During her tenure she led a statewide coalition that rewrote the Illinois Nurse Practice Act. Dr. Alexander was also a Clinical Nurse Specialist in Pediatrics at Rush University Medical Center and held a faculty appointment as an Assistant Professor in the College of Nursing. She received her Bachelor of Science in Nursing and her Master of Science degree from Northwestern University. Her PhD is in Nursing with an emphasis in Health Policy from the University of Illinois at Chicago. She has authored articles and book chapters, won several national awards and has given numerous presentations nationally and internationally. Ed Barnicott National Association of State Boards of Accountancy Ed Barnicott serves as NASBA’s Vice President of Strategic Planning and Program Management. Leslie Adrian, PT, DPT FSBPT Director of Professional Standards

Leslie Adrian is the Director of Professional Standards for the Federation of State Boards of Physical Therapy. Leslie’s responsibilities at the Federation include interacting with the jurisdictions to provide

consultation, technical assistance and training on a variety of regulatory issues as well as functioning as a staff liaison to support the efforts of the Ethics & Legislation and Foreign Educated Standards Committees. Her education includes graduate degrees in both Physical Therapy and Public Administration.

Ed Barnicott National Association of State Boards of Accountancy

Peggy Hobson Belmont, PT, MEdPresident, Virginia Board of Physical Therapy

Peggy Belmont graduated from The University of Alabama Physical Therapy Program in 1968 and earned her Master of Education, Early Childhood Special Education, from The George Washington University in 1988.

Peggy has over 33 years experience as a pediatric school-based PT including 10 years administering the Physical and Occupational Therapy Program for the Fairfax County Virginia Public Schools, where she supervised, trained and evaluated over 120 therapists. She worked with parents, educational advocates and community pediatric specialists for improving understanding of school-based therapy services for the benefit of students and ensuring the program was in compliance with current guidelines and regulations for the Individuals With Disabilities Education Act (IDEA) and other Federal and State laws, regulations and guidelines. Peggy was appointed to the Virginia Board of Physical Therapy in 2006. In 2012, she became President of the Board. In 2013, Peggy was appointed Co-chair of the APTA Section on Pediatrics, School-based Special Interest Group Task Force on PT State Practice Acts and Regulations that identified and reported on the concerns and conflicts of school-based PTs with IDEA, regulations, legal requirements and state practice acts.

Justin Berry, PT, DPT, MSJustin is the Director of the Physical Therapist Assistant Program at Northland Community and Technical College in East Grand Forks, Minnesota. He has presented on a variety of physical therapy topics, including PTA

education and professional issues, at the state, national and international levels. His research interest is in PTA education and issues.

Jean Bickel, JDJean Bickel is the government appointed member of the NJ Board of PT examiners. She is an attorney. Her undergraduate degree is from Bryn Mawr College, she also has a masters degree from Emory University and

her Law degree from Seton Hall University. She is the Assistant Director of the Division of Insurance for the New Jersey Department of Banking and Insurance. She is currently serving on the FSBPT licensure compact task force.

Mark R. Brengelman Attorney at Law, Hazelrigg & Cox LLP, Frankfort, KY

After being a graduate student at 20 to retiring from state government at 44, Mark has launched his private practice in health law after serving as a Kentucky Assistant Attorney General representing boards as

General Counsel and Prosecuting Attorney. He is a frequent participant in continuing education and a presenter for over a dozen national and state organizations; Mark is the founding presenter for “Navigating Ethics and Law for Mental Health Professionals.” Mark has now joined the law firm of Hazelrigg & Cox LLP, as partner—an established law firm tracing its history in Frankfort, Kentucky, over one hundred years.

Cindy CharltonCindy Charlton is a powerful speaker bringing her message of hope and inspiration to people of all ages. Her experiences of loss have allowed her the vision to see how precious life is. She is a survivor, living through a

traumatic illness, which took three of her limbs.

MEETING SPEAKERSAfter her own experience with illness, her husband died from cancer, leaving her a single parent with disabilities. Cindy refused to allow her disabilities to interfere with raising her children. She created a new life for her family, by continually moving forward one step at a time.

Judy ChepeusContinuing Competence Specialist, FSBPT

Judy joined FSBPT as a Continuing Competence Specialist in 2014. She previously served as the Marketing and Communications Coordinator for the Arizona Physical Therapy Association where she managed

all continuing education activity evaluation processes. Judy works to implement the tools and services of the FSBPT Continuing Competence initiative in support of the Federation’s overall mission of public protection. Judy received her Bachelor’s of Science Degree in Marketing from Arizona State University.

