Endodontic cavities are particularly challenging for bulk fill composites, as most are very deep Class I cavities. This maximises shrinkage stress potential, indicated by a high C-factor. Studies indicate that when high C-factor cavities are filled in bulk, the choice of the composite is an important element to avoiding adhesive debonding.

Unlike competing composites, SDR material retains exceptional bond strength when bulk-filled into cavities, similar to the strength it exhibits when bonded to a flat dentin surface under ideal laboratory conditions. This advantage is reflected in the correlation between µTBS results and actual failure rates. Pre-test failure rates were 100 percent for the Filtek Z100 samples and more than 70 percent for both Filtek Bulk Fill Flowable and Tetric EvoCeram Bulk Fill. SDR material showed no pre-test failures.

Additionally, the highest curing light irradiance at the cavity floor was measured for SDR material, indicating the greatest likelihood of achieving a fully cured bond at this crucialinterface. Light irradiance waslower for the other bulk-fillcomposites and lowest forthe conventional layered-fill composite Filtek Z100.

The Results

When built up on flat dentin surfaces, the tested materials exhibited no significant differences in µTBS and no pre-test failures occurred. However, all the composites except SDR material showed a large decrease in µTBS when actual Class I cavities were bulk-filled, indicating significant polymerisation stress resulting in material shrinkage.

SDR™ Material Outperforms Competing Bulk-Fill Composites for Reliably Bonded RestorationsCompared to traditional layered fill techniques, composites designed for bulk-filling can significantly reduce the time and effort for performing Class I and Class II restorations. However, many dentists have been concerned that the bulk-fill technique may lead to premature failure of the restoration due to inadequate bonding at the cavity floor or excess material shrinkage.

A recently published study shows that these concerns are unfounded when SDR material is used to bulk-fill restorations up to 4mm deep. The peer-reviewed study found that SDR material delivered the highest light irradiance at the cavity floor and significantly less polymerisation stress compared to other leading bulk-fill composites. Dentists can expect a fully cured bond and minimal shrinkage for a stronger, longer-lasting restoration, even with high C-factor cavities.

The Study Design1

The study, “Effect of Bulk-Filling on the Bonding Efficacy in Occlusal Class I Cavities,” was published in the Journal of Adhesive Dentistry 2016, 18:119–24. Authors include A. Van Ende, J. De Munck, K. Van Landuyt and B. Van Meerbeek. The study compared four composites:

• Filtek Z100 (3M Espe), a conventional light-curing composite

• Filtek Bulk Fill Flowable (3M Espe)

• Tetric EvoCeram Bulk Fill (Ivoclar Vivadent)

• SDR™ Smart Dentin Replacement Bulk Fill Material (Dentsply Sirona)

These materials were bulk-filled into 4mm deep Class I cavities. To provide a baseline for evaluating the relative strength of these restorations, the materials were also built up in bulk on flat dentin surfaces using moulds that reproduced the dimensions of a Class I cavity. All materials were bonded to the dentin using the same adhesive. Additional samples were prepared to measure the curing-light irradiance at the bottom of each composite fill using a spectrometer.

The samples were stored in water for one week, then sliced into microspecimens and tested for microtensile bond strength (µTBS). Fracture surfaces were analysed using conventional light microscopy and representative samples were further examined using scanning electron microscopy.

SDR material provides excellent cavity adaptation, even in the deepest corners of the proximal box. It self-levels, with no further manipulation required, to provide an optimum base for the final occlusal layer and to simplify placement even on maxillary posterior teeth. The unique SDR chemistry provides significantly less polymerisation stress compared to universal composites – including other bulk-fill materials – allowing layers up to 4mm thick even in high C-factor cavities.

Successful restoration of the endodontically treated tooth continues to be one of the most challenging procedures in dentistry.

This is largely because of the complexity of the process, a myriad of available treatment options and a confusing array of dental literature dealing with individual components of this multifaceted treatment equation.1

Long-term retention of the endodontically treated tooth is dependent on the collective success of the tooth canal filling and coronal restoration. Put simply, if the root canal filling or the coronal restoration is inadequate, either is equally contributive to an unsuccessful outcome.2 Therefore, the first step in developing an appropriate treatment plan for a tooth requiring root canal therapy is to determine if the tooth will be restorable. Factors that may influence this determination will include: amount of remaining coronal tooth structure after caries excavation and the ability to develop a 1.5-2mm circumferential ferrule, periodontal health, occlusion, crown to root ratio, tooth location, number of adjacent teeth, requirement to use the tooth as an abutment for a FPD or RPD, and the presence of para-functional habits.3

