WORKING WITH THE NEWBORN CHART Once a baby is born and the unit clerks have completed the admission process, the delivery nurse copies maternal history to the baby’s chart by creating a Mother-Baby Link. Once this is done you can document the baby’s initial exam, progress notes and discharge exam on the Newborn Record. The default Navigation (Home) Page contains maternal links. In order to access the newborn chart that functions for a baby’s chart in CPN you must change to Newborn View.

MOTHER-BABY LINK Low-Risk Maternity physicians, pediatricians, midwives and Family Medicine residents must complete the Mother-Baby Link learning section. The Mother-Baby Link copies relevant admission and delivery information from the mother’s chart to the baby’s chart. This minimizes duplicate charting and ensures accurate transfer of data from mom’s chart to baby’s chart. The Mother-Baby Link must be updated after any changes are made to the OB History or Delivery Record after the RN creates the initial link. The Mother-Baby Link also enables switching between the mother’s chart and the baby’s chart without having to use the Select a Patient function from the Navigation Page. Nursing staff will create the initial Mother-Baby Link before patient transfer from L+D to postpartum and it will be updated when Postpartum staff are completing their discharge paperwork.

To switch between a mother’s and baby’s chart using the Mother-Baby Link

1. From the Navigation Screen click Mother-Baby Link 2. From Current List of Linked Patients click the baby’s or mom’s name and then click Select

NEWBORN RECORD Low-risk maternity physicians, Family Medicine residents, midwives and pediatricians are responsible for documenting initial and ongoing newborn findings on the Newborn Record. Neonatologists and Clinical Assists should document using previous method (paper MPR). Residents please DO NOT complete the Date/Time fields on the Newborn Record Residents and students can perform the examination and document findings, but must not enter the Date/Time of the initial or discharge exam. Doing so electronically signs the Newborn Record and because this is considered a discharge summary, only the attending physician has the authority to sign this document. To document and sign the initial exam and enter progress notes on the Newborn Record

1. Open the baby’s chart and change to Newborn view 2. Click Newborn Record. Some fields will contain information from the maternal delivery chart,

such as Apgar scores, initial measurements and estimated gestation age 3. In the Baby at Risk box select the appropriate risk factor status at birth 4. Beside Initial Newborn Admission Assessment Done in NICU, click Yes or No. Confirm the EGA by

exam and determine if the size falls within or outside expected ranges

5. Select appropriate boxes for your assessment. If applicable, type antenatal ultrasound recommendations, description of anomalies and assessment notes Please take this opportunity to input some data into the chart- for example that the baby

has bruising due to 2 vacuum pop-offs or that you noted a deep sacral dimple 6. For ongoing assessment documentation, type in the Progress Notes box. Make sure to include

the date and time of the progress updates and your initials at the end of the progress note. Please take this opportunity to input some data into the chart- for example that you

were called re: a low BG or possible SGH

7. To automatically add your electronic signature to the Initial Examination section, enter the date and time of the initial exam. Residents and students must not enter an Exam date/time. Only the most responsible attending physician or midwife can sign the Newborn Record

To document and sign the discharge exam on the Newborn Record

1. Open the newborn’s chart. Change to Newborn View. Click Newborn Record 2. Scroll to the bottom of the Newborn Record form. The last recorded weight from the Nursing

Flowsheet appears in the Discharge Weight box. Select Normal or Abnormal and type notes in the Discharge Exam box and Discharge Diagnosis, for example, “normal newborn”

3. To sign the Discharge Examination section, complete the date and time of the Discharge Exam in Sections E and F. Residents/Students please DO NOT complete Date/Time fields Only the most responsible attending physician or midwife can sign the Newborn Record

4. If a resident performed and documented the examination, the attending physician and review and ‘sign off’ the resident’s work by entering the Date/Time Discharge Exam in Sections E+F

NOTE: Printing the Newborn Record On the postpartum unit you will not be required to complete this step- nursing will print the Newborn Record when discharge in imminent to ensure the most up-to-date documentation is captured. For this module you have the option of printing the Newborn Record (instructions on the last page of this manual) so you can visualize what the completed document looks like and how the data you entered populates the form.

NOTES REGARDING CHART COMPLETION

If you are browsing through the newborn and maternal charts you will see that various amounts of documentation have been completed. For this module does not matter which type of dyad you select as your patient- just be aware you will encounter this in the training environment and on the postpartum unit. RED Dyads- The amount of completion noted in these charts would be

similar to that of a baby who had recently been admitted to postpartum (i.e. when the initial assessment would be completed by a Newborn Care Provider)

GREEN Dyads- The amount of completion noted in these charts would be

similar to that of a baby who will be going home shortly (i.e. after nursing has completed the postpartum discharge paperwork/portion of the PNOB- when the discharge assessment would be completed by a Newborn Care Provider)
