In December of 1999, Stephen F. Lynch (then a state senator) andBoston Bruins President Harry Sinden joined Mass Hockey PresidentDan Esdale at South Boston’s Murphy Rink to announce the filing of abill in the Massachusetts legislature to create a new license plate tobenefit Mass Youth Hockey. Following his election to the United StatesCongress, Steve Lynch continued to push for the bill’s passage.Finally in 2002, Congressman Lynch, Harry Sinden, along with SenatorGuy Glodis, Senator Jack Hart, and Rep. Marty Walsh prevailed and theBoston Bruins/Invest in Youth Hockey license plate was on the road.
In the past seasons the money received from the plate sales have beenspent on Mass hockey’s Grassroots programs. The funds were distrib-uted to the Mike Cheever Grow the Game, the Border Patrol programfor our youngest players, the parental and spectator conduct code andthe youth amputee program.
Remember that these plates are for Mass Hockey. The benefits are forour programs and our children. If you and your friends and family don’thave the plates: please get them. if you already have them, pleaserenew and you will ensure continuing revenue for decades to come.
Just bring your old plates with your checkbook or credit card to anyMass Registry of Motor Vehicles office and leave with your new Bruinsplate and the knowledge that you have helped good people do goodthings.

Dear Massachusetts Hockey Members & Friends,
The past season has been very instrumental in moving Massachusetts Hockeyinto the future. We continued to implement the American Development Model.The 2nd Annual Red, White & Blue Cup was fully booked, and we will move toimplement one step closer to all cross ice games for the younger ages. Wespent the year self-evaluating the organization and how best to serve our mem-bers. The most exciting news of last year is the hiring of our new ExecutiveDirector, Kevin Kavanagh.Kevin Kavanagh is returning to his roots in Massachusetts. His experience withUSA Hockey in Membership Development growing the game, marketing andfundraising experience with other hockey organizations will prove invaluable toour organization. Some of Kevin’s concentrations will be on growing the game,improving communication to our members, and fine tuning all of our state tour-naments.As we begin a new season, I look forward to all these exciting happenings ofMassachusetts Hockey. We will continue to self evaluate, grow the game andprovide benefits to our members.

As your newly-hired Executive Director, I want to open by thanking you for the opportunityto serve Massachusetts Hockey and be a part of one of the largest and most importantpieces of the USA Hockey family. Mass Hockey has a long and storied tradition as aleader in youth hockey and with the current leadership and programs that are in place,that tradition should continue for many years to come.
It is my goal to provide our membership with a better understanding of our programs andbenefits and celebrate all that is good in youth hockey. In the coming season, I hope toupdate our current digital platform to include social media as well as monthly communica-tions that will keep our membership informed. Also, I hope to grow our relationship withseveral sponsors that will allow us to continue to grow the game and support programsthat benefit Mass Hockey. I will work to serve the membership of Mass Hockey in a waythat you can be proud of.
I come to Mass Hockey with a very hockey-oriented background, having worked forAmerican Hockey League and ECHL teams. But more importantly, I was with USA Hockeyin Colorado Springs for the past 3 ½ years working in the Membership Developmentdepartment to help grow the game. During this time, I have come to have a great under-standing of the amazing levels of volunteerism that exist in our sport. Under the leader-ship of Keri Allan and the executive board, Massachusetts has some of the best leaders inyouth hockey anywhere in the world.
I hope that you all enjoy the season that is upon us and take a moment every now andagain throughout the year to enjoy our wonderful sport.

Richard “Duke” Kumpel1930-2012
On July 22, Massachusetts Hockey lost one of its founding fathers. Richard “Duke”Kumpel . He joined Mass Hockey in 1973 (Mass Hockey’ s inaugural season) as aDistrict Director for District 10. Just one year later, he was elected as a Vice Presidentand served on the Executive Board in numerous positions since that time. Many willremember Duke for his 20+ years as the State Tournament Director. Most recently, Dukeserved proudly as Director at Large for Mass Hockey and was a fixture at meetings,spreading his joy and remembrance of a good hockey story.
Duke was born December 25, 1930 and was raised in Melrose with his sister Flo andbrother George. He graduated from Melrose High School in 1948 where he was a sprint-er on the Track and Field team . After graduating he enlisted in the Navy and served inthe SeaBees for two years. He met his bride to be, Martha who was a nurse in training atMelrose-Wakefield Hospital and they were married in 1952. They had 5 children William,Linda, Kristen, Judith and Mark. He was known as Papa Duke to his 5 grandchildren andfirst great-granddaughter.
Duke was one of the smartest men we had in Massachusetts Hockey. He used soundreasoning and judgment, never jumping to a conclusion. He always allowed the personto speak and explain their side, then would patiently and clearly explain his side and whyit was the right way to go. He never raised his voice, just politely and calmly spoke. Heonce said to his successor “We can never make a decision that hurts the kids. So whensomeone finds a new way to push the rules and we can’t stop them without hurting thekids, the next year I will make a new rule so they can’t do that again”.
In 2001 Duke was inducted into the Massachusetts Hockey Hall of Fame. In 2011 theMassachusetts Hockey Special Award was renamed the Richard “Duke” Kumpel Awardand he was its first recipient.
Duke –ismsHe greeted everyone with “How you doing young fellow”
He would begin every hockey meeting with “Ok…For those of you who do not know meI’m GOD “
In response to a tournament going off with no complications “I love you when you talkdirty to me”
One of his favorite lines was “I hate kids”,just the opposite was true.
And then “I’ll have a rum and Diet Coke”

ARTICLE I - NAMEThe name of the organization shall be Massachusetts Hockey, Inc.
ARTICLE II - PURPOSE & OBJECTIVESMassachusetts Hockey is a not for profit, tax exempt Massachusetts corporation
established pursuant to the provisions of G.L. chapter 180 and section 501 (c) (3) of theInternal Revenue Code, respectively, whose purposes and objectives are:A. to serve as the affiliate association of USA Hockey, Inc. pursuant to the terms of its
Affiliate Agreement with USA Hockey, Inc. in the Commonwealth of Massachusetts, and as such affiliate association, to be the governing body for the sport of hockey in Massachusetts;
B. to foster, advance, develop and encourage participation in the sport of hockey in Massachusetts;
C. to develop and encourage sportsmanship between all players for the betterment of their physical and social well-being;
D. to develop and improve the standards of the sport;E. to educate and train players, coaches, referees, managers, administrators, and
parents;F. to foster association with other USA Hockey Affiliates and International Ice Hockey
Federations;G. to conduct tournaments leading to the selection of teams for District, State, and
National Tournaments, and to organize and select teams for other tournaments;H. to register players, coaches, referees, managers, teams and leagues, and to certify
associations in Massachusetts;I. to be guided by the USA Hockey core values of sportsmanship, respect for the
individual, integrity, pursuit of excellence at the individual, team and organizational levels, enjoyment, loyalty and teamwork;
J. to do any and all acts necessary and desirable to further the foregoing purposes and objectives.
K. Massachusetts Hockey, an affiliate association of USA Hockey, Inc. shall abide by and act in accord with the Articles of Incorporation, By-laws, Rules and Regulations, Playing Rules and decisions of the Board of Directors of USA Hockey and such documents and/or decisions shall take precedence over and supersede all similargoverning documents and/or decisions of Massachusetts Hockey. Further Massachusetts Hockey (I) shall assist USA Hockey in the administration and enforcement of the provisions of the Articles of Incorporation, By-laws, Rules and Regulations, Playing Rules and decisions of the Board of Directors of USA Hockey, within and upon its members and/or within its jurisdiction and (II) agrees to be guid-ed by the core values of USA Hockey.
ARTICLE III - MEMBERSHIPMembership in Massachusetts Hockey shall be open to all persons and associations
approved by Massachusetts Hockey pursuant to the provisions of these Bylaw’s and shallinclude, but not necessarily be limited to, the following:A. associations certified in accordance with Massachusetts Hockey Bylaws and Rules
and Regulations;B. leagues recognized by Massachusetts Hockey to coordinate team play;C. teams registered through a Massachusetts association and in accordance with
Massachusetts Hockey and USA Hockey requirements;D. individual membership shall include but not be limited to those individuals who have
registered with USA Hockey; the immediate families and guardians of registeredplayers; directors, officers, employees, and volunteers of associations, districts, leagues and teams; and all members of Massachusetts Hockey Board of Directors;

E. honorary members elected by the Board of Directors of Massachusetts Hockey and.F. members shall be governed by the provisions of Massachusetts Hockey and/or USA
Hockey Bylaws, Rules and Regulations, Policies and Procedures, Codes of Conduct, Codes of Ethics and USA Hockey Playing Rules and shall enjoy the benefits and privileges herein as long as they shall be in good standing. Such members shall not have direct voting rights for the officers, directors, registrars and all other posi-tions in Massachusetts Hockey as the election process for the same is set out herein.
ARTICLE IV- BOARD OF DIRECTORSThe Board of Directors shall have all the powers necessary or desirable to carry out
the management of the affairs of Massachusetts Hockey. The powers and duties of theBoard of Directors shall include, but not be limited to, the power to promulgate rules andregulations, adopt policies and procedures, annually select an Auditor to perform aCertified Audit of Massachusetts Hockey accounts at the end of the each fiscal year, andestablish fees, dues and assessments, necessary for the management of the affairs ofMassachusetts Hockey.
The Board of Directors shall be comprised of members of the Executive Board,District/Divisionl Directors and Directors at Large. The majority of the Board of Directorsshall at all times be comprised of the District/Division Directors, elected Officers ofMassachusetts Hockey and non voting USA Hockey appointed Registrars.
One-third of the Directors then in office shall constitute a quorum at meetings of theBoard of Directors. Each member shall have only one vote, regardless of the number ofpositions held. No proxy votes shall be allowed. Alternate Directors may vote in theabsence of a Director from a District/Division if registered, indicating the Director beingreplaced with the Secretary prior to the meeting being called to order by the presidingofficer. Once a Director is replaced he/she is ineligible to vote during the current sessionof the meeting. All actions of the Board shall be subject to majority of the votes castunless otherwise specified in these bylaws. The presiding officer shall vote only to breaka tie. The President may surrender the Chair to vote from the floor.
There shall be at least FOUR (4) meetings of the Board of Directors annually.Special meetings of the Board of Directors may be called by the President, or upon
written request of six (6) or more Directors from separate Districts/Divisions. Specialmeetings shall be held within thirty days of receipt of such a request with a minimum ofseven (7) days notice being given to the members of the Board of Directors.
Any member of the Board of Directors may be removed for cause by vote of twothirds majority of the Board. For purposes of this provision, cause shall be deemed to bethe material non compliance by a member with the By-Laws, Rules and Regulationsand/or Policies and Procedures of Massachusetts Hockey. Material non compliance mayinclude, but not be limited to, failure to perform the duties of an elected or appointedoffice by a member, failure to attend meetings of the Board of Directors, failure to complywith or act in a manner consistent with the organization's By-Laws, Rules and Regulationsand/or Policies and Procedures, USA Hockey By-laws, Rules and Regulations, and/orPolicies and Procedures, Codes of Conduct, Codes of Ethics and USA Hockey playing rulesor any other misfeasance by a member not appropriate for such member. Any membersubject to removal for cause must be given written notice of the proposed removal by thePresident or Secretary no later than ten (10) days prior to the meeting of the Board ofDirectors at which the action shall be considered and the member shall be afforded anopportunity to be heard at the said meeting prior to any vote being taken. The Presidentor Secretary shall only issue such notice if ten (10) members of the Board of Directorshave made a written request of him to take such action. Any action taken by the Boardof Directors to remove a member shall be final with no appeal available under any of theorganization's rules of procedure.
A meeting of the Board of Directors may be held by any means of electronic or othercommunications through which all participating Directors can hear each other if and whenthe President determines the need for such a meeting or if the Board of Directors votesto conduct such a meeting.
Notwithstanding this provision, such a meeting shall not be a replacement or substi-tute meeting for any Board meetings required to be held annually or for any special meet-ing that may be called under the provisions of this By-law unless those calling such a

meeting agree to this procedure. All the provisions for notice, quorum and voting applic-able to board meetings shall apply to such meeting provided herein.
ARTICLE V - EXECUTIVE BOARDAn Executive Board consisting of the President, the Vice-Presidents, Treasurer, Secretary,Referee Coordinator, Chair of the Rules Committee, the Chair of the DisciplineCommittee, Massachusetts Hockey Director of Coaching Education, and the GeneralCounsel, and two members of the Board of Directors who are elected annually by theBoard of Directors for a one year term. shall be responsible for carrying out the direc-tives of the Board of Directors and running the day to day activites of the organization.The USA Hockey District Registrar and the USA Hockey Coach in Chief may participate inthe Executive Board meeting but may not vote. The Executive Board shall be soleyresponsible for all hiring decisions and the execution of employment contracts. TheExecutive Board shall meet as often as its members deem necessary without therequirement of any notice to the Board of Directors or the membership and it may con-duct its business as informally as it deems necessary. Each member of the ExecutiveBoard shall have one vote. All actions of the Executive Board shall be subject to majori-ty vote. Actions taken by the Executive Board shall be reported to the Board of Directorat its next ensuing meeting.A meeting of the Executive Board may be held by any means of electronic or othercommunication through which all the participants can hear each other.
ARTICLE VI- DUTIES OF THE MEMBERS OF THE EXECUTIVE BOARDElected Officers. The elected officers shall be the President, Vice Presidents, Treasurer, and Secretary.
The President shall:A. preside over all meetings of the Executive Board and the Board of Directors;B. generally supervise all the activities of Massachusetts Hockey, and ensure that all
rules and regulations and stated policies and procedures of the organization are observed;
C. have the power to rule on questions not provided for in the Bylaws until the next regular or special meeting of the Board of Directors;
D. represent Massachusetts Hockey and be its spokesperson at all public and private forums and places;
E. in the absence of the Treasurer, have the right to sign checks;F. appoint the non-elected members of the Executive Board;G. establish committees as needed from time to time;H. appoint the Chairs and members of all committees;I. be an ex officio member of all committees;J. sign all contracts for Massachusetts Hockey that are in excess of $10,000 that have
been approved by the Finance Committee except Ice Rental contracts.There shall be 5 Vice Presidents. They shall be elected to the positions of First Vice
President, Second Vice President, Third Vice President, Fourth Vice President and Fifth Vice President.The First Vice President shall, in the absence of the President, have all the power
and perform all the duties of the President. In the absence of higher-ranking officers,the 2nd, 3rd, 4th and 5th vice-presidents shall have all the powers and duties of thehigher ranking officers in their respective order of holding office.
Each Vice President shall be responsible for supervising, overseeing, organizing andreporting on the organizations committees and shall be assigned these responsibilitiesby the President in consultation with the Vice Presidents on an annual basis after theelections take place.
There shall be a Vice president for Operations who shall be responsible for the fol-lowing committees: Disabled Hockey*, Player Development*, and Tournaments*.
There shall be a Vice President for Planning who shall be responsible for the follow-ing committees: Ad Hoc, Mike Cheever Grant, High School, Long Range Planning* LongTerm Athlete Development and Sportsmanship.
There shall be a Vice President for Education who shall be responsible for the follow-

