ADDENDUM NO. (1)

August 1, 2017 This Addendum No. 1 consists of one (1) page and has the following information to be incorporated into the Request for Proposals. Proposers must acknowledge receipt of this Addendum No. 1 in their RFP submittal in Required Form A, Form of Transmittal Letter. Item #1: Proposers’ questions and/or requests and the MPEA’s responses are provided below.

Group 1: Ammonia Refrigeration

Contractor will perform on an as needed basis, repair, maintenance, upgrades and expansion of the MPEA’s industrial ammonia refrigeration systems. Contractors must demonstrate experience in the area of

ammonia refrigeration system maintenance and repairs as well as provide information on completed

projects.

Services include preventive maintenance and alterations to existing equipment. The Contractor will provide mechanical repair, millwright, alignment, tear down and overhaul of equipment as well as welding, general

electrical and control wiring.

Equipment Description

(3) 2,200 ton (nom.) ABB/Stal ammonia screw compressor trains Compressor Type. S93EB-26

Services Required

Provide preventive maintenance and non-emergency repair services for the above equipment which includes but is not limited to:

Replace compressor seals

Perform shaft alignment

Perform vibration analysis

Megger insulation test on compressor motor

Verify overload trip settings

Repair ammonia leaks

Perform oil analysis

Replace oil cooler thermostatic valve

Non-destructive testing of pipe and vessels

Slide valve repair

Periodic safety valve replacement

Contractor Qualifications and Responsibilities

a) Contractor must be able to provide service on all equipment listed within this Group.

b) Contractor must obtain all required permits.

c) Contractor must be OSHA and EPA refrigerant certified to work on any and all MPEA equipment.

d) Contractor must meet all federal and OSHA guidelines for handling and disposal of refrigerants.

e) Contractor must have a safety program that meets the requirements of both OSHA and EPA

regulations.

f) Contractor must have previous experience repairing and maintaining ammonia systems.

g) Contractor must employ certified welders qualified to work on or around ammonia systems.

h) Contractor must be able to provide knowledgeable and skilled employees available to respond

within 24 hours of contact from an MPEA supervisor.

i) Contractor must be able to make available qualified ammonia service technicians on an around

the clock basis.

j) Contractor must be able to provide a minimum of two account representatives knowledgeable

of the account, with one available at all times.

k) Contractor must demonstrate technical and procedural knowledge of ammonia systems.

l) Contractor must become familiar with the Process Safety Management (PSM) and Risk

Management (RM) guidelines as provided by the Authority.

m) Contractor must have at least 8 hours PSM Ammonia System training prior to working on all

Authority equipment.

n) Contractor must be able to write pre-startup safety review (PSSR) and management of change

(MOC) as required per the OSHA standard.

o) Contractor must maintain an injury and hazard related illness log related to its employees

working on or in the vicinity of the Authority’s ammonia system.

p) Each technician assigned to the job must be safety trained to meet OSHA, and general industry

standards.

q) Contractor must provide Material Safety Data Sheets (MSDS) for chemicals brought on site.

r) Contractor must provide daily legible service tickets for all work performed

s) Contractor must provide legible written reports for any test, analysis or inspection

t) Contractor must provide a statement outlining what type of training, if any, can be provided to

the owner’s employees either in general or regarding the specific equipment listed.

u) Contractor must complete an orientation prior to all projects. An annual refresher orientation

will be given for the Contractor on a regular basis. The Contractor and the Authority will

maintain a record of this training.

v) Contractor must provide evidence that the Contractor held site safety meetings for field

supervisors, employees and new hires.

w) Contractor must provide a copy of Contractor’s refrigeration license and/or certification.

x) Contractor must provide a copy of the Contractor’s training documentation for each employee

working on the Authority’s premises.

y) Contractor must provide evidence of Contractor’s Safety Program.

2. Contractor must provide information highlighting the Contractor’s work on engagements similar in size and complexity.

provided industrial ammonia refrigeration repair and maintenance services;

i. the name and location of the project

ii. the size and scale of the project iii. the scope of services provided on the project

4. Contractor must provide a description of experience of all staff expected to participate in the

Services.

5. Contractor must provide names and contact numbers of all sub-contractors that Contractor may employ to meet the requirements of the scope and the contract.

electrical and control wiring.

Contractor will perform on an as needed basis, repair, maintenance, and upgrades to MPEA’s Indeck water

tube boiler systems. Contractor must demonstrate experience in the area of repair and maintenance of Indeck water tube boilers as well as provide information on completed projects. Services include the preventive maintenance of, and alterations to existing equipment. The Contractor will

provide mechanical repair, refractory repair, millwright, tear down and overhaul of equipment as well as
