To opt-out of the Student Health Insurance Plan (SHIP):
1. Go to http://usm.maine.edu/uhcs
2. On the left-hand side of the screen, click the + button next to University of Maine System Student Requirements
3. Then click the link named Mandatory Health Insurance Requirement Info.
This will take you to a new page.
4. Click the Blue link that says waiver form. This will take you to the Cross Insurance website.
From this new page, click the 4th link down on the left side that reads Waive the Insurance. You will then be directed
to the Aetna site to begin the waiver process.

6. Select your campus (Click the BLUE UNDERLINED link that says 2016-2017 University of Southern Maine, enter
Waiver Form. Again, a new page will open up.
[6.] Enter your Date of birth (like this: MM/DD/YYYY), and then your MaineStreet Student ID. If you don’t know your ID
number, it is in the yellow box next to your name on your MaineStreet Student Center.
7. Click the “Get Started” button. You will now be given the option to enroll in a plan or waive. Select waive (orange
button).
8. You will then be prompted to be sure you have all of your current health insurance information.
9. Agree to the terms of the waiver policy, then press the Continue button on the right.
10. Have your Health insurance information ready. The waiver form will ask for your insurance company name, policy
number (also called your group number), and your ID number. If your insurance company is not listed, select other. If
you only have one set of numbers, use the same set in both fields. For the phone number, use whatever number is on
the back of your health insurance card. You will then have to answer two questions. The first being “Does your
current plan provide coverage for more than emergency care while in Maine?”, and the second being “Do you have
healthcare coverage that extends through the academic year?” Click the continue button on the right.
11. You will then be taken to the Waiver Summary Page. Please confirm that all of the information is correct before
submitting.
12. Submit the waiver form. You should receive a confirmation page next. Please check your UMaine System email
account for an email confirming that you have waived out of the insurance.