respective Vice President by the President.(Committees marked with a * are permanent all others are at the discretion of the
President.)The Treasurer shall have the responsibility for carrying out all of the duties usually associated with the position of Treasurer including the following:A. receive all funds due Massachusetts Hockey, depositing them in a chartered bank, and
paying the rightful obligations of the Corporation; B. sign the checks on the Corporation’s accounts;C. submit in writing a quarterly and an annual Treasurer’s Report to the Board of
Directors, containing all Massachusetts Hockey Accounts.D. provide necessary materials to the Auditor for the certified audit of Corporation books
as requiredE. review financial reports of each Massachusetts Hockey account;F. co-signature on all Massachusetts Hockey accounts, including but not limited to,
Referee Registrar; State Registrar; District Registrar(s); and all Divisional Registrars. G. Chair the Finance Committee and conduct at least three scheduled meetings of the
Finance Committee per year. The financial reports for all Massachusetts Hockey accounts shall be reviewed at one of these meetings;
H. Prepare or have prepared the annual 990 return to the Internal Revenue Service and the PC-1 to the Attorney General’s Division of Public Charities and file these reports bytheir due date. Distribute the completed reports with a copy of the Audit report attached to the Board of Directors.
The Secretary shall:A. compile and distribute minutes of all Massachusetts Hockey meetings;B. be the official custodian of the Corporation’s records;C. advise the Board of Directors of all the upcoming meetings and other important
information;D. verify eligibility of voting members present at any meeting;E. verify a quorum is present andF. prepare and file the Massachusetts General Laws Chapter 180 corporation Annual
Report annually and Corporation updates with the Secretary of the Commonwealth of Massachusetts.

Referee Supervisors and the State Tournament Coordinator.
The Chair of the Rules Committee shall:A. be appointed by the President, subject to the approval of the Executive Board;B. hold committee meetings for review of Bylaws, Rules and Regulations, and policies
and procedures;C. review and make recommendations on all proposed amendments to the By-Laws,
Rules and Regulations, Policies and Procedures to the Board of DirectorsD. publish and distribute the Official Guide to all Massachusetts Programs, annually.
The Massachusetts Hockey Director of the Coaching Education shall:A. be appointed by the President, subject to the approval of the Executive Board;B. coordinate all the coaching clinics held in Massachusetts with the USA Hockey C.E.P.
Director;C. establish dates and locations for all clinics in cooperation with the USA Hockey C.E.P.
Director, and notify all District/Divisional Registrars, Associations, and Referees-in-Chief of those dates and location;
D. receive copies of all Clinic Registration Forms and maintain a file of all attendees;E. maintain a computer file by District of all individuals successfully completing
requirements of the Coaching Education Program;F. distribute current lists of accredited coaches to the District/Divisional Registrars
periodically.
The Chair of the Discipline Committee shall:A. be appointed by the President, subject to the approval of the Executive Board;B. coordinate the Discipline process ;C. maintain records of the disciplinary actions of Massachusetts Hockey and report these
actions to the board of directors.
The General Counsel shall:A. Provide legal advice to Massachusetts Hockey.B. Chair the CORI screening CommitteeC. Administer the Physical and Sexual Abuse Polices and their associated rules.
ARTICLE VII - NOMINATION AND ELECTIONS OF USA HOCKEY DIRECTORS,OFFICERS, DIRECTORS AND APPOINTMENT OF DISTRICT/DIVISIONAL
REGISTRARSNominations of Officers and USA Hockey Directors:
There shall be a Nominating Committee as provided in Article IX hereof consisting ofno more than five nor less than three members which shall be responsible for nominatingcandidates for all elected offices of the organization. They shall conduct its business as itsmembers deem fit, and it shall meet as often as its members deem necessary. They shallprepare the official ballot for the April Meeting. Candidates desiring to be listed on theballot must submit their name and the position they seek to the chair of the nominatingcommittee, by March 20th. Nominations may be made from the floor by a motion with asecond for all elected positions. If any candidate so nominated from the floor wasdeemed not qualified by the Nominating Committee to be placed on the ballot then thechair of said committee shall state to the Board prior to any vote being taken why thatcandidate was deemed not qualified. Candidates must be former Executive Board mem-bers or a current member of the Board of Directors and must be in good standing withMassachusetts Hockey to be eligible (CORI and Conflict of Interest Forms are accurate

and submitted to the respective chairs, no outstanding financial issues and not under sus-pension). Any member who holds an appointed position from USA Hockey and is specifi-cally excluded from holding office in a USA Hockey Affiliate and/or District by USAHockey’s By Laws, Rules and Regulations and/or Policies and Procedures shall not be eli-gible to be hold any elected position in Massachusetts Hockey.
Elections of Officers and USA Hockey DirectorsElections for the offices of the corporation shall take place at the April meeting.
Voting shall be by secret ballot and candidates receiving a simple majority of the votesshall be elected. The President’s vote shall not be counted except to break a tie. All can-didates will be allowed time to make a statement to the Board of Directors prior to theballots being cast. Upon completion of the counting of the votes for each elective officethe vote tally shall be announced. If no candidate receives a majority then the candidatewith the least votes will be dropped for further voting until one person receives a majori-ty. The President shall be elected for a 2 year term. All other officers for a one year term.There shall be separate ballots for each office subject to election with the President’soffice being first voted, then the Vice Presidents, Secretary, Treasurer, and continuing inan order determined by the Chair of the Nominating Committee until all elected positionsare filled. No person shall serve as President for more than 4 consecutive years. Election of District/Division DirectorsA. District and Division Directors shall be elected annually from each District.B. Each District/Division shall elect five (5) Directors.C. District and Division elections shall be held by District/Division Registrars no later
than March 15 of each year. Notification of the election meeting shall be given in writing by the Registrar to each program or team at least three weeks prior to said election.
D. Nominees shall be a member of Massachusetts Hockey and shall be eligible only in the District/Division in which they participate or reside and shall have been in good standing for at least one year immediately prior to nomination. For the purpose of thisparagraph participation shall mean active engagement with an association in the District/Division for at least one year prior to nomination.
E. USA Hockey appointed individuals interested in running for office must resign in writ-ing to the President of Massachusetts Hockey with an effective date prior to the District/Division election. If the resigned individual is the District/Division Registrar, theUSA Hockey Registrar or designee will then run the District/Division elections
F. In District/Division elections for Directors each registered and certified association shall have one vote for each Registered Participant Member as of March 1st as report-ed in the most recent census complied by the USA Hockey Associate Registrar. In all other matters, each association shall have one vote at District meetings.
G. Directors’ terms shall run from April 1 to March 30.H. Alternate director(s), if any, shall be elected by the same process as Directors.I. Names of elected Director(s) and Alternate Director(s), if any, shall be reported to the
Secretary of Massachusetts Hockey by USA Hockey Associate Registrars no later than the March 25. Failure to report names to the Secretary in a timely fashion shall cause individuals to be excluded from voting in the next regularly scheduled, special or Annual Meeting whichever comes first. The Executive Board shall receive all credentials and forward to the Secretary a list of properly elected District/Division Directors by April 1.
Directors at Large.Executive Board members not re-elected, or re-appointed may remain on the Board ofDirectors after the expiration of their term as a Director at Large with voting privileges forone year only, except those who have been a member of the Executive Board for 15years who shall be Directors at Large with voting privileges for life.
Election of USA Hockey DirectorsThe Board of Directors shall elect a USA Hockey Director for a three year term annually, by a simple majority at the April meeting. The terms will be staggered so as tominimize the number of Directors whose term ends in any one year.

In the event of a vacancy in any elected office of the organization due to resignation,removal, death, or other cause, the vacancy shall be filled by election at the next regular-ly scheduled or specifically called meeting of the Board of Directors for the balance of theterm remaining. Upon notice of such a vacancy, the President, or if the President’s officeis vacant, the next highest ranking officer, shall request the Nomination Committee toinform the Board of Directors of the opening and to prepare for such an election. TheSecretary shall give notice of the scheduled election to the Board of Directors.
ARTICLE VIII - DUTIES OF DISTRICT/ DIVISION REGISTRARS & DIREC-TORSUSA Hockey Associate Registrars shall:A. be directly responsible to the USA Hockey Registrar;B. hold District/Division meetings with a minimum of six per year;C. distribute all materials supplied to the District/Division associations;D. check player registrations for ineligible players, overall completeness and compliance
with Massachusetts Hockey and USA Hockey forms;E. submit Tournament applications to the State Tournament Coordinator no later than 14
days after their respective due date;F. review and approve all player transfers within the District;G. coordinate District Playdowns, and qualify players and teams for participation in
District/Division playdowns.H. hold District/Division Elections in accordance with Article VII of these Bylaws;I. maintain all District/Division records, and provide to the state Registrar upon
request;J. provide monthly reports to the State Treasurer in the prescribed format;K. report rules violations to the President;L. preside, without voting, at District Hearings. Registrars may decline to serve in this
capacity.Massachusetts Hockey District/Division Directors shall:A. attend Massachusetts Hockey State and appropriate District/Division meetings;B. cooperate with the District/Division Registrars in coordinating District/Division play
downs and other business of their respective District/Divisions; C. fulfill all the fiduciary responsibilities and obligations required by virtue of being an
elected member of the Board of Directors of Massachusetts Hockey;D. be eligible for appointment as a voting member of any disciplinary panel in accordance
with Article XIV unless conflict of interest considerations dictate otherwise.
ARTICLE IX - COMMITTEESA. The permanent committees of Massachusetts Hockey shall be: Audit, Disabled,
Discipline, Finance, Nomination, CORI Screening, Player Development, Association Registration, Rules, Tournament, Roster and Long Range Planning.
B. The Chairs of the Rules Committee, Coaching Education Program, and Discipline Committee shall be appointed annually by the President subject to the approval of the Executive Board. The chairs of all other Committees and members of all Committees shall be appointed annually by the President, except that the Treasurer shall be the Chair of the Finance Committee.
C. The President may from time to time appoint other committees for specific purposes and terms.
D. The President and President elect shall be ex-officio members of all Committees.E. All committee action except action under Article XIII and XIV of these By laws shall be
subject to review of the Executive Board on its own motion or at the request of the committee Chairperson. Any such review shall be limited to a review of the committee’s decision on the record before it.
F. The Chair of the Discipline Committee in consultation with the President and Counsel, if any, shall be the sole arbiter of the definition and meaning of all terms, phrases, words and procedures as they relate to the discipline process as set out in Article XIV of Massachusetts Hockey By-laws.
G. Any committee meeting may be held by any means of electronic or other communica-tion through which all the participants can hear each other.

H. The Audit committee shall consist of at least 3 members. The majority of the membersmust be members of the Board of Directors and none of the members can have any material financial interest in any entity doing significant business with Massachusetts Hockey.The committee shall make recommendations to the Board of Directors regarding:1. selection, retention, and termination of an independent auditor,2. the compensation of the auditor,3. measures to insure that the internal controls are documented by management and
evaluated as part of the audit,4. process by which the audit committee shall review the audit and the management
letter, if any, with the auditor and work with the auditor and management of Massachusetts Hockey to resolve or recommend resolution to the board of any issues of concern arising from the audit or management letter,
5. measures to ensure that any non-audit services provided by the auditing firm conform with the standards of independence required.The committee shall prior to the board’s consideration of and action on the indepen-dent auditor’s report, the committee shall report to the board on the results of the audit.

K. The Finance Committee shall Refer to Article VI Duties of the Members of the Executive Board
L. The Rules Committee shallRefer to Article VI Duties of the Members of the Executive Board
M. The Tournaments Committee shall1. have the responsibility of coordinating all State Final Tournaments;2. oversee all State Finals and District Playdown activity;3. receive all District Playdown applications and fees;4. ensure that all established deadlines are upheld;5. award State Tournaments to the Districts at the annual meeting;6. accept Tournament sites;7. receive Tournament Compliance Forms from each Tournament site prior to the
annual Tournament meeting;8. appoint ‘on-site’ Tournament Directors to oversee each Tournament site;9. receive the reports from each Tournament Committee and each ‘on-site’ Director;
10. issue a report on all Tournaments at the Annual Meeting.N. The Nomination Committee shall review the qualifications of all candidates prior to a
candidate’s name being placed on the ballot. That review shall consist at a minimum of determining whether a prospective candidate has on file with the Treasurer a complete and up to date conflict of interest disclosure form, whether that candidate holds any other position in Massachusetts Hockey and/or USA Hockey that would disqualify him/her for holding the office sought and whether that candidate holds any other position outside of Massachusetts Hockey that would place that candidate in a conflict of interest and prevent him/her in the opinion of the Committee from exercising his/her independent judgment on behalf of the best interests of Massachusetts Hockey. In the event the Committee determines the candidate is not qualified, the Committee will not place the candidate’s name on the ballot and will report its decisionin writing or via email to the full Board of Directors and the candidate.
ARTICLE X - ANNUAL MEETINGThe Annual Meeting of Massachusetts Hockey shall be held in the month of June. The
time and place shall be designated by the President in writing to the Board of Directorsnot less than fourteen (14) days prior to the meeting.
The order of business at the Annual Meeting shall be as follows, unless changed at themeeting:A. Roll call and proof of notice.B. Minutes of previous Board Meeting.

