promotion ofthe Contest, and their respective parents,

subsidiaries, affiliates and advertising and promotion agencies as well as the immediatefamily [spouse, parents, sib]ings, and childrenJ and household members ofeach such

employee are not eligible. The Contest is subiect to federal, state, and Iocal laws and

regulations.

The contest is sponsored by Team Green Dentistry, located at 11559 Cumberland Rd., Ste.

100, Fishers, IN 46037.

Agreement to Official Rules

Participation in the Contest constitutes entrant's full and unconditional agreement and

acceptance ofthese Official Rules and the decisions of the Sponsor, which are final and

binding- Winning a prize is contingent upon being compliant with these Official Rules and

fulfilling all other requirements set forth herein.

Contest Period

The contest begins on lanuary 31, 2020 at 6:00pm Eastern and ends on February LO,zOzO

at 11:0Opm Eastern (the "Contest Period"J. Entries tlat are before or after the ContestPeriod will be disqualified. Submissions will be accepted for the duration ofthe Contest

Online: This method of entry will be available by visiting our websiteteamgreendentistry.com/loveyoursmile and following the directions provided to fill out theentry informafion, and submit.

Limit one [1) entry per person, per email address, and per household for the duration oftheContest Period, regardless of method of entry, Entries received from any person, e-mailaddress, or household in excess ofthe stated limitation wilI be void. AII entries become theproperty of Sponsor and will not be acknowledged or returned.

Prize Drawing

On or aboug February 10,2020Team Green Dentistry will select finalists, based on a panelof four iudges revievying and selecting all eligible essay and picture entries ofthe Love YourSmile Contest. Finalists will schedule a date and time on or about February 11th, 2020through February 23, 2020 to consult and take x-rays with one ofthe dentist and orassistants. On or about, February 25, 2020, the Sponsor will select a winner in a randomdrawing from among the finalists participating. The odds of being selected depend on thenumber of entries received. The sponsor will attempt to notiS/ the potential winner viatelephone or email on or about February 25, 2020. Ifthe potential winner cannot becontacted within five (5) days after the date ofthe first attempt to contact him/her, theSponsor may select an alternate potential winner in his/her place at random from theremaining non-winning, eligible entries.

7. Winner Notification

The potential winners will be notified by email, mail, or phone. Each potential Grand andFirst Prize winner [parent/legal guardian if a minor in his/her state of residence] will berequired to complete, electronically sign and submit a Declaration of Compliance within five(5) days of the date notice or a$empted notice is sent, in order to claim his/her prize. Ifapotential winner cannot be contacted, or fails to submit the Declaration of Compliancewithin the required time period (ifapplicable), or prize is returned as undeliverable,potential winner forfeits prize. Ifthe potential winner is at least 18 but still considered a

minor in his/her jurisdiction of residence, Sponsor reserves the right to award the prize inthe name ofhis/her parent or legal guardian, who will be required to sign the Declaration ofCompliance on the potential winners behalfand fulfill any other requirements imposed onwinner set forth herein. Potenrial winners must continue to comply with all terms andconditions ofthese Official Rules, and winning is contingent upon fulfilling all requirements.In the event that a potential winner is disqualified for any reason, Sponsor will award the

applicable prize to an alternate winner by random drawing from among all remainingeligible entries. Only three [3) alternate drawing will be held after which the prize willremain un-awarded. Prizes will be fulfilled approximately 8-10 weeks after the conclusionofthe Contest

Grand Prize (1J - All expenses paid for any Cosmetic Dental procedure per{ormed at TeamGreen Dentistry.

General Conditions

In the event that t}le operation, security, or administration of t}le Contest is impaired in anyway for any reasorl including but not limited to fraud, virus, bug worm, unauthorizedhuman intervention or other technical problem, or in the event the Contest is unable to runas planned for any other reason, as determined by Sponsor in its sole discretion, theSponsor may, in its sole discretion, either [a) suspend the Contest to address theimpairment and then resume the contest in a manner that best conforms to the spirit ofthese Official Rules or (bl terminate the Contest and, in the event oftermination, award theprize at random from among the eligible, non-suspect entries received up to the time oftheimpairmenL The Sponsor reserved the right in its sole discretion to disqualifu anyindividual it finds to be tampering with the entry process or the operation ofthe Contest orto be acting in violation ofthe Official Rules or in an unsportsmanlike or disruptive matter.Any attempt by any person to damage the website or undermine the legitimate operation ofthe Contest may be a violation of criminal and civil law, and, should such an attempt bemade, the Sponsor reserves the right to seek damages [including attorney's feesJ and anyother remedies for any such person to the fullest extent permitted by law. Failure by theSponsor to enforce any provision to these Official Rules shall not constitute a waiver ofthatprovision.

10. Release aad Limitation of Liability

By psnicipating in the Contest, enaants a8ree to release and hold harmless the Sponsor, and each

oftheir respective parents, subsidiaries, afliliates, advertising and promotion agencies. other

companies associated with the Contest, and each oftheir respective offlcers, directors, employees,
