2.Pumps to be provided with stainless steel intake screen (strainer marked as “none” on submittal).
3.Provide additional motor lead length if necessary so that splicing of the motor cable is not required within the pump can per spec. paragraph 2.4. Standard motor lead length is listed as 200”, which may not be sufficient to extend beyond the pump can to a pull box.
4.Please submit written certification that proposed Tnemec 140 meets the NSF 372 requirement for low lead. If not, provide alternative coating.
5.Maass Midwest pump can to extend 24” above finish grade, not 18” as shown.
6.Maass Can suction side flange shall be ANSI Class 150, not Class 300 as show.

Hermetically Sealed TypeA Hermetically Sealed Type motor utilizes windings of standard construction and insulation thickness. The windings are encased and Hermetically Sealed within the external shell casing on the outside and an internal tube or liner inside the bore. The Hermetically Sealed enclosure elimi-nates the possibility of water leakage into the winding the liquid medium circulates between the rotor and stator liner providing lubrication and cooling to the bearings.
Wet Winding TypeA Wet Winding Type motor is one in which the motor windings are in direct contact with a liquid medium. The medium is clean, clear water. A pressure balancing system prevents exchange of the motor liquid medium and well water due to thermal expansion and contraction when the motor is operating. The liquid medium fills the inside of the motor and surrounds both the stator windings and the rotor. A completely water proof insulation is used on the mag-net wire used for the stator windings. The liquid medium inside the motor air gap and coils acts as a heat transfer device by circu-lating through the windings and transferring heat to the external casing. Dissipation of this heat occurs as the well water flows at a required velocity over the external case. As is the case in all submersible type motors, the internal liquid medium is also used for bearing lubrication.

6" Canned Design Motor Features• Replaceable Plug-in Motor Lead – All 6” motor leads
are stranded copper for flexibility and strength. Leads are 150” long and field replaceable.
• Insulation – The canned motor’s stator coil is mounted in a stainless steel frame and is completely sealed in a stainless steel cylinder. The coil wire features moisture resistant insulation which is completely water-proofed for long life.
• Heat Resistant – The space between the stator's stain-less steel protective can and its frame is filled with a specially formulated patented resin that protects the motor against thermal fluctuation and internal stress.
• 95º F (35º C) Water Temperature (CANNED TYPE: 6", 5 – 50 HP) – The motors operate with a minimum flow rate of 0.5 ft./sec. (0.15 m/sec) in water temperature up to 95º F (35º C) without any derating of horsepower. This 95º F (35º C) temperature is 18º F (10º C) higher than NEMA standards.
8-10" Rewindable Water-Tight Motor Features• Insulation Wire – The coil conductor insulation mate-
rial is a specially developed denatured polypropylene, which offers excellent leak-resistant characteristics. Three barriers are applied to the copper conductors to provide complete insulation against the motor’s cool-ing fluid. This design is the result of extensive research and is of superior quality. It insures that CentriPro mo-tors will have an extremely long service life.
Motor Features - Both Designs• Sand Resistant Slinger and Lip Seals – CentriPro sub-
mersible motors feature double-row, grease packed, lip seals to prevent sand from entering the motor. A carbon steel slinger with a baked on epoxy coating pre-vents sand from entering the lip seal area. Additional protection against sand intrusion is provided by the close tolerance slinger guide with a .02” fit. This design
is field-proven in West Texas wells which are consid-ered to be some of the worst sandy well applications. Best of all, these features are standard for a CentriPro motor, so there is no need to stock one motor for clean wells and another more expensive motor for the more abrasive, sandy jobs.
• High Torque Characteristics – The stator laminations are constructed of high quality, electrical grade, silicon steel which provide high torque even when the motor is subjected to nominal voltage variations.
• Balancing – The rotor balance rings (one on each end of the rotor) allow for excellent dynamic balance for the rotating element of the motor.
• Carbon Sleeve Bearings – Two carbon, water-lubri-cated, guide bearings are used to properly align the shaft. The bearings have a large surface area to better support the shaft, reducing whipping, while acting as a steady bushing.