C. Treasurer’s Report.D. Report of the Rules CommitteeE. Report on previous year’s activities and remaining committee reports.F. Unfinished business.G. Seating of newly elected officers.H. New Business. Awarding of tournaments and setting dates.I. Selection of site for next Annual Meeting.J. Adjournment.Voting procedures at the Annual Meeting shall be the same as those set out in the third-paragraph of Article IV entitled Board of Directors.
ARTICLE XI - PARLIAMENTARY AUTHORITYAll meetings of the organization, including meetings of the Board of Directors, shall be
governed by Robert’s Rules of Order, except to the extent said Rules conflict with theseBylaws.
ARTICLE XII - INDEMNIFICATION AND EXCULPATIONMassachusetts Hockey shall indemnify and hold harmless USA Hockey, Inc., its
Directors, members, councils, committees and each member thereof, and all otherelected, appointed, employed or volunteer representatives of USA Hockey from any andall claims, liability, judgments, costs, attorneys' fees, charges and expenses whatsoever,arising from the acts and omissions of Massachusetts Hockey, except to the extent (i) thatUSA Hockey, Inc. or its aforedescribed representatives caused such claims. Liability,judgments, costs, attorneys' fees, charges or expenses by their own intentional neglect ordefault or (ii) that such acts or omissions we're the direct result of compliance with theArticles of Incorporation, By-Laws, Rules and Regulations, Playing Rules or decisions ofthe Board or Directors of USA Hockey. Further, Massachusetts Hockey understands andacknowledges that USA Hockey. Inc. and its aforedescribed representatives have assumedsuch assignment, function, office or capacity upon the express understanding, agreementand condition that they be so indemnified and held harmless to the extent described inthis by-law .Indemnification: Massachusetts Hockey shall indemnify each person who isor was a director, officer or employee, or a volunteer acting within the scope of theirauthority on behalf of Massachusetts Hockey and/or its affiliates, such person’s heirs,executors and legal representatives, against all liabilities and expenses, including judg-ments, fines, penalties and reasonable attorney’s fees and all amounts paid, other than toMassachusetts Hockey, in compromise or settlement, imposed upon or incurred by suchperson in connection with, or arising out of, the defense or disposition of any action, suitor other proceeding, whether civil or criminal, which such person may be a defendant orwith which such person is threatened or otherwise involved, directly or indirectly, by rea-son of such person’s holding or having held such position. The provisions of this para-graph shall not apply to any person who has been adjudicated in such action, suit or pro-ceeding not to have acted in good faith in the reasonable belief that his or her action wasin the best interests of the Corporation. Massachusetts Hockey shall provide no indemnifi-cation with respect to any matter settled or compromised, pursuant to consent decree orotherwise, unless such settlement or compromise shall have been approved as in the bestinterest of Massachusetts Hockey by (1) a disinterested majority of the board of directorsof the Corporation or (2) if there was no disinterested directors, by independent legalcounsel representing Massachusetts Hockey and appointed by a majority of the directorsthen in office. The right of indemnification provided in this Article shall not be exclusive ofor affect any other rights to which any director, officer, employee or volunteer may beentitled under any agreement, statue or otherwise. Massachusetts Hockey’s obligation toprovide indemnification under the Article shall be offset by and to the extent of any othersource of indemnification. Nothing contained in this Article shall affect any rights to whichMassachusetts Hockey personnel other than directors or officers may be entitled by con-tract or otherwise.
No Personal Liability: No director, officer, employee of this Corporation or a volun-teer acting on behalf of Massachusetts Hockey and/or its affiliates shall be personallyliable to Massachusetts Hockey for monetary damages for breach of fiduciary duty as adirector, officer, employee or volunteer notwithstanding any provisions of law imposing

such liability: provided, however, that the liability of a officer, director, employee or volun-teer to the extent that such liability is imposed by applicable law, shall not be eliminated(1) for any breach of such person’s duty of loyalty to Massachusetts Hockey; (2) for actsor omissions not in good faith or which involve an intentional or a knowing violation oflaw, or (3) for any transaction from which such derived an improper benefit.
The directors, officer, employees and volunteers of Massachusetts Hockey shall not bepersonally liable for any debt, liability or obligation of the Corporation. All persons, corpo-rations or other entities extending credit to, contracting with, or having any such claimsagainst Massachusetts Hockey, may look only to the funds and property of thisCorporation for the payment of such contract or claim, or for the payment of any debt,damages, judgment or decree, or any amount that may otherwise become due or payableto them from Massachusetts Hockey.
Massachusetts Hockey agrees, at all times, to be covered by the general liability insur-ance policy maintained by USA Hockey. USA Hockey shall inform Massachusetts Hockey ofthe limits of that policy, and of any changes to those limits which may be made by USAHockey at its sole prerogative. By purchasing and maintaining the aforementioned generalliability insurance policy, USA Hockey does not assume any liability for any actions oromissions of Massachusetts Hockey. Massachusetts Hockey agrees to use reasonableefforts to purchase, acquire or provide. and maintain in full force and effect at all times,directors and officers' liability insurance, and to name USA Hockey as an additionalinsured thereunder if such insurance is not obtained through USA Hockey. MassachusettsHockey retains right to obtain whatever additional insurance coverage's it may desire, atits own expense, but agrees to name USA Hockey as an additional insured thereof.
ARTICLE XIII - EQUAL OPPORTUNITY AND RESOLUTION OF GRIEVANCESMassachusetts Hockey shall provide an equal competitive opportunity, taking into
account ability, physical size, and other athletic criteria, to amateur athletes, coaches,trainers, managers, administrators, and officials to participate, consistent with the require-ments of the Amateur Sports Act of 1978, as amended, in amateur athletic competitionwithout discrimination on the basis of race, color, religion, age, sex or national origin.
Massachusetts Hockey shall provide for the prompt and equitable resolution of griev-ances of its members, including fair notice and opportunity for a hearing consistent withthe provisions of these By-Laws and the requirements of the Amateur Sports Act of 1978as amended.
Any grievance filed under this Article must be filed with the President ofMassachusetts Hockey with a copy of the grievance forwarded to the Chair of theDisciplinary Committee and the General Counsel. Only a member in good standing mayfile a grievance and for the purpose of this article. A grievance shall only be entertainedand acted upon if it deals with a material violation by a member of Massachusetts Hockeyand/or USA Hockey By-laws, Rules and Regulations, Policies and Procedures, Codes ofConduct, Codes of Ethics and USA Hockey playing rules and such violation is clearly con-trary to any practice or precedent consistent with said By-laws, Rules and Regulations,Polices and Procedures, Codes of Conduct, Codes of Ethics and playing rules.
ARTICLE XIV - SUSPENSION, EXPULSION, OTHER DISCIPLINE; APPEALSA.
1. Except in the case of suspension, expulsion, or other disciplinary action imposed by operation of the USA Hockey playing rules, disciplinary actions shall only be taken after a hearing by the appropriate authority.2. Any member of Massachusetts Hockey may be suspended, expelled or otherwise disciplined for violation of the provisions of Massachusetts Hockey and/or USA Hockey By-laws, Rules and Regulations, Codes of Conduct, Codes of Ethics, and USA Hockey playing rules.3. Suspension, expulsion or other disciplinary action may occur by operation of:
a. Massachusetts or USA Hockey By-Laws, Rules and Regulations;b. the playing rules of the game as promulgated by USA Hockey;c. administrative action taken by Massachusetts Hockey, or any association, league, district, or other organization certified and/or registered by Massachusetts Hockey with the authority to do same.

4. Additional violations for which discipline may be imposed include, but are notlimited to:a. threats or actions against officials of Massachusetts Hockey or USA Hockey;b. any conduct unbecoming or detrimental to the sport of hockey:c. allowing participation by ineligible or unregistered player, coaches, officials or
teams in sanctioned events:d. participation in events not officially sanctioned;e. clear and deliberate assault upon a game official. For the purposes of this
section a game official shall be any person present and necessary to conductthe game in the opinion of the Chair of the Discipline Committee.
f. Any player, coach or team official found guilty of a clear, deliberate physical assault upon a game official will be expelled from Massachusetts Hockey.
5. A summary suspension is permitted in those cases where an individual has been arrested for a crime alleged to have been committed, an individual has assaulted another or violated physical or sexual abuse policies of MassachusettsHockey or USA Hockey, including abuse by adults or players, provided policies are in writing and have been approved by the Board of Directors, and when an individual is named in a lawsuit against Massachusetts Hockey.Any summary suspension must be in writing and given to the individual affectedand the writing shall inform the individual of his/her right to request a hearing within seven (7) days of being notified of the suspension. Any hearing requested hereunder shall be conducted according to the provisions of paragraph B of this Article.
B. Hearing Procedures1. When a hearing is required in accordance with USA or Massachusetts Hockey By-laws, Rules and Regulations, or if a member who has been suspended, expelled or disciplined by operation of said rules, requests a hearing, or if the Chair of the DisciplineCommittee orders a hearing to be held, such hearing shall be conducted by a panel ofnot less than three nor more than five members and a presiding non voting Chair. Hearing may be held at the association, league, district or state level as appropriate.2. Hearings should be conducted as soon as possible, but not later than 30 days following the incident for Match Penalties and generally not later than 30 days from the request for a hearing or the completion of an investigation for all other incidents.3. All voting members of a Hearing Panel for District and State Hearing Panels shall be current members in good standing of the Massachusetts Hockey Discipline Committee or the Board of Directors including alternate directors. All voting members of a Hearing Panel for Association or League Hearings shall be members of Massachusetts Hockey in good standing. In all cases the members shall bereasonably disinterested persons.4. A decision should be announced within 5 days of the hearing and a written decisionwhich identifies the issues, pertinent facts, and the conclusions of the panel shall be issued within 10 days of the completion of the hearing and delivered to the parties and to the Chair of the Discipline Committee.5. Adverse decisions may be appealed to the Massachusetts Hockey Discipline Chair in accordance with C. of this section.6. All Disciplinary action imposed by association, league, district or state hearing panels for violation of USA Hockey or Massachusetts Hockey By-laws, Rules and Regulations, Codes of Conduct or Codes of Ethics shall be applicable to all Massachusetts Hockey association, league, district and state activities unless other-wise specifically limited in the written decision of the hearing panel of the first instance.7. Please refer to the Massachusetts Hockey Discipline Packet for further details regarding discipline hearings and procedures.
C. Any party to the hearing may appeal any adverse decision in writing using the form prescribed by Massachusetts Hockey within (10) ten days of the receipt of same to theDiscipline Committee which will process the appeal consistent with its practices and procedures and these Bylaws. The written decision of the panel in the first instance, ifso rendered, shall be considered prima facie evidence of the facts stated therein and

the burden of proof as to why the decision shall not be implemented shall be upon theappealing party. The only basis for appeal will be that the panel acted erroneously in interpreting or applying USA or Massachusetts Hockey Bylaws, Rules or Regulations and Codes of Conduct and Ethics to the facts. Appeals will be based solely on the record of the panel. Appeals from action taken by Massachusetts Hockey or the Discipline Committee shall be governed by the Rules and Regulations of USA Hockey.
D. Any member who does not comply with the procedures set out herein shall be deemed not to have exhausted his administrative remedies and this shall be deemed to be a defense against any legal actions taken by the member against Massachusetts Hockey, Inc. or other affiliate organization or association.
E. An appeal pursuant to the provisions hereof shall not stay the implementation of the suspension, expulsion or other disciplinary action for a violation of the playing rules. All other appeals may stay the implementations of the suspension, expulsion or other disciplinary action in the discretion of the Chair of the Discipline Committee. Notwithstanding the foregoing, a one game suspension imposed for any reason shall not be subject to an appeal. Suspensions resulting from a game misconduct penalty must be served in the next consecutive game by the suspended member, regardless of whether the suspension may again have to be served in a league.
F. Any member who has been disciplined under provisions of this Article, except for any violations of the playing rules or any one game suspension imposed for any reason, may after two years from the date of the imposition of the discipline petition the Chairof the Discipline Committee to review the matter and determine whether the disciplineshould be revised. Any decision of the Chair of the Discipline Committee hereunder shall not be subject to appeal.
G. The Discipline Committee shall be designated appeal authority to hear appeals under Article XIII and Article XIV for Massachusetts Hockey and any decisions of the committee or any panel established to hear appeals under this Article shall be final and only reviewable by further appeal to USA Hockey pursuant to USA Hockey’s dispute resolution process.
H. Any member who has been disciplined under the provisions of this Article, as a result of a failure to comply with a mandate of Massachusetts Hockey, may petition the Executive Board of Massachusetts Hockey for relief from the discipline, upon showing that the mandate has been subsantively complied with and that there was no other basis for the suspension. The Executive Board shall immediately review the matter anddetermine whether the sole and/or primarily basis for the suspension was a failure to comply with a mandate of Massachusetts Hockey and that the mandate has been substantively complied with. Upon such a finding, if it is in the best interest of Massachusetts Hockey, the Executive Board may immediately revise the discipline to an appropriate period of probation consistent with other discipline resulting from the same incident or other incidents of a similar nature. If the Executive Board determines that there were other reasons for the suspension, other than to comply with a mandate the Executive Board shall determine whether a suspension would continue tobe warranted absent the failure to comply with a mandate of Massachusetts Hockey. Any decision of the Executive Board hereunder shall be final.
ARTICLE XV - AMENDMENTSA. Proposed amendments to the By-Laws and Rules and Regulations must be
submitted to the Rules Committee in writing for their recommendation as to adoption, rejection, modification, or deferral prior to any vote being taken by the Board of Directors.
B. These By-Laws may be amended at the Annual Meeting of the Board of Directors provided a quorum exists if the proposed amendments have been presented in writing by the Rules Committee to each member of the Board of Directors at least fourteen (14) days prior to the meeting.
C. The Rules and Regulations may be amended at any regularly scheduled or special meeting of the Board of Directors provided a quorum exists. If the Rules Committee has not acted within 30 days of the submission of the proposal, the submitter may

