1. INTRODUCTION
Universiti Teknologi PETRONAS (UTP) was established with the objective to
produce well-rounded graduates who are not only technically competent but also
possess other traits such as lifetime learning capacity, critical thinking,
communication and behavioral skills, business acumen, practical aptitude and
solution synthesis ability.
In line with the vision to close the industry gap, UTP has made it compulsory for
its undergraduate students to undergo practical works in the form of a twenty
eight (28) - weeks industrial attachment prior to their completion of studies at
UTP. The industrial internship carries fourteen (14) credit hours. The Industrial
Internship Programme is coordinated by the Student Industrial Internship Unit
(SIIU) who is responsible in ensuring the success of the Industrial Internship
Programme. With an average of 500 interns per semester, SIIU has successfully
secured the placements for all students with various host companies such as
PETRONAS subsidiaries, government-linked companies (GLC), multi-national
companies (MNC), as well as major overseas corporation i.e. in US, Germany,
France, Austria, Singapore, UAE, Italy and UK.
UTP has always placed great emphasis in having a close collaboration with the
industries in which its graduates will eventually gain recognition from industry
players locally and abroad. This internship programme is a prime example of the
University-Industry collaboration.
Currently, UTP offers five engineering programmes namely Chemical, Civil,
Electrical & Electronics, Mechanical and Petroleum Engineering, and three
technological programmes namely Information & Communication Technology,
Business Information Systems and Petroleum Goescience at Bachelor degree
level.
2. PURPOSE OF INDUSTRIAL INTERNSHIP
The purpose of the industrial internship is to expose UTP students to the real
working environment so that they can relate theoretical knowledge with
application in industry. From the Industrial Internship Programme, the students
will also develop skills in safety practices, work ethics, communication,
management, etc. Furthermore, this Industrial Internship Programme will
establish close relationship between industries and UTP.
The industrial internship programme will provide opportunity to UTP’s students
to build a solid understanding of the fundamentals of business and organization
performance such as economic models of business, competitive positioning and
strategy execution. With this real life environment, students develop their ability
to assess performance, interpret trends, explore the consequences of change and
make better decisions.

3. LEARNING OUTCOMES
At the end of the industrial internship, student will be able to:
Apply theoretical knowledge in industrial application.
Acquire skills in communication, management and team-work.
Practice ethical and professional work culture.
Implement health, Safety and Environment (HSE) practices at workplace.
4. STUDENT ELIGIBILITY
Student is eligible to register internship upon meeting the following
requirements:
Obtained minimum CGPA of 2.00/4.00 AND,
Earned a minimum of 75 credit hours (Engineering and Petroleum
Geoscience) or 45 credit hours (Business Information Systems and
Information & Communication Technology) AND
Completed the following courses:
o Completed all basic Engineering & Technology courses
o Health, Safety & Environment (HSE)
o Islamic Studies / Moral Studies
o Bahasa Kebangsaan
o Professional Communication Skills
5. TRAINING APPROACH
The Industrial Internship should have the following features:
Hands-on training
Real project-based assignments
Research-based activities
Team-work activities
Leadership and management skills
Safety awareness
The suggested training areas (but not limited to) for the respective programmes
can be found in Appendix I.
6. BENEFITS TO THE INDUSTRY
Provides opportunity for the industry to assess the capabilities of potential
employee.

Facilitates and strengthens university-industry partnership and linkages.
Receives fresh, energetic, enthusiastic and productive employees.
Enhance company’s reputation amongst graduates.

