Please use this checklist as a guide to ensure you have completed and are submitting all required documents to apply for a grant. Only complete application packages will be considered for funding assistance.

Please include copies of the following with your application:

Completed and signed Landowner Information Forms (3 pages)

Completed Project Profile Forms including site plan (3 pages)

Documentation of condition of existing system prepared by a licensed septic system installer or a municipal representative

Documentation of septic age (may include a letter from the municipal building department or health unit, original site plans, permits or installation records)

Two or more cost quotations from qualified contractors, with Building Code Identification Number (BCIN) referenced

Important Things to Remember! It is the responsibility of each landowner to obtain all required permits before starting the project, i.e. from LSRCA, the municipality and/or Health Unit. Landowners will be required to provide copies of all required permits for grant reimbursement. Failure to obtain the necessary permits will result in an ineligibility to receive grant funding.

It is the responsibility of each landowner to pay all contractors up-front and in-full upon project completion. Proof of payment will be required as part of your submission for grant reimbursement.