bring the proposal to the Board of Directors by providing a written copy to each member of the Board of Directors at least fourteen (14) days prior to the meeting.
D. A two thirds majority of the votes cast is required to amend or change the By-Laws and/or the Rules and Regulations.
E. Any proposed amendment determined by the Rules Committee to be in the same or similar form and/or substance to one which was rejected during the current or priorseason cannot be placed before the Board of Directors unless it has received a two-thirds (2/3) majority vote cast by the Board of Directors recommending that it be placed before the Board of Directors.
ARTICLE XVI - FINANCE AND ACCOUNTINGA. The Treasurer of Massachusetts Hockey shall submit annually at the annual meeting of
the Massachusetts Hockey Board of Directors the recommended budget proposal of Massachusetts Hockey for the upcoming season. Said budget proposal shall be recommended by the Finance Committee after prior review for a ratification vote (approval/disapproval) by the members of the Board of Directors.
B. The treasurer shall cause an annual independent audit to be preformed on all the Massachusetts Hockey accounts. Said audit shall be presented in writing to the Board of Directors within six months of the fiscal year end of Massachusetts Hockey.
C. The treasurer shall file at the times required any and all financial forms required by all governmental agencies.
D. Any proposal or request for monies that deviate from the approved annual budget of Massachusetts Hockey in the amount of Fifteen Hundred dollars ($1,500.00) or more must be submitted in writing to the Finance Committee for a review for a recommen-dation vote (approval/disapproval). Said recommendation shall be submitted to the Board of Directors for a ratification vote. A proposal or request for monies shall not bebroken down into separate sums below Fifteen Hundred dollars ($1,500.00) in an attempt to circumvent a recommendation of the Finance Committee. Any proposal or requests for monies that are below Fifteen Hundred dollars can be presented from thefloor of a monthly meeting of the Board of Directors for approval/disapproval.
E. The Finance Committee shall keep and maintain records of any and all meetings and their recommendations. Said records shall be available upon request to any and all members of Massachusetts Hockey Board of Directors.
F. The Finance Committee shall establish, maintain and publish the Massachusetts Hockey Accounting and Financial Procedures.
G. Massachusetts Hockey Memorial Fund contributions may not exceed $1,000 per year in the name of a single individual. Fund contributions may not exceed five years, con-secutive or otherwise, in memory of a single individual.

ARTICLE I - REGISTRATION; COMPLETIONSection 1. RegistrationPersons and associations desiring membership in Massachusetts Hockey are required toregister in the category of membership sought as defined in Article III in the By-Laws ofMassachusetts Hockey and shall meet the following requirements in order to be so regis-tered:A. Associations:
Only one association shall be registered for each city and town or groupings of cities and towns in the Commonwealth of Massachusetts with populations of less than 150,000 residents. More than one association may be registered in cities or towns with populations over 150,000. Such programs shall consist of two or more groups of players who engage in competitive hockey play and may be referred to hereinafter as association(s) or town association(s).In order to be registered each such association shall:1. adopt a constitution and/or By-Laws which provide for the election of officers, a
Board of Directors, the conducting of its business and affairs in accordance with generally accepted parliamentary procedures and defines who has the right to voteand submit a copy of same to the Association Registration Committee as required;
2. ensure that membership and voting powers of its governing body consist of not less than 20% of members who have children actively skating in the association;
3. provide the right to vote to parents(s)/guardian(s) of minor players, coaches, and any other volunteers in that association in the process for nomination and election of Directors. The Officers of the Association must be elected on an annual or biannual basis by either the Associations Board of Directors or by the parent(s)/guardian(s) of minor players, coaches, and any other volunteers in the Association;
4. hold no fewer than 4 meetings per year open to all members of the association5. one of these meetings will be an annual meeting at which time
a) a written financial report for the most recent year ending shall be presented to its members, b) the election of its Board of Directors shall be held.c) Such meet ings shall be conducted in accordance with generally accepted parliamentary procedures. d) forward to the Association Registration Committee of Massachusetts Hockey, within two (2) weeks of said meeting, the following:
1) date, time and location of the meeting,2) agenda,3) minutes,4) nominees and winners of the annual election and5) method of communication (rink posting, newspaper, etc) which served as notice to the eligible members of the association regarding the meeting.
6. make copies of its By-Laws and/or Constitution Rules/Regulations, including methods of becoming an Officer or Director, method of selecting coaches, definitionof membership and voting rights, and a list of current Officers and Directors, available to all parents, coaches, players and other members of the association;
7. abide by Massachusetts Hockey's By-Laws, Rules and Regulations, and Policies and Procedures, including but not limited to, those regarding the prevention of Physical and Sexual Abuse;
8. secure and submit Individual Membership Registration forms (IMRs) from all members and prepare team rosters for all teams; and
9. pay required fees as established by USA Hockey and Massachusetts Hockey and as set out in the Fee Schedule herein.
10. maintain a file of signed Massachusetts Hockey Coaches Code of Conduct for each

Coach and Manager.11. maintain a file of signed Waiver of Liability forms for each Player, Coach and
Manager.12. No association may refuse to register a skater, who resides within the
Association’s geographic boundaries for any reason other than the following:a. A player is in arrears of its financial obligations to this or any other
Associationb. The player and or member is or has been subject to formal disciplinary
action by the Association within the last 5 seasons.c. The Association has no space available.
Adverse decisions may be appealed to Massachusetts Hockey in accordance with By-law XIII Grievance Procedure.
B. Leagues:Leagues organized for competitive play in the Commonwealth of Massachusetts shall be registered with Massachusetts Hockey.In order to be registered each such league shall:1. be comprised of associations, teams and members registered pursuant to the
provisions of these Rules and Regulations, or pursuant to the rules of any other affiliate District of USA Hockey;
2. agree to abide by Massachusetts Hockey's By-Laws, Rules and Regulation, and Policies and Procedures; and USA Hockey By-Laws, Rules and Regulations, and Playing Rules.
3. file a league registration annually with the Association Registration Committee certifying compliance with these requirements;
4. file a copy of current By-laws, Rules & Regulations with the Association Registration Committee.
C. Teams:Teams shall be registered in one of the following categories:1. Youth:-- Teams which are members of a City or Town association.2. Girls/Women's:-- Teams which are registered pursuant to the provisions in the
Registration procedures.All By-Laws, Rules and Regulations in this Annual Guide apply to Girls/Women unless specifically exempted.
3. Tier I and Tier II National Bound ;--Teams must belong to an organization that meets the following requirements:
a. Field the minimum of one team at each level Squirts to Bantams b. If the National Bound Tier I/Tier II association has Mite teams then those
Mite teams must comply with ADM cross-ice according to Massachusetts Hockey Rules and Regulations (Article VI. Section 3 Mite Games).
c. Teams that are National Bound eligible in the 2010-11 season will be exempt from the number of team requirement until the 2013-14 season
d. Must have registered as Massachusetts Hockey teams the previous two sea-sons.
e. Roster for Bantam and Midget levels may not have out-of-state player restrictions except they must comply with the USA Hockey requirements as to where they must register.
f. Meet all USA Hockey tournament requirements including, but not limited to, number of games played, roster and credential requirements.
g. Each organization may not have more than one Tier I national bound team and one Tier II national bound team at each age level.
Tier I teams a. Tier I teams can recruit players and have open tryout.b. Tier I teams have the intent to play at the highest competitive level possible.
Tier II teamsa. Tier II teams register as Tier II. Tier II denotes an intermediate level of
competition.b. Tryouts are open tryouts.
4. Club:-- Teams which do not fit into the definitions above.To be registered as club teams these teams must:

a. be comprised of a minimum of 80% Massachusetts residents;b. agree to abide by Massachusetts Hockey Policy and Rules and Regulations
regarding the prevention of Physical and Sexual Abuse and seek consents for the Criminal Offender Record Information for all relevant personnel on the team(s);
c. comply with all registration procedures of USA and Massachusetts Hockeyd. agree to be governed by the following restrictions:
1) cannot participate in Massachusetts Hockey State Championship Tournaments, USA Hockey Regional, Sectional or National Championship Tournaments;
2) cannot participate in Massachusetts Hockey Registered Leagues without the approval of the Massachusetts Hockey Executive Board; and
3) may only play against USA Hockey and Canadian Amateur Hockey Association registered teams, using USA Hockey playing rules, and registered USA Hockey referees.
D. Individual Members:Those individual members who are players, coaches and/or referees shall be registered with Massachusetts Hockey.In order to be registered a member shall:1. complete the USA Hockey Registration process and pay required registration fee;2. players registered as individual members on a youth team must play for the town
association of residence except that a resident of a city or town which does not have a registered association may join an association in another city or town. A member from a city or town which does not have a registered association who has joined another city or town association shall be deemed to be a member of that association for his/her youth hockey career and shall not be considered a non resident player (as per 3 below) in their original association. Midget level teams/players, players rostered on a National Bound Bantam team and Girls/Womenteams/players participating above the Girls/Women Tier 3 level shall not be subject to these requirements. In addition, a player may remain a member of the city/town association he/she has played for if the player’s family relocates to another city/town and that player will not be considered a non resident player as per paragraph 3 below, however once the player no longer participates in that original association he/she would be considered a non resident player (per 3 below) if he/she subsequently returns to the original association (and does not reside in the city or town).
3. Notwithstanding the provisions of D.2 requirements, a city or town association may register and roster up to three (3) members per roster who are non residents provided such member does not have outstanding financial obligations to another Massachusetts Hockey or USA Hockey affiliated association. Any player coming into a program who played elsewhere the previous season must provide a written financial release from their previous program to the new organization at the time of registration. Any disagreements on whether a player owes money to another program can be appealed to the District/Division Registrar.
4. An association described in Article I Section 1(A) may petition for a waiver from therequirements of Article I, Section 1(D)(3) to their District Registrar who shall pre-sent it at the next District Meeting or a special meeting for a vote. The meeting heldfor such a vote must occur on or before October 15th. In such a case, a vote shall be held for each individual roster not a group of rosters. A simple majority vote is required for approval. There shall be no absentee voting. District Directors and Association Representatives may attend a meeting held for such a vote in another District to state their opinion on the roster exemption, however, these individuals shall have no vote. The District Registrar shall give 7 days notice of a meeting held for such a vote, however, the Registrar is not required to communicate this to other districts. An adverse decision of the District may be appealed by the program to theRoster Committee. The District approvals, including rosters or lists of players shall be forwarded to the Roster Committee by the District Registrar within 10 days of the vote for a review . The review shall be limited to established precedents in rela-tion to creating teams from combinations of Associations, rostering out of state

players and the adverse effect of such teams on competition factors for state boundtournaments. The Roster Committee shall notify the District Registrar of its findings within 14 days. The decision shall be final with no appeal. The decision of the District (or the Roster committee, if appealed) is valid for the current hockey seasononly. Petitions for a waiver from the requirements of Article I, Section I(D)(3) filed after October 15th but not later than December 1st shall be acted on at the next District meeting
5. An association described in Article I Section 1(A) must receive and retain a form signed by all nonresident players and their parents/guardians indicating that they are aware that they are a nonresident participant and that the association may be applying for a roster exemption. The blank form will be available on the Mass Hockey web site. It is recommended that an electronic version of this form be included with association registration materials. There are three groups not requiredto complete this form: a. A player who resides in a city or town which does not have a registered Mass Hockey affiliate association who has joined another city or town association shall be deemed to be a member of that association for his/her youth hockey career andshall not be considered a non resident player in their original association. This formis not required in this case. b. Midget level players and players rostered on a National Bound Bantam team are not required to complete this form. c. Girls/Womens players participating above the Girls/Womens Tier III level are notrequired to complete this form.d. A player who relocates and who remains a member of the city/town association he/she has played for previously will not be considered a non resident player for aslong as the player stays with the original program and, therefore, need not complete this form.
6. In the event a player is registered on more than one team that participates in games leading towards a national championship and/or Super Regional champi-onhips, the USA Hockey registrar shall have the final authority to certify the eligibility of any such player solely on the basis of the following Regular –Season and Post-Season criteria.Dual Rostering Girls/Women may dual roster on a youth and female team and may continue play-ing on both teams until the end of the regular seasonDual-rostered males playing on Youth ( T-I and T-II team) may continue playing on both teams until the end of the regular season.Post –Season Play In the event both the Teams advance to a National Bound State Tournament and/or Super Regional State tournament the player at that point must declare which team they will continue with. Once that decision is made they will be deletedfrom the 2nd teams’ roster. If the player does not indicate which team they wish tobe eligible and remain rostered for, state, and national tournaments, the player will continue with the Tier-I team and thus be deleted from the T-II Roster by the USA Hockey Registrar.
E. Honorary Members:Honorary members shall not be required to register and shall be elected as honorary members by vote of the Board of Directors of Massachusetts Hockey.
Section 2. CompletionA. In General
All registrations shall be done annually at the time and in the manner prescribed by Massachusetts Hockey on forms prepared by Massachusetts Hockey and/or USA Hockey, Inc. and shall be deemed complete and accepted when stamped by an appropriate Massachusetts Hockey official.
B. PlayersPlayers shall not be considered registered and shall not participate in any on iceactivity until the USA Hockey registration process is completed and submitted to their association.