Mark W. Cornwall, PT, PhD, FAPTAProfessor, Program in Physical Therapy, Northern Arizona University

Dr. Cornwall received his bachelor’s degree in Physical Therapy from the University of Maryland at Baltimore in 1977 and a PhD in from Indiana University in 1986. He has been active in physical therapy

education and research for over 30 years. In addition, he has been active at both the local and national level of the American Physical Therapy Association and has served on the Arizona Physical Therapy Board. He is currently a Professor of Physical Therapy in the Department of Physical Therapy and Athletic Training at Northern Arizona University.

Crady deGolian Director, National Center for Interstate Compacts, Council of State Governments

Crady deGolian is the director of the National Center for Interstate Compacts, at the Council of State Governments. In his current role, Crady manages each of the National Center’s compact projects from development to

implementation. He worked extensively on The Interstate Compact on Educational Opportunity for Military Children and The Interstate Compact for Juveniles. He has also helped lead the development of several new compact efforts, including the Prescription Monitoring Program Compact, the State Authorization Reciprocity Agreement and the Electric Transmission Line Siting Compact. In addition to managing all of CSG’s compact activities, Crady has written extensively about interstate compacts, delivered national presentations on the topic and provided oral testimony before Congress about interstate compacts. Crady has been with the National Center for over four years in multiple capacities and with CSG for nearly six years. Previously, Crady worked on both the health and education policy staffs at CSG, first as a research associate and then as a policy analyst.

Originally from Louisville, KY, Crady holds a BA in history and political science from Furman University in Greenville, SC and an MA in Diplomacy and International Relations from the University of Kentucky. Crady and his wife Betsy have lived in Lexington for seven years and have one child, Cooper Crady.

Catherine Dower is a health policy consultant focusing on practice redesign and health professions regulation. She most recently served as Health Policy and Law Director at the UCSF Center for the Health

Professions, where she directed the Innovative Workforce Models in Health Care project and co-directed the Health Workforce Tracking Collaborative. For more than twenty years—with hundreds of presentations, publications, and testimony—she has been engaged in state and national efforts to reform health professions regulation and legal scopes of practice. Catherine received her undergraduate and law degrees from UC Berkeley and is licensed to practice law in California.

R. Kenneth Gordon, JDKen Gordon retired from Baker & McKenzie LLP as the Chair and Founding Member of the Global and North America Pharmaceutical and Healthcare Industry Groups in 2009. Ken currently serves on various

boards and has 20 years of experience serving on healthcare, corporate, civic and professional boards, including serving as Chairman of the Board of Community Hospital Corporation. He currently serves as the Public Member on the FSBPT Board of Delegates. His undergraduate degree is from Tufts University, and he holds a JD and an MBA from Northwestern University.

Lisa Hahn, MPADepartment of Health Professions, Richmond, VA

Lisa Hahn currently serves as the Executive Director for the Board of Long-Term Care Administrators, as well as the Board of Funeral Directors and Embalmers and the Board of Physical Therapy for the

Department of Health Professions. Ms. Hahn has over 25 years of regulatory experience. She has previously served as the Executive Director for other health regulatory boards as well as serving as the Deputy Director of Enforcement for the Department of Health Professions. Additionally, she served as the Chief for Private Security at the Department of Criminal Justice Service and was a former Richmond City Police Officer. Ms. Hahn holds a Master’s Degree in Public Administration from Virginia Commonwealth University, a Bachelor’s Degree in Criminal Justice from the University of Maryland. Lisa is the past President of an international association called CLEAR (The Council on Licensure, Enforcement and Regulation). She currently serves on the leadership team for the National Association of Long Term Care Administrator Boards (NAB). She also serves on three national committees for the Federation of the Boards of Physical Therapy. Ms. Hahn was born in Virginia. She is married and has two children. In her spare time, she enjoys reading, exercising and watching her children in their sporting activities.