7. INTERNSHIP PROCESS
7.1 PRE-INTERNSHIP
o First Industrial Internship Briefing is given to students by
second week of the semester prior to the training by Student
Industrial Internship Unit (SIIU).
o List of Internship Eligibility is announced by SIIU by second
week of semester prior to the training.
o Internship placement registration is done by signing up with
SIIU. Students are required to submit Personal Data Info (PDI)
and Resume/CV to SIIU by the fourth week of semester prior to
the training.
o UTP Cover Letter & Reply form is issued by Industrial
Internship Unit.
o Internship placement is secured at approved host companies by
students one (1) month before internship commencement date.
o Student to submit Project Title proposal (From PRE-01) to SIIU
before internship commencement.
o Supervisor will be assigned upon receiving topic proposal.
o For overseas placement, student is assisted in visa/work permit
application and travel arrangement.
o Second Industrial Internship Briefing is given in week fourteen
by SIIU.
o Student to confirm placement before the internship start.
7.2 DURING INTERNSHIP
o Confirmation of Registration at Host Company is done in
PRISM within first week of internship commencement date.
o SIIU and Department Internship Coordinator approve industrial
project topic. Upon approval, students to start industrial project
immediately.
o First Visit is conducted by UTP Supervisor between week five
to week eight of internship period at local host company.
o UTP Supervisor to submit First Industrial Internship Visit
Report (ITC-B) to SIIU.
o The ITC-B form is reviewed and analyzed by SIIU before
necessary action can be taken.
o Student shall submit the Industrial Progress Report between
week twelve to week fourteen.
o Host Company supervisor to carry out progress assessment
(from ITC-C).

o Second Visit is conducted by UTP Supervisor between week
twenty-five to week twenty-eight of training for student’s
internship evaluation.
o Lecturers shall submit all the evaluation forms to SIIU.
7.3 POST-INTERNSHIP
o Logbook and Industrial Project Report shall be submitted to
Student Industrial Internship Unit (for official stamping) and
UTP Supervisor (for evaluation) by returning intern three weeks
after the end of internship.
o Student Industrial Internship Course Evaluation form is
submitted by returning intern to Industrial Internship Unit.
o Debriefing and sharing session is conducted by respective
Department Internship Coordinators.
o Consolidation of marks for ITC-C, ITC-D, ITC-E, ITC-F and
Logbook is conducted five week after end of internship.
o Moderation of total marks is done during Internship Sub-
Committee meeting six week after end of internship.
o Final marks and grades are endorsed during Special
Examination Committee meeting seven week after end of the
internship.
o Final marks and grades are approved during Special Senate
meeting.
o SIIP grades are announced by Examination Unit.
8. DETAILS OF INDUSTRIAL INTERNSHIP PROGRAMME
8.1 HOST COMPANIES
Industrial Internship Programme will be undertaken at Host Companies
which are approved by SIIU.
New host company will be assessed by SIIU before acceptance
confirmation by student.
8.2 PROGRAMME
Student shall provide the latest Student Industrial Internship
Programme Guidelines to the host Company Supervisor for their
reference. The Host Company Supervisor will plan and then come up
with the training schedule based on the Training Domain provided in
Appendix I. The proposed industrial project topic and schedule shall be
agreed by both the UTP supervisors and Host Company Supervisor.

8.3 DURATION AND PERIOD OF INTERNSHIP
The minimum duration of the Industrial Internship will be for a total
period of 28 weeks.
8.4 CONFIRMATION LETTER
A confirmation letter will be issued by the Host Company to the
students once the internship placement has been accepted.
8.5 TRANSFER OF INTERNSHIP PLACEMENT
Students are not allowed to transfer or change to another internship
site without prior approval from the SIIU and agreement of the Host
Company Supervisor. On limited circumstances, student may be
considered for transfer after the first visit. The student must submit
Requisition to Change Host Company Form ITC-H (Appendix XIV)
supported by Host Company Supervisor and offer letter from the new
host company to SIIU.
8.6 LEAVE
Students are not entitled for any annual leave throughout the
internship period. In the event that the student has to take leave due to
emergency, he/she must seek prior approval from the Host Company.
In the case of prolonged leave (exceeding 7 days) due to
hospitalization, student must notify both SIIU and host company in
writing and provides necessary documents as evidences.
8.7 INSURANCE
Students are covered by group personal insurance throughout the
industrial internship period (Appendix II).
8.8 TRAINING ALLOWANCE
Training allowance entitlement is solely at the host company’s
discretion. Although the training allowance is not a requirement,
substantial numbers of host companies provide training allowance in
order to make its internship programme attractive and competitive.
8.9 SAFETY TRAINING
Student shall abide to safety as students are encouraged to attend safety
training as deem fit by the Host Company.