C. TeamsTeams shall not be considered registered until a USA Hockey official player roster form(Form 1-T) are submitted to the District/Division Registrar with the required fees established by USA Hockey and Massachusetts Hockey 10 days prior to the start of playdowns or November 15, whichever comes earlier, for tournament bound teams, and on or before December 1 for all other teams.
D. AssociationsAssociations shall not be considered registered until all Individual Membership Registrations (IMRs) for their members have been submitted, all team applications androsters have been submitted, all Criminal Offender Record Information consent forms have been submitted, their constitution and/or By-Laws have been submitted and all required fees have been paid.
ARTICLE II - COACHES AND MANAGERS; CERTIFICATIONSection 1. Coaches and ManagersCoaches, managers and all bench personnel shall be registered as members ofMassachusetts Hockey, Inc. and shall be identified as such on approved roster forms. A. They shall be responsible for the conduct of all members of their teams during sanc
tioned activities. B. There shall not be less than one nor more than four (4) bench personnel in the bench
area during a game and a game shall be forfeited upon a failure to comply with the minimum requirement of one, all of whom must be certified pursuant to the provisionsof Section 2 of the Article
C. All coaches and managers are required annually to read, sign and be in compliancewith the USA Hockey Code of Ethics.
D. Rostered coaches, managers and bench personnel may be changed during the season by submitting a Change/Add Bench Official form and the appropriate fee to a District/Divisional Registrar.
E. 1. All coaches must have current certification and online component verification begin-ning January 1 of the current playing season. Any coach not in possession of these requirements will be ineligible to coach for the remainder of the season. It will be the responsibility of the local Association registering the team to enforce the national Policy.2. It will be the responsibility of the Massachusetts Hockey Vice President of Education, Massachusetts Hockey Director of Coaching Education and the USA Hockey Coach in Chief for the Massachusetts district to determine if there shall be any penal-ties provided herein imposed on an individual coach or local Association who fails to provide a current up-to-date coaching CEP card with appropriate verification of level and online module.3. If the Coaches (Head or Assistant) credentials are not at the appropriate level by December 31st of the current season, the coaches cannot be on the bench or ice and must be removed from the team roster.4. The penalty for being on the bench or ice after December 31st of the current sea-son without proper certification or approved temporary card from the USA Hockey CICmay be a fine up to $100 per incident and the Association will not be eligible to partic-ipate in any State Playdown tournaments this season and the next season.5. A temporary card requires the coach to get certified by December 31st of the fol-lowing season. If the coach fails to do so the Association will be fined $100 and the Coach cannot be on the bench or ice for the remainder of the current season and the entire following season.6. Any monies collected by this will go to the Mike Cheever Grow Hockey fund.
Section 2. CertificationA. To qualify as a head coach or assistant coach on an approved roster, the following

B. Coaches who fail to comply within their inaugural season will be declared ineligible for subsequent season(s) until they first achieve the appropriate USA Hockey Coaching Education Program level endorsement.

ARTICLE III REFEREESOnly Level 2, 3 and 4 referees shall be eligible to referee Peewee, Bantam, Midget,Girls/Women and Junior Massachusetts Hockey tournaments.All referees must report all Match penalties within 48 hours, in writing, to his/her DistrictReferee Supervisor, including attaching the relevant game sheet if possible. The DistrictReferee Supervisor in receipt of the referees’ report will forward a copy to the DistrictRegistrar of the offending party and the Chairman of the Discipline Committee within 48hours. Referees failing to report Match penalties may be subject to suspension fromMassachusetts Hockey.
ARTICLE IV - NON-REGISTERED AND INTERNATIONAL COMPETITIONMembers shall not play any team not recognized by or registered with MassachusettsHockey or any team, which has been suspended and/or expelled by MassachusettsHockey or USA Hockey, Inc. Members shall not play in international competition withoutsecuring written permission from the USA Hockey Registrar submitted on the forms and inthe manner prescribed by said Registrar and after having paid the required fee as set outin the Fee Schedule herein.
ARTICLE V - TEAM CLASSIFICATIONSTeams in Massachusetts Hockey shall be classified in accordance with USA Hockey Rulesand Regulations. See Rules and Regulations III Classification.
ARTICLE VI - A GAMESection 1. Games And ScrimmagesThe following conditions shall constitute a game: A. When two teams occupy the same ice surface for the purpose of competitive play,
including scrimmages, at least for the minimum time and all rules prescribed by USA Hockey.
B. Any tournament or exhibition game played with teams from other USA Hockey Districts or teams approved by USA Hockey from other countries.
Section 2. Referee RequirementsA. All Playdown games are under the jurisdiction of Massachusetts Hockey and must be
officiated by Massachusetts Hockey registered referees.B. Score sheets are required for all games played. It is mandatory that all games leading
to District Playdowns including Tier IV playdowns be played with Massachusetts Hockey Referees. Before the beginning of the game. the referees, SHALL PRINT HIS/HER NAME ON THE GAME SHEET. At the end of the game the referees SHALL SIGN AND/OR INITIAL THE GAME SHEET.
Section 3. Mite GamesA. For the 2012-2013 Season:
1. All Mite Teams including but not limited to Tier I/II National Bound and Club notregistered as Mite B,C, D or novice and 8 & Under Girls Tier I – Must play a minimum of 20 cross-ice games. The game requirement can be fulfilled with league or non league games.
2. Youth Mite B, C, D, Novice and 8 & Under Girls Tier II, Tier III – All games must be cross-ice.
3. For all Youth Mites and 8 & Under Girls ( Tier I, II, III) – Team must submit the cross–ice schedule to their District/Division Registrar. All practices must be organized using the ADM format in accordance with the USA Hockey ADM Guidelines
B. For the 2013-2014 and subsequent seasons:

1. All Youth Mite and 8 & Under Girls games must be cross-ice.2. All Youth Mite and 8 & Under Girls must submit their cross-ice schedule to their
District/Division Registrar. All practices must be organized using the ADM formatin accordance with the USA Hockey ADM Guidelines.
3. Home Town Associations, Tier I and Tier II National Bound Team Associations and Club Team Associations must be 100% compliant with this cross ice requirement or the program will not be eligible for the National Bound or State Bound tourna-ments at all age levels.
C. Recommended Guidelines for ADM Cross-Ice Games:1. At least two games with at least two teams each per sheet of ice.2. 4V4 without goalies with mini nets or 3V3 with goalies and intermediate/regular
size nets.3. Groups formed from both teams (Game Group, Fun zone Group and Rest Group4. Rotate Groups every 1.5 to 2 minutes.5. Lines should match the other teams’ competition6. No referees7. Home team is responsible for communicating with visiting team and agreeing on
format of zones.
ARTICLE VII - STATE CHAMPIONSHIP TOURNAMENTSSection 1. National And State Bound ChampionshipsMassachusetts Hockey in the Youth and Girls/Women categories will recognize the follow-ing classifications for State Champions provided a minimum of four teams have applied.In the Youth categories that represent at least 3 Districts.
As of 2013-2014, programs with Mite Teams must be compliant with Article VI. Section 3.Mite Games in order to be eligible for any National Bound or State Bound Tournaments inall age levels.
A. NATIONAL TOURNAMENT BOUND TEAMS; Only Teams from these Massachusetts Hockey categories are eligible to compete in these Tournaments heading for a National Tournament site.Youth Tier I Tier I Teams and City/Town Association Teams.Youth Tier II Tier II Teams, and City and Town Association Teams in Peewee, Bantam, Midget 15-16 and Midget 17-18 Girls Tier I ( 13-14 years of age, 15-16 years of age, 17-19 years of age).Girls Tier II ( 13-14 years of age, 15-16 years of age ,17- 19 years of age).Women's (18 years of age or older)

Section 2. Tier ClassificationsA. Classification for Youth State Tournaments are as follows:TIER I:-National Bound/Super Regional Bound
Tier I State Tournament will consist of Tier I Teams and any city/Town Association teams electing to enter. The format will be determined by the State Tournament Coordinator.
TIER II: National Bound/Super Regional BoundTier II Teams and any City/Town Association team electing to enter. The format will bedetermined by the State Tournament Coordinator.
Tier II Non National Bound1. Any registered Massachusetts Youth Hockey team except a Massachusetts Hockey
Tier I or Tier II National Bound team and Massachusetts Hockey Club teams may compete in the State Tournament at the proper Tier II classification. A Tier II team is considered to be the first, or highest rated, team in a City/Town association.
2. A Tier II team electing not to compete in the District playdowns and State Tournamentwill not penalize the Tier III and/or Tier IV teams from competing at the proper classification, subject to the approval of the District, if the District Registrar has a viable roster and at least ten (10) game sheets for the non-competing Tier II team.
3. A team competing in Tier II Division L (Large) shall be so classified where in thecurrent playing season the association rostered, in the case of Midgets and Bantams more than 36 players at each level respectively, and in the case of Peewees and Squirts more than 48 players at each level respectively.
4. A team competing in Tier II Division S (Small) shall be so classified where in the current playing season the association rostered, in the case of Midgets and Bantams fewer than 37 players at each level respectively, and in the case of Peewees and Squirts fewer than 49 players at each level respectively.
5. Any team in Division S may apply for participation in the Division L District playdowns and State Tournament competition.
6. For purposes of total player census for Division S or L classification at the Tier II level, all players rostered in house leagues, other travel teams and on Yankee Conference teams shall be included for purposes of paragraphs 3 and 4 above.
TIER III:Except as provided in paragraph 2 of this section, any second or third team from a association that has a team competing in Tier II in that same youth classification for that year’s tournament (see rules under Tier II part 2 above for exception).
TIER IV:Any third or lower team from a association that has teams eligible for Tier II and Tier III competition in that same youth classification for that year’s tournament. Also included shall be any team playing in a city/town recreation league or combination of players from a recreation program, subject to eligibility requirements and upon approval of the STC and the District Registrar for that team’s district.
Any team, except a Tier I or II National Bound team, may petition its District to play inone tier lower if the team plays in a lower tier in their League and/or they feel they wouldnot be competitive in the tier these rules require them to enter. The petition must be sub-mitted to their District/Division Registrar. The petition must document the basis for thischange in tier. Documentation must show the record, the division they play in theirleague(s), or other information to document their reasons for the petition. The petitionmust be approved by a unanimous vote of the District. Any association making a travestyof this will be censured the following season.Classification must be declared on the team application form. The home District shall beconstrued to be the District in which the team registers. Any team, which competes in aninter-district or inter-community league, must return to its home District for district play.District/Division teams shall, if directed by the Massachusetts Hockey State ChampionshipTournament Coordinator (hereinafter in these rules referred to as the “STC) enter inter-district/Division playdown activity to reduce the number of entries for the tournament.B. Classifications for Girls/Women State Championships
National Bound Girl’s TeamsNational Bound teams must belong to associations that meet the following requirements:

1. Tier I teams a. Tier I teams can recruit players and have open tryouts. Each organization may field no more than one team at each age level that can advance to a National Championship. No player shall be solicited nor shall the possibility of playing Tier I hockey be discussed with any player, prior to the conclusion of the USA Hockey National Tournaments except for existing Tier I player for the particular Tier I organization for which that player played during the existing season and then only by that Tier I organization.b. Roster for 17-19 and 15-16 should not have out-of-district player restrictions. Rosters will be limited to a maximum of four (4) out-of-district players for levels 13-14 and 11-12 . Residency is based on where the players parent resides. An appeal can be made for a waiver if the number exceedsfour. c. Tier I teams have the intent to play at the highest competitive level possible.d. Tier I teams should include as a minimum or build their programs to include: strength & conditioning, on-ice skills, tactics & habits, hydration and recovery, mental preparation, training elite athletes, training and opportunity for upward mobility.e. Each Association may only enter 1 team at each age level in the National BoundState Tier I Tournament.
2. Tier II teamsa. Tier II teams register as Tier II. Tier II denotes an intermediate level of competition.b. Tryouts are open tryouts. Rosters will be limited to a maximum of four (4) out-of-district players for all levels: 17-19, 15-16, 13-14, and 11-12. Residency is basedon where the players parent resides. An appeal can be made for a waiver if the number exceeds four.
c. Tier II teams or clubs must refrain from openly recruiting outside their defined boundaries or outside their member organizations.
d. Each Association may only enter 1 team at each age level in the National Bound State Tier II Tournament. All entries will be reviewed for appropriate competitive level by a committee of the Girls/Womens Registrar and the Division Directors. An adverse decision my be appealed to the Tournament Committee.
3. National Bound Girl’s TeamsNational Bound teams must belong to associations that meet the following requirements:a. Field the following minimum number of teams:
i. 11-12 & 1 team &ii. 13-14 1 team &iii. 1516 1 team or 17-19 1 team
b. Associations must abide by rules and Regulations Article VI. Section 3, Mite Games
c. Provide a minimum of one (1) weekly on ice training sessiond. Appoint a Coaching Directore. Meet and maintain full compliance with Massachusetts Hockey and USA
Hockey By-Laws and Rules & Regulationsf. Notify players at time of signing of the Massachusetts Hockey rule covering
Dual registration.3. National Bound Senior Teams
Teams that are properly registered and are in full compliance with Massachusetts Hockey and USA Hockey By-Laws and Rules & Regulations are eligible to compete in National Bound Tournaments.State Bound TeamsAll other teams that are properly registered and meet all other Massachusetts Hockey and USA Hockey By-Laws and Rules & Regulations are eligible.
Section 3. Playdowns to Select Teams for State ChampionshipsA. Girls/Women - All teams who submit a Playdown Application and meet the require-
ments of the tournament rules shall participate in playdowns to reduce the number of teams to enter the State Championship Tournament unless otherwise directed by the