• Water-Filled Design – The internal cooling water is mixed with antifreeze and antirust liquid to allow the motor to be stored at –22º F (–30º C). Two water plugs are provided for filling or draining the anti-freeze fluid during maintenance. These plugs are also used to top off and to check the water level.
• Complete Corrosion and Water-Tight Protection – All main motor components are made of stainless steel: in-cluding the can housing (water tight type motors have baked epoxy coated carbon steel housings), shaft and bolts. All other motor parts are finished with the baked epoxy coating.
• Baked Epoxy Coating – All external and internal cast iron parts are coated with a baked epoxy resin that pro-vides excellent resistance to water and corrosion.
• Quality Control – All CentriPro submersible motors are manufactured and tested under the most stringent quality control procedures, providing long service life and trouble-free operation.

DESCRIPTION OF WATER TIGHT INSULATION WIRE
The reliability of submersible motors depends on their insulation characteristics. CentriPro 8-10” motors are the result of years of con-tinuous research and development. Our re-windable water-tight motors feature excellent insulation characteristics thanks to their new patented, water-tight, insulated magnet wire.
The insulation material is a specially developed denatured polypropylene applied over a spe-cial enamel layer. An external nylon sheath is applied over this polypropylene layer for extra mechanical protection. These three barriers are applied to copper conductors for complete insulation from the motor’s cooling fluid. This insures that CentriPro motors will have an ex-tremely long service life.

The field proven KINGSBURY design thrust bearing creates a wedge of water between the pivot shoe and carbon disc. Our innovative design permits high thrust loads to be placed on the bearings while showing no measurable wear after several years of severe duty operation. This allows for long pumping life, virtual trouble free operation and low maintenance. For all 6" motors, the 300 lbs. maximum continuous up thrust is absorbed between the upper carbon sleeve bearing and the rotor balance ring. For all 8" – 10" motors, the 1000 lbs. maximum continuous up thrust is carried between the upper slide plate and the separate up thrust carbon bearing.

WARRANTY - Company warrants title to the product(s) and, except as noted with respect to items not of Company’s manufacturer, also warrants the product(s) on date of shipment to Purchaser, to be of the kind and quality described herein, and free of defects in workmanship and material. THIS WARRANTY IS EXPRESSLY IN LIEU OF ALL OTHER WARRANTIES, INCLUDING BUT NOT LIMITED TO IMPLIED WARRANTIES OF MERCHANTABILITY AND FITNESS, AND CONSTITUTES THE ONLY WARRANTY OF COMPANY WITH RESPECT TO THE PRODUCT(S). If within one year from date of initial operation, but not more than eighteen months from date of shipment by Company of any item of product(s), Purchaser discovers that such item was not as warranted above and promptly notifies Company in writing thereof, Company shall remedy such nonconformance by, at Company’s option, adjustment or repair or replacement of the item and any affected part of the product(s). Purchaser shall assume all responsibility and expense for removal, reinstallation, and freight in connection with the foregoing remedies. The same obligations and conditions shall extend to replacement parts furnished by Company hereunder. Company shall have the right of disposal of parts replaced by it. Purchaser agrees to notify Company, in writing, of any apparent defects in design, material or workmanship, prior to performing any corrective action back chargeable to the Company. Purchaser shall provide a detailed estimate of the material, labor costs associated with proposed remedy for expeditious review and approval by the Company. ANY SEPARATELY LISTED ITEM OF THE PRODUCT(S) WHICH IS NOT MANUFACTURED BY THE COMPANY IS NOT WARRANTED BY COMPANY and shall be covered only by the express warranty, if any, of the manufacturer thereof. THIS STATES PURCHASER’S EXCLUSIVE REMEDY AGAINST COMPANY AND ITS SUPPLIERS RELATING TO THE PRODUCT(S) WHETHER IN CONTRACT OR IN TORT OR UNDER ANY OTHER LEGAL THEORY, AND WHETHER ARISING OUT OF WARRANTIES, REPRESENTATIONS, INSTRUCTIONS, INSTALLATIONS OR DEFECTS FROM ANY CAUSE. Company and its suppliers shall have no obligation as to any product which has been improperly stored or handled, or which has not been operated or maintained according to instructions in Company or supplier furnished manuals. PATENTS - Company shall pay costs and damages finally awarded in any suit against Purchaser or its vendees to the extent based upon a finding that the design or construction of the product(s) as furnished infringes a United States patent (except infringement occurring as a result of incorporating a design or modification at Purchaser’s request) provided that Purchaser promptly notifies Company of any change of such infringement, and Company is given the right at its expense to