• Acts as the “Front Door” to obtaining prenatal care and services, along with OHP. Patient Navigation/Case management services.

OMC tracking forms

Each site sends a one-page tracking form to the state OMC office for each client.

Problems:Required faxing and maintaining reams of paper documents

All information had to be manually input from paper forms into a database

Required OMC staff to call/email local sites to get any missing information
