NOTIFICATION OF ADDENDUM ADDENDUM NO. 2 DATED 4/29/2011 Ladies/Gentlemen: Attached please find an addendum on the above captioned project. Included in the attachment is an adendum notification which details the changes and the respective proposal pages which were added and/ or changed. Except for new bid insert pages, it is unnecessary to return any of the pages attached. Bid insert pages must be returned with the bid proposal submitted to the Department, unless your firm is submitting a bid using a computer print out. The computer print out must be changed to reflect the new bid item information. Contractors and material suppliers, etc. who have previously been furnished informational proposals are not being furnished a copy of the addendum. If you have a subcontractor on the above project, please advise them of this addendum. Acknowledgment of this addendum is not requested if your company has been issued a proposal stamped “This Proposal Issued for Informational Purposes.” You are required to acknowledge receipt of this addendum on the Addendum Acknowledgement form contained in your bid proposal by placing a mark in the box next to the respective addendum. ADDENDUM NO. 2

Ladies/Gentlemen:

Attached please find an addendum on the above captioned project. Included in the attachment is an adendum notification which details the changes and the respective proposal pages which were added and/or changed.

Except for new bid insert pages, it is unnecessary to return any of the pages attached.

Bid insert pages must be returned with the bid proposal submitted to the Department, unless your firm is submitting a bid using a computer print out. The computer print out must be changed to reflect the new bid item information.

Contractors and material suppliers, etc. who have previously been furnished informational proposals are not being furnished a copy of the addendum. If you have a subcontractor on the above project, please advise them of this addendum. Acknowledgment of this addendum is not requested if your company has been issued a proposal stamped “This Proposal Issued for Informational Purposes.”

In the event of a called evacuation, emergencies, impending adverse weather or as directed, do not perform any work without written authorization. The District reserves the right to suspend all work in support of evacuations or emergencies occurring from other parts of the state. Any work performed, other than work directed by the Department, is unauthorized work in accordance with Item 5. Sweep, clean and remove any construction waste, surplus materials or debris from the roadway and right of way at the end of each day unless otherwise approved. The work performed will not be measured or paid for directly, but will be subsidiary to pertinent Items. The asphalt season for the placement of asphalt surface courses is March 15 to November 15 unless otherwise approved. Cut existing pavement using a saw or other approved method to ensure a neat transverse and/or longitudinal line to assure a smooth tie-in with new pavement. Cut to a minimum depth of the final lift thickness. The work performed will not be measured or paid for directly, but will be subsidiary to pertinent Items. Promptly pick up and properly dispose of paper and other materials used for pavement joints. Stencil the National Bridge Inventory (NBI) number on each bridge and bridge class culvert. Use 3” letters or numbers. Use stain and color as approved. Paint will not be permitted. Locate the NBI number on the outside beam immediately adjacent to the abutment on the downstream end, on the outside headwall upper right-hand corner, or as directed. The work performed will not be measured or paid for directly, but will be subsidiary to pertinent Items. All pavement markings shall be in accordance with the latest edition of Texas MUTCD.

This project will have a US Coast Guard Bridge Permit. The following requirements apply:

1. Do not deviate from the approved plans without approval from the Commander, Eighth Coast Guard District.

2. Obtain approval from the USCG District Commander prior to implementation of construction of false-work, cofferdams or other obstructions.

3. Conduct all work in a manner that does not impede navigation or impair the depths of the channel.

4. Notify the USCG District Commander of any and all events affecting navigation during construction in a timely manner.

5. Promptly clear the channel of all obstructions due to construction as deemed necessary by the USCG District Commander.

6. Remove in their entirety, all parts of the Fender System not utilized in the completed system, for the bridge across the Copano Bay, mile 0.0, west of Harvey Lock. The waterway shall be cleared to the satisfaction of the USCG District Commander.

Notify the USCG District Commander three (3) weeks prior to beginning work. Submit a construction plan for approval, containing a description of the equipment, such as barges and tugs that may occupy the waterway, and how they may affect navigation, and a detailed work schedule.