STC.B. Youth - Each District shall hold a playdown tournament to select their entry/entries in
the State Championship Tournament except Tier II National Bound State Championships.
Section 4. Tournament Application and Playdown TimetablesTo enter these tournaments each team must submit to their District/Division Registrar aPlaydown Application by the dates specified on the Application Form.A. All Youth District Midget II and III, and Bantam II National Bound playdowns must be
completed by the date shown in the Calendar of Events. All other Bantam and Midget playdowns may also be held prior to December 1st at the option of the District.
B. Youth District playdowns for all Squirt, Peewee, Bantam II, III, IV and Midget III mustbe completed by the date shown in the Calendar of Events.
Section 5. Youth District PlaydownsA. District/Division Playdown Tournament Committees
The appropriate Registrar shall appoint the District/Division Playdown Tournament committee. They shall be responsible for all playdown activity, and will notify the STC verbally and in writing of the complete playdown results.
B. At every District Playdown game, there shall be in attendance at least one member of the District Playdown Committee, and it shall be their responsibility to establish and monitor player sign-in procedures to prevent any irregularities. Any association making a travesty of this will be prohibited from entry in the playdowns the following season.
Section 6. Tournament HostsA. The STC shall award the State and Northeast Sectional Tournaments to the Districts
at the Annual June Meeting. The District will award the tournament to the Host/Co-host by the September State Meeting. The Host shall submit to the STC the compliance form for approval no later than the October State meeting. District/Divisions or Hosts failing to meet the above dates are subject to immediate forfeiture of that tournament. The STC may on the above dates award the tournament to a new candidate who shall submit the compliance form by the November State meeting.
B. The Tournament Host shall pay all fees for that State tournament. The Semi Final and Final games for the Tier I and Tier II Bantam and Midget National Bound Tournaments shall have accredited medical personnel as outlined in the USA Hockey Regional Playoffs Rules. Massachusetts Hockey strongly recommends this policy for all Tournaments
C. All State Tournaments shall be held on the dates scheduled in the Massachusetts Hockey Annual Guide.
D. District/Divisional Teams entering the State Finals shall have a representative attend the Pre-Tournament meeting to obtain the schedules and other pertinent information, meet the host, give the tournament a check and receive an overview of the Tournament Rules by the committee and Hosts.
E. Host teams for State, Sectional and National tournaments shall not participate in games leading to that tournament, unless invited by the STC.
Section 7. Tournament SeedingA. The STC shall set the number of teams in each non National Bound Tournament using
as a guide 20% of the playdown applications for that level the previous year. Tournaments will be 4, 6, 8, 10, 12, or 16 teams.
B. Tournaments consisting of 16 teams will be divided into 4 Divisions with the teams playing everyone in their division in a round robin. The team in each division with the best record (see section L below) shall advance to the Semi-Finals.
C. Tournaments consisting of 12 teams will be divided into 3 divisions with the teams playing everyone in their division in a round robin. The division winners plus the second place team with the best record (see Section L below ) shall advance to the Semi-finals. With the National Vs American and the Olympic Vs second place team. If the second place team is from the Olympic Division then the American winner will play the Olympic winner and the National winner will play the second place team.
D. Tournaments consisting of 10 teams will be divided into 3 Divisions. An Olympic division consisting of 4 teams and an American and a National Division consisting of 3 teams each. The teams in the Olympic Division will play each other in the round robin.

The teams in the American and National Division will play the teams in the other Division in the round robin. The Olympic Division winner plus the top 2 teams from the combined American and National divisions and the remaining team with the best record (See Section L. Below) shall advance to the semi-finals, with the Olympic winner playing the team with the second best record from the combined American National Divisions and the best team from the American National divisions playing the remaining team.
E. Tournaments consisting of 8 teams shall be divided into 2 divisions with the teams playing everyone in their division in a round robin. The 2 teams in each divisionwith the best records (see section L below) shall advance to the semi finals with the winner in each division playing the runner-up from the other division.
F Tournaments consisting of 6 teams shall be divided into 2 divisions with the teams playing everyone in the other division in a crossover round robin. The 4 teams with the best records (see section L below) will advance to the semi finals with the team with the best record playing the team with the 4th best record and the teams with the2nd and 3rd best records playing each other.
G. Tournaments consisting of 4 teams will not be divided into divisions. They will play each other in the round robin (3 games each). the 2 teams with the best record (see section L (below) shall advance to the Finals.
H. The National Bound Championship Tournament format will consist of eight (8) teams, one from each of the Zone Playdowns, two Zone Finalists (determined by the STC based on the number of teams entered in that tournament), and one (1) host team. The zone winners, finalists and host teams shall be placed in two divisions, with three playoff games for each team in all classifications. If less than 8 teams are entered in any age group the STC may set the number of teams at the state tourna-ment to 4 or 6.

L. In the case of a tie, Division winners will be determined by the following tie-breaking formulas.
Note: In order to break the tie, the following formulas will be followed until one rule determines the position of all teams that are tied.
1. The results of the games played between the teams (Head to Head) tied in the following order:a. The points acquired in these games.b. Least goals against.c. Dividing the goals scored in these games by the goals scored against
(GF divided by GA), the position being determined in order of the greatest

quotient. A quotient involving dividing by zero (0) has higher standing than a quotient dividing by any number other than zero. Where two or more teams have no goals against and the quotient tie breaker is required, the teams shall be ranked high to low in descending order of “goals for”Note: Forfeits-If a team forfeits any of its games, and their games become involved in any tie-breaking formula to determine who will advance to the next level of play, the team forfeits all games in the round robin play and the gamesare recorded as 1 - 0 victories for the non-offending team.
2. If after applying the formulas of 1 a, b, and c, the tie still exists, 1a, 1b and 1c shall be applied using all games played by the teams tied. The formulas shall be applied in the order 1 a, b, c and 2.
3. If the above procedure does not break the tie, then their position will be deter-mined by the number of penalty minutes assessed in all games they played. Their position being determined in order of the least penalty minutes.
Section 8. Tournament Rules and RequirementsA. No team participating in District Playdowns or State Tournament shall be required to
play more than two (2) games per day, and no team shall be scheduled to play a second game in one day against a team playing its first game on that day, except in 6team and 12 team State Championship Tournaments or with specific prior approval of the STC or his designee. It is recommended that there be a minimum of four (4) hours between games on the same day, and twelve (12) hours between games played on consecutive days. The playing schedule and times of games must be approved by the STC or his or her designee.
B. Unless otherwise agreed by the Team and Tournament Director, the home team shall wear light/white jerseys and the visiting team shall wear dark colored jerseys in all games. If the home team has not been established by the Tournament Committee, itshall be determined by the toss of a coin. The team traveling the furthest distance shall call. The Host will provide the jerseys or pullovers as required for the team wear-ing the inappropriate colors.
C. All State Tournament games shall be played stop time. D. USA Hockey Playing rules and Massachusetts Hockey variations shall prevail. All
round robin games shall be 12-minute periods. All Tier I and Tier II National Bound semifinals and finals games will have fifteen (15) minute periods for Peewees, 16 minutes for Bantams and 17 minutes for Midgets. All other State Playdown and State Tournament games shall be no less than twelve (12) minute periods.

E. In case of a tie in the semi or final games, at the end of regular play, teams will be allowed a three minute rest period. All divisions are to play to a sudden death conclusion.
F. In all Bantam and Midget National Bound Tournaments, Semi-Final and Final games the ice should be resurfaced after every two (2) regular periods of use, if possible. In all State Tournaments the 1st overtime period, if required will be played following a 3-minute rest period. If additional overtimes are required, the ice may be resurfaced, and again if possible after each set of 4 overtime periods with 1 minute rests between periods until a conclusion is reached.

G. Tournament fees will be established by STC and announced at the January State meeting if possible.
H. All disciplinary actions in playdowns or state final games will be acted upon using the Massachusetts Hockey regular process (Article XIV of the Massachusetts Hockey By-Laws ). There will be no hearings conducted at Playdown or State Tournament sites.
I. Suspensions resulting from a game misconduct imposed during a playdown or tournament game must be served in the next state playdown or tournament game. If there is an intervening non-playdown, non-state tournament game, the suspension willalso apply to that intervening game. Any member incurring a Match Penalty during playdowns, regional or state tournaments shall be ineligible for such play for the balance of the games leading to a State Championship.
J. State Champions will be considered to represent Massachusetts Hockey in the USA Hockey national and any other Tournaments designated by Massachusetts Hockey if they receive the approval of the Massachusetts Hockey STC and Executive Board.
K. In the event a state champion is not selected through tournament competition at any level of play, for any reason, the Massachusetts Hockey STC, in consultation with the Executive Board, shall name a team to represent Massachusetts in all national and other competitions.
Section 9. ProtestsProtests must be filed by a rostered team official of the protesting team. Protests must befiled with the on-site Massachusetts Hockey Tournament Committee representative(s), inwriting, within one (1) hour after the completion of the game in question. On ice judg-ment calls and playing rules are not cause for protest.Specific Requirements:
1. Written protest must contain all the facts and may only protest one stated issue or challenge the qualifications of one player. Multiple protests may be submitted if multiple issues and/or player qualifications are being protested. However each is considered a separate protest requiring a separate fee.
2. Protest will be accompanied by a fee of $100.00 cash, and will be returned if the protest is upheld.
3. All parties must be duly notified and be present in person at the protest hearing orno appeal will be allowed the absentee.
4. A hearing of the protest will be heard by a committee designated by the STC.5. If an appeal on the outcome of the protest committee’s decision, is necessary, the
offended party must file the appeal within (1) hour of the decision, in writing, accompanied by a fee of $100.00 cash.
6. All parties must be duly notified and present in person at the appeal hearing; the appeal being heard by a committee of at least 3 members of the Massachusetts Hockey Tournament Committee.
7. The decision of the appeals committee shall be final.8. All parties will be given the final decision at the conclusion of both the protest and
the appeal hearings, a follow up of the decision, in writing, will be sent within 10 days.
Section 10. RefereesA. Referees fees shall be negotiated at the Massachusetts Hockey Annual Meeting and
submitted to the Board of Directors for approval. A two (2) man system may be used for all games in the District/Division playdowns, State Tournament, Regional and othertournaments designated by Mass Hockey, with the exception of Tier I and Tier II National-bound Semi-final and Final games which will utilize a 3 man system.
B. The USA Hockey Referee-in-Chief and the Massachusetts Hockey Referee Registrar shall provide a list of Tournament-certified referees for all State Tournament sites. The Referee-in-Chief and The Referee Registrar shall assure availability of officials for each game played. The District Referee Supervisor shall provide a list of assigned Referees matched to the tournament game schedule to the respective District/Divisional Associate Registrar. All minor officials will be the responsibility of the Tournament Host Committee.
C. The STC or his/her designee must approve all referees used for all State

Tournaments, Sectionals (if held in Massachusetts), and any other tournaments designated by Massachusetts Hockey.
Section 11. Team and Roster RequirementsA. All Teams entering District/Division playdowns must be prepared to compete through
the State Tournament, New England Sectional (if there is one) and National Tournament (if there is one) or other Tournament site as directed by Massachusetts Hockey. If a team withdraws prior to the Tournament its association may be held ineligible to participate in the following year’s tournament. Withdrawing teams/associations must pay a $100.00 fine payable to Massachusetts Hockey and shall never theless be responsible for their tournament financial obligation if a paying, qualified replacement team cannot be found.
B. If a team fails to appear in time to start a game, the game may be forfeited provided the other team takes the ice to claim the forfeit. Forfeiting team, coach, team officials and associations may be subject to additional disciplinary action. The forfeiting association will be responsible for all tournament fees plus a penalty fee equal to the tournament fee made payable to Massachusetts Hockey within 14 days of the forfeit. In the event of exceptional circumstances, the forfeit fee may be timely appealed to the Tournament Committee.
C. In order to qualify for a State Tournament, prior to the start of the Tournament each team must have competed, as a team and each player including roster change players:GIRLS/WOMEN — at least fourteen games, with each player having played in at least ten of those games, except in the case of State Championship Tournaments held before December 1, each team must have competed in nine games, with each player having played in at least six of these games.YOUTH —
1.at least fifteen games, with each player having played in at least ten of those games, 2. in the case of non National Bound State Championship Tournaments held before December 1, each team must have competed in nine games, with each player having played in at least six of those games.3. in the case of National Bound State Championship Tournaments held before December 1, each team must have competed in seventeen games, with each player having played in at least seven of those games.
D. It shall be the responsibility of each team to make sure copies of the accepted stamped roster (with names, birth dates and signatures), Birth Certificates, approved waivers, game sheets and team personnel change forms (if applicable) are available at the site for each playdown and final site game. Any player who is unable to obtain a birth certificate will be allowed to register if the player is able to supply satisfactory proof of birth date. These include only hospital records, immigration papers or affidavits sworn before an officer of the court.
E. Only eligible players and team officials listed on the approved official Team roster formor approved supplemental roster form and approved roster change forms may participate in District/Divisional non National Bound Playdowns, State, Regional and other Tournaments designated by Massachusetts Hockey except injured players on non-national bound teams may be replaced by players on a lower level team of the Association. Hand written players or team officials will not be accepted on any approved roster.
F. Each team may dress 20 players (Massachusetts Hockey strongly recommends 2 goaltenders — see Section 12 of this Article) equipped and ready to play. No more than four (4) registered and accredited team officials in good standing will be permitted on the bench. At least one properly accredited team official must be on the benchat the start of the game. Electronic communication in any form to or from the bench by team officials will not be permitted.
Section 12. GoaltendersA. Any alternate goaltender that meets the requirements in Section 11.C of this Article
for his/her original team may return to his/her lower division or classification team if he/she does not enter the new team's game. The alternate goaltender shall only enter the game in the event of illness or injury to the regular goaltender during the Tournament Game. The alternate goaltender who enters the game due to illness