settle such charge and to defend or control the defense of any suit based upon such charge. THIS PARAGRAPH SETS FORTH COMPANY’S EXCLUSIVE LIABILITY WITH RESPECT TO PATENTS. BUYER DATA - Timely performance is contingent upon the Purchaser supplying to the Company, when needed, all required technical information, including drawing approval, and all required commercial documentation. NUCLEAR - Purchaser represents and warrants that the product(s) covered by this contract shall not be used in or in connection with a nuclear facility or application. NONCANCELLATION - Purchaser may not cancel or terminate for convenience, or direct suspension of manufacture, except on mutually acceptable terms. DELAYS - If Company suffers delay in performance due to any cause beyond its control, including but not limited to act of God, war, act or failure to act of government, act or omission of Purchaser, fire flood, strike or labor troubles, sabotage, or delay in obtaining from others suitable services, materials, components, equipment or transportation, the time of performance shall be extended a period of time equal to the period of the delay and its consequences. Company will give to Purchaser notice in writing within a reasonable time after Company becomes aware of any such delay. STORAGE - Any item of the product(s) on which manufacture or shipment is delayed by causes within Purchaser’s control, or by causes which affect Purchaser’s ability to receive the product(s), may be placed in storage by Company for Purchaser’s account and risk. SHIPMENT - The term “shipment” means delivery to the initial carrier in accordance with the delivery terms of this order. Company may make partial shipments. Company shall select method of transportation and route, unless terms are f.o.b. point of shipment and Purchaser specifies the method and route and is to pay the freight costs in addition to the price. When terms are f.o.b. destination or freight allowed to destination, “destination” means common carrier delivery point (within the continental United States, excluding Alaska) nearest the destination. For movement outside the United States, Company shall arrange for inland carriage to port of exit and shall cooperate with Purchaser’s agents in making necessary arrangements for overseas carriage and preparing necessary documents. SPECIAL SHIPPING DEVICES - On shipments to a destination in the continental United States or Canada, Company has the right to add to the invoice, as a separate item, the value of any special shipping device (barrel, reel, tarpaulin, cradle, crib and the like) used to contain or protect the product(s) invoiced, while in transit. Full credit will be given on the return to Company of the device in a reusable condition, f.o.b. destination, freight prepaid. TITLE AND INSURANCE - Title to the product(s) and risk of loss or damage shall pass to Purchaser at the f.o.b. point, except that a security interest in the product(s) and proceeds and any replacement shall remain in Company, regardless of mode of attachment to realty or other property, until the full price has been paid in cash. Purchaser agrees to do all acts necessary to perfect and maintain said security interest, and to protect Company's interest by adequately insuring the product(s) against loss or damage from any external cause with Company named as insured or co-insured. TERMS OF PAYMENT - Unless otherwise stated all payments shall be Net Thirty (30) Days and in United States dollars, and a pro rata payment shall become due as each shipment is
made. If shipment is delayed by Purchaser, date of readiness for shipment shall be deemed to be date of shipment for payment purposes. If at any time in Company’s judgment Purchaser may be or may become unable or unwilling to meet the terms specified, Company may require satisfactory assurances or full or partial payment as a condition to commencing or continuing manufacture or making shipment; and may, if shipment has been made, recover the product(s) from the carrier, pending receipt of such assurances. TAXES - Any applicable duties or sales, use, excise, value added or similar taxes will be added to the price and invoiced separately (unless acceptable exemption certificate is furnished). LIMITATION OF LIABILITY - Neither Company nor its suppliers shall be liable, whether in contract or in tort or under any other legal theory, for loss of use, revenue or profit, or for cost of capital or of substitute use or performance, or for incidental, indirect, or special or consequential damages, or for any other loss or cost of similar type, or for claims by Purchaser for damages of Purchaser’s customers. Likewise, Company shall not, under any circumstances, be liable for the fault, negligence, or wrongful acts of Purchaser or Purchaser’s employees, or Purchaser’s other contractors of suppliers. IN NO EVENT SHALL COMPANY BE LIABLE IN EXCESS OF THE SALES PRICE OF THE PART(S) OR PRODUCT FOUND DEFECTIVE. GENERAL - (a) Company will comply with all laws applicable to Company. Compliance with OSHA or similar federal, state or local laws