Operate all water-going vessels, barges, etc. in accordance with USCG regulations. The U.S. Corps of Engineers Notice to Proceed permit number for this project is SWG-2010-00887. TxDOT will not issue a Notice to Proceed until the USCG Bridge Permit is issued. TxDOT will ensure that the Contractor complies with all conditions of the USCG Bridge Permit.. Do not initiate activities in a project specific location (PSL) associated with a U.S. Army Corps of Engineers (USACE) permit area that have not been previously evaluated by the USACE as part of the permit review of this project. Such activities include, but are not limited to, haul roads, equipment staging areas, borrow and disposal sites. Associated defined here means materials are delivered to or from the PSL. The permit area includes all waters of the U.S. or associated wetlands affected by activities associated with this project. Special restrictions may be required for such work. Be responsible for any and all consultations with the USACE regarding ALL activities, including project specific locations (PSL) not YET evaluated by the USACE. Provide the department with a copy of all consultations or approvals from the USACE prior to initiating activities. Proceed with activities in PSLs that do not affect a USACE permit area if the USACE determines that the PSL is non-jurisdictional or that the USACE granted proper clearance for the jurisdictional areas or as part of the permit review of this project, the USACE previously evaluated the PSL. Take sole responsibility for documenting any determinations that activities do not affect a USACE permit area. Maintain copies of determinations for review by the department or any regulatory agency.

Project Number: Sheet County: ARANSAS Control: 0180-03-035, ETC. Highway: SH 35 Document and coordinate with the USACE, if required, prior to any excavation hauled from or embankment hauled into a USACE permit area by either (1) or (2) below.

(1) Restricted Use of Materials for the Previously Evaluated Permit Areas. Document both the project specific location (PSL) and their authorization. Maintain copies for review by the department or any regulatory agency. When an area within the project limits has been evaluated by the USACE as part of the permit process for this project:

a. Suitable excavation of required material in the areas shown on the plans and cross sections as specified in Item 110 is used for permanent or temporary fill (Item 132, Embankment) within a USACE permit area;

b. Suitable embankment (Item 132) from within the USACE permit area is used as fill within a USACE evaluated area; and,

c. Unsuitable excavation or excess excavation [“Waste”] (Item 110) that is disposed of at a location approved by the Engineer within a USACE evaluated area.

(2) Contractor Materials from Areas Other than Previously Evaluated Areas. Provide the department with a copy of all USACE coordination or approvals prior to initiating any activities for an area within the project limits that has not been evaluated by the USACE or for any off right of way locations used for the following, but not limited to, haul roads, equipment staging areas, borrow and disposal sites:

a. Item 132, Embankment, used for temporary or permanent fill within a USACE permit area; and,

b. Unsuitable excavation or excess excavation [“Waste”] (Item 110, Excavation) that is disposed of outside a USACE evaluated area.

This project requires permits with environmental resource agencies. There is a high probability of encountering environmentally sensitive areas on Contractor designated project specific locations (PSL) for this project (haul roads, equipment staging areas, borrow pits, disposal sites, field offices, storage areas, parking areas, etc.). Listings of regulatory agencies the Contractor may need to contact for this project are provided herein. Environmental issues and PSL coordination requirements will be addressed in permits. The locations of the underground utility installations are unknown. Confirm the location of any installations which may exist. Assume full responsibility of the notification to the utility involved in case of conflict or damage and be responsible for damage that occurs due to negligence. This work is subsidiary to the various bid items of the contract. Assume ownership for all designated waste material and dispose of it at a place off of the right of way, as approved by the Engineer.

Highway: SH 35 For the purposes of satisfying requirements set forth by the U.S. Army Corps of Engineers Permit No. SWG-2010-00887 the contractor performing work on behalf of TxDOT will be referred to as the permittee. As part of the requirements under SWG-2010-00887, the permittee shall collect and relocate oyster reef permanently impacted during bridge pile construction. TxDOT has estimated approximately 1,660 cubic yards of oyster reef material will be relocated as a result of the bridge pile construction. The permittee will be required to clean the collected oyster reef material of sediment and debris prior to the material relocation. The permittee shall relocate collected oyster reef material to a previously approved oyster reef restoration site in Copano Bay authorized under the Harte Research Institute permit SWG-2010-00246. The permittee will coordinate all efforts with TxDOT during collection and relocation to ensure requirements under Permit SWG-2010-00887 are completed. The permittee shall not deposit collected oyster reef material until the permittee has received GPS coordinates (latitude/longitude) from TxDOT. Based on data from NMFS and the Harte Research Institute, the relocation site is within three miles from the current bridge location in varying depths of water from two to four feet. The relocated oyster reef will be placed to construct a 3-dimensional reef complex with high vertical relief to enhance habitat and protect an eroding shoreline while minimizing sedimentation. Because complex reef structures provide habitat for numerous fish and invertebrate species, the permittee will construct the reef as a series of reef ridges where “hills and valleys” are required design parameters. The permittee shall use silt curtains during construction around oyster reefs to minimize sedimentation in the surrounding water column. ITEM 2 It is recommended that prospective bidders examine the specified work locations with the Engineer to view the nature of the work, the need for close coordination with the various utilities, traffic control considerations, and other factors influencing the prosecution of the work. ITEM 5 Field verify all dimensions and notify Engineer prior to initiating any work. Verify the locations of utilities, underground or overhead, shown within the limits of the right-of-way. If pieces of the felt fall into the water or on the ground during demolition operations, carefully collect them and transport them to the ACM Removal Site. Notify the Department when concrete elements have been placed in the ACM Removal Site. Piling/Column Removal: Cut or break piling/columns as required by other Items but avoid disturbing the mastic. Transport piling or column sections with mastic on them to the ACM Removal Site. Not all of the piling or columns have the mastic. Final Disposal: The Department will notify you when the ACM have been removed. ACM removal may be done incrementally or all at once depending on the space available at the removal site. Once the ACM have been removed, dispose of the remaining concrete elements as necessary. ITEM 500 "Materials on Hand" payments are not considered when determining partial payments. If a Hurricane Watch is issued for this area, begin removing all rigging including cables, scaffolding and secure all materials and equipment to locations agreeable to both parties. 4586 Dynamic Monitoring and Analysis of Driven Piling