James D. HeiderExecutive Director, Oregon Physical Therapist Licensing Board

Jim graduated from Central Washington State University in 1976 with a bachelor’s degree in Banking and Finance. Following graduation, he worked thirteen years for a national financial institution initially as a senior staff

accountant, eventually transitioning into the human resource division as manager of employee services overseeing payroll, benefits and training. In 1990, Jim accepted a position as Administrative Manager for a large Portland branch of a global insurance brokerage firm. As part of the management team, Jim was accountable for human resources, operations and administrative management. During his tenure, he earned the designation of Senior Professional in Human Resources, as awarded by the Society of Human Resource Professionals. In a planned effort to complete a balance in his career, in 2003, Jim left the private sector to accept a position in the public sector as Executive Director for the Oregon Physical Therapist Licensing Board. Per an Oregon Board directive Jim became involved with the Federation. Over the past several years he has served on numerous task forces and committees. In recognition of his accomplishments and contributions, in 2006, the Federation awarded Jim an Outstanding Service Award; in 2009 he was awarded the President’s award and, in 2010, he again was recognized with another Outstanding Service Award. Jim’s commitment and achievements, both with the Oregon member Board and the Federation, were the preface to his appointment to the Federation Board of Directors where he will complete his first full term in September 2014.

Heidi Herbst Paakkonen, MPAContinuing Competence Product Manager, FSBPT

Heidi joined FSBPT as its Continuing Competence Product Manager in 2011. She previously served as the Executive Director of the Arizona Board of Physical Therapy, and as the Executive Director of the

Arizona Physical Therapy Association. Her 13 years of experience in the public policy topic area of physical therapy continuing competence includes writing and implementing requirements; program development, enforcement and auditing, volunteer development and support, promoting awareness and quality assessment. Heidi earned her Masters Degree in Public Administration from the University of Wyoming.

Shiri Ahronovich Hickman, JDState Legislative and Policy Manager, Federation of State Medical Boards

As the FSMB’s State Legislative and Policy Manager, Shiri works directly with the FSMB’s member boards to improve the quality, safety, and integrity of health care by promoting high standards for physician licensure and practice.

Shiri supports state medical and osteopathic boards achieve their legislative, regulatory and policy agendas, and supports the FSMB in addressing policy issues such as telemedicine, licensure portability, electronic health records and special licensure categories. Shiri has a BBA from the College of William and Mary, a law degree from the University of Baltimore School of Law and admitted to practice law in Washington, D.C.

MEETING SPEAKERSJonathan Jagoda, MPP Director, Federal Government Relations, Federation of State Medical Boards (FSMB)

Jonathan Jagoda, Director of Federal Government Relations, Federation of State Medical Boards (FSMB) will discuss the development of the Interstate Medical Licensure Compact that will expand access to care,

facilitate multi-state practice and telemedicine, while ensuring state medical regulatory authority in the protection of the public. Mr. Jagoda will also be discussing the FSMB’s recently adopted Model Policy for the Appropriate Use of Telemedicine Technologies in the Practice of Medicine, which recommends the appropriate standards of care when delivering health care services directly to patients via telemedicine.

Mary T. Keehn PT, DPT, MHPEMary T. Keehn, PT, DPT, MHPE received her Physical Therapy degree and a Master’s Degree in Health Professions Education from the University of Illinois at Chicago (UIC), and a Doctor of Physical Therapy from the

Massachusetts General Hospital Institute of Health Professions. She was a faculty member and Director of Rehabilitation Services at the University of Illinois at Chicago for over 30 years. Dr. Keehn served on the Illinois Physical Therapy Licensing and Disciplinary Board from 2007 - 2013. She is currently a PhD student at UIC and also holds the position of Associate Dean for Clinical Affairs at UIC.

Other professional activities include serving as an onsite visitor for CAPTE and providing consultation services in rehabilitation, education

and administration through her consulting practice, Rehab Knowledge Exchange. Mary chairs the Foreign Education Standards Committee for FSBPT.

Julie KernanVice President, Global Account Management, Prometric

Julie Kernan serves as the Vice President of Global Account Management at Prometric. In this role, Ms. Kernan is responsible for leading a team of client services professionals to support Prometric’s global

client portfolio, which includes the Federation of State Boards of Physical Therapy and the NPTE Examinations. Her previous positions at Prometric have included vice president, executive director and director positions in the sales and marketing arenas.

Prior to joining Prometric, Ms. Kernan served as director of sales for Kelly Services, Inc., the $5 billion staffing organization. Ms. Kernan holds a bachelor’s degree from Indiana University in Bloomington, Indiana, where she focused on human resources and business courses. In addition to her education, Ms. Kernan is very active in the testing industry through ICE and serves on the Board of House of Ruth Maryland.

