En nuestra profesión, la curiosidad científica es algo inherente y casi
imprescindible para disfrutar de su ejercicio. El deseo de averiguar la causa de algo
que no entra dentro de la normalidad, constituye el motor que nos permite
abandonar nuestros límites, dispuestos a explorar y conocer más sobre el tema en
busca de una respuesta.
El laboratorio nos ofrece la oportunidad de utilizar la curiosidad a modo de
herramienta para encontrar respuestas y a la vez expandir el conocimiento y
experiencia adquiridos a nuestro entorno.

Esta cualidad que nos lleva (o debería llevarnos) a preguntarnos constantemente “por qué “, no es otra
cosa que la premisa básica que viene sustentando al método científico desde sus primeros pasos con Galileo.
Otra ventaja añadida es que el proceso de investigar, nos ayuda a no olvidar.
Los seres humanos somos esencialmente curiosos, y esa virtud ha favorecido el descubrimiento de
grandes inventos, como la penicilina por el gran Alexander Fleming, o la radioactividad que le valió el Premio
Nobel a Marie Curie. Si bien muchos pensaréis que estos descubrimientos fueron fruto de la casualidad y no
de la curiosidad, he de decir que es necesario tener la voluntad y la disposición para explicar lo que a priori
no entendemos, de otra manera el hallazgo no pasa de ser un suceso anecdótico que se pierde en el olvido.
Albert Einstein afirmaba: “No tengo un talento especial, solo soy apasionadamente curioso”.
Desde Laboratory Medicine at a glance queremos exprimir vuestra curiosidad científica. Queremos
fomentar la investigación de todo lo que no encaja, que compartáis con nosotros vuestras imágenes más
curiosas para que aprendamos todos, para servir de inspiración a otros.

El líquido sinovial (LS) es un dializado del
plasma que recubre interiormente las articulaciones.
Se forma mediante un proceso de ultrafiltración a
partir de la red vascular presente en el tejido sinovial
y en el cual destacan dos tipos de células conocidas
como sinoviocitos tipo “A” y “B”. Los de tipo B tienen
función secretora y agregan, entre otras sustancias,
mucopolisacáridos con alto contenido en hialuronato
que le proporciona al LS su viscosidad característica
mientras que los de tipo A tienen función fagocítica.
El estudio del LS es solicitado con frecuencia al
laboratorio clínico para orientar el diagnóstico hacia
una posible artropatía por cristales, infecciosa u otro
tipo de disfunciones reumatológicas como monoartritis
o derrame articular. Según recomendaciones del
Colegio Americano de Reumatología el estudio del LS
se agrupa en:
Estudio macroscópico, referido a las
características físicas como volumen, color,
viscosidad…etc.
Estudio microscópico, que incluye el recuento
celular y la búsqueda de cristales mediante
luz polarizada o tinciones.
Estudio microbiológico, con tinción de Gram y
cultivos.
Estudio bioquímico.
Cada uno de estos estudios proporciona
información complementaria que permite valorar el
estado de la articulación y orientar hacia el
diagnóstico.
El estudio de cristales en LS es básico para el
diagnóstico de la artropatía por microcristales y
constituye el método de referencia para el diagnóstico
de la gota. Los tipos de cristales responsables de la
artritis gotosa son el urato monosódico monohidrato
Synovial fluid (SF), is a dialysate of plasma that
internally covers the cavities of synovial joints. It is
formed by ultrafiltration of plasma in the synovial
capillaries.
There are two types of cells on the synovial
lining known as type "A" and "B" synoviocytes. The
type A cells have phagocytic functions while the type
B cells with secretion functions, add to the SF among
other substances, mucopolysaccharides with high
hyaluronate content that gives it the characteristic
viscosity.
The SF analysis is frequently requested to the
clinical laboratory to guide the diagnosis of crystal
arthropathies, infections and other rheumatological
dysfunctions such as monoarthritis or joint effusion.
According to the recommendations of the
American College of Rheumatology the SF evaluation
includes:
Macroscopic study, refers to physical
characteristics such as volume, color,
viscosity, etc.
Microscopic study, includes the cell count and
the examination for crystals with polarized
light or stains.
Microbiological study, Gram stain and
cultures.
Biochemical analysis.
Each one of these studies provides complementary
information that allows clinicians to assess the state of
the articulation and guide to correct diagnosis.
The analysis of crystals in SF is basic for the diagnosis
of microcrystal arthropathy, it is considered the
reference standard for the diagnosis of gout.

