As people age, their skin shows signs of shrinkageand laxity. Whereas shrinkage occurs due to lossof volume from fat compartments (particularlynoticeable in the face), laxity occurs as a result of

reducedcollagenandelastinstructuralsupport, causingglobalskinsaggingonthefaceandbody.Bothofthesechangesoftenleadtoanunattractive, tired lookthatmakespeoplewant toappear more youthful and rested. This can result in reducedself-confidence, low self-esteem, reduced opportunities inthe workplace and/or in relationships, and psychologicaldifficulties(e.g.depression).

Dermalfillersarevitalproductsfortheaestheticpractitioner’s‘toolbox’ to help restore skin structure and replace elementsthat are being drained away with the ageing process. As anaesthetic practitioner, I routinely use a volumising dermalfillerinconjunctionwithneurotoxintocreateasmootherskincanvas and restore volume balance, to help the patient lookmore rested with improved outward vitality. Hyaluronic acid(HA)dermalfillersareusedinmanyofmypatientswithverysuccessfuloutcomes;theypromotehydrationandgivetheskina‘drinkfromtheinside’.Theyaresafe,generallywell-tolerated,suit a wide range of skin types and are cost-effective, as theproductsavailablehaveagoodlongevityinsitu.

When choosing which product to inject, aestheticpractitionersarespoiltforchoice.Personally,Ialwayskeepmyeyesopentonewfillersthatbecomeavailableandpayattentiontohowtheyshowimprovementscomparedwithothers.Safetyandefficacyofproductsissoimportantaspractitionerswanttodeliverthebestandmosteffectivetreatmentsfortheirpatients.Practitionersalsowanttominimisesideeffects,suchaspain,swelling, infection and lumps, as well as deliver a treatment

thatlastsareasonableamountoftimesothatthepatientfeelsthattheyhavespenttheirpennieswisely.

Introducing PerfecthaI was introduced to the Perfectha range of dermal fillers inJune  2014 when I attended the FACE conference. Havingalready used and become very familiar with Ellansé andSculptra, it seemed an obvious choice to try the new dermalfillerofferingfromSinclairPharma’sportfolio.Afterall,ItrustandrespectthecompanyandhavehighregardforbothEllanséandSculptra.

On the surface, Perfectha may seem like ‘just another oneofthoseHAfillers’.However, whenlookingatitmoreclosely,Perfectha comes in all shapes and sizes to perfectly suit eacharea of the face that I would consider treating. I like thepackagingandtheeasywayinwhicheachproductglidesoutoftheneedlewithgentlepressureonthesyringe.ThismeansmyhandsarenotfeelingclumsyandIcanconcentratemoreonfollowingthefacial linesIamchasingwithmyneedle, orthehollowsIamtryingtofillandrevolumise.

Perfecthaisalsoasaferangeoffillers, whichisparticularlyimportanttome. Iamreassuredbythefact that, since2007,over2.5millionsyringeshavebeensoldworldwideandhaveaverylowriskofsideeffects.Thewordhasspreadaboutthesedermal fillers in the press and endorsement by celebrities, includingMichelleHeatonasthenewfaceofthebrand, allowsformyclients to feelateasewhen they realise that someonewhom they identify with has been very happy with theirinjection experience. In fact, I am now seeing patients whoaskforPerfecthabyname;thisisimpressiveforadermalfillerrangethatisstillconsideredayoungsterintheUKmarket.

Mechanism of actionPerfectha’scross-linked(E-Brid)technologyisuniqueasitsgeliscomposedofahighlycovalentbondedelasticgelcombinedwith a small amount of non-cross-linked HA. This mixtureallows for a streamlined injection, as well as an impressivevolumisingandliftingcapacitywherethefiller isplaced. The

non-cross-linked HA aids in ensuring the product integratewithin the target treatment zone and helps Perfectha flowmoresmoothlyoutoftheneedlewheninjected.Thisleadstogreaterpatientcomfort.

ThesafeaspectofusingaHAfillersuchasPerfecthaisthatitshowlydegradesintoinertsubstancesnotharmfultothebody, andcanbedissolvedwithhyaluronidaseinjectionsifanyposttreatmentadjustmentisneededifsymmetryisnotmatching.Injection depthThePerfectharangeallowsforaestheticpractitionerstohaveachoiceinthedepthlevelwheretoplacetheproductwithinthe skin. Perfectha Fine Lines and Complement are greatat smoothing out more superficial lines, especially those onthe lower cheeks and upper lip areas. Perfectha Derm is veryversatile intermsofwhereitcanbeused(e.g. lips,undertheeyes).PerfecthaDeepismypersonalfavouriteasIfeelitsworksreallywellinthelipsandnasolabialareas,marionettelinesandevencheeks.PerfecthaSubskinisusefulfordeepinjectionsandlargevolumereplacement,aswellasrecontouringofthebonefrom age-related resorption or fat compartment shrinkage.Thehandy1mlsyringesareapersonalfavouriteofminewhendoingdeepcheekaugmentation.