during any operation or use of the product(s) is the sole responsibility of Purchaser. (b) The laws of the State of New York shall govern the validity, interpretation and enforcement of any contract of which these provisions are a part, without giving effect to any rules governing the conflict of laws. (c) This document and any other documents specifically referred to as being a part hereof, constitute the entire contract on the subject matter, and it shall not be modified except in writing signed by both parties. Unless otherwise specified, any reference to Purchaser’s order is for identification only. Assignment may be made only with written consent of both parties. ACCEPTANCE - The determination of compliance with performance guarantees will be based on results of factory tests under controlled conditions with calibrated instruments and tested per standards of the Hydraulic Institute. INSPECTIONS/EXPEDITING - The Company wishes to clarify that it will have to restrict access to agreed upon reasonable times and only for the purpose of conducting those inspections agreed upon. We request 72 hours notice prior to each visit. We request notification prior to visits to our subcontractors. PRICES - The prices stated herein will remain firm for the period up to the stated date of shipment providing the shipment is not delayed by the customer. If shipment is delayed by the customer beyond the shipment date quoted herein, the prices will be based on the prices in effect at time of shipment, including storage and material handling costs. In no event shall the adjusted price be less than the original order price, including change orders. Prices are F.O.B. Shipping Point, unless otherwise specified. When price includes transportation and other charges pertaining to the shipment of goods, any increase in transportation rates and other charges will be for the account of the Purchaser. There will be an extra charge for any test other than that which may be normally run by the Company, or for any test performed to suit the convenience of the Purchaser. CONTROLLING PROVISIONS - These terms and conditions shall control with respect to any purchase order or sale of the Company’s products. No waiver, alteration or modification of these terms and conditions whether on Purchaser’s purchase order or otherwise shall be valid unless the waiver, alteration or modification is specifically accepted in writing and signed by an authorized representative of the Company. If this transaction involves EXPORT, the following additional terms and conditions shall apply: PACKING - Equipment will be packed, boxed or crated in accordance with the Company’s standard commercial practice, for underdeck export shipment, unless otherwise agreed. LETTER OF CREDIT - Unless otherwise specified in writing, payment shall be made by irrevocable letter of credit in form acceptable to Company, confirmed by a major USA bank, acceptable to the Company and providing for payment in full in United States dollars against presentation of United States inland shipping documents and invoices, such letter of credit to be established prior to Company’s acceptance of the order. The letter of credit shall also provide that in the event Company is, for any reason beyond its control, prevented from making shipment from Company’s factory or delivery at the port of embarkation, a certificate of manufacture of the whole or any part of the goods shall constitute delivery of such whole or any part of the goods and payment in full of any and all drafts drawn against the letter of credit for the goods so “delivered” shall be made upon presentation of such certificates of manufacture in lieu of United States inland shipping documents. In the event that Company is prevented by law, or otherwise, from making shipment from Company’s factory or delivery at port of embarkation of the goods or any part thereof, on completion of manufacture, Company reserves the right to place the goods in storage for the Purchaser’s account and risk. Any charges incurred in this connection will be for the account of the Purchaser at cost and will be payable upon demand. COMPANY AS AGENT - If Company makes or arranges for ocean shipment, Company shall act as agent for the Purchaser and reserves the right to procure full insurance coverage, including war risk insurance, at the expense of the Purchaser. All expenses incurred in this connection will be payable upon demand to the Company. If Company as agent applies for or secures manufacturing, financing, exporting or other licenses required by the United States Government, or any department thereof, Company shall make such applications or secure such licenses solely as agent for the Purchaser, and assumes no responsibility therefore

EQUIPMENT RECOMMENDATIONS
Listed below are general equipment guidelines for the application of this product. Job site conditions may require modifications to these guidelines to achieve the desired results. Ensure brush and roller is well loaded to avoid air entrapment. Multiple coats may be needed to achieve adequate film build.
