And, with a packed programme of clinical education and CPD also planned, this year’s show is the perfect opportunity for contractors to hone their pharmaceutical care skills. PSNC has told contractors: “A quality conversation between the patient and the pharmacist is the most important element of the [NMS] consultation. This requires a two way discussion between the patient and the pharmacist, rather than the pharmacist delivering information in a didactic manner to a passive patient.”

Extended funding for the community pharmacy New Medicine Service (NMS), announced by PSNC in mid-March, will allow

pharmacies in England to recruit new patients to the service up until September 30th and to receive payment for these patients even where the service is completed in October or November 2013.

To fund the extension of the NMS until September, PSNC has secured a further £20 million in funding. This takes the additional contract funding secured by PSNC for the NMS to £130m since the service was first announced in 2011.

PSNC says the decision to extend the NMS after September 2013 lies with the emerging NHS Commissioning Board and that this will take into account any emerging data from the Department of Health (DH) commissioned evaluation of the service. This is currently due to report in September, but may be extended to ensure that enough patients are recruited to it.

The NMS was originally commissioned until the end of March 2013 and in its first twelve months, the service was delivered to more than half a million English patients.

New pharmacy funding arrangements effective from April 13 improve practice and advance funding payments, PSNC has said in a funding update that also reveals that that the Department of Health is considering a rethink of the current margins survey-based system. Noting the Department’s preference for a more transparent system where all NHS earnings are accounted for, PSNC warns that the revised arrangements are being considered as part of the on-going Cost of Service Inquiry update negotiations.

Boots UK opened its new distribution centre in February 2012, investing over £50 million in the facility; this included the purchase of the existing 460,000 sq ft building – the area of six football pitches – its total refurbishment and the automated handling systems. The building accommodates the retailer’s e-commerce fulfilment operation, its Christmas gift operations and its growing international retail distribution, using a multichannel system designed and installed by KNAPP.

Automated shuttle storageAt the heart of the solution are 12 of KNAPP’s OSR Shuttle™ systems, which provide automated storage combined with goods-to-person order picking at eight ergonomically designed Pick-it-Easy workstations. KNAPP also supplied its own warehouse management and control systems to optimise

all warehouse movements and processes. In addition, Boots contracted KNAPP to provide a team of resident engineers in order to maintain the handling solution. The system has increased time efficiency by 65% compared to the previous manual operation, while also improving order traceability and providing a significant reduction in variable costs. Expertise in pharma wholesaleKNAPP’s client base in the pharmaceutical wholesale sector is impressive, with successful solutions supplied to The Co-operative Pharmacy, Celesio, Phoenix, Alliance and Sangers in the UK, as well as many leading international brands including Anzag in Germany; Aragofar and Hefagra in Spain; Medlog in Portugal; and Multipharma in Belgium. KNAPP clients in other industries include John Lewis, British Gas, Clarks, Avon Cosmetics, Bausch + Lomb, Coca-Cola, L’Oreal and Sony.

Analysis by PSNC of a sample of 224,000 patients receiving the NMS by October 2012 found that:

• 31.9% of non-adherent patients became adherent to 31.5% of their medicines after the NMS intervention

• Pharmacists gave 366,702 pieces of healthy living advice to patients while they provided the service

• 54.4% of NMS interventions were provided for patients prescribed medicines for hypertension, with 26.4% asthma/COPD, 11.3% diabetes and 7.9% anticoagulants/antiplatelet medicines

•Patients on asthma and COPD medicines had the lowest incidence of side-effects, but those side- effects caused non-adherence at nearly twice the rate than in the other disease states

• 99.6% of NMS patients were recruited directly by pharmacies, with the remainder referred by GP practices.

Welcoming the extension of the NMS, Sue Sharpe, PSNC chief executive, said: “This funding, which represents additional money outside of core contract funding, will be welcome for contractors. All negotiations are difficult at the moment and PSNC worked very hard to make the case to the DH and the NHS CB for the continuation of the NMS. The positive conclusions we were able to draw from our analysis provided the main evidence base for use by DH and the Board in their decision on re-commissioning the service. While this analysis will only be one factor in the NHS CB’s decision about the long-term future of the NMS, we are hopeful that the DH-commissioned evaluation will reflect our results.”

Isle of Wight contractor and PSNC Service Development Subcommittee member Gary Warner added: “Ongoing widespread provision of the service is vital to demonstrate pharmacy’s commitment to supporting medicines optimisation. We will be doing all that we can to ensure that the service is able to continue

beyond September, but the NHS CB will also want to see contractors’ continued commitment to providing the service to all eligible patients to deliver the maximum benefit for individuals and the NHS.”

In a new report, Paying the Price. Prescription Charges and People with Long-Term Conditions, the Prescription Charges Coalition, says: “28-day prescribing may be costing more to both the NHS and the individual. Numerous initiatives to reduce medicine waste and improve the use of medicines have not addressed the impact of charges.”

The report urges prescribers to think about a patient’s exemption status when deciding the prescription duration. The report concludes: “For stable, long-term conditions, three or six monthly prescribing may be the best approach. This will enable people to get on with their life while managing their condition and reduces the cost of unnecessary appointments.”

