To all TPSS Employees. This year’s Cafeteria Plan Open Enrollment Period has begun! For your convenience, attached is a schedule of where Taylor & Sons will be during the enrollment and a copy of this year’s Employee Benefits Booklet. You may also link to the Employee Benefits Portal through the Tangipahoa Parish School Board website. Log in to: www.tangischools.org . You would then click on “Departments”, then click on “Payroll”, then click on “Insurance Info” http://www.tangischools.org/Page/11974 A screen will be displayed giving an overview of products, premiums, claim forms, etc. This year, we will be replacing Reliance Disability with Symetra Disability. Additionally, Reliance Group Term Life will be replaced with Mutual of Omaha Group Term Life. Also new this year is an accident plan with Combined Accident. Premiums are as low as $9.96/month! A reminder, if you are enrolled in the Flexible Spending Plan, YOU MUST RE-ENROLL whether or not you will be changing your monthly contribution. ► Open Enrollment is your opportunity to make changes to your insurance benefits for the 2014-2015
Benefit Year.
► Every employee is asked to update their information, including but not limited to, address and
phone numbers, with the School Board Office. During Open Enrollment, Taylor & Sons will
assist the School Board Office in verifying and collecting this information. If any changes occur
after Open Enrollment, please report these changes to the School Board Office. You are
responsible for notifying insurance carriers of these changes. Change forms can be obtained
from Taylor & Sons.
What is Section 125, Pre-taxing of Premiums? Also referred to as the Cafeteria Plan, premiums of certain
benefits can be reduced from your salary before taxes are applied. Ask your benefits representative to review
the example below. There is no cost to participate. Pre-tax elections cannot be changed until the next open
enrollment unless you experience a life changing event as defined by the Federal Government (i.e., marriage,
divorce, legal separation, birth/adoption, death or a change that otherwise affects the eligibility status of your
benefit). These are called “qualifying events”. You also have certain rights to revoke elections under FMLA.
It is your responsibility to contact the School Board Office in a timely manner (within 30 days of the event) to
act and preserve any rights.
EXAMPLE: The following illustration is an example of how the Section 125 program can help you make the
most of your paycheck. This illustration is based on an employee with a gross salary of $20,521.08, married
with 3 federal withholding allowances. The monthly salary is $1,710.09. Qualified benefit expenses (including
hospitalization, dental, cancer, and certain types of life insurance) equal $196.92. (Please note your individual
results will vary.)

All full-time employees, their spouses, and/or children to age 26 are eligible for coverage. ID cards and a
benefit brochure will be provided to those who enroll. Tangipahoa Parish School Board gives their
employees an option between two different plans. Both plans have the exact same benefits EXCEPT with
the lower option plan (IN-NETWORK BENEFITS ONLY) you MUST go to an in-network dentist or
NO benefits are paid.

The Tangipahoa Parish School Board provides employees the opportunity to purchase vision coverage
through AlwaysCare Vision. One exam per year is covered in full after a $10 co-payment, if you go to an
AlwaysCare provider. If you go to an out-of-network provider, the exam is covered up to $35.
TASC (Medical & Child Care Reimbursement) Flexible spending account can help you save 30% on Medical Expenses and Child Care.
MEDICAL
You can set aside up to $2,500 per plan year for medical expenses for you and your family.
CHILD CARE
Set aside up to $5,000 per household for child care ( or care for your older tax dependents) per calendar year.

You need to use up your election each year. The funds don’t roll forward.
Once Open Enrollment has closed, you generally can’t change your election.
You have to re-enroll in these programs each year, even if you don’t want your election amount to change.

Fidelity Life Insurance with Long-Term Care
Almost 50% of us will, at one time or another, utilize home health or experience a
confinement to an assisted living facility of some sort . Confinement costs as much as $5 ,000
per month or more! Long-term care is typically difficult to purchase due to the strict
underwriting procedures used by insurance carriers.
This plan provides the following unique benefits:
Guaranteed Issue (no health questions) for employees who have not applied for
coverage previously
Life insurance protection: Benefits paid tax-free to your beneficiary upon death .
Future insurabil ity option: If you enroll in the minimum, which is as low as $4.00
per week, then you can always increase benefits at each annual enrollment
regardless of your health.
Premiums are level to age 120. Even if you leave your employer you can maintain
the coverage at the same rates.
Long-term care benefits are paid up to 75 months. Most plans today only pay
benefits up to five years. Benefits are received tax -free and paid in addition to any
other sources.

Loyal American Critical Illness Insurance Critical Illness Insurance covers you and/or family in the event you have a heart attack, stroke, major human
organ transplant, end stage renal failure, paralysis, coma, severe burns or cancer. It gives you a tax-free benefit
up of $5,000 up to $50,000.
As an employee of Tangipahoa Parish School Board, it is Guaranteed Issued up to $10,000 during open
enrollment. This means absolutely no health questions or past medical questions.

Easy to apply: employee, single-parent coverage or two-parent family coverage available.
The policy is portable.
Employees can apply for a benefit amount of $5,000 up to $50,000.