(artropatía más frecuente), el pirofosfato cálcico
dihidratado, que provoca la conocida seudogota o
condrocalcinosis (segunda causa más frecuente de
artropatía), y con menor frecuencia oxalato cálcico,
apatita y otros fosfatos cálcicos básicos y lípidos
(principalmente colesterol).
Diferenciar los cristales de urato monosódico y
de pirofosfato es de gran importancia ya que son los
cristales más prevalentes. Su identificación requiere
de la ayuda de un microscopio con luz polarizada y
sobre todo de personal con experiencia porque su
forma y características ópticas pueden crear
confusión:
Cristales de urato monosódico (UMS). Tienen
forma de aguja (esporádicamente de
esferulito) e intensa birrefringencia con
elongación negativa de forma que los
cristales paralelos al eje del compensador se
observan de color amarillo y los
perpendiculares azules.
Cristales de pirofosfato (PP). Pueden presen-
tar varias morfologías, aunque la más típica
es de bastones cortos. Son cristales con
birrefringencia débil y elongación positiva, de
manera que los cristales paralelos al eje del
compensador se observan azules y los per-
pendiculares amarillos.
Los cristales deben ser rastreados con un
objetivo de 10x y evaluados como mínimo con el
objetivo de 40x, prestando especial atención a las
áreas celulares. El examen completo del líquido
requiere usar el objetivo 100x ya que pueden existir
microcristales en gran cantidad que causen la
patología y pasen desapercibidos.

arthropathy), calcium pyrophosphate that causes
Pseudogout or chondrocalcinosis (second most
common) and less frequently calcium oxalate, apatite
and other phosphates basic calcium and lipids (mainly
cholesterol).
Differentiating the crystals of MSU and
pyrophosphate is of great importance since they are
the most prevalent crystals. Its identification requires a
polarizing microscope and above all of trained
personnel to recognize the shape and optical
characteristics.
MSU crystals: Needle-shaped crystals with
an intense negative birefringence (crystals
parallel to the compensator axis appear
yellow and if they are perpendicularly blue).
Calcium pyrophosphate crystals (CPP). The
CPP crystals are rhomboidal and positively
birefringent (crystals parallel to the
compensator axis appear blue and if they are
perpendicular yellow).
The crystals must be tracked with a 10x
objective and evaluated at least with the objective of
40x, paying special attention to the cellular areas. The
complete examination of the liquid requires the use of
the 100x objective to evaluate the presence of
microcrystals in large quantities that can cause
pathologies and go unnoticed.
We present a case of a 50-year-old man who
presented with intense pain, increased volume and
local heat in the left knee of 24 hours of evolution.
Serum colouring gradually faded out until the
fourth day. The patient’s progress was favourable,
probably because no serious organ failures were
presented. On the fifth day, the patient was
discharged.

Se presenta un caso de varón de 50 años que
acude por dolor intenso, aumento de volumen y calor
local en rodilla izquierda de 24 horas de evolución. Se
realiza punción de la rodilla accediendo por el receso
prepatelar externo y se obtiene líquido de aspecto
turbio, recuento de leucocitos: 86.000 cel/µL y
abundantes cristales compatibles con urato
monosódico.
We present a case of a 50-year-old man who
presented with intense pain, increased volume and
local heat in the left knee of 24 hours of evolution.
Puncture of the knee is performed by external
prepatellar and cloudy liquid is obtained, white blood
cell: 86,000 cel/µL and abundant crystals compatible
with monosodium urate.