o Basic Offshore Safety Induction & Emergency Training
(BOSIET) AND NIOSH Safety Passport
A provision for safety training (BOSIET and NIOSH) and
medical check-up as request by any student is subject to
recommendation and approval by both Host Company and
Industrial Internship Unit. Upon approval, Industrial Internship
Unit will bear 50% of the cost of BOSIET / NIOSH and
medical check-up only. Cost of travel, lodging and meals
throughout the training/site visit shall be borne by student.
The student must submit the Requisition for BOSIET /NIOSH
Form ITC-G (Appendix XIII) a month before the training to
SIIU.
o Green Card
Green Card is an integrated program that involves the
registration and accreditation of Construction Personnel to
enhance safety levels at Construction Sites. Those Construction
Personnel who have been registered with CIDB and issued the
Green Card are automatically covered by a special Insurance
Scheme that insures the Construction Personnel against death
and accidents.
Green Card is mandatory for all Civil Engineering students
before going for internship. Training is organized by Civil
Engineering Department every semester.
8.10 VISITS BY THE UTP (ACADEMIC) SUPERVISORS
o UTP Supervisor will visit the Host Company Supervisor and
students at the training site twice during each industrial
internship session (within Malaysia). For overseas attachments,
the visit by the UTP Supervisor will be during the internship
evaluation period.
o UTP Supervisor is responsible to establish communication with
the students and host company throughout the internship period.
o A first visit report (form ITC-B) must be submitted by UTP
Supervisor to SIIU upon first visit completion.
9. RESPONSIBILITIES
9.1 RESPONSIBILITIES OF THE HOST COMPANIES
Host Companies play important roles in ensuring the success of the
industrial internship programme. They are expected to provide the
following support:

9.1.1 HOST COMPANY SUPERVISOR (HCS)
Plans and schedules the overall internship programme to
ensure that it provides maximum opportunity for students
to obtain useful experience related to their field of study
(Section A of Appendix V).
Selects and assigns a qualified staff to be students’
Host Company Supervisor.
Assists students to find accommodation and
transportation (if necessary).
Provides briefing on the background of the
organization/company.
Discusses with the students the details of their internship
programme expected outcomes and industrial project.
Supervises and guides students in their industrial projects.
9Assesses student’s logbook (Section B of Appendix V)
and checks their detailed reports (Section C of Appendix
V).
Evaluates students' performance using FORM ITC–E
(Appendix X) and industrial project progress report using
form ITC-C (Appendix VIII).
Evaluates students’ oral presentation at the end of the
Industrial Internship programme using FORM ITC-D
(Appendix VIV).
Provides the necessary facilities, equipment, tools and
guidance to students.
Provides necessary recommendations to improve the
programme.
9.2 RESPONSIBILITIES OF UTP
9.2.1 STUDENT INDUSTRIAL INTERNSHIP UNIT (SIIU)
Facilitate the internship programme so that it can
complement the learning outcomes.
Provides list of approved companies to students for
selection.
Confirms the placement of students with the Host
Companies.
Sends students’ resume to the Host Companies (if
necesssary).

Assists students on visa/work permit application for
internship
Coordinates the following visits by UTP supervisors:
─ Pre-Visit and briefing to the Host Company’s
management (if necessary).
─ Organize and arrange for First Visit of UTP
Supervisors to Host Companies.
─ Second Visit.
Reviews the industrial internship matter and schedule
with the students before, during and after the industrial
internship programme.
Finalizes students’ grading in Internship Committee
Meeting.
Organizes special assignment such as UTP –Industry
Dialogue, Forum or Sharing Session.
9.2.2 DEPARTMENT INTERNSHIP COORDINATOR
Address internship matters/updates at respective
departments.
Compile students’ scores from the FORM ITC-C, ITC-D,
ITC-E, Logbook and Industrial Project Report (ITC-F).
Promotes and establish UTP-Industry relationship via
internship.
Encourage contribution of industrial project towards final
year project.
9.2.3 UTP SUPERVISORS
Establish and maintain communication with students and
Host Company Supervisor throughout the duration of the
internship period.
Assess students’ performance during their Industrial
Internship programme.
Develop and maintain good rapport with the Host
Companies.