Nancy R. Kirsch, PT, DPT, PhDNancy R. Kirsch, PT, DPT, PhD, FAPTA received her PT degree from Temple University, her Masters in Health Education from Montclair University, Certificate in Health Administration from Seton Hall University, her PhD

concentration in ethics from Rutgers University (formerly UMDNJ) and a Doctor of Physical Therapy from MGH. She owned a private practice

for over 20 years and currently practices in a school based setting. In addition she is the Director of the Doctor of Physical Therapy Program at Rutgers, The State University of New Jersey. Nancy has been a member of the New Jersey Board of Physical Therapy Examiners since 1990 and was chairperson of the board for 12 years. Nancy is currently Vice President of the FSBPT Board. Nancy has also been active in the American Physical Therapy Association since she was a student. She served the New Jersey Chapter as Secretary and President, and as a delegate and chief delegate to the House of Delegates. She served the national association as a member of the ethics document revision task force. She also served a five year term on the APTA Ethics and Judicial Committee and the APTA Reference Committee. She received the Lucy Blair Service Award and was elected a Catherine Worthingham Fellow from National APTA and received the President’s Award from the FSBPT. Dr. Kirsch writes a monthly column in PT in Motion Magazine, called Ethics in Practice.

Cindy Krafft PT, MS, COS-C CEO Kornetti & Krafft Healthcare Solutions, President—Home Health Section APTA

Cindy Krafft PT MS COS-C has been actively involved in Home Health since 1994 from the clinical, leadership and consultation perspectives. She is currently the CEO of Kornetti & Krafft Healthcare Solutions, the

President of the Home Health Section of the APTA and the past Chair of the Therapy Advisory Committee for the National Association of Home and Hospice Care.

Alan Chong W. Lee, PT, PhD, DPT, CWS, GCSMount St. Mary’s College Associate Professor

Dr. Alan C. Lee is an associate professor at Mount St. Mary’s College in Los Angeles. Dr. Lee has served as the secretary of Telerehabilitation SIG for the American Telemedicine Association. He is the telehealth

group leader for Frontiers in Rehabilitation Science and Technology (FiRST). His telehealth research includes international pediatric teleconsultation in physical therapy and he co-authored the Blueprint for Telerehabilitation Guidelines. Dr. Lee maintains clinical practice at Scripps Mercy Hospital in San Diego with dual board-certifications in geriatrics and wound management.

Kathleen A. Luedtke-Hoffmann, PT, MBA, PhD Associate Professor—School of Physical Therapy, Texas Woman’s University, Member—FSBPT Ethics & Legislation Committee

Dr. Luedtke-Hoffmann will present the work completed thus far by FSBPT’s Ethics & Legislation committee to research current telehealth legislation currently in effect and develop guidelines for state

jurisdictions considering provision of physical therapy using of telehealth. Additionally, Dr. Luedtke-Hoffmann will discuss the challenges to enacting telehealth as a member of the Texas Board of Physical Therapy Examiners.

MEETING SPEAKERSDan ManzExecutive Director Essex Rescue

Dan Manz is the Executive Director for Essex Rescue in Essex Junction, VT. He supervises all operations and staff for this paramedic level private not-for-profit ambulance service providing 9-1-1 EMS

coverage to five communities including Vermont’s second largest city. He previously worked in the Office of Public Health Preparedness and EMS for the VT Department of Health. He was VT’s EMS Director for 25 years and has been in EMS for more than 35 years working as an EMT, volunteer squad leader, hospital communications technician, EMS regional coordinator, EMS trainer and State EMS Director. Much of his work has been in rural areas including Maine, Vermont and Saudi Arabia. He served as a Board member for the National Registry of EMTs for seven years. Dan has been active in the National Association of State EMS Officials (NASEMSO), serving as their President for two years and representing the association in several national projects including the HCFA Negotiated Rule Making process and development of the National EMS Scope of Practice Model. Currently he is chairing a NASEMSO group of national partner organizations charged with implementing the National EMS Education Agenda for the Future: A Systems Approach. He is also working with NASEMSO on a project to develop a national EMS licensure compact. Dan enjoys hiking, fishing and distance running.

Rick Masters, JDSpecial Counsel, National Center for Interstate Compacts, Council of State Governments

Rick Masters is Special Counsel to the National Center for Interstate Compacts (‘NCIC’), which is affiliated with the Council of State Governments (‘CSG’), providing legal guidance concerning the law

and use of interstate compacts, including application and enforcement as well legislative drafting of proposed compact statutes in conjunction with the various projects in which the NCIC is involved. Rick is also a recognized subject matter expert in the field of interstate compacts and provides legal advice to several other compact governing boards and agencies many of which are affiliated with CSG. He has testified frequently before state legislative committees concerning a wide variety of compact legislation and has also provided testimony to the U.S. Congress concerning compact consent legislation and related interstate compact legal issues. Rick has also been counsel of record in a number of federal and state cases involving important interstate compact issues including a recent published decision of the U.S. Court of Appeals for the Tenth (10th) Circuit upholding the validity of the regional low-level radioactive waste compacts to which most of the states are members.