BRUSH: Use a good quality solvent resistant, natural or synthetic.
ROLLER: Use a good quality natural or synthetic 3/8” short-nap roller cover with phenolic core.

GENERAL SURFACE PREPARATION
All surfaces must be sound, dry, clean and free of oil, dirt, grease, wax, mildew, loose or flaking paint and other surface contaminants.
Remove loose, peeling, flaking or scaling paint by scraping, sanding or wire brush.
Area should be sheltered from airborne particulates and pollutants. Avoid combustion gases or other sources of carbon dioxide that may promote amine blush. Ensure good ventilation during application and curing. Provide shelter to prevent wind from affecting spray patterns. For tank lining, dehumidification equipment is recommended.
Surface temperature must remain at least 5°F above dew point temperature.
For good results, an SSPC-SP 5 (NACE No. 1) white metal blast or SSPC-SP 10 (NACE No. 2) near-white metal blast is recommended. For moderate exposure (non-immersion), an SSPC-SP 6 (NACE No. 3) commercial blast should be used.

APPLICATION: Apply by spray, brush or roller. APPLY ONLY when air and surface temperatures are between 50 - 122°F and the surface temperature is at least 5°F above the dew point temperature.
DRYING TIME: Under normal conditions, dries to touch in 4 hours and to recoat in 8 hours. Low temperature, high humidity, poor ventilation and thick films will retard drying.
CLEAN UP: Clean up skin, clothing, paint tools or spills immediately with recommended thinner, carefully observing cautions on paint and thinner labels. Dried paint may be removed by scraping. Clean Up with 21064 or 21092 Thinner, MEK or Xylene

SPECIAL SHIPPING DEVICES - On shipments to a destination in the continental United States or Canada, Company has the right to add to the invoice, as a separate item, the value of any special shipping device (barrel, reel, tarpaulin, cradle, crib and the like) used to contain or protect the product(s) invoiced, while in transit. Full credit will be given on the return to Company of the device in a reusable condition, f.o.b. destination, freight prepaid. TITLE AND INSURANCE - Title to the product(s) and risk of loss or damage shall pass to Purchaser at the f.o.b. point, except that a security interest in the product(s) and proceeds and any replacement shall remain in Company, regardless of mode of attachment to realty or other property, until the full price has been paid in cash. Purchaser agrees to do all acts necessary to perfect and maintain said security interest, and to protect Company's interest by adequately insuring the product(s) against loss or damage from any external cause with Company named as insured or co-insured. TERMS OF PAYMENT - Unless otherwise stated all payments shall be Net Thirty (30) Days and in United States dollars, and a pro rata payment shall become due as each shipment is
made. If shipment is delayed by Purchaser, date of readiness for shipment shall be deemed to be date of shipment for payment purposes. If at any time in Company’s judgment Purchaser may be or may become unable or unwilling to meet the terms specified, Company may require satisfactory assurances or full or partial payment as a condition to commencing or continuing manufacture or making shipment; and may, if shipment has been made, recover the product(s) from the carrier, pending receipt of such assurances. TAXES - Any applicable duties or sales, use, excise, value added or similar taxes will be added to the price and invoiced separately (unless acceptable exemption certificate is furnished). LIMITATION OF LIABILITY - Neither Company nor its suppliers shall be liable, whether in contract or in tort or under any other legal theory, for loss of use, revenue or profit, or for cost of capital or of substitute use or performance, or for incidental, indirect, or special or consequential damages, or for any other loss or cost of similar type, or for claims by Purchaser for damages of Purchaser’s customers. Likewise, Company shall not, under any circumstances, be liable for the fault, negligence, or wrongful acts of Purchaser or Purchaser’s employees, or Purchaser’s other contractors of suppliers. IN NO EVENT SHALL COMPANY BE LIABLE IN EXCESS OF THE SALES PRICE OF THE PART(S) OR PRODUCT FOUND DEFECTIVE. GENERAL - (a) Company will comply with all laws applicable to Company. Compliance with OSHA or similar federal, state or local laws during any operation or use of the product(s) is the sole responsibility of Purchaser. (b) The laws of the State of New York shall govern the validity, interpretation and enforcement of any contract of which these provisions are a part, without giving effect to any rules governing the conflict of laws. (c) This document and any other documents specifically referred to as being a part hereof, constitute the entire contract on the subject matter, and it shall not be modified except in writing signed by both parties. Unless otherwise specified, any reference to Purchaser’s order is for identification only. Assignment may be made only with written consent of both parties. ACCEPTANCE - The determination of compliance with performance guarantees will be based on results of