or injury to the starting goaltender may return to his/her regular team. Coaches violating this rule are subject to suspension.
B. Massachusetts Hockey strongly recommends all teams entering District/Division Playdowns, State, and other Massachusetts Hockey designated Tournaments, have two (2) goaltenders dressed and ready to play. The host will not be required to furnish alternate goaltenders for such playdowns or tournaments held in Massachusetts. Alternate goaltenders must come from teams in the same association as the team to which he/she is moving up.
Section 13. MiscellaneousA. Player and/or team eligibility can be resolved by the Massachusetts Hockey
Tournament Committee at any time.B. There shall be no all-star teams selected at any State Tournament or Playdown sites.C. In the event of problems not covered by the existing rules, the STC may make any
determination in the best interest of the Tournament.D. Any 17-18 Midget National Tournament Bound players, who are rostered on a
dormant split-season 17-18 Midget team, are permitted to be rostered on a Junior team and be returned to their Midget team on or before February 15. These players file a Roster Designation Form with their District Registrar and the Junior Registrar before playing their first game with the Junior team. Players must complete the Roster Designation Form, which states their full intention of rejoining their Midget split season team no later than February 15.
ARTICLE VIII - OTHER TOURNAMENTSAll Senior and Junior Tournaments shall be held at the direction of and under the formatprescribed by the State Championship Tournament Coordinator.Any independent or invitational tournament held in Massachusetts must secure the sanc-tion of Massachusetts Hockey and USA Hockey. One of the requirements necessary forobtaining a Massachusetts Hockey sanction is that only Massachusetts Hockey, USAHockey, CAHA and IIHF registered teams be allowed to enter or compete in the tourna-ment.Any registered team participating in a non-sanctioned tournament, may be declared ineli-gible for the next Playdown, State or National Tournament.
ARTICLE IX - AWARDSHat Trick, Playmaker and Zero Club awards (one per player, per season) will be issued toplayers who earn them during regular season, State Finals and Tournament Games, pro-viding the roster of the team has been registered with the Massachusetts Hockey. A copyof the official game sheet must be submitted to Massachusetts Hockey on the properAward Form (see Massachusetts Hockey forms in the back of the Annual Guide ). Gamesheets copies will be returned only upon request.
ARTICLE X - TRAVEL PERMITSTeams traveling outside the State must obtain a travel permit. The only exceptions will bethe following;A. Single games played in bordering States,B. practice ice time,C. league play.Travel permits will only be issued by your USA Hockey Registrar or Associate Registrar foryour District/Division. Permit applications my be obtained from the Mass Hockey web site(www.mahockey.org) or the Massachusetts Hockey Annual Guide. The completed applica-tion with a copy of the team’s Roster if necessary may be either emailed as an attach-ment to the Associate Registrar (preferred) or mailed. The Associate Registrar willapprove the permit and return it. Associations/team officials found in violation of theseprocedures shall be subject to disciplinary action.
ARTICLE XI – ASSOCIATION MERGERS AND DIVISIONSMergers and/or divisive reorganizations of existing associations must be approved by theMassachusetts Hockey Board of Directors.

Any request for a merger or division shall be made in writing to the President and shallidentify the proponents of the merger or division, define the proposed geographic area tobe merged or divided and give a brief statement of the purpose of the merger or divisionincluding, but not necessarily limited to, the number of players affected by the proposedaction, the affect on any existing association and the financial plans of the proponents.The President shall inform any affected existing associations(s) of the proposal and shallrefer the matter to the Association Registration Committee for an investigation of themerits of the proposal.The Association Registration Committee may in its discretion interview the proponentsand any affected existing associations and the affected Districts and shall determine,whether through hearing or written submissions, whether the proposal is consistent withMassachusetts Hockey’s By-laws, Rules and Regulations and Policies and Procedures andin the best interest of Massachusetts Hockey. After the committee completes its review ofthe request, it shall report its findings and recommendations to the Board of Directors foraction. The Committee shall so report no later than Sixty (60) days after the request hasbeen referred to it.
ARTICLE XII - LOCAL RULINGSSection 1. Problem ResolutionAll Associations must attempt to resolve their problems within their District, and theDistrict recommendation shall be considered and given probative weight prior to casesbeing presented to the Massachusetts Hockey Board of Directors.Section 2. Learn-to-Skate/Initiation program Equipment RequirementAll Learn to Skate and Initiation Program players must wear HECC approved helmets.Section 3. Rules InterpretationIf when both teams are on the ice, and a team official(s) for any reason shall pull his/herteam from the ice, rule 632 (Refusing to start play) in the USA Hockey Rule Book shallapply. If the Referee shall impose a Match Penalty under this rule, and the team official(s)has been found guilty of such actions, the game in question shall be forfeited and theteam official(s) may be suspended for one year as the result of a hearing.
ARTICLE XIII - SEXUAL AND PHYSICAL ABUSE SCREENINGSection 1. IntentIt is the intent of these rules and regulations is to reduce the risk of a MassachusettsHockey minor becoming the victim of sexual or physical abuse. A minor shall be any indi-vidual under the age of eighteen (18) years of age.
Section 2. Applicability All members of Massachusetts Hockey who have routine access to minors shall bescreened under the provisions of this Article XIV.A. Members shall include, but not necessarily be limited to, officers, members of Boards,coaches, managers, referees, instructors, and staffs, whether or not they receive compen-sation for the performance of their duties, in all organizations associated withMassachusetts Hockey, registered town Associations, club teams, Tier I National Boundteams, Tier II National Bound teams, girls/women Associations and Festival or develop-ment camps.B. Members may, in their discretion, apply the requirements of these rules to other volunteer personnel not carried on rosters or registered through USA and not specificallymentioned herein.C Any member required under these rules to be screened who does not consent to besubject to the application of these rules shall not be allowed to participate in activities ofMassachusetts Hockey and its members.D. All organizations, associations or members shall be responsible for ensuring their par-ticipant member comply with the provisions of this Article.
Section 3. OffensesA. The person has been convicted (including crimes the record of which has been
expunged and pleas of "no contest") of any crime of violence against minors, or any

crime which indicates the person may pose a risk to the safety and well-being of children under his/her direction and/or stewardship, including but not limited to:1. Murder in any degree, manslaughter, arson2. indecent assault and battery on a child under fourteen,3. indecent assault and battery on a mentally retarded person,4, indecent assault and battery on a person who has obtained the age of fourteen,rape,5. rape of a child under sixteen with force,6. rape and abuse of a child,7. assault with intent to commit rape,8. assault of a child under sixteen with the intent to commit rape9. kidnapping of a child under sixteen,10. open and gross lewdness and lascivious behavior,11. unnatural and lascivious acts with a child under 16,12. distribution and trafficking of narcotics or other controlled substances within ten
(10) years of the date of the screen, provided there are no intervening disqualifying offenses,
13. intent to commit any of the above crimes; and14. like violation of the law in another state; or
B. It is determined that: he/she has been adjudged liable for civil penalties or damagesinvolving sexual or physical abuse of children, is subject to any court order involving sexu-al abuse or physical abuse of a minor, including, but not limited to domestic order of pro-tection, has had his/her parental rights terminated for reasons involving sexual or physicalabuse of children; orC. Massachusetts Hockey or its members comes into possession of verified informationthat he/she has a history with another organization (volunteer, employment, etc) of com-plaints of physical or sexual abuse of minors, resigned, been terminated or been asked toresign from a position, whether paid or unpaid, due to complaint(s) of sexual or physicalabuse of minors.
Section 4. ProcessThe Massachusetts Criminal Offender Record Inquiry (CORI”) Committee shall be solelyresponsible for implementing the screening process. The committee shall in its discretionand from time to time publicize the procedure to be used to complete a screen on theMassachusetts Hockey web site.
Each member to be screened shall at the time of his/her registration with USA Hockey orsuch other time as the Committee shall designate use the screening tool provided by theCommittee on the web site to complete his/her screening. Members may be required tobe screened for each distinct function served in his/her various positions in MassachusettsHockey and it associated organizations and members.
It shall be the responsibility of the lead official or a designated representative of associat-ed organizations and members to insure all members required to be screened completethis process.
Section 5 Committee. The CORI Screening Committee shall:A. Be responsible for the screening process;B. Forward to the Massachusetts Corrections History System Board all information
required to obtain CORI information. Forward all information required to obtain CORI information;
C. Review the CORI information returned; D. If relevant information, as defined in Section 4 of this Article is revealed, request that
the President of Massachusetts Hockey (or his/her designee), notify the individual thatrelevant CORI information has been received that would exclude him/her from partici-pating in Massachusetts Hockey and that he/she has ten (10) days to request an opportunity to dispute the accuracy or relevance of said information. Upon failure to make such a request or to satisfactorily explain why this information is not accurate or

relevant, the individual shall be notified in writing by the President of Massachusetts Hockey (or his/her designee) that he/she is excluded from participation in Massachusetts Hockey. Notification shall be in writing, and indicate the individual is not in compliance with Massachusetts Hockey Policy. A copy of this notification shall be forwarded to the individual’s parent organization and the cognizant registrar.
E. Maintain and update a database of information was received and reviewed including posting names of those screened on the Massachusetts Hockey web site and verifying that those individuals registered with USA Hockey are screened.
F. Comply with all legal requirements concerning disclosure, storage, retention and handling of CRI information received;
G. With approval of the Executive Board, audit members to gather data on compliance with these rules, seek better methods for implementation and assist members in implementing these rules.
Section 6. AdministrativeA. Members who refuse to consent to screening may appeal according to the provisionsof section 7 hereinafter.B. All organizations, associations or members that are responsible for ensuring their par-ticipant member comply with the provisions of this Article and fail to do so shall be sub-ject to suspension at the annual meeting of Massachusetts Hockey. There shall be noappeal of such a suspension by the board of Directors.C. In the event of a conflict between the provisions of this Article and any other Article inthese Rules and Regulations, the provisions of this Article XIV shall take precedence.
Section 7. Appeal ProcessMembers who are prohibited from participation in Massachusetts Hockey pursuant to theprovisions of this Article or who refuse to consent to be screened may appeal this actionto the Committee in writing within ten (10) days of receipt of notice of such prohibition.The Committee or a designated subcommittee thereof shall hear all appeals within thirty(30) days of receipt of the request at a place and time and in a manner determined bythe Committee consistent with the practices of Massachusetts Hockey. A decision shall berendered in writing within ten (10) days of the completion of the hearing.
The individual may appeal any adverse decision of the Committee, in writing within ten(10) days of the receipt of same to the President of Massachusetts Hockey who willprocess the appeal consistent with its practices and procedures and these rules and regu-lations. A written decision signed by the members of the Committee rendering same shallbe considered prima facie evidence of the facts stated therein, and the burden of proof asto why the member shall not be prohibited from participation in Massachusetts Hockeyshall be upon the member in any hearing before the panel set up by the President whosedecision shall be final.
Section 8. Complaint ProcessComplaints concerning violations of the provisions of this Article and/or of MassachusettsHockey's Sexual and Physical Abuse policy statements shall be in writing addressed to thePresident of Massachusetts Hockey and shall be signed by the individual making the com-plaint. The complaint shall state in full and complete detail the basis of the complaintconcerning the alleged violations.Upon receipt of such a written complaint, the President shall refer the matter to the CORICommittee for its review. All credible complaints of sexual and/or physical abuse shall bereferred to the appropriate governmental authorities by the President after the CORICommittee’s review.
The CORI Committee or a designated subcommittee may, in its discretion recommendMassachusetts Hockey initiate administrative action against the individual complained ofpursuant to the applicable sections of its By-Laws and these rules and regulations.
The individual may appeal any adverse decision or other action of the CORI Committee in

writing within ten (10) days of the receipt of same to the President of MassachusettsHockey who will process the appeal consistent with its practices and procedures andthese rules and regulations. A written decision signed by the members of the CORICommittee rendering it shall be considered prima facie evidence of the facts stated there-in and the burden of proof as to why the individual shall not be prohibited from participa-tion in Massachusetts Hockey shall be upon the individual in any hearing before the panelset up by the President whose decision shall be final.Section 9. Awareness ProgramMassachusetts Hockey may from time to time incorporate into its various literature andclinic programs educational/awareness information as developed by the CORI Committee,or other authoritative sources, as pertinent information becomes available on the mattersof physical and sexual abuse and this screening process.
Article XIV Medical WaiversMedical waivers to play down one age level may be granted by Massachusetts Hockeyafter review by qualified personal. Medical conditions that might indicate that a playershould be allowed to play down include but are not limited to small stature, cancer orheart disease. To apply for such waiver fill out the Medical Waiver request form and theRelease and Waiver of Liability form which can be found on our web site. Forward bothcompleted forms to you District/Divisional Registrar who will forward it to the appropriateperson for a determination.The following restrictions apply to the waived player.
The Waiver is for only one season and will not be automatically renewedWaivers will not be issued to players on Tier 1 or Tier 2 teams.Waivers will only be issued to players on Tier 3 Teams if it is the lowest level team in the programWaivers will be issued to players on Tier 4 TeamsWaived Players cannot participate in District Playdowns or State Championship Tournaments
Waived players can:Participate in league play only if the league approves the participation at the lower agelevelParticipate in Invitational Tournaments only if the Tournament Director approves it.