Visit the Host Companies and review the industrial
project schedules with Host Company Supervisor.
Complete the 1st Industrial Internship Visit Report form
after the first visit using FORM ITC-B (Appendix VII).
Evaluate the students’ oral presentation at the Host
Companies using FORM ITC-D (Appendix VIV).
Review the students' performance with the Host
Company supervisors FORM ITC-E (Appendix X).
Assess the industrial project reports submitted by students
at the end of the internship programme using FORM ITC-
F (Appendix XI).
Provides necessary recommendations to improve the
programme.
9.2.4 UTP STUDENTS
To confirm an industrial project topic within two (2)
weeks after the programme start.
Send the industrial project schedules to the Industrial
Internship Unit within three (3) weeks after the
programme starts.
Adhere to the UTP and Host Company’s rules and
regulation (please refer General Plant Rules and
Regulations in Appendix XVI).
Follow the Host Company’s safety procedures.
Maintain good discipline and personal conduct.
Listen to and follow their supervisor’s instruction.
Meet work schedules (Section A: Appendix V) on time.
Discuss with supervisor on the training schedule (if
necessary)
Update their logbook regularly and present it to the Host
Company Supervisor for weekly review and endorsement.
Submit industrial project progress report to host company
supervisor in week twelve to week fourteen.

Be responsible and committed in acquiring as much
knowledge and skills as possible during the internship.
Complete and submit the logbook and industrial project
report to the SIIU at the end of the internship programme.
Perform oral presentation to the Host Company and UTP
supervisors during the second visit.
Make the necessary recommendations to improve the
programme.
10. OFFENCES & PENALTIES DURING INDUSTRIAL
INTERNSHIP
Students undergoing Industrial Internship programme must adhere to the rules
and regulations, as specified by the respective host company/industry and also
comply with the Students’ Disciplinary Rules & Regulations of Universiti
Teknologi PETRONAS and Academic Policies and Procedures.
Internship students committing inappropriate conduct during the course of
soliciting internship placement and during the period of undergoing industrial
internship programme will be subjected to the following penaltie as per Table 1.

11. ASSESSMENTS OF STUDENT INDUSTRIAL INTERNSHIP
PROGRAMME
Students will be assessed on their general attitude and receptivity to the working
environment in addition to being assessed for performing specific technical tasks.

11.1 ORAL PRESENTATION (FORM ITC-D)
Students are required to make an oral presentation describing the gist
of their industrial internship, experience gained during the training and
significant contribution that they have given to the host company. The
UTP and Host Company Supervisor will jointly evaluate the oral
presentation. The evaluation will be based on the following criteria in
the ITC-D Form (Appendix VIV):
o Visual and Non-Verbal Communication
o Content
o Clarity of presentation
o Questions and Answers
11.2 ASSESSMENT OF STUDENTS’ PERFORMANCE &
COMPETENCY (FORM ITC-E)
This ITC-E Form (Appendix X) contains the assessment of students’
performance during the Industrial Internship Programme. The nineteen
(19) attributes are evaluated based on the students’ performance, task
completion and competencies. Please refer to Appendix VII for more
details.

11.3 LOG BOOK
The logbook contains weekly and detailed reports prepared by the
students and endorsed by the Host Company Supervisor (Section B and
Section C: Appendix V). The students should describe the tasks or
projects they are undertaking, how these tasks contribute towards
achieving the Host Company's objectives, the problems and challenges
they face, the lessons learned from the experience, and provide any
comments or recommendations they wish to make. The logbook should
also record any points discussed with the Host Company Supervisor in
relation to the internship programme.
11.4 INDUSTRIAL PROJECT REPORT (FORM ITC-E)
The purpose of the report is to record all the relevant activities that
contribute towards developing the students' cognitive and practical
skills. Students will have the opportunity to exercise their writing skill
and to clearly communicate their ideas and experience.
It is compulsory for students to prepare and submit the Industrial
project report. It must be submitted to SIIU within three (3) weeks
after the official industrial internship date ends. Late submission will
result in a 50% deduction of marks specifically for the Industrial
project report. However, students who submit the Industrial project
report two weeks after the due date, no mark will be given for their
Industrial project report. No matter how late, students must submit the
Industrial project report before the new semester begins. Should a
student fail to submit the Industrial project report until the new
semester begins, the whole industrial internship exercise will be
considered incomplete.

11.4.2 Report Format
o Language
The Industrial project report must be written in
acceptable and formal English.
o Font and Spacing
All text should use Times New Roman regular font-
style size 12 with 1.5 line spacing.
o Numbering
All pages must be numbered in proper sequences from
Introduction to the end of the report including pages on
figures and tables. Page number must be positioned at
the bottom and centered.

