Before Receiving Requests

Students should speak with their professors about the following items (at minimum) If you are able to accommodate the student for their

exam.▪ If yes, there is no need for the student to submit a request.

If the student has a conflict caused by extended time on exam, you both should discuss alternate times▪ Any date/time that differs from the professor’s schedule

requires professor approval.

Receiving A Request

When you receive the following email, click the link named “The Office of Disability Services Accommodated Exam Request Web Site.” You will then be directed to our online accommodated

exam request application. Follow these next few steps to provide ODS with

important information regarding the administration of your exam.

The direct link to the application is https://apps.adn.missouri.edu/ExamAccommodation.

Log in using your pawprint and password used to log into myZou.

Click Here

One of your students has submitted an Accommodated Exam Request form.  Please login to complete your portion of the form.  Once completed, this information will be sent to the Office of Disability Services.  Thank you for your assistance with this process.

Accessing Requests

The direct link to the application is https://apps.adn.missouri.edu/ExamAccommodation.

Log in using your pawprint and password used to log into myZou.

Click “Edit This Form” of the request to review the submitted information.

Scroll down to the “Instructor’s Instructions” section

Click “Edit Instructor Section” to provide pertinent information related to your exams.

Information

Editing Requests Verify the dates/times listed for accuracy. Exam dates and times should be exactly the

same as your class unless otherwise discussed with your student. Any date/time that differs from your schedule

requires professor approval. If the dates the student submitted are not

accurate or needs updating, you can make the modifications. Select from “Exam,” “Quiz,” “Midterm” or “Final”

then number the exam or quiz if applicable. Select the date and time from the drop down menus.

Click “Add”

Editing Requests Click “Save and Return Later to Finish” button

before entering any additional information once you have made the date/time change/ additions.

You may then click “Edit Instructor Section” to enter information for administering your exams.

Submitting a Request “Save and Sign as Completed”

Sends professor email requesting they enter necessary info.

“Save and Return Later to Finish,” Saves the info but will NOT submit NOT on the schedule with ODS

“Cancel” Will clear any info entered and not submitted

Denying Requests ODS will deny the request if:

Does not provide 9 days’ notice Does not provide a exam time Requested on weekend or at an unapproved time

At such time, the student will need to take the exam with class.

Changing an Exam Date/Time

Revisions can only be made by ODS Send requests to MUDSExams@missouri.edu

providing ▪ Course▪ Student▪ original exam dates/times▪ updated dates and times

ODS must receive approval from your instructor. If less than 9 days’ notice is provided, you may be

denied.

Error Messages

If you receive an error message: Take a screenshot (Alt+Print Screen) Paste into an email or attachment Send to MUDSExams@missouri.edu

REMINDERS…

Emails will be sent to University email accounts informing you of needed approvals.

Click on the link provided which states “The Office of Disability Services Accommodated Exam Request Web Site”

Exams will be returned at 8:00am, 11:30am, or 3:00pm.

Exams started/completed after 3:00pm will be delivered on the next business day.

REMINDERS…

Exams during Finals Week will only be administered at 7:30am (Monday thru Friday) 10:00am (Monday thru Thursday) 12:30pm (Monday thru Thursday) 3:00pm (Monday thru Thursday) 5:30pm (Monday thru Thursday) 8:00pm (Monday thru Thursday)

9 Days notice is required for all regular exam requests.

Finals Week requests - due 3 weeks prior to

start of Finals Week.

Arrive on time (if not 10-15 minutes early).

If you are 5 minutes or more late, you must take your exam with your instructor.

All needs must be taken care of prior to start of exam

Once your exam has started, you will NOT be permitted to leave the exam site.

Bring photo ID.

If no ID, your exam will not be administered. At such time, you must take your exam with your

instructor.

No Shows

Do not arrive for exam + No communication = No Show

Receive email reminding you of scheduled exam with our office & of the ODS “no show” policy.

2nd “No Show” for the same course

Dropped from scheduled for that course for remainder of scheduled exams for current term

ODS Policies for Exams…

Academic Integrity violations will be reported to instructors.

All cell phones, notes and other non-exam related materials should be left in your backpack.

To keep track of time, be sure to bring a watch.

Not all classrooms on campus have functioning clocks.
