Product Description Transfer of a Collocation site to an assuming CLEC from a
vacating CLEC. Offered for Caged Physical, Cageless Physical, Virtual, and ICDF
Collocation. All other types of Collocation to be transferred will be handled on an Individual Case Basis.
Two types of Collocation Transfer of Responsibility: Transfer without working circuits Transfer with working circuits
Transfer with working circuits is offered if administration changes are required and does not involve translations activity.

Ordering Prior to submitting a Collocation Transfer of Responsibility
request: Assuming CLEC is required to have an Interconnection
Agreement with Qwest. Also will need to amend any previously-existing ICA if it does not include services to be transferred.
Vacating and Assuming CLEC must sign the Collocation Transfer of Responsibility amendment.
Process begins by the assuming CLEC submitting the Collocation Transfer of Responsibility Application form and submitting to colo@qwest.com and rfsmet@qwest.com.

Ordering continued… In addition to the application form the following information will
need to be included: Collocation Transfer of Responsibility Spreadsheet, if there
are working circuits - providing detail on each working circuit to be transferred.
Qwest Transfer Authorization Agreement, signed by the vacating and assuming CLECs.
Letter of Authorization, if the Collocation site has a CLEC-to-CLEC (Direct) arrangement, signed by the assuming CLEC and the CLEC partner.
Email verification from the vacating CLEC representing that all of its end users have been notified of the transfer, if there are working circuits.

Ordering continued… Qwest will review the required documentation, check the financial
status and confirm that no one is waiting in the queue for Collocation space in the requested Central Office.
Qwest will initiate a quote to the assuming CLEC, who is required to pay 100% as acceptance of the quote.
If the transfer is with working circuits, the CLEC must submit LSRs. Circuits submitted via an ASR will be processed based on the information provided on the Collocation Transfer of Responsibility Spreadsheet.
At the completion of the transfer, the vacating CLEC, assuming CLEC and Qwest will sign the Qwest Services Transfer Agreement.

Rate Elements Vacating CLEC will not incur charges for the transfer of the
Collocation site. The quote for the assuming CLEC will reflect the following:
Nonrecurring charges associated with the transfer of the Collocation site:
– Assessment Fee– Network Systems Administrative Fee
Nonrecurring charge(s) for processing Interconnection circuits, if applicable.

Product Description Allows access to Unbundled Network Elements (UNEs)
without collocating physical equipment in a Qwest Wire Center FC Collocation takes place on a Termination Block
or Termination Panel engineered by Qwest Allows termination of a copper or fiber cable at a
demarcation point Available for cross connection to DS0 and DS1 level UNE
services. DS3 levels of service, or DS1 utilizing fiber are available via the Bona Fide Request (BFR) Process

Facility Connected Collocation

Product Description Allows CLECs to reduce primary or secondary DC power feeds
to their collocation site Primary feed may be reduced to a minimum of 20 amps Secondary feed may be reduced to 0 amps while holding cabling
and fuse position for future use (pending availability) Provides an option to hold power cabling and fuse or breaker
position on a power board or Battery Distribution Fuse Bay (BDFB) for future power augment requests
Where shortage of fuse position is imminent, CLEC has option to energize secondary feed to minimum 20 amps, or relinquish held fuse position to Qwest

Ordering Procedure CLEC must sign the DC Power Reduction Amendment CLEC completes power section of the Collocation Application
and submits to Qwest as described in the Collocation PCAT Qwest Collocation Project Management Center (CPMC) will
notify CLEC of any Application deficiencies within 10 calendar days of receipt
Walk through of collocation site is performed to determine work required to perform power reduction

Billing Billing to CLEC for initial power will be reduced upon receipt of
payment for the charges quoted for the DC Power Reduction Reduced billing will be made effective back to the date of
acceptance of the Power Reduction Application Recurring billing for the Power Maintenance Charge will
terminate upon power restoration or fuse position return (for secondary feed)