If the tooth has been judged restorable and has received adequate root canal therapy, the next treatment planning decision involves the need for a post and core, or just a crown build-up. A post or dowel has been historically placed to retain the foundational core and to add retention of the crown that would have normally been gained from coronal tooth structure.4 Determining factors at this stage include evaluating the height and thickness of remaining dentin after tooth preparation, the number of dentin walls remaining, and the final occlusal scheme.5

In clinical situations where there is little dentin remaining (less than half the coronal tooth

structure; zero to two dentin walls remaining after caries excavation and tooth preparation), the use of a post is indicated. When there are no walls of dentin remaining a cast post and core cemented with resin cement is an appropriate treatment plan. When one or two walls of dentin remain or in anterior situations, a prefabricated metal or fiber post should be considered.1,6

With two or more walls remaining or when greater than one half of the coronal tooth structure remains the dentist may choose to forgo a post and simply use composite to place a crown build-up. In selecting the material best suited for a build-up material, dentists must consider the size and geometry of the preparation and access to enable light transmission. In areas where light transmission is difficult or impossible, a dual-cure composite like Fluorocore 2+ is indicated. However, in areas where the tooth can be isolated with a sectional or circumferential matrix and it can be accessed with a curing light, SDR (Smart Dentine Replacement) is an excellent material choice because of its cavity adaptation and bulk-filling properties. Because SDR can be placed in 4mm increments large cavity forms can be restored in fewer procedural steps. In areas where the core build-up will be placed into function, SDR should be capped with a 2mm layer of hybrid composite (like ceram.x® universal). Also, because of its self-leveling handling and high radiopacity, SDR can make the process easier, not only during the placement steps but also when evaluating the restoration on post-operative radiographs.

In most cases the last step in restoring the endodontically treated tooth involves the decision to place an indirect restoration to achieve cuspal or full coverage. In general, cuspal or full coverage is recommended to prevent fracture and increase long-term survival.7,8

When a local Dentist in our area was looking to retire, I decided to introduce myself. As we had similar

dental ideologies he invited me to work the odd day in his rooms to catch his patient overflow. This quickly turned into a full time position with the opportunity to eventually purchase the practice. It was obvious that the dated premises needed upgrading, so I started researching what equipment would suit me best.

Dental friends of mine had chosen Dentsply Sirona products for their practices in the past, and had informed me of how it had transformed their businesses, but at that time I was a recent graduate on a limited budget so I struggled on with what equipment I had inherited. However, their enthusiasm stayed with me and I wanted to see how Dentsply Sirona technology could steer my new practice towards my vision. Knowing the quality reputation of Dentsply Sirona products, I called in a local representative for an informal chat. It was one of the best decisions I have made in my career. Not only has the Dentsply Sirona business partnership worked for me, but the business model in terms of the return on investment has allowed me to successfully transform my practice to a place I enjoy working in. As a company, Dentsply Sirona has given me wonderful support with information, installation and training, together with outstanding ongoing service.

Previously, I had never quite understood the real value of “work flow” until my wife observed that I wasn’t arriving home exhausted anymore. I still had energy to go out for dinner during the week, help out at home and even take Phoebe, our dog, for a walk. At the practice I have noticed that the quality of my work has improved. My preps are clearer and the treatment cases I am doing have become more comprehensive. Patients have mentioned that they feel more relaxed; opening themselves to previously shelved dental work. My newly installed Teneo chair has contributed to patients having a positive “dental experience”. Dentsply Sirona has an in-depth understanding behind the science of ergonomics and engineering and has implemented this knowledge into the Teneo. It is evident from the first use by the way the chair positions the patient back into a relaxed pose. As Geoff, the Dentsply Sirona equipment specialist says “it holds the patient in a cuddle, like a mother holds a baby”. The design

allows for the convenience of equipment use, which has lowered stress levels for myself, the staff and the patient. The Teneo is totally programmable and allows me to control the entire chair, from the automatic drive on the delivery system, including the touchless LED overhead light, the adjustable assistant’s unit, the handpieces and a wireless foot controller. This is the key as I am twisting less, leaning less and stretching over for equipment less. I have been able to program and control the chair to suit my method of working; the chair fits into my work flow and not the other way round. It has become an extension of me. In addition, the well thought out design allows for all handpieces, the built in scaler, the curing light, the endodontic and implant handpieces etc. to be built into the chair. They are not an add-on or an after thought. The response, weight and feedback I am getting through the equipment allows for greater control and less guess work, therefore more precision and less fatigue. From the patient’s perspective little enhancements get the greatest responses, such as the water rinse, handpiece water and spray all being warmed, the cuspidor coming towards them when they rinse out, the back massage feature and the ability to save a patient specific position. My assistants love the built in automatic cleaning system and I am at peace knowing that the water lines are kept pristine with no biofilm contamination.