High-quality outcomesFollowingtreatmentwithPerfectha,Iquicklyrealisedthatmypatients were delighted with the results. As the products areassociatedwithminimalswelling, thereisashorterperiodofdowntime. The products also create natural-looking resultswhere the client’s face moves in an ‘undone way’ insteadof appearing‘stiff’.

ByusingabiphasicHAfiller,Ialsofeeltheresultsaremorepredictable than a monophasic filler and there is less risk ofover-correction than with monophasic fillers. When Park etal(2014)comparedtwomainstreambiphasicHAdernalfillers(one of which was Perfectha) with a monophasic HA, theresultsshowedthebiphasicHAfillershadlessriskofswellingthanthemonophasicdermalfillers.Itiseasierfortheaestheticpractitionertogaugewhentostopinjectingwhenthisdegreeofreal-timecorrectionisachieved.

Lip enhancement I really like using Perfectha Deep on small, thin lips as itgives a support that exceeds other ranges for this type of lip.Perfectha Fine Lines is effective at plumping up smoker’slinesandrestoringlostvolume.Thekeytoanaturalresult ischoosingtherightproductchoice,avoidingtheover-injectionofvolume,andmassagingandmouldingtheproductintotheareawithdigitalpressureafterinjection.Icontinuallyreassesspatients from the front and side views when I inject and useupto0.6mlofproduct in theupper liparea;more thanthisamountcandistort‘normal’balancedcontours.

PefecthaDermandDeeparesuitableproductstorestorelostvolumetothelipbody.Againmassageisanimportantstep,aswellasensuringthepatientisawarethatthelipareawillhavesomeswellingforafewdayswhiletheproductsettlesdown.

Thegoldenratioandanexperiencedeyeshoulddictatewhichvolume of product to inject. Aesthetic practitioners shouldalwaystryandtrusttheirexperiencedaestheticeyeandavoidbeingledbythepatientwhomaybeaftera‘hyperinflated’un-natural look. The hydration effect of injecting Perfectha intothe lip area produces a natural glow to the skin, which addstremendouslytopatientsatisfactionwiththetreatment.

Non-surgical rhinoplastyPerfectha Deep and Perfectha Derm are suitable for non-surgicalrhinoplasty.Bothcomeina1 mlsyringeandalthough

thefullsyringemaynotbeneededattheinitialtreatment, itisgoodpractice to followupwithyourpatient 1–2weeks later.Anaestheticisnotrequiredastheprocedureisquickandsmallvolumes injected slowly. Perfectha is injected and then easilymoulded to smooth out the nasal deformity. This is where agoodaestheticpractitioner’saestheticeye isvital, as theyareeffectivelyreshapingthenoseandasmallchangemakesabigdifferencetotheface.

This relatively straightforward procedure can be carriedover a lunchtime without much disruption to the patient’sschedule.Minimalbruising, temporaryrednessandafewdays

of soreness are to be expected. The instant psychological liftand confidence boost is easily achieved after this enhancingtreatment, as a small change to the nose—a central focalpointtotheface—makesabigdifferencetohowpeoplelook.Personally, I feel the treatment is excellent value for moneyconsideringthefillerusuallylastover18monthsandisasimpleandquickprocedurewithlimiteddowntime.

Case studiesCase study oneA 46-year-old non-smoker presented to the clinic with clearsundamagetoherskin.Althoughshehadgoodnaturalcheekvolume,hermainconcernrelatedtohernasolabiallines.Asthepatientwantedaninstantresultwithgoodlongevity,Idecidedtoinject1.5mlofPerfecthaDeeponeachside,usinganeedle.Thisprocedurewasdone12monthsagoandthepatientfeelsthattheproducthasnotshownanysignsofshrinkage.SheisdelightedwiththeresultsandhasrequestedPerfecthaforherfutureHAinjectiontreatments.SheisalsoembarkingonskinpeeltreatmentstocombatthesundamageandcomplimenttheHAtreatment.

Case study twoThis patient was a 63-year-old non-smoker. She waspostmenopausal and had thinner, looser skin related to theageing process and a lack of hormones. Her volume loss wasasymmetrical; it was worse on the right of her mouth andlinesoccurringatthesideofhermouthweremakingherlooktired.Forthispatient,IusedaneedletoinjectPerfecthaDerm(0.6mlinleftsidenearmouthedgeand1mlinrightside)andDeep (0.7ml in nose to mouth line on right). Although thepatienthadsomebruisingontherightsiderelatedtofrailtyofskin,thefinaloutcomewasexcellentandthepatientwasveryhappywithherresults.SheisnowconsideringhavingPerfecthaSubskintohelprecontourherjawlineand-prejowlsulcusarea.

Long-lasting resultsWith Perfectha, I feel confident I can hit my target 100% ofthe time (or near enough!). After having immersed my clinicwithPerfectha,Ihaven’tlookedbackandcertainlyfeelthatmydermal filler patient results have improved. The long-lastingresults are essential when my clients are wanting value formoney.Perfecthadeliversthisandinturnreflectsonmewellas an aesthetic practitioner. Without a doubt Perfectha hasa lottodowiththis, butI liketothinkmaybemyinjectablesexperiencehassomethingtodowithittoo!