POLICIES OF MASSACHUSETTS HOCKEY
PLAYER MOVEMENTIt is the policy of Mass Hockey that players play in their age appropriate classification. Inexceptional circumstances, a player (except for 8 and under) may be rostered in a higherage group with the approval of their association/program. Approval is required at the startof each season.
SEXUAL AND PHYSICAL ABUSEIt is the policy of Massachusetts Hockey that there shall be no sexual and/or physical
abuse of any minor participant involved in or associated with any and all activities orga-nized and sponsored by Massachusetts Hockey by its officers, directors, employees, mem-bers or independent contractors.
Sexual abuse of a minor participant shall be deemed to occur when an officer, director,employee, member or independent contractor touches a minor participant for the purposeof causing the sexual arousal or gratification of either the minor participant or the officer,director, employee, member or independent contractor. Sexual abuse of a minor partici-pant also occurs when a minor player touches an officer, director, employee, member orindependent contractor for the sexual gratification of either the minor participant or theofficer, director, employee, member or independent contractor, if the touching occurs atthe direction, request or with the consent of the officer, director, employee, member orindependent contractor.
Physical abuse shall be deemed to occur when an officer, director, employee, memberor independent contractor has physical contact with a minor participant that intentionallycauses or intentionally creates a threat of immediate bodily harm or personal injury.Physical abuse shall not include physical contact that is reasonably designed to coach,teach or demonstrate a hockey skill or technique. Permitted physical conduct mayinclude, but is not necessarily limited to, shooting pucks at a goaltender, demonstratingchecking and other hockey skills and communicating with or directing participants duringthe course of a game or practice, by touching him in a non-threatening, non-sexual man-ner.
It shall not be a defense to a complaint of sexual or physical abuse that the partici-pant consented, that his/her age was unknown or mistaken or that the act did not takeplace at a hockey function.
It shall be the policy of Massachusetts Hockey to refer all complaints of such sexualand/or physical abuse to the appropriate governmental authorities. MassachusettsHockey may, in its discretion, initiate administrative action against the individual com-plained of pursuant to the applicable sections of its Constitution and By-Laws and maypermanently ban or suspend said individual from Massachusetts Hockey if the allegation isproven to be true.
COACHING CODE OF ETHICSAll coaches who have not previously signed the Coaches Code of Ethics with theirProgram will be required to sign this form prior to starting their coaching activities. Thelocal Program will keep the forms on file.
CONFLICT OF INTERESTOfficers and Directors of the corporation shall not engage in any activity that violates theirduty of loyalty to the corporation and presents a conflict of interest in their position withthe corporation without disclosing the involvement in such activity to the President of thecorporation and without securing approval of the Involvement in such activity from thePresident, Board of Directors, Committee, District or Division as appropriate.
A conflict of interest exists when an individual's actions and/or relationships present thepotential for improper personal gain or advantage or an adverse effect on the interests ofthe corporation as a result of the individual's position in the corporation. Conflicts of inter-est relate to and arise from, including but are not necessarily limited to, self dealing and

interested transactions between the corporation and its Officers and Directors, the pur-chase and sale of goods and services, the use of corporate assets for a private benefit,the interference with or appropriation of a corporate opportunity, the acceptance of gifts,entertainment and/or favors and failure to deal fairly with the corporation and itsDirectors and members.
Additionally, Officers and Directors shall exercise reasonable care not to disclose confiden-tial information acquired in their official capacity with the corporation, the disclosure ofwhich might be adverse to the corporation and no Officer or Director shall use confiden-tial information for his or her own personal gain.
This conflict of interest policy shall apply to the Officers and Directors of the corporationand their family members and any other organization under their control. For purposes ofthis policy, a family member is a spouse, parent, child or spouse of a child, brother, sisteror spouse of a brother or sister. Control exists if an Officer or Director or family memberowns, directly or indirectly, at least fifty (50%) percent of the stock ownership or mem-bership interests of another organization or has the right and/or ability to control themanagement and policies of another organization.
Officers and Directors who anticipate having a conflict of interest shall report the conflictto the President of the corporation, or to an individual or committee designated by thePresident, immediately upon identifying the conflict. The President or his/her designeeshall report the conflict to the Board of Directors, committee or District or Division that isconsidering, or has considered, the transaction to which the conflict applies. The transac-tion shall be approved by a majority vote of the deliberating body but in any case theOfficer or Director who has the conflict may not speak or vote on the matter and shall notbe counted for purposes of a quorum. The record of the proceedings shall reflect therequired disclosures and the Officer or Director's ineligibility to vote.
In the event that there is any inconsistency between this Conflict of Interest Policy andthe laws of the Commonwealth of Massachusetts, the laws of the Commonwealth shallgovern. By virtue of their position as Officers and Directors of the corporation, theirinvolvement in monthly and annual meetings of the corporation and their receipt of copiesof the corporation's By-Laws and Rules and Regulations, the Officers and Directors of thecorporation acknowledge their understanding of and agreement with this Conflict ofInterest Policy.
Any violation of this Conflict of Interest Policy shall be dealt with using the removal proce-dures as set forth in Article IV, entitled "Board of Directors", of the corporation's By-Law.s.
GRANDFATHERINGWithout explicit exemption in the rule none of these Rules and Regulations have any wayshape or form a “Grandfather Condition or Clause”.
USA HOCKEY ASSOCIATE REGISTRARSUSA Hockey Associate Registrars are cautioned against accepting too many jobs andresponsibilities in Massachusetts Hockey. The fact that they have leadership qualities isthe reason for their appointment as District/Divisional Registrars. Two major problemsdevelop from taking on additional duties: 1) not enough time to do one job properly; and2) a conflict of interest when matters of controversy arise.
COACH HELMET USAGE ENFORCEMENT POLICY1. Third Offense- Two game suspension. At the option of the local suspending authority,the offending coach may be allowed to practice with the team during the suspension aslong as he/she wears a helmet.2. Fourth Offense - As per USA Hockey Bylaw 10 (C). Notification on helmet Policy viola-tion pending a hearing by the affiliate or suspending authority - disciplinary hearing mustbe held within 7 days of notification of the violation. Suspension should not be less than30 days.

Massachusetts Hockey, Inc. takes seriously its obligations to preserve information relatingto litigation, audits and investigations. The Sarbanes-Oxley Act makes it a crime to alter,cover up, falsify or destroy any document to prevent its use in an official proceeding.Failure on the part of the Board members and volunteers to follow this policy can result inpossible civil and criminal sanctions against Massachusetts Hockey, Inc. and possible dis-ciplinary action against responsible individuals (up to and including expulsion from theorganization). Each employee has an obligation to contact the President of a potential oractual litigation, external audit, investigation, or similar proceeding involvingMassachusetts Hockey, Inc. The information listed in the retention schedule below isintended as a guideline and may not contain all the records Massachusetts Hockey Inc.may be required to keep in the future. Questions regarding the retention of documentsnot listed in this chart should be directed to the President.
From time to time, the President may issue a notice, known as a “legal hold” suspendingthe destruction of records due to pending, threatened, or otherwise reasonably foresee-able litigation, audits, government investigations, or similar proceedings. No recordsspecified in any legal hold may be destroyed, even if the scheduled destruction date haspassed, until the legal hold is withdrawn in writing by the President.

INTRODUCTIONAll board members and volunteers are to observe highest standards of business and per-sonal ethics in the conduct of their duties and responsibilities. As representatives ofMassachusetts Hockey, Inc., we must practice honesty and integrity in fulfilling ourresponsibilities and comply with all applicable laws and regulations. Set forth below isMassachusetts Hockey Inc.’s policy with respect to reporting good faith concerns aboutthe legality or propriety of Massachusetts Hockey’s actions or plans.
Reporting of Concerns or ComplaintsIt is the responsibility of all board members and volunteers to comply with MassachusettsHockey Inc’s By Laws, Rules and Regulations, Policies, Code of Ethics and Conduct andapplicable law and to report violations or suspected violations in accordance with thisWhistleblower Policy.
ConfidentialityMassachusetts Hockey Inc. will treat all communications under this policy in a confidentialmanner, except to the extent necessary 1) to conduct a complete and fair investigation, or2) for review of Massachusetts Hockey Inc.’s operations by the Massachusetts HockeyInc. Board, its audit committee, Massachusetts Hockey Inc’s independent public accoun-tants and Massachusetts Hockey Inc.’s legal counsel.
RetaliationMassachusetts Hockey Inc. will not permit any negative or adverse actions to be takenagainst any individual for making a good faith report of a possible violation of its By Laws,Rules and Regulations, Policies, Code of Ethics and Conduct or applicable law, even ifthe report is mistaken, or against any individual who assists in the investigation of areported violation. Retaliation in any form will not be tolerated. Any act of alleged retalia-tion should be reported immediately and will be promptly investigated. Anyone who retali-ates against someone who has reported a violation in good faith is subject to discipline upto and including expulsion from the organization. This Whistleblower Policy is intended toencourage and enable individuals to raise serious concerns within Massachusetts HockeyInc. prior to seeking resolution outside the organization.
How To Report Concerns or ComplaintsA complaining individual may communicate suspected violations of its By Laws, Rules andRegulations, Policies, Code of Ethics and Conduct, applicable law, or other wrongdoing oralleged retaliation by contacting Massachusetts Hockey Inc.’s Chair of the DisciplineCommittee. If you wish to remain anonymous, it is not necessary that you give your nameor position in any notification.
Whether or not you identify yourself, for a proper investigation to be conducted, pleaseprovide Massachusetts Hockey Inc. with as much information as you can, sufficient to doa proper investigation, including where and when the incident occurred, names and titlesof the individuals involved, and as much other detail as you can provide.Illustrative Types of ConcernsThe following is a non-exhaustive list of the kinds of improprieties that should be reported:• Supplying false or misleading information on Massachusetts Hockey Inc.’s financial orother public documents, including its Form 990.

federal or state law or regulations.• Altering, destroying or concealing a document, or attempting to do so, with theintent to impair the document’s availability for use in an official proceeding or otherwiseobstructing, influencing or impeding any official proceeding, in violation of federal or statelaw or regulations.• Embezzling, self-dealing, private inurnment (i.e., Massachusetts Hockey Inc.earnings inuring to the benefit of a director, officer, or volunteer) and private benefit (i.e.,Massachusetts Hockey Inc. assets being used by anyone in the organization for personalgain or benefit).• Paying for services or goods that are not rendered or delivered.• Using remarks for actions of a sexual nature that are not welcome and are likelyto be viewed as personally offensive, including sexual flirtations; unwelcome physical orverbal advances; sexual propositions; verbal abuse of a sexual nature; the display of sex-ually suggestive objects, cartoons, or pictures; and physical contact of a sexual or particu-larly personal nature.• Using epithets, slurs, negative stereotyping, and threatening, intimidating, orhostile acts that relate to race, color, religion, gender, national origin, age, or disability.• Circulating or posting written or graphic material in the workplace that deni-grates or shows hostility or aversion toward an individual or group because of race, color,religion, gender, nationality, age, or disability.• Discriminating against an employee or potential employee due to a person’srace, color, religion, sex, sexual orientation, national origin, age, physical or mental impair-ment, or veteran status.• Violating Massachusetts Hockey Inc.’s By Laws, Rules and Regulations,Policies, Code of Ethics and Conduct or applicable law.

Online Registration allows individual Members to register withUSA Hockey. The Waiver of Liability is completed during onlineregistration; copies of Waivers are NOT required for those regis-tering online.
Instruct your members to register online and send theirConfirmation number/page to you. Let your members know the following fees are collected duringonline registration: USA Hockey fee of $40 for age 7&over and $0for 6&under and MASS fee of $8 for age 7&over, $0 for age6&under (birth year 2006 and later). Program Registrar opens Cyber Sport software and selectsClaim Online RegistrationSelect appropriate Member Type and scan barcode or enter num-ber on the Confirmation page for all Participants being processedClick Send/Receive to link the USA Hockey database to down-load requested records. All data the Participant entered duringthe Online Registration process will be downloaded into the LocalProgram’s software. New records are added and existing records,with changes, are updated. Each record is marked asTransmitted thereby completing the registration process andallowing that Participant to be placed on a Team Roster.
Important: You must Transmit all Players and Coaches asinstructed above. You do not need to do data entry, createtransmittal disks or collect USAH and MASS fees whenusing Online Registration. The online registration process is notcomplete until you transmit the confirmation numbers throughCyber Sport to USA Hockey. You may then assign your playersand coaches to teams and create the Official Player RosterForm.
Note: You may use online processing and still create traditionalbatch transmittals if necessary. Contact your Registrar for proce-dures to follow for submitting disk transmittals.

At least ONE of the rostered coachesshould be Certified at Level 3 to begin the season from Squirtteams up to Midget teams and from Girls U10 up to Girls U19.
The software will prompt you to make this change for all TierI and Tier II teams — you will need to remember to assign a Head Coach to allother Youth and Girls/Women’s teams. MASSACHUSETTSprefers a Head Coach be designated for all Youth, HighSchool and Girls/Women’s teams. That “Head Coach” wouldpreferably be the highest certifiedCoach listed on the Team Roster

Non-US Citizen InformationInternational Ice Hockey Federation regulations require that allnon-US citizen players on youth and girls/women’s classificationteams be transferred by means of a letter of approval. There isno fee to process this written transfer. Under USAH Forms inyour software, there are two Youth Transfer Forms – one forCanadians and a separate one for all other non-US citizens.Complete the appropriate form for all non-US citizen youth andgirls/women’s players in your program before games areplayed.
In addition to a completed Transfer, non-US citizen youth and all

female players must also provide proof of resident status in theUS. This information should be kept with the team registrationmaterials and should be provided prior to regular season games.Additional information is available on the Youth Transfer Forms inyour Cyber Sport.
New this year: When a non-US citizen’s transfer is completeand proof of resident status has been approved by the nationaloffice, the player’s record will be marked as ‘Transfer Complete’in the national database. You will be able to update your softwarewith this information and the player’s record will be marked as‘Transfer Complete’ on the Personnel screen in your Cyber SportSoftware. To update, click on the Update Personnel and CEP but-ton on the main screen of your software. There is a new columnon the Player Roster and on the Credential Verification sheet thatidentifies non-US citizens. If the symbol is an ‘O’, the transfer isnot complete. If the symbol includes a ‘T’, the Transfer isComplete.