11.. DDeessccrriippttiioonn.. This item consists of (1) conducting the dynamic pile monitoring during initial pile driving and during pile restrikes and (2) the services of a dynamic testing consultant to provide dynamic analyses using Case Pile Wave Analysis Program (CAPWAP) and Wave Equation analyses programs. The CAPWAP and Wave Equation analyses shall be performed for the purpose of obtaining ultimate pile bearing capacity, pile stresses, pile integrity, and pile driving system efficiency. The Contractor shall supply all material, equipment, and labor to perform the dynamic monitoring. The Contractor shall obtain the dynamic measurements with the Pile Driving Analyzer (PDA) of monitor piles and furnish the necessary dynamic data to the dynamic testing consultant. 22.. EEqquuiippmmeenntt.

A. The Pile Driving Analyzer (PDA) shall be capable of four channels of strain data acquisition and four channels of acceleration data acquisition. The PDA shall conform to the requirement of ASTM D-4945.

B. To prepare the pile for sensor attachment, a drill of sufficient power, operated by either a

DC battery or a generator, shall be furnished. A hammer drill is required for preparation of concrete piles.

3. SSccooppee ooff WWoorrkk. The Contractor shall furnish equipment, materials, and labor necessary for conducting dynamic pile monitoring and contacting dynamic testing consultant for providing the CAPWAP and Wave Equation analyses.

A. Dynamic Pile Monitoring. This work consists of obtaining dynamic measurements with the Pile Driving Analyzer (PDA) of monitor piles during initial pile driving and during pile restrikes. Dynamic monitoring involves attaching at least two strain transducers and two accelerometers to the 24 inch square pile, and at least four strain transducers and four accelerometers to the 54 and 66 inch cylindrical pile near the pile head during initial driving and during restrike testing. A cable or wireless transmission connects the sensors near the pile head with the Pile Driving Analyzer located a safe distance from the pile, but not more than 330 ft from the pile. The cost of equipment mobilization or any delays due to dynamic pile monitoring shall be subsidiary to pertinent bid items. When dynamic monitoring is specified, the piles shall be lengthened by a distance of 2.5 times the pile diameter to allow access to the top of the pile at the end-of-driving. The additional length on the piles to be monitored is included in the total pile lengths shown in the plans.

During pile driving operations the pile driving stresses induced by the Contractor's hammer shall be monitored with the pile driving analyzer to ensure that the pile is not being damaged. When necessary, the Contractor shall reduce the driving energy transmitted to the pile.

Pile Restrike. Pile restrikes for monitor piles may be performed in accordance with the time intervals specified below unless otherwise shown in the plans. Monitor piles with dynamic monitoring shall have a 1-day restrike after the initial pile installation with the Pile Driving Analyzer monitoring the driving.

Field Report. Within one day of dynamic pile monitoring, the Contractor shall prepare a hand written daily field report summarizing the dynamic monitoring results. As a minimum, the daily reports shall include the calculated driving stresses, transferred energy, and estimated pile capacity at the time of testing. Daily field reports shall be transmitted to the Engineer.

B. Dynamic Analysis. The Contractor shall secure the Dynamic Testing Consultant (DTC)

to perform either a Case Pile Wave Analysis Program (CAPWAP) analysis or a Wave Equation analysis or a combination of both, for each occurrence of dynamic monitoring. The minimum requirement of the DTC shall be a Master Level PDA tester as certified by the Foundation QA. The Contractor shall furnish the necessary dynamic data obtained from the dynamic monitoring to the consultant who shall use the results from the CAPWAP data to assess pile installation stresses and integrity, as well as to predict the pile’s static bearing capacity and resistance distribution. This information will be used to verify the Pile Driving Analyzer’s Case pile capacity assumptions and to determine the distribution of soil static resistance, quakes, and damping factors required for the wave equation analysis. The dynamic testing consultant shall use the CAPWAP results to establish the relationship between stroke, energy, and blow count in the wave equation. The dynamic testing consultant shall submit two copies of the results to the Department’s Geotechnical Section within 1 week of receiving the data unless otherwise directed by the Engineer. Additional production piles may be monitored if deemed necessary by the Engineer. The cost of additional analyses shall be at the contract unit price for dynamic analysis unless it is determined that the monitoring is necessary because of Contractor error.