Voluntary Long Term Income Protection insurance covers employees who become disabled as a result of an
injury or sickness for a period beginning after, and extending beyond, what is normally covered by Short Term
Disability insurance. It replaces a percentage of the income employees would have earned had they been able
to continue working. This is important to an employer as a benefit that will attract and keep good employees.
In addition, it is important because salary continuance can be an administrative headache and expensive.
Who is eligible?
All active, full-time employees.
When do my benefits begin?
You may select from the following elimination period options:
Option 1 - 14 days for injury, 14 days for sickness*
Option 2 - 30 days for injury, 30 days for sickness*
Option 3 - 60 days for injury, 60 days for sickness
Option 4 - 90 days for injury, 90 days for sickness
Option 5- 180 days for injury, 180 days for sickness
* If you are hospital confined as an inpatient for your disability and have selected an elimination period of 30 days or
less, benefits begin immediately. Inpatient means an individual who is physically confined for an overnight stay, as a
registered bed patient in a hospital or institution, as defined in the policy or plan.
What is the benefit amount?
You may elect a monthly benefit in increments of $100 from a minimum of $300, up to a maximum benefit of $10,000,
not to exceed 66.67% of your covered salary. If at any time the monthly benefit you have chosen exceeds 66.67% of your
covered salary, your benefit amount will be reduced to the highest increment for which you are eligible.

Eligibility Employees: Each active, full-time employee working 30 or more hours per week, except any person working on
a temporary or seasonal basis.
Dependents: You or your spouse must be insured in order for dependent children to be covered.
Dependents are:
Your legal spouse under age 70.
Your unmarried financially dependent children* age 14 days to 20 years (to 26 years if full-time student).
* Natural and adopted children upon finalization of adoptions; stepchildren and foster children living with you.
Upper age limits do not apply to handicapped children.
A person may not have coverage as both an Employee and Dependent.
Only on insured spouse may cover Dependent Children.
Benefit Amount Employee & Spouse: Choose from a minimum of $10,000 to a maximum of $500,000 (in $10,000 increments)
for yourself and/or spouse. The benefit amounts chosen need to be the same.
Eligible Dependent Children: Age 14 days to 6 months: $1,000; Age 6 months to 20 years of age (26, if full-
time student): Choice of: Option 1: $5,000 Option 2: $10,000.
Choose one benefit amount for all eligible children in family.
Guarantee Issue Employee: under age 60: $10,000 Age 60 but under age 70: $0 Age 70 or older: none
Spouse: under age 60: $10,000 Age 60 or older: none
Guarantee Issue is subject to underwriting rules and is not available in all circumstances.

Terms, definitions and other useful information:
Accidental Death & Dismemberment Insurance Benefit – the total benefit for which an individual is insured under this
plan is subject to the maximum benefit if death is due to an accident (confirmed by coroner) or if loss is a defined (read
policy for details) bodily dismemberment.
Accredited School – (as related to dependent coverage of eligible children) an accredited post-secondary institution of
higher learning for full-time students beyond the 12th grade level.
Active Employment – you are working for your employer for earnings that are paid regularly and that you are performing
the material and substantial duties of your regular occupation. You must be working at least the minimum number of
hours as described under each plan. Your worksite must be the employer’s usual place of business; an alternative work
site at the direction of your employer, including your home; or a location to which your job requires you to travel.
Normal vacation is considered active employment. Temporary and seasonal workers are excluded from coverage.
Annual Enrollment Period – means a period of time before the beginning of each plan year.
Beneficiary – the primary person/entity you name to receive the benefits of the policy upon your death.
Contingent Beneficiary – the secondary person/entity you name to receive the benefits of the policy upon your death.
Coordination of Benefits – a policy will coordinate benefits (pay reduced benefits) when you receive sick pay, or other
income such as state compulsory benefits, other group insurance or other retirement system benefits, as described in
Louisiana Revised Statutes 17:1202. Your policy contains specific information with which you should be familiar.
Disability – most insurance companies have their own specific definition of “disability”, but most consider one disabled
when a person is unable or limited to perform the material or substantial duties of your regular job due to sickness or
injury, and you have a (specified) loss in monthly earnings due to the same sickness or injury. Review your disability
policy. It contains specific information with which you should be familiar.
Elimination Period – the period of continuous disability which must be satisfied before you are eligible to receive
benefits.
Evidence of Insurability – a statement of your (or your dependent’s) medical history which can be used by an insurance
carrier to determine if you (or your dependent) is considered a good risk for coverage. Cost of such medical history varies
from carrier to carrier, provider to provider and in some cases done at no charge.
Grace Period – the period of time following the premium due date during which the premium payment may still be paid
to keep the policy active.
Term Llife Insurance – life insurance for a specific period of time at a specific rate; no cash value build up; pays only a
death benefit.
Universal Life Insurance – permanent life insurance that features a cash value.
Rider - an addition to an insurance policy sold as an addendum to enhance benefits to include other features or cover
other dependents.
Waiting Period – the continuous period of time that you must be in active employment in an eligible group before you are
eligible for coverage under a plan.
