• Carrier State: “fecal excretors” asymptomatic-->majority of patients
• Diarrhea without colitis: mild, 3-4 loose BM/d +/- cramps
• Colitis : more severe systemic, profuse diarrhea, fever, leukocytosis, abd pain

• Fulminant colitis:– 2-3% of patients, esp elderly, more and more today

DIAGNOSIS

TREATMENT
1. Discontinue offending agent or modify to less offensive agent )successful in 20% to 25%(.
2. Replace fluids and electrolytes.
3. Avoid antiperistaltic agents: may worsen diarrhea or precipitate toxic megacolon.
Severe Disease
• One point each was given for age >60 years.
• Temperature >38.3ºC
• Serum albumin <2.5 mg/dL )25 g/L(
• Peripheral white blood cell count >15,000 cells/microL.
• Endoscopic evidence of pseudomembranous colitis
• Treatment in the intensive care unit.
Severe Disease
• Oral Vancomycin )125 mg four times daily( should be initiated promptly for severely ill patients.
• Severely ill patients with ileus may have markedly delayed passage of oral antibiotics from the stomach to the colon. These individuals may benefit from the addition of intravenous Metronidazole at a dose of 500 mg every eight hours
Severe Disease
• Vancomycin Enema
• Cholestyramine
Prevention
• Restrict the use of antibiotics to the treatment of diseases in which they are essential.
• Alcohol-based hand disinfection does not eliminate C. diff spores, so hand washing is absolutely essential.
• People who are hospitalized with C. difficile have a private room or share a room with someone who has the same illness.
• Hospital staff and visitors wear disposable gloves and gowns while in the room.
