What Is the NCLEX-RN®?

If you want to be licensed as a registered nurse in the United States or any of its territories, you are required to pass the NCLEX-RN (National Council Licensure EXamination-Registered Nurse) examination.

At its most basic, the exam is meant to determine if you are qualified to safely perform the responsibilities accorded to an entry-level registered nurse – this “qualification” is

meant to protect the safety of the general public as well as to assure you have the skill set required to practice positive and safe medicine.

The exam measures core competencies in a variety of areas that have been determined by the licensing authorities (The National Council of State Boards of Nursing (NCSBN) to be essential knowledge and praxis for entry-level RNs. The exam is a computer-adaptive test, otherwise known as a CAT.

A few facts about the exam:

It is given in English only. It is administered by Pearson VUE at their network of Pearson Professional Centers in the U.S. and abroad. The content is developed by practicing nurses, educators and regulators.

What Does the NCLEX-RN Test for?

The NCLEX-RN tests for levels of cognitive ability, like knowledge, comprehension and the ability to apply and analyze that knowledge. Most of the questions deal with application and analysis, since what is required of nurses is to be able to think critically and process complex ideas. As the test taker, you must analyze and apply information given in the test questions.

For example, a question that tests for cognitive ability might go something like this:

A patient who has a T5 spinal cord injury is complaining about a terrible headache and is showing signs of anxiety. The nurse notices the patient is perspiring and is bradycardic and hypertensive. What is the most appropriate nursing intervention that should be given initially?

talk to patient about their feelings of anxiety contact physician give the patient an analgesic check patient for bladder distention

The right answer is d, check patient for bladder distention. You are required to analyze the information given and to decide the best way to intervene initially as a nurse. You must know the symptoms of autonomic dysreflexia and possible causes, like full bowel or bladder.

What Is the Test Plan of the NCLEX-RN?

The National Council of State Boards of Nursing comes up with a Test Plan for NCLEX-RN by conducting a Practice Analysis study every three years.  The most recent study was done in 2005, which formed the basis for changes to the 2007 test plan. Over 6,000 newly licensed nurses were surveyed and asked about the frequency and priority of performing 150 nursing care activities.

The framework of the NCLEX-RN is derived from the analysis of the data collected from this study.  The National Council of State Boards of Nursing retained the "Client Needs" structure of the NCLEX-RN exam and effected minor changes in a couple of sub-categories to provide clarity and reduce redundancy. The questions that make up the new Test Plan address the levels of cognitive ability, patient needs, and integrated concepts and processes that an entry-level registered nurse has to be able to perform, so that patients receive safe and conscientious nursing care.

 The four major sections and subsections that deal with Patient Needs are:

Safe and Effective Care Environment Health Promotion and Maintenance Psychosocial Integrity Physiological Integrity

Let’s take a closer look at each.

Safe and Effective Care Environment

The two subsections under a Safe and Effective Care Environment are Management of Care (7-13%) and Safety and Infection Control (5-11%). Management of Care requires that a nurse collaborate with a multidisciplinary healthcare team in order for the patient to receive integrated, cost-effective care.

A question covering Management of Care might be something like this:

A patient in a medical unit is agitated and incoherent. The nurse in charge thinks that the patient is having a reaction to medication. The best intervention for this patient, one that ensures a safe environment, is to:

use a multidisciplinary approach to plan care ask the family to remain with the patient ask the physician to call for restraints and sedation get a nurse to watch the client

It is the nurse’s responsibility to give safe and conscientious care to the patient, so a is the correct answer since b, c, and d rely on other people.

Safety and Infection Control refers to the requirements that keep patients and health care providers safe from environmental hazards.

A question covering Safety and Infection Control might be something like this:

A patient being given total parenteral nutrition (TPN) is being taken care of by a nurse. What action can the nurse take that will lower the chances of the patient getting an infection from this therapy?

utilizing aseptic technique while handling TPN solution and tubing. making assessments of vital signs every 4 hours giving acetaminophen (Tylenol) before changing cental line dressing showing the client how to do a Valsalva maneuver while receiving intravenous tubing changes.