The top, bottom and right margin are 25 mm except the
left margin which is 40 mm. All paragraphs should
start from the left margin.
o Number of copies
Student must submit one (1) hard cover copy & one (1)
CD of the Industrial project report to UTP Supervisor
within three (3) weeks after the official industrial
internship date ends. Upon marking the Industrial
project report, the hard copy and the CD will be kept in
the UTP’s Resource Centre.
o Hard bound copy
The Industrial project report must be properly bound
with hard cover. The hard cover color for each
Programme is as per Table 3:

The spine of the Industrial project report should
include students’ name and ID number, duration of
internship and the phrase ‘Industrial Internship
Industrial Project Report’.
o Front hard cover format
Format on front hard cover should include the phrase
‘Industrial Internship Industrial Project Report’,
duration of internship (month & year only), host
company’s name, student’s name, ID number and
programme.
12. STUDENTS’ FINAL GRADING
The Internship Sub-Committee which consists of Head of Industrial Internship
Programme (Chairman) and respective Department Internship Coordinators will
verify and certify the status of the Industrial Internship Progamme undertaken by
the students. The final grades will be given as per Table 4 below :

It is compulsory for all UTP students to have at least one health insurance policy. With the
increasing costs of medical expenses, students will definitely benefit from the health
insurance plan. The insurance plan selected was carefully created for students and encompasses two categories:

GENERAL PLANT RULES AND REGULATIONS FOR STUDENTS
INTRODUCTION
These rules and regulations form part of the conditions of students internship. Students should read them carefully and make sure they understand them.
(i) Any student who contravenes the Plant Rules will be reported immediately by his immediate Supervisor to the respective Manager or other Authorized Executive.
(ii) Where a serious offence is alleged, the student may be suspended from internship without allowance pending investigation. Where the offence is found to be not proven, the student will be reinstated and the period of suspension will be counted with the allowance (wherever applicable).
(iii) The Human Resource (HR) Training Executive together with the Industrial Relation Executive will investigate the circumstances of the offence and relevant witnesses will be called up to give evidence. The student concerned will have the right to give his/her version of the incident.
(iv) Where the HR Training Executive, in conjunction with the Industrial Relation Executive confers that the offence is proven, disciplinary action will be taken as follows, either :
a) Verbal warning
b) Written warning
c) Suspension without allowance
d) Dismissal with or without notice
(v) On a case to case basis, the gravity of the offence and the previous record of the students will determine the measure of the disciplinary action to be taken but as a guide for student, the offences liable for immediate dismissal with or without notice are pointed out in the text. However, it is not possible to cover all circumstances but there will be other serious offences which similarly may lead to immediate dismissal with or without notice. Such dismissal will not in any way prejudice the possibility of subsequent legal proceedings.
(vi) Apart from disciplinary offence, inefficiency and negligence will lead to a verbal warning, written warning or dismissal depending on the seriousness of the offence and the record of the student.
(vii) The management reserves the right to amend the existing rules or introduce new rules to meet the changing circumstances.

GENERAL RULES AND REGULATIONS
RULE 1 : GENERAL CONDUCT ON PLANT PREMISES
YOU MUST NOT :-
(i) Smoke in any place where signs prohibits you from doing so.
(ii) Sleep whilst on training/duty.
(iii) Commit any immoral act on the premises.
(iv) Commit any criminal offence.
(v) Fight with or strike any other employee.
(vi) Adopt an insubordinate attitude to any Supervisor or Executive.
(vii) Fail to obey any order given by the trainer or any Authorized Executive.
(viii) Incite or aid another student to commit any of the above offences.
PENALTY - If you commit any of the above offences, you are liable to dismissal with or without notice.
RULE 2 : HOUR OF TRAINING WORK
Training/working hours will be in accordance to the times of office hours/shift duty as determined by the OPU/subsidiary from time to time for specified jobs.
YOU MUST :-
(i) Observe the office working hours which specified for your training.
(ii) Work shift as required.
(iii) Be ready to start training/work at your appointed place and time in your work dress.
(iv) Be prepared to work overtime when the job requires it.
(v) Report immediately to your Line Trainer/Supervisor if you are late for work.
YOU MUST NOT :-
(i) Arrive late for training/work.
(ii) Stop training/work before the normal finishing time.
(iii) Leave your assigned training/work until relieved where you are undergoing continuous or semi-continuous shift.
PENALTY - If you commit any of the offences listed above, you are liable to dismissal with or without notice.
RULE 3 : TIME RECORDING
YOU MUST :-

