Introduction In an effort to streamline the Holter Enrollment Process CardioNet has updated the Online Holter System. This Practice

Reference Guide is intended to walk you through the process as well as highlight the new features and changes.

Enrolling a Patient How to submit an online enrollment for Holter Services using the Consolidated Holter Form.

1. Log onto access.cardionet.com

2. Enter User Name and Password

3. Select “ENROLLMENT” Tab

4. Select “Holter Enrollment” Tab

Update: All Holter prescriptions, regardless of product/ device type are now enrolled through one consolidated online enrollment form. The Data upload will be done through a separate process.

5. Enter Patient Demographics

6. Select “Continue”

7. Enter Physician and Practice Information

8. *Enter Holter Information*

9. Enter Insurance Information

10. Select “Sign and Submit”

*Holter Information Section Updates* (Step 8)

To enter a device that is not in your inventory:

1. Select [SN NOT LISTED]

2. Select Holter Device Model

3. Enter Serial Number

4. Proceed with Enrollment (Sign and Submit)

Update: Pacemaker field has been added

to the holter enrollment. This field is

required.

*Update(s): All Holter Devices associated with the Practice will appear in the drop down menu. A new feature allows users to enter a Serial # for a Holter Device that is not listed in the Practice Inventory. This feature is not available for CardioKey or ePatch.

Message 1

Upon entering the Device Serial Number manually, the system will generate one of two messages.

Message 1: Requesting a Confirmation. The system is

asking the user to confirm that the device serial

number entered is correct.

Message 2

Message 2: Warning to User. The system is indicating

that the Device Serial # entered is associated with an

existing enrollment, and the Holter file for that

Enrollment has not been uploaded.

Holter Upload Instructions for Uploading a Holter File via the CardioNet Access Site.

1. Log onto access.cardionet.com

2. Enter User Name and Password

3. Select “ENROLLMENT” Tab

4. Select “Holter Upload”

Update: Holter File Uploads are now entered separately from the enrollment form. Prior to uploading the file the online enrollment must be completed.

5. Select “Browse”

6. Identify appropriate file and select “Open”

7. Select “Retrieve”

CardioKey/ ePatch

8. Select corresponding enrollment from list of patients

a. This step does not apply to CardioKey or ePatch

b. If Serial # was entered manually, it will appear in this list

Update: CardioKey and ePatch have the device serial #’s imbedded into the file name. As a result for those device types only, the patient file will tie to the upload upon selecting “Retrieve” in the previous step (Step 7).

Patient Demographic information will appear upon selecting Holter enrollment, or will appear automatically for CardioKey or ePatch after selecting “Retrieve”

9. Confirm that patient information is accurate

10. Select “Upload”

11. Confirm Holter Upload is for the correct patient

12. Select “OK”

13. Diary Entry

To submit diary entries manually proceed to step 14

If the patient did not provide a diary proceed to step 18

14. To submit diary entries manually select “Enter Diary Entries Manually”

15. Enter information in to all fields

16. Select “Add Entry” (Continue for each entry)

17. Select “Upload”

18. Select “Submit” to Complete

Locating Reports Locating reports through CardioNet Access.

1. Log onto access.cardionet.com

2. Enter User Name and Password

3. Select “Reports” Tab

4. Select appropriate Report Search Tab

To locate a report entered through the Rozinn Platform:

1. Select “Historical Holter Search Tab”

2. Enter search criteria

3. Select Search

Historical Holter Search Tab: Reports for Holter Enrollments entered via the Rozinn Platform. Search Tab: Reports for Holter Enrollments submitted through CardioNet Access.

To locate a report entered through CardioNet Access:

1. Select “Search”

2. Enter/ Select Search Criteria

Data Clearing (ePatch Only) IMPORTANT: DATA MUST BE CLEARED FROM THE EPATCH BEFORE USED ON A NEW PATIENT.

PRIOR TO DATA CLEARING, ENSURE THAT THE FILE WAS SUCCESSFULLY UPLOADED TO THE APPROPRIATE PATIENT FILE.

1. Under “Computer” locate and select “EPATCH” drive

2. Select the EPS file

3. Click the “Delete” key on your keyboard

4. Click “Yes” to permanently delete
