Advances in Hair Transplantation

› Orentreich first used the punch graft technique for hair restoration.

› Scalp flaps and scalp reduction then came in.

› Modern day hair transplantation then came into being first being the strip technique or FUT.

› FUE then came into vogue.

Background

› The strip method is performed by taking a strip from the hormonally resistant area over the back of scalp.

› The strip is then dissected into individual follicular units.

› Slits are made over the bald area and then the follicular units are individually transferred.

› This method is highly reliable.

› At present around 90% hair transplantations are performed using this technique.

FUT (Follicular Unit Transplant)

› Linear scar over the back of the scalp a problem in those who keep their hair short.

› Long healing time.

› Pain & paresthesia over the donor site.

› Difficulty in scalp closure in people with inelastic scalp.

Downsides of FUT

› TRICHOPHYTIC CLOSURE attempts to make the scar less visible by a specialized method of closure wherein hairs start growing through the scar itself. If the scalp elasticity is limited, this might not be possible however.

› Partial longitudinal FUT extracts partial longitudinal follicular units that can be used as complete units.

› Individual follicular units are extracted using sharp punches.

› There is no linear scar which is useful for people who keep short hair.

FUE (Follicular Unit Extraction)

FUE

› Patient has to be completely shaven off during donor harvesting; not useful in women.

› Higher rate of follicular transection.

› Slow & expensive.

› Can result in spotted scars.

Downsides of FUE

› Robotic hair transplants: harvesting of follicular units using robots.

› Use of micropunches in harvesting.

› Body hair transplantation using the concept of FUE.

› Beard hair grafts.

Advances in FUE

› Use of platelet rich plasma (PRP) and growth factors isolated from the patient’s own blood are injected into the transplanted and donor areas.

› Claims to reduce healing time, stimulates stem cells and growth of transplanted hair.

› Scientific evidence regarding these claims are lacking at present.

Biofollicular transplants & PRP

› FUT is still the most commonly used technique and gives reliable results.

› FUE in experienced hands is a good option but is expensive and is overhyped at present.

› It is better not to waste your money on PRP!

› Finally, do a detailed analysis and then go ahead with hair transplantation.
