Parents provided paper survey immediately following every IEP/EP meeting;

Provided in English, Spanish and Creole;Options for parents to complete:

Complete immediately, seal, and return to ESE Program Specialist to send via school mail to ESE;

Complete at home, seal, and return to ESE Program Specialist to send via school mail to ESE;

Complete at home and mail directly to ESE; Complete anonymously and add comments.

PARENT Individual Education Plan (IEP) SATISFACTION SURVEY

2008-2009

Before you leave today, please complete the following survey regarding your experience at your child’s IEP meeting on a scale of 1 to 5 with 1 “strongly disagree” and 5 “strongly agree”. Your input is valued and necessary for our continuous improvement of services to students with disabilities. Thank you for participating.

1. The meeting was held at a mutually agreeable time with the school and with me.

2. I understood the reason for the meeting I was invited to attend. 3. I was given the opportunity to provide input prior to the meeting

through the “parent planning notes” found on the meeting notice.

4. I was satisfied with the communication from the school prior to the meeting.

THE MEETING 5. I felt my input was valued as a team member. 6. There was input from the appropriate people on the team (general

education teacher, special education teacher, etc.)

7. My concerns and questions were addressed at the meeting. 8. The team considered my child’s strengths. 9. I was comfortable expressing my opinions during the meeting. 10. At the end of the meeting, I had a clear understanding of the services

that would be provided to my child.

11. The participants at the IEP meeting were prepared and informed. BENEFIT OF SPECIAL EDUCATION

Surveys Available

Two notebooks placed in Board office with actual surveys that were returned to ESE.

ESE Staff called parents who listed phone number who indicated they had questions or concerns.

Input from parents was valuable to schools and department.

2009-2010 Survey

To increase response rate for 2009:Online survey will be available in

school office for parents to complete immediately following the IEP/EP meeting;

Online survey may be completed from home following the meeting; or

Paper form will be available upon request.
