The Basics

•Fill out form completely and accuratelyF ll • Full name

• Person ID (Employee number or PERNER)

• Actual classification/job name (Ex. Office Technician)

• Actual classification/job code (Ex. 24102828 or 2828)• Hours of assignment

• Location, location code and position control number. , p• Position control number (obtain from your Fiscal Specialist)  

• THIS IS NOT the budget confirmation number

• Appropriate signatures• Site contact information 

Request for Personnel Action (RPA)

Key Information

• Dual purpose formp pSubmit to Budget for all FUNDING changes

Submit to Classified Employment Services for HIRING and ASSIGNMENT changes.

• If the FUNDING change impacts the ASSIGNMENT, your Fiscal Specialist will advise if it should be submitted to Classified Employment Services as well. 

• FUNDING modifications (same assignment level) does not trigger an reassignment from one position control number to another. FUNDING changes are handled by the Fiscal Specialist.

Request for Transfer (Change of Work Location)

Request for Transfer (Change of Work Location)

Key Information

• Request for Transfer form is an employee request to have his/her name added to the transfer list for currentclassification.

• Submitting a Request for Transfer form DOES NOT trigger an automatic transfer. Employee’s name goes out with eligibility list.

A R  f  T f  f  CANNOT b  HELD   • A Request for Transfer form CANNOT be HELD nor DISAPPROVED.

• There are deferral time limits that are specified in collective bargaining agreement.

Request for Bilingual Differential

Request for Bilingual Differential 

Key Information

• The Request for Bilingual Differential is at the discretion of the administrator, NOTthe employee.p y

• Permanent employees and Teacher Assistants are eligible to receive a bilingual differential; substitute and restricted status employees ARE NOT eligible.

• If administrator is approving employee for a bilingual differential, ALWAYS confirm that employee has successfully tested for language skill and submit with Nepotism form at hire.

• The bilingual differential DOES NOT follow the employee.

Th  bili g l diff ti l i   ff ti    th  fi t d   f th   th f ll i g • The bilingual differential is effective on the first day of the month following submission of form. There are NO EXCEPTIONS or RETROACTIVE effective/start dates.

• A Request for Bilingual Differential WILL NOT be processed for a classification when a language skills specific classification exists for the same class. (Ex. Parent Resources Assistant versus Parent Resource Assistant‐Spanish Language).

Classified Substitute Unsatisfactory Report

Key Information

• Complete form thoroughly and accurately.

• Accurately report the dates of assignment. 

• Use the “COMMENTS” section to clarify or specifically describe the unsatisfactory service.

• The administrator or office manager MUST discuss report with substitute and provide a copy to the substitute employee.

• Remember that the substitute pool is only as good as the substitutes who work in it.

REMINDER

Complete the Classified Substitute Unsatisfactory Report thoroughly, discuss report with the substitute employee, and provide a copy of the report to the substitute employee.

Let’s Wrap•Fill out forms completely and accurately

The forms discussed today can be found on the Personnel Commission website.  From the main LAUSD.net website, click on “Offices”, then “Personnel Commission”.  The classified forms are under the “Quick Links” section to the right of the webpage.