Rick has been involved in extensive research and writing in the field of interstate compacts and has published a wide variety of law review articles, bench books used by state court judges, and other publications concerning the law and use of interstate compacts. He was recently asked by Kentucky Governor Steve Beshear to serve as a Special Justice to the Kentucky Supreme Court and was appointed by the Governor in November 2012 to serve a four (4) year term as a Commissioner on the Executive Branch Ethics Commission.

Sherri Paru received her PT degree from Ithaca College in 1991. Since 2002, Sherri has been the Clinical Advisor and Investigator for the Oregon Physical Therapist Licensing Board. She is a Certified

Jeff Rosa is the current Executive Director of the Ohio Occupational Therapy, Physical Therapy and Athletic Trainers (OTPTAT) Board. Jeff has been with the Board since October 2003. Prior to joining the Board,

he served as the Legislative/Regulatory Specialist with the Ohio Board of Nursing. He also spent six years as a budget analyst with the Ohio Legislative Service Commission, the Ohio General Assembly’s non-partisan research office. Jeff has been involved with the Federation since

2004. He has been a member of the FSBPT Board of Directors since 2008 and previously served as the Chair of the FSBPT Council of Board Administrators in 2005 and 2008 and was a member of the FSBPT Finance Committee and Education Committee. Jeff received the Outstanding Service Award from the FSBPT in September 2006.

Practice Nurses, Professor Safriet was selected as the Inaugural 23 Honorary Fellow of the American Association of Nurse Practitioners, and inducted as an Honorary Fellow in the American Academy of Nursing. Her most recent work is published in the groundbreaking IOM report The Future of Nursing: Leading Change, Advancing Health entitled Federal Options for Maximizing the Value of Advanced Practice Nurses in Providing Quality, Cost Effective Health Care. Professor Safriet continues to teach, lecture and publish on the regulation of health care providers, especially concerning scope of practice and reimbursement.

Independent PT Licensing Board in Virginia. She has served as Chapter President of the Virginia Physical Therapy Association, Chair of the Direct Access Task Force, a member of the Legislative Committee and is currently serving as Chief Delegate to the APTA House of Delegates for Virginia. She has been active on several FSBPT Task Forces including Continuing Competency and Foreign Credentialing, as well as participating in various FSBPT activities. She has been a credential reviewer for FCCPT and is currently a member of their QRC Committee. Nancy White, PT, DPT, OCS is Senior Director of Clinical Practice and Research at the American Physical Therapy Association. In this position, she provides leadership to advance the scientific practice of physical

therapy through initiatives such as the development of clinical practice guidelines, integration of evidence into practice and collection of data to support the value of physical therapist practice. For the past year, Nancy has been APTA’s staff lead for the Choosing Wisely initiative and has been a member of the Integrity in Practice staff team.

Prior to joining APTA in 2009, Nancy spent 30 years in direct clinical practice and management. She is a Board Certified Orthopaedic Clinical Specialist and has been an active APTA member since 1979. She served in leadership positions in the Virginia Chapter, the Orthopaedic Section and the Foundation for Physical Therapy. She has a Master of Physical Therapy degree from the University of Alabama in Birmingham, and a DPT from Marymount University.

James A. Wollock, PhDUniversity of Wisconsin—Madison (UW)

James Wollack is an Associate Professor of Educational Psychology at the University of Wisconsin—Madison (UW), and serves as the Director of the UW Testing & Evaluation Services and the UW System Center for

Placement Testing. Dr. Wollack’s scholarly interests focus most squarely on issues associated with test security and test

construction, having published numerous articles in scholarly journals and delivered many presentations at professional conferences. He is also the co-editor of the Handbook of Test Security (2013, with J. Fremer). Dr. Wollack frequently consults with state testing programs and testing organizations (including serving on the FSBPT Technical Advisory Panel), serves as an expert witness to certification agencies on cases involving suspected cheating on high-stakes exams, and presides over cases of alleged student academic misconduct at the University of Wisconsin.

Dr. Wollack is currently on the Board of Directors for the National College Testing Association and the Executive Committee for the Conference on the Statistical Detection of Potential Test Fraud. He recently completed a term on the Board of Directors for the National Council on Measurement in Education and is a past President of the Measurement Services Special Interest Group of the American Education Research Association (AERA).
