‘Ceri’s Family’ has proven to be a low cost, innovative, creative tool that has captured the imagination of a diverse range of individuals including service users and the general

public, gaining widespread support.

It has an extensive range of applications to further develop and promote joint working, address health inequalities and ensure meaningful engagement with our local communities

which will lead to services that improve the health and well-being of the people of Denbighshire.