(i) Clock “IN” at the beginning and “OUT” at the end of each training day or shift.
(ii) Report immediately to your Line Trainer/Supervisor if you accidentally clock the wrong card or if you find that someone has clocked yours.
YOU MUST NOT :-
(i) Clock other person‟s card.
(ii) Allow anybody else to clock your card.
(iii) Make any entry on or deletion in your or any others student‟s time card.
(iv) Tamper with the time clocks in any way.
PENALTY - If you commit any of the offences listed above, you are liable to dismissal with or without notice.
RULE 4 : ENTERING AND LEAVING THE PLANT AND SECURITY CHECK
YOU MUST :-
(i) Only enter or leave the Office or Plant through an authorized gate.
(ii) Produce your entry pass when required to do so to anyone appointed by the Management.
(iii) Accept the right of the Security Guards to search you when entering or leaving the Plant or during working hours in the presence of a responsible person authorized by the OPU/subsidiary.
(iv) Be only in the specified places as indicated in your security pass card.
(v) Accept the right of access of an authorized official of the OPU/subsidiary to inspect any locker provided.
YOU MUST NOT :-
(i) Leave the plant other than at the normal stopping time without a leave slip or permission from your trainer/supervisor or a Medical card from Medical Officer authorizing the Security to pass you out.
(ii) Take any material out of the Plant unless you have an official note giving you permission to do so.
(iii) Bring any vehicle, bag, basket, tin, parcel or other container into the premises, if deemed against the OPU‟s/subsidiary‟s rules and regulations.
PENALTY - You are liable to immediate dismissal with or without notice if you commit an offence.
RULE 5 : LEAVE, ABSENCE AND HOLIDAYS
YOU MUST :-
(i) Observe the gazetted holidays of the OPU/subsidiary or host company.

(i) Take any leave. However, in the case of emergency, obtain prior permission from your Line Trainer/Supervisor and if this is not possible you must telephone your Line Trainer/Supervisor immediately to give your reason.
PENALTY - If you are absent without prior permission or valid reason or proof, you are liable to immediate dismissal with or without notice.
RULE 6 : MEDICAL LEAVE
YOU MUST :-
(i) Inform you the Line Trainer/Supervisor immediately if you are sick and unable to work.
(ii) Inform the Line Trainer/Supervisor before 5.00 p.m. if you are on night shift and cannot attend.
(iii) Notify the Line Trainer/Supervisor 24 hours before returning to training/work if you are on shift.
(iv) Report immediately to your Line Trainer/Supervisor on returning to training/work after absence by reason of sickness or other cause.
(v) A certificate must be obtained from any registered medical practitioner to cover the period of sickness. The certificate has to be submitted to the Head of Training through your Line Trainer/Supervisor.
PENALTY - If you are absent without leave, you are liable to immediate dismissal.
RULE 7 : PANTRY AND AMENITIES
YOU MUST :-
(i) Take your meals only during the recognised meal breaks and in the OPU‟s/subsidiary‟s premises either in the canteen, pantry or mess rooms provided, unless otherwise authorised.
(ii) Take care of the pantry or mess room furniture and facilities and dispose of rubbish in the trays or bins provided.
YOU MUST NOT :-
(i) Eat your meals in any part of the Plant premises other than the canteen, pantry or mess room or otherwise without prior permission of the management.
(ii) Leave your training/job to take a meal where you are in charge of a continuous or semi-continuous process without the permission of your Supervisor.
PENALTY - If you commit any of the above offences, you are liable to suspension without allowance.

RULE 8 : USE OF PLANT AND EQUIPMENT
YOU MUST :-
(i) Use or operate any OPU‟s/subsidiary‟s property, buildings, fittings, plant, goods, machinery, tools, stores, materials in any stage of their use or manufacture unless it is in the course of your training/duty.
(ii) Do private work either for yourself or other people on the OPU‟s/subsidiary‟s premises.
PENALTY - If you cause deliberate damage to the OPU‟s/subsidiary‟s property or guilty of a gross negligence resulting in damage to the OPU‟s/subsidiary‟s property or financial loss, you are liable to immediate dismissal with or without notice.