A. Dynamic Pile Monitoring. The number of dynamic monitoring to be paid for will be the number of dynamic monitoring occurrences on monitor piles ordered and completed. Dynamic monitoring during initial pile driving shall be considered as a single monitoring occurrence. Additionally, monitoring during pile restrike shall be considered as a separate monitoring occurrence. Dynamic monitoring occurrences that are necessary because of Contractor error shall not be measured for payment. Quantities of dynamic pile monitoring and dynamic analysis will be shown on the plans.

Steel Pipe Piling

1. Description. Furnishing of steel pipe piling in place, fabricated, reinforced, and filled with concrete must be in accordance with these specifications and plans, of the type and size shown on the plans, and in the locations as required by the plans.

Furnish the length of piling as indicated on the plans or authorized by the Engineer.

2. General. All materials and construction methods must conform to the requirements of this Item, and to the pertinent requirements of the following Items:

3. Material. Metal for the steel pipe pile must conform to ASTM A588, ASTM A572 Grade 50, or ASTM A1008 Grade 50, Class 2 . Hydrostatic testing of pipe is not required.

For concrete placement in pipe pile, supply Class “SS” concrete with a Coarse Aggregate Grade 4, 5 or 6.

4. Coating. Shop apply protective coating on pipe piling and structural steel elements as specified in Item 407.2 Materials, Paragraph 5

5. Storing and Handling. Store and handle steel pipe piling to prevent damage to the piling and coating system. Place the stored piling on blocking to prevent contact with the ground. Keep piling clean and fully drained at all times. Use a sufficient amount of blocking to prevent deflection in the stored piling. Utilize fabric slings or other methods as approved by the Engineer to handle the steel pipe pile.

6. Construction. The length of a steel pipe pile may be built up in sections prior to the driving operation. The minimum length of a section measured between splices shall be 25 feet. Splices must be made with complete penetration groove welds in accordance with Item 448, “Structural Field Welding,” for structural steel construction. Weld the sections together in a manner that will fully develop the section of pile and also form a water-tight joint. The Contractor is advised that accurate alignment of pile splice is critical and misalignment between sections should not be more than 1:200.

The Contractor must provide equipment and facilities necessary for the proper inspection of the piling. Any damaged, improperly driven, or otherwise defective piling will be evaluated by the Engineer and be removed and replaced if deemed necessary.

At the completion of driving, remove all soil from the pile by augering and high pressure washing, or other methods approved by the Engineer. Capture all soil and water removed from the inside of the pile and properly disposed of the material. No discharge of these materials into the surrounding water will be allowed.

Provide reinforcing steel in the piling as indicated on the plans. Care shall be taken to hold the reinforcement to true position in the pipe when placing concrete. Use concrete blocks or other suitable devices to prevent the displacement of the reinforcing steel.

Continuously place concrete in a single operation. If the interior of the pile is dry, concrete can be allowed to free fall an unlimited distance if it does not strike the reinforcing cage or sides of the hole during placement. When free fall is used, provide a hopper with a minimum 3-ft.-long drop tube at the top of the shaft to direct concrete vertically down the center of the shaft. Do not use a shovel or other means to simply deflect the concrete discharge from the truck. If the interior of the pile contains standing water, place concrete with a pump or tremie in accordance with the “Underwater Placement” provisions of Item 416, “Drilled Shaft Foundations”.

7. Measurement. Steel pipe piling will be measured by the foot of acceptable piling in place after all cut-offs and builds-ups have been made. Cut-offs and build-ups will not be measured for payment.

If the Contractor elects to drive piling deeper than required to meet the specified penetration requirements, no measurement will be made on the portion below the elevation at which penetration was first obtained.

8. Payment. The work performed and materials furnished in accordance with this Item and measured as provided under "Measurement" will be paid for at the unit price bid for "Steel Pipe Piling", of the specified size.

No payments will be made for cut-offs and build-ups.

The unit price bid will be full compensation for furnishing and driving the steel pipe pile, including alignment holes, pilot holes, and jetting, placing reinforcing steel, concrete and anchor plate, and for all materials, tools, labor, equipment and incidentals necessary to complete the work.