Presentamos una imagen de una extensión de
sangre periférica, teñida con May-Grünwald Giemsa
(Figura 1) en la que se observan eritrocitos con
múltiples inclusiones de gránulos azulados, pequeños
e irregulares, que ocupan una porción del hematíe.
Las inclusiones descritas también se observan en
eritroblastos, adoptando disposición perinuclear.
Estos gránulos, sugerentes de Cuerpos de
Pappenheimer, son morfológicamente distinguibles de
otras inclusiones visibles con dicha tinción (Howell-
We present an image of a peripheral blood
smear, stained with May-Grünwald Giemsa (Figure 1).
This picture shows erythrocytes with multiple
inclusions of bluish, small and irregular granules that
occupy a portion of the red blood cell. The inclusions
described are also observed in erythroblasts, adopting
a perinuclear disposition. These granules, suggestive
of Pappenheimer bodies, are morphologically different
from other visible inclusions with the same type of
staining (Howell-Jolly and basophilic stippling). This

de hierro intracelulares evidenciados posteriormente
mediante la tinción de Perls (Figura 2).
La presencia de los cuerpos de Pappenheimer
no es patognomónica de ninguna enfermedad en
concreto, sin embargo, puede orientar el diagnóstico
etiológico de:
Anemia sideroblástica congénita o síndrome mielodisplásico tipo ARSA.
Esplenectomía.
Talasemias.
Drepanocitosis.
Tóxicos (Saturnismo, Isoniazida).
Las imágenes pertenecen al caso de una
paciente de 84 años remitida a Urgencias ante el
hallazgo de valores críticos de hemoglobina
detectados en la analítica de rutina. A su llegada a
Urgencias presentaba deterioro del estado general,
sin fiebre, hipotensión sin taquicardia, palidez muco-
cutánea y sin signos de sangrado activo. Destacaba
la presencia de una herida en sacro con supuración
marronácea verdosa y maloliente.
Analíticamente, llamaba la atención la
presencia de una anemia severa macrocítica,
arregenerativa (Hemoglobina: 4,5 g/dL,VCM: 102 fL y
reticulocitos: 5,55 %, pero con índice de producción
reticulocitaria de 0,8) sin presencia de otras
citopenias. El frotis de sangre periférica mostró una
intensa anisopoiquilocitosis de serie roja, presencia de
eritroblastos, punteado basófilo y cuerpos de
Pappenheimer.
Ante este hallazgo, se amplió el perfil analítico
inicial con estudio del metabolismo del hierro desde el
laboratorio, el cual mostró un secuestro férrico con
niveles de ferritina elevados (1291 ng/mL), hierro
sérico normal pero con inadecuada utilización

pathognomonic of any specific disease; nevertheless,
it can help to the etiological diagnosis of:
Congenital sideroblastic anemia or refractory anemia with ring sideroblasts, a disorder classified as myelodysplastic syndrome.
Splenectomy.
Thalassemias.
Drepanocytosis.
Toxic (Saturnism, Isoniazide).
The presented peripheral blood smear image
belongs to an 84-year-old female patient who was
admitted to the Emergency Department after being
detected with critical hemoglobin value in a routine
laboratory test. Once in the Emergency Department,
patient showed deterioration of clinical condition, no
fever, hypotension without tachycardia and
generalized pallor without signs of active bleeding.
The patient also presented a lesion in the sacrum with
a greenish-brown and malodorous suppuration.
Initial screening tests showed the presence of
severe macrocytic aregenerative anemia
(Hemoglobin: 4.5 g / dL, MCV: 102 fL and
reticulocytes: 5.55%, but with a reticulocyte production
index of 0.8), without other cytopenias. The peripheral
blood smear showed intense anisopoikylocytosis of
the red blood cells, erythroblasts presence, basophilic
stippling and Pappenheimer bodies.
According to these findings, the iron profile was
added by the laboratory revealing ferric sequestration
with elevated ferritin levels (1291 ng /mL), normal
serum iron levels and decreased reticulocytic
hemoglobin (19.7 pg) which evidenced an inadequate
use and availability of it. Other blood biochemistry
tests showed acute renal failure (GFR CKD-EPI: 30