The report was issued as England raises prescription charges by 20p to £7.85 on April 1.

In new advice from the BMA on avoiding excessive prescribing, GPs have been told that prescribing intervals should be in line with the medically appropriate needs of the patient, taking into account the need to safeguard NHS resources, patient convenience, and the dangers of excess drugs in the home.

The GP guidance also cites a review of prescribing durations that recognises that shorter prescriptions are associated with significant increases in dispensing and other transaction costs, together with reductions in compliance in previously stable patients, and an increase in dissatisfaction amongst patients because of travel costs and time to obtain regular medicines. This review also states that shorter duration prescribing can place significant and unnecessary workload on the doctor and surgery staff.

KNAPP’s automated warehouse solution for Boots UK scoops five awards

Cost and inconvenience are making more than one in three people (36%) unhappy with their repeat prescription schedule, a prescription charge action group has concluded.

The material handling system supplied by warehouse automation specialist, KNAPP, to Boots UK at its new distribution facility in Burton-upon-Trent has won no less than five prestigious retail and logistics awards.

Key findings from the report include:

•35% of respondents who pay for each prescription have not collected at least one item due to the cost, with three quarters of this group reporting that their health got worse as a result. 10% said that they ended up in hospital as a direct consequence of not taking their medication

•29% of respondents who paid for their prescriptions and did not have a Prescription Prepayment Certificate reported not taking their medicine as prescribed either occasionally or often. Over half of these gave the cost of the prescription as the main reason for not taking their medicine as prescribed.

Earlier this year PSNC confirmed that contractors were unlikely to see finalised funding for 2012-13 before the end of March, due to the need to embed updated COSI findings, which was first published in 2011, into a new multi-year funding agreement.

To mitigate the effect of the delayed funding negotiations, PSNC has announced that effective from April, 2013, there will be:

•An increase in the practice payment payable by 8p per item

•100% advance payments (for the May statement onwards)

•Introduction of the Government’s Supply Chain Finance Scheme

•Introduction of new methadone fee arrangements

•A new requirement to ensure prescriptions have a prescriber code.

PSNC Chief Executive Sue Sharpe said: “Although there will be no ‘big bang’ on April 1st when the organisations of the reformed NHS take over from the existing bodies, for community pharmacies there are going to be a number of important changes.

“The increase in practice payments will be welcome

for businesses. The move to 100% advance payments will provide a boost to payments due at the beginning of June, and the introduction of the Government’s supply chain finance scheme will also be helpful for many contractors.

“Community pharmacies will also want to be aware of some new requirements in the reformed NHS, for example the need to check that prescriptions have a cost centre code on them, and they will need to understand the new endorsing arrangements for methadone prescriptions. ”

The important changes included in the 2013 NHS (Pharmaceutical and Local Pharmaceutical Services) Regulations 2013, which came into force in April are:

Market entrythe 2012 market entry provisions are carried forward into the 2013 regulations but contractors should note that pharmacy applications that meet a current or future need identified in the pharmaceutical needs assessment (PNA) or are based on the premise of ‘unforeseen benefits’ are subject to approval by the

NHS Commissioning Board (NHSCB). This amendment is intended to ensure the best use is made of NHS resources in the new NHS structure, in which the

PNA is written by the local health and wellbeing board, an organisation that is not responsible for the cost of funding the pharmaceutical services

Publication deadline for PNA extendedthe deadline for Health and Wellbeing Boards to review the PNA will be extended to April 2015

Providing fitness to practice informationwhen making an application for inclusion in a pharmaceutical list, the applicant no longer has to re-submit fitness to practise details to the NHSCB that has already been provided to the NHSCB or primary care trust

Methadonethe item level fee for oral liquid methadone prescriptions changes to £2.50 per prescription. From April 2013 contractors will additionally be able to claim a fee of 55p per additional dose packaged separately. The Department of Health (DH) has amended the terms of service to enable the pharmacist to decide the appropriate container in which to supply methadone

Prescriber identificationunder new terms of service, pharmacies will be able to refuse

to dispense prescriptions that do not contain cost-centre information. PSNC advises that pharmacists will be expected to check whether the prescriber’s cost centre identity information is included on the prescription form, and take steps to establish the relevant identity and enter it onto the prescription where it is absent. Negotiations are to take place to decide the appropriate remuneration for this addition to the pharmacy workload

Nominationelectronic prescription service (EPS) nominations from patients prior to a change of ownership will continue following a change of ownership. However, within a deadline of six months patients must be informed that the ownership has changed and asked whether they wish to continue to nominate the pharmacy

Educationpharmacy contractors, along with other primary care contractors, have a new duty to co-operate with Health Education England (HEE) over matters relating to education and training

Local Healthwatchcontractors have a duty to allow access to the premises by Local Healthwatch organisations

Safeguardingthe reference to safeguarding children and vulnerable adults included in the previous clinical governance arrangements is replaced with a requirement to have appropriate safeguarding arrangements for service users
