以品管專案改善門診調劑調撥藥品成功大學附設醫院藥劑部藥師 徐宛琳、葉鳳英、李珮 、張鳳梅、張慧真
摘要
調劑作業中藥師接收處方之後,須依序判讀處方,並遵循三讀五對完成調劑、交
付藥品及適當指導用藥。當各調劑檯藥品品項或數量不足,在各藥檯間相互調撥藥品
也時有所見。以醫學中心門診服務量而言,藥師深刻感受到調撥次數增加不僅降低調
劑效能,也可能影響藥事服務品質。於101年3月由門診調劑組藥師自發性組成品管圈
(QCC,Quality Control Circle)1-3,經由問題結構探討出主要原因、圈員腦力激盪擬定
可行對策,執行後目標達成率為106%,經過效果確認並納入標準化作業。此活動不
僅改善調劑作業中各檯藥品調撥次數,進而提升藥師調劑流暢性;也透過品管圈凝聚
團隊夥伴之情誼,提升溝通協調等能力,是本專案最佳附加效益。

Abstract
Pharmacist has to evaluate the prescription's appropriateness in dispensing practice, and offer adequate medication consultation for patients. Due to high dispensing volume in outpatient department (OPD) in the national tertiary teaching medical center. It is challenging that inadequate drug supply for each dispensing station. This not only increases drug requisitions (17 times/day) for each section in the pharmacy unit, decreases efficiency in dispensing and affecting quality of pharmaceutical services. Quality control circle (QCC) for solving this problem was initiated by OPD pharmacists in March 2012, we discussed the dominant factors and developed reasonable plans to decrease drug requisitions (target: 8.5 times/day). After 9 months of intervention, the achievement rate was 106% (8 times/day). According to this outstanding outcome, we setup a new policy of drug requisition. Additional benefits of the QCC were that pharmacists improved fluency in dispensing, cohering partnership, and enhance communication and coordination capabilities.