factory tests under controlled conditions with calibrated instruments and tested per standards of the Hydraulic Institute. INSPECTIONS/EXPEDITING - The Company wishes to clarify that it will have to restrict access to agreed upon reasonable times and only for the purpose of conducting those inspections agreed upon. We request 72 hours notice prior to each visit. We request notification prior to visits to our subcontractors. PRICES - The prices stated herein will remain firm for the period up to the stated date of shipment providing the shipment is not delayed by the customer. If shipment is delayed by the customer beyond the shipment date quoted herein, the prices will be based on the prices in effect at time of shipment, including storage and material handling costs. In no event shall the adjusted price be less than the original order price, including change orders. Prices are F.O.B. Shipping Point, unless otherwise specified. When price includes transportation and other charges pertaining to the shipment of goods, any increase in transportation rates and other charges will be for the account of the Purchaser. There will be an extra charge for any test other than that which may be normally run by the Company, or for any test performed to suit the convenience of the Purchaser. CONTROLLING PROVISIONS - These terms and conditions shall control with respect to any purchase order or sale of the Company’s products. No waiver, alteration or modification of these terms and conditions whether on Purchaser’s purchase order or otherwise shall be valid unless the waiver, alteration or modification is specifically accepted in writing and signed by an authorized representative of the Company. If this transaction involves EXPORT, the following additional terms and conditions shall apply: PACKING - Equipment will be packed, boxed or crated in accordance with the Company’s standard commercial practice, for underdeck export shipment, unless otherwise agreed. LETTER OF CREDIT - Unless otherwise specified in writing, payment shall be made by irrevocable letter of credit in form acceptable to Company, confirmed by a major USA bank, acceptable to the Company and providing for payment in full in United States dollars against presentation of United States inland shipping documents and invoices, such letter of credit to be established prior to Company’s acceptance of the order. The letter of credit shall also provide that in the event Company is, for any reason beyond its control, prevented from making shipment from Company’s factory or delivery at the port of embarkation, a certificate of manufacture of the whole or any part of the goods shall constitute delivery of such whole or any part of the goods and payment in full of any and all drafts drawn against the letter of credit for the goods so “delivered” shall be made upon presentation of such certificates of manufacture in lieu of United States inland shipping documents. In the event that Company is prevented by law, or otherwise, from making shipment from Company’s factory or delivery at port of embarkation of the goods or any part thereof, on completion of manufacture, Company reserves the right to place the goods in storage for the Purchaser’s account and risk. Any charges incurred in this connection will be for the account of the Purchaser at cost and will be payable upon demand. COMPANY AS AGENT - If Company makes or arranges for ocean shipment, Company shall act as agent for the Purchaser and reserves the right to procure full insurance coverage, including war risk insurance, at the expense of the Purchaser. All expenses incurred in this connection will be payable upon demand to the Company. If Company as agent applies for or secures manufacturing, financing, exporting or other licenses required by the United States Government, or any department thereof, Company shall make such applications or secure such licenses solely as agent for the Purchaser, and assumes no responsibility therefore

NOTES Gear shall be manufactured by RSE-Sierra, a Myers Power Products Company. Seismic certification, if required, shall be furnished by others. Engineered drawings shall be furnished in the time frame quoted. Short circuit/coordination and arc flash studies shall be furnished by others. Field Testing shall be furnished by others. Allowance for field service covers no more than 8 hours per day, between 7 am and 6 pm Monday through Friday, holidays excepted. Transportation, lodging and per diem, if appropriate, will be added to the invoice. Startup services shall be furnished by others. Utility CT-meter compartments include provisions for CTs and for meters. CTs and meters not listed elsewhere in this BOM shall be furnished by others. - System Inergrator not part of scope (by others.) - Seismic Calculation is not included. STANDARD TERMS AND CONDITIONS
1. This Quotation becomes void after 30 days unless otherwise noted in the body of the quotation. 2. Price does not include any applicable Sales tax. 3. Price does not include Shipping, unless otherwise noted. 4. Product Lead times are estimates only. GTH relies heavily on the word of manufacturers and package carriers for these delivery dates.
