1.1. MENGENAL PARAMETER YANG DIBUTUHKAN DALAM PEMBERIAN BANTUAN NAPAS BUATAN
PERSENTASI OXYGEN [FiO2], adalah kadar oxygen yang terkandung dalam gas yang dibutuhkan oleh paru pasien pada saat pasien tarik napas [INSPIRASI]. Dengan adanya MIXER pada ventilator, maka FiO2 dapat diatur dari 0,21 s/d 1. Atau bisa juga diartikan sebagai Persen, yaitu dari 21% s/d 100%. MIXER adalah sebuah alat pencampur 2 macam gas OXYGEN. Yang pertama yaitu gas Oxygen murni [100%] yang biasa disebut sebagai Oxygen tabung atau Oxygen sentral yang sudah disediakan outletnya di setiap ruangan di dalam rumah sakit. Dan yang kedua adalah gas dari UDARA luar [AIR] yang di-TEKAN yang sering disebut sebagai UDARA TEKAN [21%], untuk menghasilkan gas 21% [AIR] ini digunakan alat yang disebut kompresor. Pada umumnya rumah sakit menggunakan SENTRAL AIR [Udara Tekan].

BUATAN TIDAL VOLUME [Vt], adalah banyaknya jumlah gas oxygen yang masuk ke dalam paru-
paru. Satuannya dalam mililiter atau Liter. Besarnya volume yang diberikan kepada pasien berbeda-beda, tergantung dari BERAT BADAN-nya [BB]. Pada pasien BAYI biasanya digunakan 5-7 mL/kg x BB. Kalau pada pasien ANAK 7-8 mL/kg x BB. Sedangkan untuk pasien DEWASA antara 8-10 mL/kg x BB. Di dalam menentukan berapa nilai Tidal Volume yang akan diberikan pada pasien, harus diperhatikan juga kondisi klinis dari paru pasien.
CONTOH, Ada pasien Bayi dengan Berat Badan 3 kg, maka Tidal Volume yang diberikan adalah 3 kg x 5-7 mL/kg. Jadi Tidal Volumenya antara 15-21 mL. Kalau kondisi elastisitas [Compliance] parunya baik, bisa diberikan 15mL, tapi kalau kondisi Compliance parunya menurun/memburuk, maka Tidal Volumenya bisa
diberikan sampai 21 mL.

• Gambar Ambu Bag yang di atas kondisinya belum ditekan, jadi TEST LUNG nya belum mengembang.
• Ketika Ambu Bag ditekan maka terjadi aliran gas Oxygen dari BAG menuju TEST LUNG, sehingga TEST LUNG mengembang dan berisi gas Oxygen yang disebut Tidal Volume.

PRESSURE/TEKANAN, karena adanya elastisitas [Compliance] pada paru-paru, dan dengan masuknya sejumlah gas Oxygen di dalam paru, maka akan menimbulkan tekanan [PRESSURE] pada dinding paru-paru tersebut. Satuannya dalam cmH2O.
Contoh, Ada Pasien Anak dengan Berat Badan 10 kg, maka untuk mencapai Tidal Volume 70 mL minimal dibutuhkan tekanan sebesar 10 cmH2O untuk elastisitas paru yang normal. Tapi kalau kondisi elastisitas parunya menurun/memburuk, maka tekanan yang diberikan menjadi lebih besar [>] dari 10 cmH2O. Batas tekanan maksimum yang masih mungkin bisa diberikan pada pasien Bayi dan Anak antara 30 s/d 35 cmH2O. Selain faktor elastisitas [Compliance], ada juga faktor hambatan [Resistance] seperti lendir yang menghambat jalan masuk udara ke dalam paru-paru. Dua faktor ini harus selalu diperhatikan, disamping kondisi klinis lain di paru. Di bawah ini ada beberapa gambar dan keterangan yang

RESPIRATORY RATE [RR], adalah banyaknya bantuan napas yang diberikan oleh mesin ventilator pada pasien dalam waktu satu menit. Satuannya dalam Breath/Menit [BPM]. Pada pasien Bayi antara 30-60 BPM, pada pasien Anak antara 20-30 BPM, dan pada pasien dewasa antara 12-20 BPM. Pada saat kita menentukan nilai RR pasien, tidak hanya ditentukan dengan apakah dia adalah pasien Bayi, Anak, atau Dewasa. Tapi juga ditentukan oleh kondisi klinisnya saat itu. Misalnya PCO2-nya tinggi, PaO2-nya rendah, dll.
INSPIRATORY TIME [Ti], adalah lamanya waktu yang dibutuhkan selama pasien tarik napas, dalam satuan detik. EXPIRATORY TIME [Te], adalah lamanya waktu yang dibutuhkan selama pasien buang napas, dalam satuan detik.
I:E RATIO, adalah perbandingan antara INSPIRATORY TIME dengan EXPRIRATORY TIME. Untuk menentukan nilai perbandingannya disesuaikan dengan kondisi klinis pasien. Normal I:E = 1:2 pada umumnya. CONTOH, Ada pasien Anak dengan Berat Badan 10 kg, RR yang diinginkan 30 BPM, I:E = 1:3. Maka kita dapat menghitung 1x Napas-nya adalah 2 detik, Ti-nya adalah 0,5 detik, dan Te-nya adalah 1,5 detik.Sebaiknya RR, Ti, Te, dan I:E RATIO ditentukan berdasarkan kondisi klinis pasien saat itu.

PEAK FLOW [PF], adalah maksimum kecepatan aliran gas oxygen yang masuk ke dalam paru-paru. Satuannya adalah Liter Per Minute [LPM], nilainya tergantung dari besarnya Tidal Volume paru pasien [Vt] yang dibagi dengan lama waktu tarik napasnya [Ti].CONTOH, Ada pasien anak dengan Berat Badan 20 kg, RR-nya 20 BPM, dan I:E RATIO-nya 1:2. Kondisi compliance parunya normal. Maka Tidal Volume-nya adalah 7 mL/kg x 20 kg = 140 mL. Lama waktu tarik napasnya [Ti] adalah [60/20]x1/[1+2] = 1 detik.