evidenciada por una hemoglobina reticulocitaria
disminuida (19,7 pg).
El resto de la bioquímica sanguínea reveló un
fracaso renal agudo (FG CKD-EPI: 30 mL/min),
importante síndrome inflamatorio (PCR: 220,3 mg/L) y
procalcitonina ligeramente elevada (0,54 ng/mL) con
niveles de lactato en la gasometría venosa periférica
de 3,4 mmoL/L. Estos hallazgos orientaron el
diagnóstico hacia un proceso séptico con probable
foco cutáneo y anemia severa multifactorial con
requerimiento transfusional.
Ingresó en la planta de Medicina Interna donde
bajo tratamiento antibiótico de amplio espectro,
presentó mejoría clínica con descenso de los
reactantes de fase aguda, así como recuperación
parcial de la función renal.
Sin embargo, la sobrecarga férrica se mantuvo
a pesar de la resolución del cuadro infeccioso. Este
hallazgo junto al rendimiento parcial de la transfusión
y los rasgos displásicos descritos en el frotis de sangre
periférica nos hizo ampliar el estudio con una tinción
de Perls que confirmó la presencia de cuerpos de
Pappenheimer con disposición anular.
El cuadro clínico y los hallazgos analíticos
encaminaron el diagnóstico a un posible síndrome
mielodisplásico, por lo cual se realizó interconsulta a
hematología. Debido a la fragilidad de la paciente, la
etiología multifactorial de la anemia y la decisión de
una adecuación del esfuerzo terapéutico se
mantuvieron los cuidados en domicilio, con evolución
tórpida y éxitus de la paciente.

peripheral venous gasometry. These results lead the
diagnosis towards a septic process with probable
cutaneous focus and a severe multifactorial anemia
with transfusion requirements.
The patient was transferred to the Internal
Medicine Department, showing clinical improvement,
decrease of the acute phase reactants, as well as
partial recovery of renal function after the broad
spectrum antibiotic treatment.
However, iron overload was maintained after
the resolution of the infectious symptoms. This finding,
along with the partial response to the blood transfusion
and the dysplastic features described in the peripheral
blood smear made us complete the study with a Perls
stain. The smear revision confirmed the presence of
Pappenheimer bodies with an annular arrangement.
Clinical presentation and analytical findings
leaded the diagnosis to a possible myelodysplastic
syndrome; therefore a consultation to the hematology
department was carried out. Due to the weakness of
the patient, the multifactorial etiology of the anemia
and the decision to limit the therapeutic effort, it was
decided to maintain hospital-based home health care,
followed by torpid evolution and patient´s death.

Se presenta el diagnóstico de la Atrofia
Muscular Espinal (AME) realizado en el laboratorio de
Genética Molecular mediante estudio de MLPA
(Multiplex ligation-dependente probe amplification)
con la Salsa P060-B2 (MRC-Holland®, Netherlands),
además existe la Salsa P021 como confirmatoria(1).
Esta técnica nos permite conocer el número de copias
de los genes de la supervivencia de la neurona motriz
(SMN1 y SMN2) de una manera rápida y fiable
(sensibilidad 95% y especificidad 100%). Es un
método diagnóstico muy interesante puesto que existe
un tratamiento específico para AME-I y II. El fármaco
Spirinza®(2) consiste en un oligonucleótido
antisentido que bloquea la región del intrón 7 del gen
SMN2 facilitando la activación del exón 7 del SMN2 en
el ARNm del SMN2, generando una proteína
funcional.
AME es una enfermedad neuromuscular
causada por la degeneración de las neuronas motoras
alfa de médula espinal, presenta una herencia
autosómica recesiva, con una incidencia estimada de
1/6.000-1/10.0000 nacimientos, y una frecuencia de
portadores de 1/40-1/60(3). La proteína de
superviviencia de las neuronas motoras (SMN), que
participa en el movimiento normal de los músculos y
el control de extremidades, abdomen, cabeza y cuello,
está codificada por dos genes localizados en el
cromosoma 5q13.2, SMN1 y SMN2. El gen SMN1 se
transcribe produciendo una proteína SMN estable,
mientras que el gen SMN2 difiere en el nucleótido
c.840 C>T provocando un sitio nuevo de splicing y, por
tanto, origina una pequeña cantidad de proteína SMN
inestable que no contiene el exón 7(4)(5). El 95-99%
de los pacientes de AME carecen de ambas copias del
gen SMN1 debido a una deleción en homocigosis o
una conversión génica de SMN1 en SMN2.

