If you are referred to a specialist, upon completion of your appointment, go to the Referral Managment Center (RMC) to process your referral. Located in Main Clinic on the second floor with TOPA• Bring your planner/calendar to set appt• RMC will provide instructions and directions
If you cannot get to RMC, send message via MiCare (RelayHealth) or call (618) 256-WELL (select option for RMC) to process your referral
Can also answer questions and provide assistance for Prime patients (Prime enrollment is highly encouraged)

Eye exams (preventive) does not include contact exam, contacts, or eye glasses
Prime ADFM – Age 3 and up – 1 per year Annual Cancer Screenings (based on age & risk factors)
Routine Well Woman Exam (PAP smears) – age 18 or sexually activeMammograms – women age 39 or >Colonoscopy – age 50 w/ certain risks factors

Medical Records/Patient Administration
Medical records are property of the U.S. Govt; service members not allowed to hand carry, but may obtain copies thru Release of Information office (basement)
• If you’re referred, the RMC will fax pertinent clinical information to your off-base specialist
• Please give Radiology 48 hours notice, if possible, to make copies of any special X-rays that you need to take to an off-base provider
CHCS registration into local MTF clinic database prior to appointments being made (accomplished through Patient Admin located at 310 W Losey St, bldg 1530, room B230, second floor of the MTF.
