demonstration projects for people with chronic conditions Payment: tiered to reflect the severity/number of

chronic conditions Not limited to per-member per-month (PMPM) Chronic conditions include: a mental health

condition, a substance use disorder, asthma, diabetes, heart disease and obesity

based health teams to support primary care providers Payment: tiered to reflect the severity/number of