Atrophy (AME in Spanish language) performed in the
Molecular Genetics laboratory by means of an MLPA
study (Multiplex Ligations Probe Amplification) by
SALSA MLPA-P060-B2 (MRC-Holland®,
Netherlands), furthermore SALSA MLPA-P021 is used
to confirm the diagnosis(1). This method allows us to
know the copy number of the survival genes of the
motor neuron (SMN1 and SMN2) in a fast and reliable
way (95% sensitivity and 100% specificity). At present
this diagnostic method is important due to the fact that
there is a new treatment for SMA type I and II. The
drug Spirinza®(2) consists of an antisense
oligonucleotide that blocks the region of intron 7 of the
SMN2 gene, this facilitates the activation of exon 7 of
SMN2 in the mRNA of SMN2, generating a functional
protein.
AME is a neuromuscular disease that is caused
by the degeneration of alpha spinal cord motor
neurons. It presents an autosomal recessive pattern of
inheritance and it has been estimated that the annual
incidence of 1/6.000 to 1/10.000 births and a carrier
frequency of 1/40 – 1/60 individuals(3). SMN protein is
encoded by two genes located on chromosome
5q13.2 (SMN1 and SMN2) and involves in the normal
movement of muscles and control of limbs, abdomen,
head and neck. The SMN1 gene produces a stable
SMN protein, whereas the SMN2 gene differs from
SMN1 at nucleotide c.840 C>T, this fact generates a
new splicing site, and therefore a small proportion of
unstable SMN protein that does not contain the exon
7 is created(4)(5). 95-99% of AME patients lack both
copies of the SMN1 gene due to a deletion in
homozygosis or a gene conversion of SMN1 in SMN2.
According to a Consensus of Standards
document published in “Journal of Child
Neurology”(6), AME is classified by the time the
symptoms, the muscle activity achieved, and the