This question covers content regarding asepsis. Answer b will show signs of infection but isn’t related to prevention or lowering risk of infection. Answers c and d don’t have anything to do with infection. Therefore, a is the right answer because utilizing aseptic technique while handling TPN solution and tubing is paramount in the prevention of infection.

Health Promotion and Maintenance

There are two subsections under Health Promotion and Maintenance: Total Growth and Development (7-13%) and the Early Detection and Prevention of Disease (5-11%).

An example of questions covering Total Growth and Development is the following:

Which activity would be the most beneficial for good health among patients in an older adult day care center?

walking three to five times a week for 30 minutes cycling three times a week for 20 minutes sculpting once a week for 40 minutes gardening every day for an hour

The best answer is a. Walking, a 30 minute session three to five times per week, is one of the most beneficial forms of exercise for an older person.

Questions that come under the subsection Early Detection and Prevention of Disease would be structured something like this:

A nurse has given instructions to a patient on how to prevent thrombophlebitis. Which patient’s reply demonstrates an understanding of the ways to prevent this condition?

“I have decreased my fluid consumption to one glass of water a day.” “I avoid sitting or standing in one position for prolonged periods.” “I am taking oral contraceptives to avoid pregnancy, which can cause venous stasis.” “I’m glad I don’t need to wear those ugly stockings.”

B is the correct answer. One of the ways to prevent thrombophlebitis and venous stasis is to avoid sitting or standing for long periods of time.

Psychosocial Integrity

Coping and Adaptation (5-11%) and Psychosocial Adaptation (5-11%) are the two subsections under Psychosocial Integrity.

Coping and Adaptation are questions pertaining to the nurse’s ability to help a patient cope and adapt to their condition.

The following example is the kind of questions that fall into this category:

A hospice nurse is visiting someone who is dying of ovarian cancer. The patient says to her, “If I can just live long enough to attend my daughter’s graduation, I’ll be ready to die.” This patient is experiencing which phase of coping?

Acceptance Bargaining

Isolation Depression

This patient is experiencing Bargaining, answer b. It is the phase of coping when a dying person tries to bargain with fate or God.

Psychosocial Adaptation is the other subsection under Psychosocial Integrity. It pertains to the nurse’s ability to deal with a patient’s maladaptive behaviors and/or chronic mental illnesses. An example of these kinds of questions is as follows:

A nurse visiting an elderly male patient sees that he is restricted to his room by his daughter-in-law. The elderly man replies to the nurses suggestion that they go into the living room and join the family by saying, “I’m in everyone’s way; my son needs for me to stay here.” What is the most important action for the nurse to take at this point?

Tell the son that restricting his father to his room is inhuman. Suggest respite care and senior citizens’ resources to the patient and daughter-in-law. Stay neutral and don’t say anything until you are asked for help. Make a suggestion to the patient and daughter-in-law to investigate the option of a nursing home for the patient.

The correct answer is b. Much of the abuse of the elderly comes from care giver burnout and stress. The nurse is responsible for helping families understand community support systems that are available to them.

Physiological Integrity

Basic Care and Comfort (7-13%), Pharmacological and Parenteral Therapies (5-11%), Reduction of Risk (12-18%), Physiological Adaptation (12-18%) are the four subsections under Physiological Integrity.

Basic Care and Comfort covers the ways that a nurse should give comfort and be able to assist the patient in daily activities. The following is an example of these kinds of questions:

A patient has a weak right leg. Based on this knowledge the nurse decides that the patient would best benefit by…

straight leg cane walker lofstrand crutch wooden crutch

The right answer is a. A patient with only a little weakness should use a straight leg cane. The nurse directly helps the patient in his daily activities by providing him with a straight leg cane.

Pharmacological and Parenteral Therapies refer to a nurse’s ability to manage and give care by administering medication and parenteral therapies. An example of these types of questions is as follows:

A patient diagnosed with chronic angina pectoris is being cared for by a nurse. The patient receives sotalol (Betapace), 80 mg PO daily. Which of the following would occur if the patient is having side affect due to the medication?

bradycardia difficulty swallowing dry mouth diaphoresis

The correct answer is a. Sotalol (Betapace) is a beta-adrenergic blocking agent. Bradycardia, palpitations, difficulty breathing, irregular heartbeat, signs of congestive heart failure, and cold hands and feet are all side effects of this condition.