This bid does not include electrical or mechanical installation. For example: Running conduit, pulling wire, mounting hardware and or instrumentation, unless otherwise stated above.
5. Fieldwork not included in the contract will be billed at standard rates. Unless otherwise stated above, fieldwork including start-up, instrumentation calibration, service, etceteras, will be billed at $125.00 per hour plus travel time, mileage, and expenses during normal business hours (8:00 a.m. – 5:00 p.m.). Over-Time rates are billed at $187.50 per hour plus travel time, mileage, and expenses. Holiday rates are billed at $250.00 per hour plus travel time, mileage, and expenses. Any exceptions must be stated and accepted in writing.
6. Acceptance of this quotation shall be in the form of a Purchase Order or signed contract, and shall not be modified without a written change order request.
7. Acceptance of this quotation acknowledges your acceptance of these terms in their entirety, regardless of other contract documents and their implied terms.
8. The terms of this quotation are Net 30 days from the date of invoice unless otherwise noted. Interest and late charges shall accrue at 1.5% per month on any unpaid balances.
9. Progress billing will be invoiced per an attached schedule if applicable. Typically this includes an invoice for 10% of the contract amount upon receipt of approved submittals, for engineering services.
10. Please note that GTH requires a complete set of customer supplied Project Specifications. 11. Please be advised that it is GTH’s standard practice to Lien all Jobs/Projects in excess of $5,000.00. A $100.00 filing fee may apply. 12. Start-Up assistance, Commissioning, and or Field Work may be suspended if invoices are past due. 13. GTH may not be able to respond to Warranty calls if invoices are past due. 14. Unit Price Breakouts are provided as estimates only. Prices for Change Order Additions or Subtractions may vary, and require a separate
quotation. 15. Returns, exchanges and/or cancellation may be subject to 25% fees. 16. On-site Training Terms: 50% at booking and 50% Net 30 days from the date of invoice. Please note a 50% fee will be assessed for
cancellations made within 1 week of date of training. 17. Any Custom Order, or Special Order made to your unique specifications, must be accompanied by a written and signed Purchase Order,
and cannot be cancelled once an acknowledgment has been issued. In addition, we cannot accept returns of Custom or Special Order items.
18. Warranty: Components are warranted by the manufacturer to be free of defects in materials and workmanship and are inspected prior to shipment to insure that they comply with the manufacturer’s stated specifications. Terms of the warranty are for a period of one (1) year from the date of shipment. Any component failing to meet specifications will be repaired or replaced, at our option, free of charge, provided the following conditions are met:
A. Notification of the nature of the defect is submitted within one (1) year of invoice date. B. Authorization to return the defective component is received. C. The component is returned freight charges prepaid to our Anaheim, CA. facility. The replacement component will be shipped freight
allowed. Correction of such defects by repair or replacement shall constitute the fulfillment of our obligations. We cannot be liable for consequential damages, direct or indirect, resulting from defect or delay. There is no provision for a labor allowance to cover the trouble- shooting, removal and reinstallation of any component that failed during the warranty period. Under no circumstances will claims be honored for repairs or rework of components without prior written approval of the manufacturer. Components damaged from improper installation, handling, storage, or operation, are specifically excluded from the warranty. The above warranty will pass through to the end user from the manufacturer.