Se presenta el caso de un paciente varón de 40
años originario de Mali que acude a su centro de salud
de forma rutinaria.
En el hemograma destaca una hemoglobina de
14,1 g/dL (Valor de Referencia [VR]: 13,5-17,2) con
eritrocitosis (5,58 x106 hematíes/ul, VR: 4-5) y una
disminución del Volumen Corpuscular Medio [VCM]
(71,3 fL; VR: 80-100) y de la Hemoglobina
Corpuscular Media [HCM] (25 pg; VR: 27-33). El
estudio del metabolismo del hierro resultó normal.
En el análisis hematimétrico (Sysmex XN
Series, Kobe, Japan) se detectan unas poblaciones
leucocitarias normales, pero destaca una población de
eritroblastos de 3,6 células por cada 100 leucocitos
(Figura 1) y una reticulocitosis (3,3%; VR: 0,5-2). El
analizador muestra las alarmas “NRBC_Present” y
“Fragments?”. A la vista de los resultados obtenidos,
se realiza un frotis de sangre periférica.
El frotis de sangre periférica muestra una
marcada anisopoiquilocitosis con abundantes formas
dismórficas (dianocitos y excentrocitos) y algunos
eritroblastos (4 por cada 100 leucocitos) compatibles
con una hemoglobinopatía (Figura 2a), por lo que se
amplió el estudio de hemoglobinas por HPLC
(Cromatografía Líquida de Alta Resolución).
Los estudios de Hemoglobinopatías (Figura 2b)
realizados por HPLC (Variant II, BioRad) mostraron
una HbF normal (0.2%; VR:<1%), una HbA2 normal
(3%; VR: 2.5-3.5) y una HbC elevada (93.8%; VR:
0%), consistente con una hemoglobinopatía C
homozigota.
We present the case of a 40-year-old male
patient from Mali who routinely visits his health center.
In the complete blood count highlights a
hemoglobin of 14.1 g/dL (Reference Value [RV]: 13.5-
17.2) with an erythrocytosis (5.58 x106 RBC/ul, RV: 4-
5) and a decreased Mean Corpuscular Volume [MCV]
(71.3 fL; RV: 80-100) and Mean Corpuscular
Hemoglobin [MCH] (25 pg; RV: 27-33). Iron profile was
normal.
In the complete blood count analysis (Sysmex
XN Series, Kobe, Japan), normal leukocyte
populations are detected, but an erythroblasts
population of 3.6 cells per 100 leukocytes was founded
(Figure 1) together with a reticulocytosis (3.3%; RV:
0.5-2). The analyzer displays the flags
"NRBC_Present" and "Fragments?". Given these
results, a peripheral blood smear was performed.
Examination of peripheral blood smear shows a
marked anisopoikilocytosis with abundant dysmorphic
forms (target cells and eccentrocytes) and some
erythroblasts (4 per 100 leukocytes) compatible with a
hemoglobinopathy (Figure 2a), so we added a variant
hemoglobins study by HPLC (High Performance
Liquid Chromatography).
Hemoglobin variant studies (Figure 2b)
performed by HPLC (Variant II, BioRad) demonstrated
a normal HbF (0.2%; RV:<1%), normal HbA2 (3%; VR:
2.5-3.5) and elevated HbC (93.8%; VR 0%), consistent
with homozygous hemoglobinopathy C.

La hemoglobina C es una hemoglobina variante
con una mutación en el gen de la beta globina en la
misma posición que la hemoglobina S, pero con
diferente cambio de aminoácido (α2β26GluLys)1.
Esta hemoglobina es originaria del Este de
África, al este del Río Níger. En el caso de Mali, de
donde es originario el paciente, la prevalencia en
algunas zonas es mayor al 10%. Esta alta prevalencia
se ha relacionado con el efecto protector que produce
sobre las infecciones por Plasmodium falciparum2.
En estos pacientes podemos encontrar desde
una concentración normal de hemoglobina hasta una
anemia leve o moderada (8 g/dl). Habitualmente
existe una microcitosis marcada, así como un CHCM
en el límite alto de la normalidad. Debido a la hemólisis
crónica, el recuento de reticulocitos suele estar
levemente elevado, normalmente 2-4%, y los
pacientes pueden presentar esplenomegalia y
colelitiasis.
Hemoglobin C is a variant hemoglobin with a
mutation in the beta globin gene at the same position
as hemoglobin S, but with a different amino acid
change (α2β26GluLys)1.
This hemoglobin is native from East Africa, at
the east of the Niger River. In the case of Mali, where
the patient originates from, the prevalence in some
areas is greater than 10%. This high prevalence has
been related to the protective effect it produces on
Plasmodium falciparum infections2.
In these patients we can found from a normal
concentration of hemoglobin to a mild or moderate
anemia (8 g / dl). There is usually a marked
microcytosis, as well as a MCHC at the high limit of
normality. Due to chronic hemolysis, the reticulocyte
count is usually slightly elevated, usually 2-4%, and
also patients may present splenomegaly and
cholelithiasis.

En el estudio de HPLC se suele encontrar una
hemoglobina C que corresponde a casi la totalidad de
hemoglobinas. La hemoglobina Fetal puede estar
ligeramente elevada, pero no suele superar el 3%.
In the HPLC study, a hemoglobin C is usually
found that corresponds to almost all the amount of
hemoglobins. Fetal hemoglobin may be slightly
elevated, but usually does not exceed 3%.