Other chair manufacturers claim to have all encompassing chairs, but Dentsply Sirona have excelled with the Teneo. Other beneficial features include having the endo system with apex locator and implant handpieces built into the chair and the red band handpiece being configured next to the other handpieces ready to go when required. The Sirocam intra oral camera is also installed in the delivery unit, allowing a patient to view their mouth on the screen mounted on the chair. There is no need to haul out a separate endo system or apex locator from the cupboard, a separate surgical unit or wheel in the implant trolley, it’s all built in, right where I need it and within easy reach.

All in all, I am delighted with my Teneo and cannot speak highly enough of Dentsply Sirona’s professional service in all aspects of bringing this technology into my practice.

There are so many tasks to take care of in themodern sterilisation department. On a busy day, it can be a challenge to get all these tasks completed, whilst ensuring that all guidelines and standards are adhered to.

The design of the sterilising room should follow some basic principles. Workflow should be easily identified in a flow that moves from:

• Dirty: Pre-cleaning area (including thermal disinfection); and Ultrasonic

• Clean: Drying area; Packaging area; Articles awaiting sterilisation; and Steriliser

• Sterile: Cooling area for articles waiting storage

• Storage: area for clean & sterilised instruments

Working within the guidelines does not mean that style is sacrificed. Beautiful spaces with optimal workflow can be achieved, making the sterilising room a functional stylish space in your practice.

Tips when designing a reprocessing room

• Take a look at the size of the reprocessing room and consider if the capacity is sufficient for the size of the practice. The reprocessing room should be large enough to carry the workload required

• Make sure that the bench space is at least 600mm to 700mm deep. This will allow the sterilisers to comfortably sit on the bench with enough room for a technician to manoeuvre the units during routine maintenance

• Ensure that there are enough power and data points for current and future needs, and possible additional devices

• Consider the power needs of the sterilisation equipment (15amp for premium model autoclaves)

• Ensure that the sterilisers are placed in a position not more than 2 metres from the reverse osmosis system and not more than 1 metre from a drain for direct draining of the waste water

• Ensure that there is sufficient airflow and that the units are not boxed in by overhead cupboards

• Make sure that there is a compressed air line in the sterilisation room for the inclusion of a handpiece maintenance unit

A diode laser uses one or more laser diodes (semiconductors) as a medium to generate the laser beam. The wavelength of the rays is the decisive factor as this will determine how the laser energy is absorbed by the tissue.

Absorption = effective laser treatment. The cutting and disinfection capacity of a laser largely depends on the energy that is absorbed in the tissue. Choosing a wavelength which is absorbed to the maximum extent into haemoglobin and melanin means faster more effective laser treatment.

With its very high absorption into haemoglobin at 445 nm, the SiroLaser Blue is ideal for all soft tissue applications. Clean deep fast incisions without contacting the tissue.

While there’s still an important role for hand scaling to play in oral care, the opportunity to use ultrasonic scaling can make a real difference to both patient and clinician.

For oral hygiene care, ultrasonic scalers can have a significant impact on the disruption of biofilm. For the clinician, the scalers offer an opportunity to reduce the pain and discomfort of daily hand scaling. In this article, hygienist Cathy Hendrickson explains how the Cavitron Touch with Steri-Mate 360 offers unprecedented comfort and control throughout every procedure, for increased scaling confidence.

Using the Cavitron Touch from Dentsply Sirona has increased my confidence in scaling.

It is powerful yet gentle and very easy to use. I do not get as tired during long root-planing appointments, as I feel I do not have to do as much hand scaling as before.

I had been using a different ultrasonic scaler, but the Cavitron Touch is far more powerful. Even when using older inserts, my tactile sense has improved and is greater even with fine calculus. It has made a huge difference, shortening the time needed for appointments.

More difficult patients are easier to treat and actually enjoy treatment, because they do not come away from the appointment with sensitivity or pain. I recently had a patient who said,

“I love what you are doing with that. I can feel how deep you go and yet it is not painful. I love it!”

Even patients who are sensitive to cold water can tolerate treatment with the Cavitron Touch because the water flow adjustment is on the handpiece and the lower the flow of water, the warmer the water feels.

I also like the touch technology of the foot pedal: touching the pedal engages the handpiece and touching it again turns it off. And because it is cordless, I feel I don’t have to stretch my leg to reach it. I can put it wherever I want to make it comfortable for me. The main unit has a touch screen where I am able to select the power setting along with other options that make it very user-friendly. The touch screen also has all the options lit up very nicely, which makes it very easy to read. With a touch, you can change what you are doing: power-scaling options and settings, purging, rinsing, other settings, etc. I really like using the Cavitron Touch. Using it for the first time on a patient had me sold. It is certainly a product that makes my job easier and more fun!