Reduction of Risk is the third subsection under Physiological Integrity. It pertains to the ability of the nurse to help reduce a patient’s risk of complications or health problems relating to existing conditions, treatments or procedures. An example of these types of questions would be:

A patient with acute myocardial infarction is receiving therapy with alteplase recombinant (tPA). The nurse makes an assessment for any possible complications that could arise due to this treatment. What assessment information would the nurse record as indicating a possible complication?

The right answer is b, epistaxis. A major side effect of tPA therapy is bleeding.

The fourth subsection under Physiological Integrity is Physiological adaptation, which addresses the nurse’s ability to give care to patients with acute, chronic or life-threatening physical health conditions. A question that would address this area would be:

A nurse goes over the blood gas results of a patient with Guillain-Barre syndrome. After analyzing the results, the nurse concludes that the patient is experiencing respiratory acidosis. Which of the following vonfirms the nurse’s conclusions?

pH 7.50, Pco2 30 mm Hg pH 7.35, Pco2 40 mm Hg pH 7.25, Pco2 50 mm Hg pH 7.45, Pco2 52 mm Hg

The right answer is c. The normal pH is 7.35 to 7.45. The normal Pco2 is 35 to 45 mm Hg. The pH is down and the Pco2 is up in respiratory acidosis.

A Note about Alternate Question Types

The majority of questions we have looked at so far have been the standard four-option multiple choice questions. While these make up the largest part of the NCLEX-RN, the exam also contains a variety of alternate questions types. These “alternate” questions are often used by the testing authority as experimental to determine suitability for future use. In other words, these question types may or may not count toward your overall score – so it’s best to treat them as real questions and answer accordingly. Let’s take a brief look at the question types:

Hot Spots Fill-in-the-Blanks Multiple Responses

Hot Spots will ask you to provide your answer by pointing and clicking your cursor on the appropriate screen location (e.g., on a specific location on the human body that answers a question).

Fill-in-the-Blanks will provide you with a statement that contains a missing word or number. You are asked to fill in the blank with the appropriate response to correctly complete the sentence or situation.

Multiple Responses will provide a question or situation that contains multiple answers that are needed for a fully correct response.

Say ‘Goodbye’ to Stress

Nothing can put the kibosh on all your best laid plans and preparation like stress. High anxiety, nervousness, and worry are natural – this is one of the most important tests you’ll ever take… so don’t be too hard on yourself if you’re feeling an unnatural level of apprehension. But, you don’t want your stress to take over.

There are many ways to help you alleviate – if not completely eliminate – your stress before and during the actual exam:

Exercise – With its natural benefits, exercise is an obvious choice to help you deal with your stress. Whether it’s walking 20 minutes a day or any other form of exercise you prefer, getting in some good cardiovascular time that also gives your muscles a chance to

move about and stretch out is always beneficial. In the days and weeks leading up to your exam, make sure exercise is a regular part of your daily preparation.

Diet – You know the old saying “You are what you eat,” well this is true for your test prep, too. Try to eat healthy foods that are low in sugar and processed carbohydrates. The initial sugar rush of candies and sodas may give you a burst of energy, but the inevitable crash makes learning and retention difficult. This is true of all stimulants, including coffee. So, be sparing in your use of such foods and beverages to power you up. Eat vegetables and unprocessed foods to allow your body to fire on all cylinders.

Sleep – Getting a good night’s rest has never been more important. Recent studies show that sleep deprivation is cumulative, so even if you go to bed early the night before the exam, if you’ve been staying up late studying for days or weeks previous, you’re going to feel sluggish and emotionally unprepared come exam day. Get eight hours of sleep every night in the days and weeks leading up to the exam. You’ll be amazed at how good your body feels when you wake – refreshed, alive, and ready for success!

Balance – Perhaps the most overlooked aspect of good preparation is balance. In the days and weeks before the exam, if you’re spending every minute cramming, you’re likely going to arrive on test day and be so tired, so overloaded with information, that you’re going to perform poorly. How rewarding would that be – to do all that work and end up not passing?

It’s vitally important for you to be balanced in your approach to preparation. A good program of exercise, healthy foods, the proper amount of sleep, AND a regular study routine will give you the best opportunity for success.
