서론
코르티코스테로이드제(스테로이드)는 피부과질환에서 가
장 많이 사용하는 약제로, 전신 스테로이드제와 국소 스테
로이드제로 구분한다. 역사적으로는 1950년 Hench 등[1]은
류머티즘질환에 대한 스테로이드의 치료효과에 대한 연구
로 노벨상을 받았고, 1951년 Sulzberger 등[2]은 염증성 피
부질환에 스테로이드를 처음으로 사용하였다. 국소 스테로
이드제는 크게 4가지 기전, 즉 혈관수축, 항염증, 면역억제,
증식억제 반응을 통해 작용한다[3]. 국소 스테로이드제의 세
기는 혈관수축 반응 정도에 따라 7단계로 나뉘며, 환자의 나
이, 도포 부위, 병변의 중증도, 환자의 순응도 등을 고려하
여 적절하게 선택하여 사용하여야 한다. 부작용으로는 피부
위축과 모세혈관확장, 튼살, 피부궤양, 여드름모양 반응, 색
소 변화, 감염, 알레르기 접촉피부염 등 다양하게 발생할 수
있다[4]. 이러한 부작용을 최소화하고 효과를 증대시키기 위
해서는 적절한 사용방법을 숙지해야 한다.
스테로이드의 기전
국소 스테로이드제는 아직 기전은 명확하지 않으나, 히스
타민 프로스타글란딘(prostaglandin)과 같은 혈관확장 물
질의 분비 억제를 통하여 얕은 진피의 모세혈관을 수축시
켜 홍반을 감소시키고, 이를 통하여 항염증 작용을 나타낸
다[5,6]. 이러한 혈관수축 능력, 즉 피부를 희게 하는 능력에
따라 역가가 결정된다.
스테로이드의 항염증 작용은 비특이적이나 인지질분해
효소(phospholipase)-A2 억제 및 IL-1,2,3,6이나 TNF-α

Topical corticosteroids are widely prescribed and used. As interest in the use of corticosteroids grows, people are increasingly exposed to misinformation that causes an excessive fear of side effects. Topical corticosteroids mainly have vasoconstrictive, anti-inflammatory, immunosuppressive, and anti-proliferative effects. Corticosteroids are prescribed based on the patient’s age, the site and severity of the lesion, and patient compliance. Adverse reactions include skin atrophy, telangiectasia, hypopigmentation, and steroid-induced acne. To prevent the side effects of topical corticosteroids and to minimize phobias about topical corticosteroids in the future, we suggest providing proper information and education for both physicians and patients about the safety, efficacy, and proper use of topical steroids. In this article, we review basic information about topical corticosteroids and guidelines for their use. Key Words: Steroids; Topical; Utilization; Therapeutics; Adverse effects

같은 사이토카인(cytokine)의 생성과 방출을 억제함으로써
나타난다. 코티솔(cortisol)이 세포 내의 스테로이드 수용체
(steroid receptor)와 결합하면 수용체에 붙어 있던 열충격
단백질이 떨어져 나가고 코티솔과 수용체가 핵 내로 들어
가 DNA의 글루코코르티코이드 반응성 성분(glucocorticoid
responsive element)에 결합하여 mRNA 전사가 일어나고
그 결과로 아넥신(annexin) 1 (lipocortin-1)의 생성이 증가
된다[7,8]. 아넥신1은 phospholipase-A2를 억제시켜 인지
질이 arachidonic acid로 되는 것을 막아 항염작용을 일으
킨다. 반면 포도당 신생 작용을 증가시켜 당을 높이기도 한
다. 이러한 작용을 전사촉진이라고 한다. 두 번째 작용기전
은 핵 내로 들어간 코티솔과 수용체가 p50이나 p65에 붙어
NF-kB가 DNA에 결합하는 것을 억제함으로써 항염작용을
일으키는데 이를 전사억제라고 한다[8]. 또한 여러 염증세포
에 영향을 미쳐 염증을 조절하는데, 혈액 내 중성구는 증가
시키는 반면에 염증 부위의 중성구는 감소시킨다. 또한 단핵
구, 호산구, 림프구도 감소시킨다. 특히 B세포 보다 T세포를
감소시키는 효과가 크며, B세포 감소에는 고용량의 스테로
이드가 필요하다[9,10].
스테로이드제는 이외에도 피부에서 비만세포의 고갈, 호
중구의 화학주성 저해 및 랑게르한스세포의 감소를 유발하
여 면역억제 작용을 나타내는 것으로 알려져 있다. 또한 여
러 종류의 세포에서의 DNA 합성과 유사분열을 억제하는 항
증식 작용도 있다.
국소 스테로이드제의 종류 및 분류
스테로이드는 기본적으로 6각링 3개, 5각링 1개에 17개의
탄소 원자로 이루어진다. 스테로이드제의 강도, 미네랄 코
르티코이드(mineralcorticoid) 효과, 작용시간(생물학적 반
감기), 대사 등은 이들 분자구조에 따라 달라진다. 예를 들
어, triamcinolone acetonide는 hydrocortisone 보다 100배
강력하다. 각질층을 통하여 잘 흡수되게 하기 위해서는 기제
내에 비극성의 친유성 형태가 좋으며 기제에 따라서도 임상
적 효과가 다르게 나타난다.
Table 1은 국소 스테로이드제를 역가별로 분류한 표이다.
먼저 국소 스테로이드는 역가에 따라 효능이 매우 강력한 제
제로부터 매우 약한 제제까지 7군으로 분류할 수 있다[11].
같은 역가의 국소 스테로이드제라 하더라도 제형에 따라 강도
가 달라질 수 있으며, 일반적으로 연고가 크림이나 로션에 비
해 역가가 높다. 연고는 다른 제제에 비해 밀봉효과가 커 작용
이 증강되나 제형의 특성상 가려움이나 모낭염이 유발되는 경
우도 있다. 로션의 경우 넓은 부위에 쉽게 펴 바를 수 있으며
얼굴 등에 선호된다. 크림의 경우 가장 널리 사용되며 건조한
병변에 바르기가 유리하다. 젤이나 거품은 털이 있는 부위에
유용하다. 따라서 국소 스테로이드를 적절하게 처방하려면 우
선 국소 스테로이드제의 역가에 대해 숙지하고 있어야 하며,
성분과 무관하게 표기된 농도에 따라 역가가 달라지는 것으
로 오인하는 환자도 있어 정확한 역가를 알려주는 것이 좋다.
