General Points
Any changes in your program need to be reflected in the online application at the time of the change. It is the responsibility of the CN Administrator to ensure these changes take place.
Any changes made in the Sponsor or Site Applications after approval may change the application status to unapproved. If this occurs please be sure to contact NCDPI to get it re-approved by sending an email.
We strongly encourage the use of all NCDPI prototype documents which can be found in the system Download Forms section.

General Points
If you select “No” for any question regarding a prototype form it will produce a Checklist item and you must submit the document to the regional consultant for approval prior to submitting it to NCDPI with your packet.
Your 2009-10 Application/Agreement must be submitted and approved by July 1. 2009-10 Child Nutrition claims for reimbursement will start the effective date of approval.
There are 3 sections in the application process and you must complete each section before you can get to the next. Those sections are:
